1.Effect evaluation of auricular plaster therapy based on the pattern/syndrome differentiation in patients with constipation induced by strong opioid drugs for pain

Objective To explore the effect of auricular plaster therapy on associated symptoms and quality of life in patients with constipation induced by strong opioid drugs for pain.Methods A total of 60 patients selected were randomly divided into the intervention group and the control group with 30 cases in each group.The intervention group was treated with auriculotherapy based on the pattern/syndrome differentiation after detecting the auricular sensitive acupoints' distribution in patients with opioid-induced constipation after receiving opioid therapy.BFI,self-made scale on symptoms accompanied by constipation and PAC-QOL were used to collect data about constipation symptoms and the level of quality of life before or after the intervention,and still with a subset of the intervention group followed for more than two weeks.Results The BFI and PAC-QOL scores of the intervention and control group were lower after intervention,and the decline score of the two groups showed significant difference.But for the self-made scale,the score of each item was slightly lower compared with that before treatment,which showed no statistically significant difference between groups.A follow-up of 2 weeks showed that the long-term efficacy of auricular plaster therapy was significantly lower than before,but was less desirable than short-term.Conclusions Scientific and reasonable choice of acupoint prescription according to syndrome differentiation can effectively improve symptoms accompanied by constipation and patients' quality of life.
2.Nomenclature standardization of radiotherapy in cervical cancer

Objective:To standardize the naming of organ at risk (OAR) and target area during cervical cancer radiotherapy based on AAPM TG-263.Methods:After self-programming of Matlab software to implement the reading and resolution of radiotherapy structure files, the naming of each substructure was automatically output, recorded and restored. After naming all substructures, the structure names were classified by keywords. According to TG-263, a standard naming conversion table of OAR and target area was developed, and the classified structure names were standardized through procedures. Finally, the standardized named radiotherapy structure files were output and imported into the treatment planning system (TPS).Results:The radiation structure of 144 patients with cervical cancer was successfully transformed and displayed correctly in TPS. Before the transformation, the naming of OAR and target area lacked of uniform norms and standards, and the naming of the same structure significantly differed. After the transformation, 43 naming methods of OAR and 74 naming methods of the target area were unified into 20 and 8 naming methods, which were more convenient for staff understanding and communication.Conclusion:The standardization of cervical cancer radiotherapy structure naming can reduce the inconsistency of naming and provide reference for the standardized naming of pelvic tumors.
3.Investigation analysis of current exercise status and its obstacle factors in patients with cancer-related fatigue

Objective To probe into the exercise status of digestive tumor patients in cancer-related fatigue, analyze its obstacle factors, in order to provide evidence for clinical nursing care. Methods Self designed general information questionnaire and tumor patient′s cinesipathy questionnaire in 216 digestive tumor patients of Digestive Tumor Surgery Department in Jiangsu Chinese Medicine Hospital for investigation of different degree cancer-related fatigue. Results A total of 155 of 216 patients had excises, but 66 cases reached the excises intensity of American Cancer Society in 2012 published guidelines recommend movement standards;"do not know that exercise can reduce fatigue degree", "fear campaign negative impact on disease and treatment","tired", "lack of motor skills" and "no exercise habits" were the obstacle factors for cancer-related fatigue carrying out exercise therapy. Conclusions Medical personnel should be aware of the impact of cancer-related fatigue on the quality of life in the cancer patients, regulate assessment and manage fatigue, carry out sleep interventions and guide behavior, and assess the management effects scientifically.
4.Literatures analysis on intervention of caner-related fatigue in CNKI

Objective To probe into the current status quo and issues of intervention research about cancer-related fatigue in cancer patients.Methods The key words were “cancer-related fatigue”,“fatigue”,“tumor”that searched literatures from CKNI and statistically analyzed by excel and other software.Results A total of 1 53 intervention research literatures were found out which involved excise therapy,psychological nursing, health education,Chinese medicine nursing,self care,etc.The quality of literatures were low,including 5 literatures with Jadad assessment score-3 points,1 7 literatures having 2 points,and the others with 2 points below.Conclusions There are too many intervention studies on cancer-related fatigue of patients with cancer chemotherapy,but the quality of literatures are unsatisfied.Therefore,the methodology should be strengthened for intervention studies of cancer-related fatigue,so as to improve research quality and satisfy evidence-based demands.
5.Meta-analysis of auricular-plaster therapy intervening with anxiety in adults

Objective? To review the intervention effects of auricular-plaster therapy on adults with anxiety systematically. Methods? Data about random controlled trails of auricular-plaster therapy in treating anxiety were collected from Cochrane, PubMed, Embase, CBMdisc, CNKI and Wanfang databases. The quality of literatures were assessed and the data were collected by two persons independently. Multiple-time-point Meta-analysis was used for anxiety, while endpoint Meta-analysis was used for quality of sleep and life. RevMan 5.3 was used for statistical analysis. Results? Totally 15 literatures were included in this study, involving 890 subjects. Meta-analysis showed that auricular-plaster therapy could ameliorate anxiety continuously with high degrees within 2 months (P< 0.01), but there were no notable effects yet beyond 2 months (P> 0.05). Auricular-plaster therapy could improve the quality of sleep effectively (P<0.001). There was no evidence that auricular-plaster therapy could improve the quality of life (P>0.05). Conclusions? Auricular-plaster therapy can ameliorate patients' anxiety and improve their quality of sleep, but considering the limited quality and quantity of studies included, the findings of our study will need to be tested by multicenter and high-quality random controlled trails with large samples, especially its long-term intervention effects.
6.Dosimetric effects of volumetric modulated arc therapy plans for lung cancer caused by different dose algorithms and radiation field settings

