1.The effect of perindopril and metoprolol in the treatment of geriatric chronic heart failure

Objective To evaluate the clinical therapeutic efficacy of perindopril and metoprolol in the treatment of chronic heart failure in old patients( ≥60 years old).Methods 136 old patients( ≥60 years old) with chronic heart failure were randomly divided into the treatment group(n =68) and the control group(n =68).The control group received the routine treatment with digitalis,diuretics and nitroglycerin.The treatment group was given perindopril and metoprolol orally besides the routine treatment.The perindopril was taken orally 2mg per time,and once a day,and gradually raised to maximum does 8mg per time,once a day.The metoprolol was taken orally 25mg per time,twice a day,also gradually raised to maximum dose 25mg per time,twice a day.4-8 weeks were a treatment cycle.The cardiac function,left ventricular ejection fraction ( LVEF),the 6-minute walk and plasma brain natriuretic pep-tide (BNP) were observed before the beginning of the therapy and after,respectively.Results All patients improved their cardiac function,LVEF,plasma BNP and the 6-minute walk significantly after the therapy ( t =2.56,3.23,2.48,2.53,2.79,3.31,all P < 0.05 ).The improvement of the treatment group was more significantly than that of the control group ( t =2.87,3.12,2.59,all P < 0.05 ).The total effective rate of treatment group was 94.1%,significantly higher thar 70.6% of control group( x2 =5.96,P <0.05 ).Conclusion The therapy of perindopril and metoprolol in old patients with chronic heart failure could improve ventvicular remodeling and the effect.
2.The current status and development strategies of chronic disease management in China

The chronic non-communicable diseases ( NCDs) cause a very big threat on the health of residents . Therefore , chronic disease management has always been an important part of China's health policy , and hence comes to be the main focus of our work .Through the literature research and related policies analysis , this paper reviews the chronic disease management in China from the macro-policy, meso-coordination between institutions and organiza-tions, and micro-patients and population levels of chronic disease management process to sort out induction .Based on the actual situation of the chronic disease management in China , this paper proposes policy recommendations to fur-ther implement the management of chronic conditions of China .Finally, several development proposals:China's fu-ture chronic disease management works should focus on refining theoretical model and frameworks ;constructing an in-tegrated network through combining the treatment of chronic disease management services with the implementation of healthcare reforms and the tiered-medical systems development; and accelerate the upgrading of chronic disease management's“terminal” capacity-building .
3.Advances in plant anthocyanin transport mechanism.

Anthocyanin biosynthesis is one of the thoroughly studied enzymatic pathways in biology, but little is known about the molecular mechanisms of its final stage: the transport of the anthocyanins into the vacuole. A clear picture of the dynamic trafficking of flavonoids is only now beginning to emerge. So far four different models have been proposed to explain the transport of anthocyanins from biosynthetic sites to the central vacuole, and four types of transporters have been found associated with the transport of anthocyanins: glutathione S-transferase, multidrug resistance-associated protein, multidrug and toxic compound extrusion, bilitranslocase-homologue. The functions of these proteins and related genes have also been studied. Although different models have been proposed, cellular and subcellular information is still lacking for reconciliation of different lines of evidence in various anthocyanin sequestration studies. According to the information available, through sequence analysis, gene expression analysis, subcellular positioning and complementation experiments, the function and location of these transporters can be explored, and the anthocyanin transport mechanism can be better understood.

4.Application of quality coutrol circle in reducing the incidence of adverse events of instrument management

Objective To explore the effect of quality control circle(QCC)on reducing adverse event incidence of the instrument management.Methods QCC group of 7 nurses in the central sterile supply department was set up for QCC training and then analyze the causes of adverse events in the instrument management and finally make countermeasures to apply continuous quality improvement.Result The adverse incidents were significantly decreased from 69 before QCC to 24 after QCC.(χ2=33.694;P<0.01).Conclusion QCC is effective in the reduction of the adverse events in instrument management and improvement of surgical safety.
5.Application of emotional capital theory in activating the nursing hmnan resource

