1.Systemic inflammatory response and the prognosis of esophageal cancer

Systemic inflammatory response has a major role in the development and progression of cancer,and increased systemic inflammatory response usually correlates with poorer survival.Recently the evaluation index of systemic inflammatory response including neutrophil-lymphocyte ratio,C-reactive protein,glasgow prognostic score,platelet-lymphocyte ratio.Several previous studies show that these indexes are the independent prognostic factors for esophageal cancer.Research on the systemic inflammatory markers may also be valuable for the clinical treatment and prognosis of esophageal cancer.
2.Treatment of distal femoral fracture with retrograde intramedullary interlocking naill——report of 30 cases

Objective To retrospectively report 30 cases of distal femoral fractures treated with retrograde intramedullary interlocking naill( GSH nail). Methods From February 1999 to December 2002, 30 cases of distal femoral fractures were treated with GSH nails.According to AO/ASIF classification system, there were 11 cases of type A1, 7 A2, 3 A3, 5 C1, 3 C2 ,and 1 C3 .The follow up period ranged from 6 to 54 months. Results The average time for bone healing was 16 weeks. According to Neer s knee rating scale, there were 21 excellent cases and 5 good; the excellent and good rate was 86.6% . Conclusion It's a good method to treat type A& C (AO/ASIF) distal femoral fractures with GSH nail.
3.Comparison between Traditional Thoracotomy and Video-assisted Thoracoscopy for Coagulated Hemothorax

Objective To investigate the value of video-assisted thoracoscopic surgery(VATS)in the treatment of coagulated hemothorax.Methods A prospective randomized controlled study.Between July 2005 and July 2007,62 patients with coagulated hemothorax were enrolled in this study.The patients were randomly divided into VATS and traditional thoracotomy groups(31 in each).The pathophysiological parameters of the two grups were compared.Results No significant difference in the sex,age,property and location of the wound,and rate of complicated injuries was noticed between the two groups.The mean operation time,duration of postoperative drainage,and postoperative hospital stay of the VATS group were significantly shorter than those in the traditional group [(52.1?24.4)min vs(120.2?47.2)min,t=-7.136,P=0.000;(1.7?0.7)d vs(4.8?1.8)d,t=-8.937,P=0.000;and(12.6?2.4)d vs(18.0?8.9)d,t=-3.262,P=0.002;respectively];and the mean intraoperative blood loss and the volume of thoracic drainage of the VATS group were significantly lower than those in the traditional group [(137.1?14.6)ml vs(203.2?53.4)ml,t=-6.648,P=0.000;and(181.3?37.9)ml vs(253.9?64.0)ml,t=-5.435,P=0.000,respectively].No significant difference in the rate of complications existed between the two groups(0 vs 1 case,?2=0.000,P=0.000).Conclusion Video-assisted thoracoscopy is feasible and safe for coagulated hemothorax and is worth being widely used.
4.Efficacy of PVP with high viscosity bone cement and PKP with low viscosity bone cement for osteoporotic vertebral compression fractures

Objective By comparing clinical efficacy of percutaneous vertebroplasty with high viscosity bone cement and percutaneous kyphoplasty with conventional low viscosity bone cement for osteoporotic vertebral compression fractures,to explore the clinical value of treatment of osteoporotic vertebral compression fractures with high viscosity bone cement.Methods In this prospective study,100 patients with one-level OVCF and older than 70 years were diagnosed and treated in our hospital from June 2014 to December 2015,which were randomly divided into 2 groups (50 patients in each),applied unilateral percutaneous vertebroplasty with high viscosity bone cement and bilateral percutaneous kyphoplasty with conventional low viscosity bone cement,respectively.Collected data of basic clinical informations (including age,gender,body mass index,etc.),preoperative and postoperative visual analog scales (VAS),Oswestry disability questionnaire (ODI),operative time,intraoperative X-ray times,the amount of bone cement injection,leakage of bone cement,anterior vertebral height and kyphosis (Cobb angle) of preoperative and postoperative,and statistical analysis.Results There were significant improvement of both VAS and ODI scores of the postoperative 1 day,3 months and 12 months compared with preoperative,but there was no significant difference between two groups;The operative time,intraoperative fluoroscopy times,bone cement injection amount and intraoperative bone cement leakage of high viscosity bone cement PVP group were significantly less than those of low viscosity bone cement PKP group (P<0.05);the anterior vertebral height and kyphosis improved,and there was a significant difference between preoperative and postoperative of low viscosity bone cement PKP group (P<0.05).Conclusion The efficacies of unilateral PVP with high viscosity bone cement and bilateral PKP with conventional low viscosity bone cement for osteoporotic vertebral compression fractures are the same,which can relieve pain and improve function of life significantly.However,the high viscosity bone cement PVP is simpler surgical procedure,with shorter operative time,less intraoperative radiation,less bone cement leakage rate.The domestic bone cement injection device can significantly reduce the cost of surgery,it worth to be widely used for clinical application.
5.Factors affecting dyslipidemia among adult residents in Hefei City

