1.Effect of the composition and structure of the drug delivery device on the drug release rate.

To investigate the effects of the property of drugs and the structure of drug delivery devices on the drug release rate, the effects of sealing methods, length, thickness and drug-loading manner of the silicone tubes on the drug release rate were examined using progestin, testosterone, estradiol as the delivery drugs. The results showed that the property of the drug was the crucial factor to the drug release rate. The sealing methods, length, thickness and drug status of the silicone tubes had significant effects on the drug release rate and the effects were closely related to the property of the drugs. In addition, our newly developed glass-silicone tube has larger drug deposition capability and smaller drug release area, offers an effective and convenient method for the sustained drug delivery with quick release traits in vivo.

2.Short-term results of incremental penile girth enhancement using liquid injectable silicone: words of praise for a change.

AIMTo report our experience with penile girth augmentation using liquid injectable silicone.
METHODSBetween August 2003 and July 2006, 324 men (mean age 35 years, range 19-65 years) received a series of liquid silicone subcutaneous injections between the penile skin and the corpora cavernosa on the dorsal and lateral aspects of the penile shaft, under local anesthesia. Digital photographs taken pre- and post-procedure (n = 324), and penile contour measurements (n = 30) yielded objective results. Subjective results were derived from patient and partner testimony of satisfaction. Follow-up averaged 20 months (range 1-36 months).
RESULTSThree hundred and twenty-four procedures were primary augmentations. Most men (61%) were married, 7% were accompanied by their partners, and 93% were circumcised. The mean measured penile circumference was 9.5 cm (7.5-11.5 cm) pretreatment and 12.1 cm (10.3-15.3 cm) post-treatment (mean increase of 27% in circumference and 0.84 cm in diameter). Patient and partner satisfaction was already expressed after the first two treatments. Sexual activity could be resumed after 8 h. Complications (mild bruising) were easily resolved.
CONCLUSIONPenile girth augmentation using liquid injectable silicone yields very satisfactory short-term results with no immediate or short-term complications.

3.Clinicopathologic Review of Pulmonary Silicone Embolism with Special Emphasis on the Resultant Histologic Diversity in the Lung: A Review of Five Cases.

It is known that the subcutaneous injection of silicone can lead to severe pulmonary complications, followed in some patients by respiratory failure. Currently, silicone is being increasingly applied in the field of plastic surgery and, unfortunately, the illicit injection of silicone fluid by uncertified practitioners is not uncommon in Korea. We offer a critical pathologic review of 5 cases of pulmonary silicone embolism following illegal injection to the vaginal wall, four of which were fatal and came to legal autopsy. Our findings again confirm that subcutaneously injected silicone can gain access to the pulmonary vascular tree and cause pulmonary embolism. The histologic changes observed in the lung are variable and include four patterns i.e., the mere presence of silicone emboli, congestion and hemorrhage, acute pneumonitis, and diffuse alveolar damage despite the severe critical course in all cases. We were unable to find any histologic pattern that correlates well with the clinical course. Apart from producing emboli in the pulmonary vessels, subcutaneous injection of silicone can obviously cause serious pulmonary disease due to its ability to induce acute to induce acute pneumonitis and even possibly acute respiratory distress syndrome.

4.The penis prolongation and augmentation combined with autologous granular fat injection and silicone implantation.

OBJECTIVETo prevent the retraction of the penis after prolongation and augmentation.
METHODSAfter all the superficial and part of the deep suspensory ligament amputation, we implanted the silicon sheet (the length 2.3-3.6 cm, the width 1.5-2.5 cm, the thickness 2-3 mm) and injected autologous granular fat (30-48 ml) into penis.
RESULTS16 patients (age 22-63 years, averagely 38 years) underwent this kind operation, the prolongation length is 1.8-5.1 cm, the average was 2.91 cm, the increased diameter of penis was 0.6-1 cm, the average is 0.85 cm, the following period is 3 months to 2 years. The results are satisfactory with the penis retraction less than 8%, and less than 10% decrease in diameter.
CONCLUSIONSThis method is an ideal way of the penis prolongation and augmentation, the implantation of the silicon sheet is effective way to prevent the retraction of the penis.

5.Twenty-three cases of recurrent dacryocystitis treated with modified silicone mold implantation.

OBJECTIVE:
To observe the surgical therapeutic effects of dacryorhinocystotomy under nasal endoscope combined with the application of modified silicone mold implantation for postoperative recurrent dacryocystitis.
METHOD:
The research method is to conduct the dacryorhinocystotomy under nasal endoscope to 23 cases(24 eyes) of recurrent dacryocystitis, in the process of which modified silicone mold is implanted under the nasal mucosa.
RESULT:
During a follow-up of 6-12 months to the 23 cases (24 eyes), 18 eyes were cured, 6 eyes were getting setter and there was no recurrence.
CONCLUSION
It's concluded that the modified silicone mold implantation can avoid the recurrent atresia of the aperture created by the dacryorhinocystotomy, which proves that the combination with the modified silicone mold implantation in dacryorhinocystotomy is an effective treatment method of curing recurrent dacryocystitis.

6.A False Positive 18F-FDG PET/CT Scan Caused by Breast Silicone Injection.

We present here the case of a 40-year-old woman with a greater than 10 year prior history of bilateral breast silicone injection and saline bag implantation. Bilateral palpable breast nodules were observed, but the ultrasound scan was suboptimal and the magnetic resonance imaging showed no gadolinium-enhanced tumor. The 18F-FDG PET/CT scan showed a hypermetabolic nodule in the left breast with a 30% increase of 18F-FDG uptake on the delayed imaging, and this mimicked breast cancer. She underwent a left partial mastectomy and the pathology demonstrated a siliconoma.