Objective To study the function of emotional capital theory in activating the nursing human resources. Methods We used emotional capital theory together with the use of incentives and humane management of emotional exchange to meet the individual demands of nurses, in order to accumulate good for the intrinsic emotional capital of the hospital. At the same time, attention was paid to detail services and vigorously carried out the moving services to accumulate external emotional capital for the hospital. Results After the implementation of emotional capital management, the satisfaction rate of the nurse improved 3.6% than before, and the satisfaction rate of the patients increased from 94.2% to 98.4%.In addition,the nursing complaints redueed from six down to two, the written recognition received from patients increased from 3 to 8, the quality of the care integrated service score increased from 93.3 points to 97.8 points on average, and the nursing errors reduced from 16 to 8. Condusions Use of emotional capital management theory was conducive to activate the nursing human resources, improve the quality of the nursing service and the efficiency of management.
6.Study on Anaphylactoid Reaction Induced by Shuanghuanglian Injection

Objective To observe the anaphylactoid reactions of Shuanghuanglian injection, determine the serum histamine level of sensitized animal, and to investigate its correlation with anaphylactoid reactions.Methods The guinea pigs were randomly selected to observe the anaphylactoid reactions using Shuanghuanglian injections in the different injection speed or different doses, and the serum histamine and the serum IgE of sensitized animals were determined by ELISA. Results The anaphylactoid reactions were induced by Shuanghuanglian injections in the different conditions of injection speed and doses in guinea pigs. There were no obvious difference about the serum IgE level between Shuanghuanglian group and saline group were injected in injection speed and doses. The serum histamine level was significantly increased in Shuanghuanglian group both different conditions of injection speed and doses, and it related to doses and speed. Conclusion Shuanghuanglian injection can cause anaphylactoid reactions in guinea pigs, and the changes of serum histamine level are related to injection speed and doses of Shuanghuanglian injection.
7.Survey on the relationship between dietarytherapy compliance of diabetes mellitus patients with pulmonary tuberculosis and glycemic control

Objective To investigate the relationship between dietarytherapy compliance of diabetes mellitus patients with pulmonary tuberculosis and glycemic control.Methods According to the three levels (good,medial and bad) of dietary therapy compliance,136 diabetesmellituspatients with pulnonary tuberculosis were divided into three groups signed by group 1 (good compliance),2 (medial compliance) and 3 (bad compliance).Questionnaire survey was appliedto respectively investigate the blood glucose level,body mass index,the sputum negative conversion rate and loci absorption rate of the patients in those groups.Results Of the 136 patients,group 1 had 32 (23.5%) patients,while group 2 had 62 (45.6%)patients,group 3 had 42 (30.9%) patients.Among these three groups,the fasting blood glucose (FBG) was (8.36±1.62) mmol/L,the postprandial 2h blood glucose (PBG) was (10.43±1.69) mmol/L,the HbAlc was (8.15±0.99)%,the body mass index (BMI) was (20.46±1.68) kg/m2,the sputum negative conversion rate was 55.9% (76/136),foci absorption rate was 46.3% (63/136).The comparisons of the three groups on FBG (F=34.01,P< 0.01),PBG (F=30.77,P< 0.01),HbA1c (F=32.35,P< 0.01),BMI (F=16.29,P<0.01),the sputum negative conversion rate(x2=10.49,P < 0.05),foci absorption rate(x2=7.47,P < 0.05) had statistical significance.Thecourse of diabetes (x2=18.14,P< 0.01) and patients' literacy (x2=21.04,P< 0.01) were both influence factors of dietarytherapy compliance.Conclusions Dietarytherapy compliance of diabetes mellitus patients with pulmonary tuberculosis is low and the glycemic control of those patients is poor.The situation of glycemic control and the therapeutic effect,the education about health to patients should be enhanced to make the dietarytherapy compliance better.
8.Design, synthesis and antitumor activities of novel E-substituted 2,3-diaryl propenoic acyloxy phosphonate derivatives.