Objective:
To investigate the prevalence and influencing factors of dyslipidemia among adult residents in Hefei City, so as to provide the evidence for formulating the dyslipidemia control strategy.
Methods:
A total of 10 812 permanent residents at ages of 18 years were randomly sampled from 5 counties (districts) in Hefei City using a multi-stage stratified cluster random sampling method. Participants' demographic features, dietary behaviors and development of chronic diseases were collected using questionnaire surveys, and the height, body weight, waist circumference and blood lipids were measured. The prevalence of dyslipidemia was analyzed, and the factors affecting the development of dyslipidemia were identified using a multivariable logistic regression model.
Results:
Totally 10 812 valid questionnaires were recovered, with an effective recovery rate of 92.02%. The participants had a male to female ratio of 0.85∶1, a mean age of (51.89±12.56) years. The prevalence of dyslipidemia was 25.20%, and hypertriglyceridemia and low high-density lipoprotein cholesterol (HDL-C) were predominant types of dyslipidemia, with prevalence of 12.70% and 10.20%, respectively. Multivariable logistic regression analysis showed that urban areas (OR=1.340, 95%CI: 1.219-1.474), age of 40 years and older (40 to 59 years, OR=1.181, 95%CI: 1.047-1.333; 60 to 88 years, OR=1.321, 95%CI: 1.154-1.513), alcohol consumption (OR=1.205, 95%CI: 1.093-1.329), insufficient physical activity (OR=1.217, 95%CI: 1.111-1.332), static behavior duration of 3 h/d and longer (OR=1.126, 95%CI: 1.027-1.234), overweight (OR=1.162, 95%CI: 1.045-1.291), obesity (OR=1.310, 95%CI: 1.097-1.564), central pre-obesity (OR=1.236, 95%CI: 1.102-1.387), central obesity (OR=1.414, 95%CI: 1.247-1.603), hypertension (OR=1.279, 95%CI: 1.107-1.477), diabetes (OR=2.192, 95%CI: 1.820-2.640), medical insurance (OR=0.594, 95%CI: 0.395-0.893) and self-rated “very good/good” oral health (OR=0.731, 95%CI: 0.576-0.927) were factors affecting the development of dyslipidemia.
Conclusion
The risk of dyslipidemia is high among adult residents living in urban areas, middle-aged and elderly residents, residents with alcohol consumption, insufficient physical activity, overweight, obesity, central pre-obesity, central obesity, hypertension and diabetes.
6.Efficacy analysis of open reduction internal fixation in 81 cases with rib fracture

Objective To investigate the effecacy of open reduction internal fixation in cases with rib fracture. Methods The pain scores,respiratory related clinical indicators change of 81 rib fractuure cases underwent open reduction internal fixation surgery were ob-served. A comparison was made between the surgical group and the non-surgical group of 37 cases on pain scores trend and hospital stay. Re-sults All operations were successfully completed and postoperative recovery was good. Postoperative pain scores and respiratory rate was sig-nificantly lower(P=0. 000),Oxygen saturation was significantly higher(P=0. 002),the proportion of expectoration difficult,dyspnea and ac-tivities inconvenience were significantly lower(P<0. 05). Pain scores of surgical group significantly decreased after surgery peak hours that was the fourth day hospitalized,but the overall decline of pain scores in non-surgical group was slow. The hospital stay between the surgical group and the non-surgical group was not significantly different(P=0. 084). Conclusion The efficacy of open reduction internal fixation is significant to rib fracture with exact surgery indications,and appropriate timing of surgery is the fourth day after admission.
7.The abnormal expression of microRNA-25 in the serum of gastric cancer patients and its clinical signification

Objective To investigate the abnormal expression of microRNA (miRNA)-25 in the serum of gastric cancer patients and its clinical significance. Methods In Xijing Hospital of Digestive Diseases,Fourth Military Medical University,86 gastric cancer patients with operation and completed follow-up data,70 gastric adenoma patients and 80 healthy controls were selected as study objects.Total RNA was isolated from the serum. After the stable and sensitive miRNA-25 absolute quantity detection method established,the serum levels of miRNA-25 in gastric carcinoma patients,gastric adenoma patients and healthy controls were tested according to this method. The expression differences of miRNA-25 in the serum of patients with gastric cancer and gastric adenoma and healthy controls were analyzed with statistic analysis,and the correlation between miRNA-25 expression level and clinic pathological features of gastric cancer was also analyzed. Results The expression level of miRNA-25 in the serum of gastric cancer patients (135. 6 fmol/μg total RNA) was significantly higher than that of gastric adenoma patients (67. 7 fmol/μg total RNA) and healthy controls (62. 2 fmol/μg total RNA)(P<0. 01). The receiver operating characterisstic curve of miRNA-25 indicated that serum miRNA-25 with good specificity and sensitivity in gastric cancer diagnosis (AUC=0. 827). The serum level of miRNA-25 in gastric cancer patients with lymph node metastasis [(148. 3±10. 2) fmol/μg total RNA] or clinicopathological stage Ⅲ /Ⅳ patients [(146. 7±9.5) fmol/μg total RNA] was significantly higher than that of gastric cancer patients without lymph node metastasis [(120. 3±10. 1)fmol/μg total RNA] or clinicopathological stage Ⅰ/Ⅱpatients [(119. 4±12. 2) fmol/μg total RNA] (P<0.05). The correlation statistical analysis result indicated that there was no significant difference in survival period between serum miRNA-25 highly expressed and lowly expressed gastric cancer patients (P>0. 05).Conclusion Serum miRNA-25 testing maybe helpful in diagnosis and prognosis of gastric cancer.
8.Diagnosis and treatment of delayed gastric emptying after resection of esophageal and gastric cardia carcinoma

