1.The effect of perindopril and metoprolol in the treatment of geriatric chronic heart failure

Objective To evaluate the clinical therapeutic efficacy of perindopril and metoprolol in the treatment of chronic heart failure in old patients( ≥60 years old).Methods 136 old patients( ≥60 years old) with chronic heart failure were randomly divided into the treatment group(n =68) and the control group(n =68).The control group received the routine treatment with digitalis,diuretics and nitroglycerin.The treatment group was given perindopril and metoprolol orally besides the routine treatment.The perindopril was taken orally 2mg per time,and once a day,and gradually raised to maximum does 8mg per time,once a day.The metoprolol was taken orally 25mg per time,twice a day,also gradually raised to maximum dose 25mg per time,twice a day.4-8 weeks were a treatment cycle.The cardiac function,left ventricular ejection fraction ( LVEF),the 6-minute walk and plasma brain natriuretic pep-tide (BNP) were observed before the beginning of the therapy and after,respectively.Results All patients improved their cardiac function,LVEF,plasma BNP and the 6-minute walk significantly after the therapy ( t =2.56,3.23,2.48,2.53,2.79,3.31,all P < 0.05 ).The improvement of the treatment group was more significantly than that of the control group ( t =2.87,3.12,2.59,all P < 0.05 ).The total effective rate of treatment group was 94.1%,significantly higher thar 70.6% of control group( x2 =5.96,P <0.05 ).Conclusion The therapy of perindopril and metoprolol in old patients with chronic heart failure could improve ventvicular remodeling and the effect.
2.Design, synthesis and antitumor activities of novel E-substituted 2,3-diaryl propenoic acyloxy phosphonate derivatives.

According to the super-position principle of the reinforcement of biological activities, a series of novel E-substituted 2, 3-diaryl propenoic acyloxy phosphonate derivatives were designed and synthesized. And the structures of the target compounds were confirmed by IR, 1H NMR, 13C NMR and elemental analysis. Furthermore, the cytotoxicities of all compounds on A-549, SGC-7901 and EC-109 in vitro were evaluated by MTT assay, and some of them showed good antitumor activity. Among the active compounds, especially, the IC50 value of compound 3e was (12.7 ± 1.9) μmol x L(-1) against A-549 cells, similar to cisplatin [IC50 = (8.0 ± 1.5) μmol x L(-1)], compounds 3g and 3k had better inhibition effect on EC-109 cells growth, with the IC50 values of (9.5 ± 1.8) μmol x L(-1) and (11.5 ± 0.9) μmol x L(-1) respectively, and compounds 3i and 3k exhibited good cytotoxic property on A-549, SGC-7901 and EC-109, which were worth further investigation.
3.Etiologic Analysis of Rotavirus Infection in Adults with Acute Gastroenteritis in Shanghai Changning District from 2010 to 2013

Background:Acute gastroenteritis is the second largest public health problem in the world. Rotavirus(RV)is one of the pathogens of acute gastroenteritis in adults,researches focusing on RV infection may provide the basis for prevention and control of the disease. Aims:To determine the epidemiological characteristics of RV infection in adults with acute gastroenteritis in Shanghai,China. Methods:From Jun. 2010 to Dec. 2013,stool samples and clinical data in adults with acute gastroenteritis in a designated hospital in Shanghai Changning District were collected. ELISA and PCR were used to detect RV infection and its genotypes. Results:A total of 1 554 eligible stool samples from acute gastroenteritis patients were recruited,of them 691 were males and 863 were females,the mean age was(46. 19 ± 15. 59)years old. RV was detected in 189 patients with a detection rate of 12. 2% ,163(10. 5% )were categorized as group A RV and 26(1. 7% ) were group B/ C RV;the most common genotypes in group A RV were G9(30. 1% )and G1(25. 2% ). Watery stool and vomiting were more prevalent in RV-positive patients than in RV-negative patients(P < 0. 05). The detection rates in years 2010,2011,2012 and 2013 were 12. 2% ,14. 9% ,6. 8% and 16. 3% ,respectively. When analyzed by age group,the detection rate was significantly lower in 18-39 years group than those in 40-59,60-79,and ≥80 years groups(8. 7% vs. 14. 8% ,14. 2% ,and 17. 1% ,P < 0. 05). The peak of epidemic was from Nov. to next Feb. Conclusions:RV infection in adults with acute gastroenteritis is more popular in middle aged and elderly people and shows a winter seasonality in Shanghai,China. The most common genotypes of group A RV are G9 and G1.
4.Pulmonary function of preterm infants with bronchopulmonary dysplasia from 0 to 6 months