According to the super-position principle of the reinforcement of biological activities, a series of novel E-substituted 2, 3-diaryl propenoic acyloxy phosphonate derivatives were designed and synthesized. And the structures of the target compounds were confirmed by IR, 1H NMR, 13C NMR and elemental analysis. Furthermore, the cytotoxicities of all compounds on A-549, SGC-7901 and EC-109 in vitro were evaluated by MTT assay, and some of them showed good antitumor activity. Among the active compounds, especially, the IC50 value of compound 3e was (12.7 ± 1.9) μmol x L(-1) against A-549 cells, similar to cisplatin [IC50 = (8.0 ± 1.5) μmol x L(-1)], compounds 3g and 3k had better inhibition effect on EC-109 cells growth, with the IC50 values of (9.5 ± 1.8) μmol x L(-1) and (11.5 ± 0.9) μmol x L(-1) respectively, and compounds 3i and 3k exhibited good cytotoxic property on A-549, SGC-7901 and EC-109, which were worth further investigation.
9.Etiologic Analysis of Rotavirus Infection in Adults with Acute Gastroenteritis in Shanghai Changning District from 2010 to 2013

Background:Acute gastroenteritis is the second largest public health problem in the world. Rotavirus(RV)is one of the pathogens of acute gastroenteritis in adults,researches focusing on RV infection may provide the basis for prevention and control of the disease. Aims:To determine the epidemiological characteristics of RV infection in adults with acute gastroenteritis in Shanghai,China. Methods:From Jun. 2010 to Dec. 2013,stool samples and clinical data in adults with acute gastroenteritis in a designated hospital in Shanghai Changning District were collected. ELISA and PCR were used to detect RV infection and its genotypes. Results:A total of 1 554 eligible stool samples from acute gastroenteritis patients were recruited,of them 691 were males and 863 were females,the mean age was(46. 19 ± 15. 59)years old. RV was detected in 189 patients with a detection rate of 12. 2% ,163(10. 5% )were categorized as group A RV and 26(1. 7% ) were group B/ C RV;the most common genotypes in group A RV were G9(30. 1% )and G1(25. 2% ). Watery stool and vomiting were more prevalent in RV-positive patients than in RV-negative patients(P < 0. 05). The detection rates in years 2010,2011,2012 and 2013 were 12. 2% ,14. 9% ,6. 8% and 16. 3% ,respectively. When analyzed by age group,the detection rate was significantly lower in 18-39 years group than those in 40-59,60-79,and ≥80 years groups(8. 7% vs. 14. 8% ,14. 2% ,and 17. 1% ,P < 0. 05). The peak of epidemic was from Nov. to next Feb. Conclusions:RV infection in adults with acute gastroenteritis is more popular in middle aged and elderly people and shows a winter seasonality in Shanghai,China. The most common genotypes of group A RV are G9 and G1.
10.Changes in expression of NF-κB, IL-6 and TNF-α in spinal cord in a rat model of bone cancer pain

Objective To evaluate the changes in expression of NF-κB, IL-6 and TNF-α in spinal cord in a rat model of bone cancer. Methods Seventy-two female SD rats weighing 150-180 g were randomly divided into 3 groups (n = 24 each): control group (group C);sham operation group (group S) and bone cancer pain group (group BP). Bone cancer was induced by intra-tibial inoculation of 1 × 105 Walker 256 breast cancer cells. Paw withdrawal threshold to mechanical stimulation was measured with yon Frey filaments. The expression of NF-κB p65, IL-6 and TNF-α mRNA in the spinal cord was determined by RT-PCR and the expression of NF-κB p65 by immuno-histochemistry and NF-κB p65 positive cell count was determined. Results The paw withdrawal threshold was significantly lower and the expression of NF-κB p65, NF-κB p65 mRNA, IL-6 mRNA, TNF-α mRNA and NF-κB p65 positive cell count in the spinal cord were significantly higher in group BP than in group C and S ( P <0.05 or 0.01 ). Conclusion Intra-tibial inoculation of Walker 256 breast cancer cells activates NF-κB in the spinal cord, leading to the increased release of IL-6 and TNF-α and mechanical hyperalgesia.