Objective To investigate the causes, diagnosis and methods of prevention and treatment of delayed gastric emptying (DGE) after resection of esophageal and gastric cardia carcinoma. Methods The datas of 19 patients who had DGE after resection of esophageal and gastric cardia carcinoma in hospital from January 1993 to December 2009 were retrospectively analyzed. Results Three cases were confirmed as mechanical DGE and these patients were removed obstruction and cured by operation in time. Sixteen cases were functional DGE and were cured by conservative treatment,gastric function recovered after 7-19 d.Conclusions According to clinical manifestation,upper gastrointestinal radiography and endoscopy,the diagnosis of DGE after resection of esophageal and gastric cardia carcinoma can be confirmed. The majority of DGE are functional and the patients could be cured by conservative treatment. But the patients who have mechanical obstruction have to operate in time. The best way to prevent DGE is to improve preoperative nutrition status,enforce standard surgical manipulation,maintain effective gastrointestinal decompression and strengthen postoperative dietary control.
9.Effect of video-assisted thoracoscopic surgery in thoracic disease

Objective To investigate the effect of video-assisted thoracoscopic surgery( VATS) in thoracic disease,and the feasibility to carry out VATS for basic hospital. Methods The data of VATS treatment were collected to compare the differences between study group and control group,and evaluate the the feasibility to carry out VATS for basic hospital. Results The operation time was (100. 75±22. 72) min, blood loss was (54. 27±26. 21) mL,postoperative drainage was (920. 67±171. 99) mL. The postoperative complications were fewer,post-operative hospital stay was shorter,incision time was shorter(P=0. 000) and pain scores was lower(P=0. 000) in study group than that in control group. Basic hospital has the capacity to conduct this technical. Conclusion VATS is feasible to carry out in basic hospital.
10.Effects of fresh lumbar osteoporotic vertebral compression fracture on spinopelvic sagittal parameters and distribution of Roussouly types

Objective To investigate the effects of fresh lumbar osteoporotic vertebral compression fracture (OVCF) on spinopelvic sagittal parameters and distribution of Roussouly types.Methods One hundred and eight patients with one-level fresh lumbar OVCF and 110 patients with simple osteoporosis (OP) were diagnosed and treated at Department of Orthopedic Surgery,Shougang Hospital from January 2016 to August 2017.The basic clinical data of all the patients were documented and their lumber spines were classified by Roussouly types.The 2 groups were compared in terms of spinopelvic sagittal parameters such as pelvic incidence (PI),pelvic tilt (PT),sacral slope (SS),and lumbar lordosis (LL) and Roussouly types.The correlation between Roussouly types and fracture segments was observed in OVCF group.Results There was no significant different in PI between OVCF and OP groups (50.35° ± 11.00° versus 51.96° ± 11.73°) (P > 0.05).PT in OVCF group (18.79°± 9.51°) was significantly larger than that (16.19°± 9.03°) in OP group while SS (31.56° ± 7.88°) and LL (40.22°± 12.29°) in the former significantly smaller than those in the latter (35.77° ± 8.82° and 47.89° ± 13.20°,respectively) (P < 0.05).Roussouly types Ⅰ,Ⅱ,Ⅲ and Ⅳ in OVCF group were 35,40,28 and 5 cases,respectively,with types Ⅰ and Ⅱ accounting for 69.5%;Roussouly types Ⅰ,Ⅱ,Ⅲ and Ⅳ in OP group were 17,30,49 and 14 cases,respectively,with type Ⅲ being predominant (44.5%).There was a significant difference between the 2 groups in distribution of Roussouly types (P <03.05).In OVCF group,fractures of L1 and L2 vertebrae accounted for 82.4%.There was a significant correlation between fracture segments and lumbar Roussouly types (P =0.034).Conclusions Patients with fresh lumbar OVCF tend to have a forward gravity and sagittal disequilibrium,leading to posterior pelvic rotation,increased PT,decreased SS,and downward shift of the apex of the lumbar curve.Their Roussouly classification is mainly type Ⅰ or type Ⅱ.