Objective:Through analyzing pulmonary function and clinical characteristics of preterm infants with bronchopulmonary dysplasia (BPD) from 0 to 6 months, the characteristics of pulmonary function in infants with BPD were investigated.Methods:A retrospective study was conducted on 85 infants hospitalized in the Department of Neonatology of the First Affiliated Hospital of Shantou University Medical College from December 2015 to December 2017, including 25 preterm infants with BPD (BPD group), 30 preterm infants without BPD (preterm control group), and 30 term infants without respiratory diseases (full-term control group), respectively.Pulmonary function were tested in preterm infants at 37 to 41 weeks of corrected age as well as at 6 months of corrected age, and full-term infants at 3 days to 1 week after birth as well as at 6 months old.The outcomes of respiratory system diseases at 6 months of corrected age in two groups of preterm infants were followed up.Results:(1) While preterm infants at 37 to 41 weeks of corrected age and full-term infants at 3 days to 1 week after birth, time to peak tidal expiratory flow/expiratory time(TPTEF/TE)and volume to peak tidal expiratory flow/exhaled volume(VPTEF/VE) of BPD group were lower than those of the other two groups ( P<0.05), while the differences between preterm control group and full-term control group is not significant.VPTEF, peak expiratory flow (PEF), tidal expiratory flow when 75% of tidal volume reminds in the lung (TEF75%), TEF50%, TEF25% in BPD group and preterm control group were lower than those in the full-term group ( P<0.05). The differences between BPD group and preterm control group were not significant ( P>0.05). The BPD group had significantly lower TPTEF/TE, VPTEF/VE and ratio of tidal expiratory flow and tidal inspiratory flow when 50% of tidal volume reminds in the lung (TEF50%/TIF50%) and higher VPTEF, PEF, TEF25% at 6 months of corrected age than those at 37 to 41 weeks of corrected age ( P<0.05). While at 6 months of corrected age, the BPD group had higher PEF than that in the preterm control group ( P<0.05). There was no statistically significant difference between that in the occurrence of lower respiratory tract infections (43.3% vs.16.7%), wheezing (21.7% vs.8.3%), rehospitalization (39.1% vs.16.7%) between BPD group and preterm control group within 6 months of corrected age ( P>0.05). Conclusion:Infants with BPD had small airway obstruction at 37 to 41 weeks of corrected age and may not improve significantly at 6 months of corrected age.
5.Interferon-α modulates HBV specific T-cell response to inhibit viral gene expression in chronic HBV infection mouse model

Objective To explore the antiviral effect and mechanism of interferon-α (IFN-α) in chronic HBV infection mouse model by hydrodynamic tail vein injection. Methods Chronic HBV infection mouse model was constructed with C57BL/6j mice by hydrodynamic tail vein injection of pAAV-HBV1.2 plasmid. Serum IFN-α was determined by ELISA after injection of IFNα- expressing plasmid (i.e., plasmid pKCMvint.IFN-α-2a) or control plasmid pKCMvint. HBsAg and HBeAg levels were assayed on Abbot ARCHITECT i2000SR. Total lymphocytes in liver or spleen were counted and the frequency and function of CD8+T cells and HBV-specific CD8+T cells were analyzed by flow cytometry. Results Serum IFN-α expression level was significantly higher in the animals injected with pKCMvint.IFN-α-2a plasmid than in control group (P<0.01). Serum HBsAg decreased quickly 12 days after injection and significantly lower than control group. Serum HBeAg was undetectable after IFN-α expression. Interestingly, the frequency of CD8+T cells in the liver of animals injected with pKCMvint.IFN-α-2a plasmid was significant higher than control group (P<0.05), while total liver lymphocytes and HBV-specific CD8+T cells had a similar trend. Consistently, the HBV-specific CD8+T cells in IFN-α-expressing animals showed higher level of producing IFN-γ, TNF-α and IL-2 than control group. IFN-γ production was significantly different between IFN-α- expressing group and control group (P<0.05). Conclusions IFN-α can increase the frequency and function of liver CD8+T cells to inhibit HBV gene expression in chronic HBV infection mouse model.
6.Investigation of the Soochow University model of comfortable medical procedures during peri-anesthesia period for patients undergoing radical resection of lung cancer through thoracoscope

Objective:To construct a Soochow University model of comfortable medical procedures during peri-anesthesia period for patients undergoing radical resection of lung cancer through thoracoscope.Methods:In this prospective study, eight hundreds and sixty patients undergoing radical resection of lung cancer through thoracoscope were enrolled in our hospital in 2018.During peri-anesthesia period, the 860 patients were randomly(by the random number table method)divided into the control group(the group C receiving routine medical procedures, n=430)and the comfort group(the group S receiving the comfortable medical procedures of Soochow University model, n=430). The scores of Visual Analogue Scale(VAS), Richmond Agitation-Sedation Scale(RASS), and Thirsty Analogue Scale(TAS)were recorded at 5 min after extubation(T1), at out of the post-anesthesia care unit(T2), at 1 h(T3), 6 h(T4), 12 h(T5), 24 h(T6), 2 d(T7)and 3 d after surgery(T8)in two groups.The incidences of postoperative sore throat(POST), nausea and vomiting(PONV), catheter-related bladder discomfort(CRBD), thirst and hypothermia were recorded at T2, T6 and T8, respectively.Meanwhile, the peri-anesthesia comfort questionnaire(PCQ)and peri-anesthesia satisfaction questionnaire(PSO)were completed at T6 and T8.Results:The incidences of POST(48.6% vs.16.2%), PONV(24.9% vs.13.0%), CRBD(78.8% vs.20.9%)and thirst(74.9% vs.20.0%)were higher in the group C than in the group S at T2( P<0.05). The comfort score and satisfaction score were lower in the group C than in the group S at T6(3.14±1.04 vs.4.92±1.42, 2.67±0.89 vs.3.30±1.01)and at T8(3.84±1.83 vs.5.05±1.77, 2.74±0.84 vs.3.26±1.06)( P<0.05). Conclusions:The Soochow University model of comfortable medical procedures during peri-anesthesia period for patients undergoing radical resection of lung cancer through thoracoscope has been successfully constructed, and this model can reduce the incidence of complications, and improve the comfort and satisfaction during peri-anesthesia period.
7.Association of 5-HTR1A gene C- 1019G polymorphism with antidepressant efficacy :a meta-analysis

OBJECTIVE To evaluate the correlation between 5-HTR1A gene C- 1019G polymorphism and the efficacy of antidepressants. METHODS PubMed,Embase,Cochrane Library ,CNKI,Wanfang database ,CBM and VIP database were searched for domestic and foreign literatures on the correlation between 5-HTR1A gene C- 1019G polymorphism and antidepressant efficacy. The retrieval time limit was from the inception to February 2022. According to different outcome measures of drug response,Stata 14.0 and RevMan 5.4 software were used for meta-analysis of efficacy group and remission group ,respectively. RESULTS A total of 18 literature were included. The combined results showed that among recessive gene model in valid group , the correlation of 5-HTR1A gene C- 1019G polymorphism with the efficacy of antidepressants had statistically significance in Asian population(GG vs. CG+CC ,OR=0.751,95%CI=0.585-0.964,P=0.024). There was statistical significance in the correlation of 5-HTR1A gene C- 1019G polymorphism and the efficacy of antidepressants in China (GG vs. CG+CC ,OR=0.677,95%CI= 0.508-0.901,P=0.007). There was no statistical significance in the correlation of 5-HTR1A gene C- 1019G polymorphism and the efficacy of antidepressants in remission group (P>0.05). CONCLUSIONS In the effective group ,5-HTR1A gene C- 1019G polymorphism is correlated with the efficacy of antidepressants in the Asian population and the Chinese population ;while in the remission group ,it is not proved that this polymorphism is correlated with the efficacy of antidepressants.
8.Research progress in decision-making aids for breast cancer patients.

Decision-making aid for cancer patients is of great significance in the diagnosis and treatment for diseases. Breast cancer is one of the most common malignant tumors in women all over the world, and breast cancer patients have become the main target population for decision-aided research. Application of decision-making assistance for patients in Western countries has developed to a certain extent, while relevant research in China is still at the early stage. There are kinds of intervention forms for patients' decision aids, including traditional brochures and videos, decision aids systems, decision coaching, multidisciplinary breast cancer teams, etc. The tools for decision-making quality evaluation include the patients' awareness for decision-making, participation, decision-making conflict, decision-making satisfaction, decision-making regret, which can provide important guidance for the application of decision-making aid treatment in breast cancer patients in the future.

9.Research progress in the relationship between depression and survival outcome in patients with lung cancer.

In recent years, with the high incidence of lung cancer and mortality, the disease-relevant mental health problems such as depression began to attract attention. A growing number of studies have also begun to hypothesize and confirm the relationship between depression and survival or mortality in patients with lung cancer, and have made some progress in basic research, prospective cohort research and intervention research. In basic research, cancer can cause depression due to mediating the production of inflammatory factors, and the genotype of tumor epidermis growth factor receptor (EGFR) can explain the high mortality and risk of depression in patients with lung cancer from a certain point of view. Different studies in prospective cohort studies argue that depression is an important predictor of survival in patients with lung cancer and need to be further studied. In the aspect of intervention research, although some studies have confirmed the potential of antidepressants in anti-tumor oxidative therapy, there is no enough evidence in psychological intervention and drug intervention to prove their effectiveness in improving the survival outcome of lung cancer. In the future, it is necessary to further explore the possible mechanisms for antidepressants and psychological intervention to improve the survival time of the patients.

10.Study on Toxic Effect of the Extract from Tripterygium hypoglaucum on Embryo-fetal Development of New Zealand Rabbits

OBJECTIVE:To study the toxic effect of the extract from Tripterygium hypoglaucum on pregnant animal and embryo-fetal development . METHODS :Successfully mated New Zealand female rabbits were randomly divided into solvent control group,positive control group (cyclophosphamide,20 mg/kg)and T. hypoglaucum extract high-dose ,medium-dose and low- dose groups(15.0,7.50,3.75 g/kg,by the amount of crude drug )according to their body weight ,with 18 rabbits in each group at least. The female rabbits in solvent control group and T. hypoglaucum extract groups were given water or the corresponding T. hypoglaucum extract solution from 6th to 18th day of pregnancy ,5 mL/kg,once a day. Positive control group was given cyclophosphamide subcutaneously into the neck from 10th to 13th day of pregnancy ,1 mL/kg,once a day. According to the related requirements of Technical Guidelines for the Study of Drug Reproductive Toxicity ,the general situation ,body weight ,body weight increase and food intake of female rabbits were observed and recorded during the experiment ,and euthanasia was carried out on 28th day of pregnancy ;the relative indexes of main organs ,fetal uterus ,placenta uterus ,placenta,ovary,implantation gland , absorption fetus ,stillbirth,live fetus and corpus luteum were observed and recorded after anatomy ;the relative indexes of body weight,appearance,visceral and skeletal alterations of the fetus were detected . RESULTS :Compared with solvent control group , the body weight ,body weight increase ,food intake ,main organs ,pregnancy of pregnant rabbits ,as well as reproductive function,embryo formation ,fetal growth and development ,appearance,visceral and skeletal development indexes in T. hypoglaucum extract groups had no significant abnormal changes (P>0.05);above indexes in the positive control group had significant changes (P<0.05),and significant maternal toxicity and embryotoxicity were found. CONCLUSIONS :T. hypoglaucum extract 15.00-3.75 g/kg(by the amount of crude drug )have no significant maternal toxicity ,embryotoxicity or fetal development toxicity to New Zealand rabbits.