Objective:To analyze the dosimetric differences of volumetric modulated arc therapy (VMAT) plans for lung cancer caused by different dose calculation algorithms and radiation field settings and thus to provide a reference for designing clinical VMAT plans for lung cancer.Methods:This study randomly selected 20 patients with lung cancer and divided them into four groups of VMAT plans, namely, a group adopting two fields and two arcs based on the AAA algorithm (2F2A_AAA), a group employing two fields and two arcs based on the AXB algorithm (2F2A_AXB), a group using two fields and two arcs based on the MC algorithm (2F2A_MC), and a group adopting one field and two arcs based on the MC algorithm (1F2A_MC). Then, this study evaluated the target coverage, high-dose control, dose homogeneity index (HI), conformity index (CI), and organs at risk (OARs) of the plans using different algorithms and radiation field settings.Results:The planning target volume (PTV) results of two fields combined with two arcs (2F2A) of three groups using different algorithms are as follows. 2F2A_MC achieved better results in both D1% and V 95% (the relative volume of the target volume surrounded by 95% of the prescribed dose) of planning gross target volume (PGTV) than 2F2A_AAA (D1%: t=-2.44, P=0.03; V95%:z=-2.04, P=0.04) and 2F2A_AXB (D1%: t=2.34, P=0.03; z=-3.21, P < 0.01). 2F2A_AXB outperformed 2F2A_AAA ( z=-3.66, P < 0.01) and was comparable to 2F2A_MC in terms of the CI of PGTV. Regarding OARs, 2F2A_AXB and 2F2A_MC decreased the V5 Gy of the whole lung by 0.68% ( z=-2.69, P=0.01) and 3.05% ( z=-3.52, P < 0.01), respectively compared to 2F2A_AAA. 2F2A_AXB achieved a whole-lung Dmean of 1776.44 cGy, which was superior to that of 2F2A_MC ( t=2.67, P=0.02) and 2F2A_AAA ( t=8.62, P < 0.01). Compared to 2F2A_AAA and 2F2A_MC, 2F2A_AXB decreased the V20 Gy of Body_5 mm by 1.45% ( z=-3.88, P < 0.01) and 2.01% ( z=-3.66, P < 0.01), respectively. The results of the two groups with different field settings showed that 1F2A_MC was superior to 2F2A_MC in both the CI of PTV1 and the HI of PTV2 (CI: t=2.61, P=0.02; HI: z=-2.20, P=0.03). Moreover, 1F2A_MC increased the Dmean of the whole lung by 26.29 cGy compared to 2F2A_MC ( t=2.28, P=0.04). Conclusions:Regarding the design of VMAT plans for lung cancer, the MC algorithm is suitable for the target priority and the AXB algorithm is suitable for the OAR priority. When only the MC algorithm is available, it is recommended to choose 1F2A in the case of target priority and select 2F2A in the case of OAR priority.
7.Application of Jacobian determinant of reverse deformation field to evaluation of deformation registration algorithm

Objective:To effectively quantify and evaluate the quality of different deformation registration algorithms, in order to enhance the possibility of implementing deformation registration in clinical practice.Methods:The Jacobian determinant mean (JDM) is proposed based on the Jacobian determinant (JD) of displacement vector field (DVF), and the Jacobian determinant error (DJDE) is introduced by incorporating the JD of the inverse DVF. The optical flow method (OF-DIR) and fast demons method with elastic regularization (FD-DIR) were tested on nasopharyngeal and lung cancer datasets. Finally, JDM and DJDE with the Jacobian determinant negative percentage (JDNP), inverse consistency error (ICE) and normalized mean square error (NMSE) were used to evaluate the registration algorithms and compare the differences evaluation indicators in different tumor images and different algorithms, and the receiver operating curve (ROC) was analyzed in evaluation.Results:In lung cancer, OF-DIR outperformed FD-DIR in terms of JDM, NMSE, DJDE and ICE, and the difference was statistically significant( z = -2.24, -4.84, t = 4.01, 6.54, P<0.05). In nasopharyngeal carcinoma, DJDE, ICE and NMSE of OF-DIR were superior to FD-DIR, and the difference was statistically significant ( t = 4.46, -7.49, z = -2.22, P<0.05), but there was no significant difference in JDM ( P>0.05). In lung cancer and nasopharyngeal carcinoma, JDNP of OF-DIR was worse than that of FD-DIR, and the difference was statistically significant ( z = -4.29, -4.02, P<0.01). In addition, DJDE is more specific and sensitive on ROC curve (AUC=0.77), and has different performance result for tumor images at different sites. Conclusions:The JDM and DJDE evaluation metrics proposed are effective for deformation registration algorithms. OF-DIR is suitable for both lung cancer and nasopharyngeal carcinoma, while the influence of organ motion on the registration effect should be considered when using FD-DIR.
8.Trails about auricular acupressure in improving the postoperative gastrointestinal peristaltic function:a Meta-analysis

Objective To systematically review the effect of auricular acupressure on the recovery of gastrointestinal peristaltic functioning after the surgery .Methods The randomized controlled trials ( RCT) and controlled clinical trail on auricular acupressure after the surgery were collected using the databases of Cochrane Library, JBI Library,PubMed,EMbase,CBMdisc,China National Knowledge Infrastructure (CNKI) and Chinese BioMedical Literature Database ( CBM ) . After selection and critical appraisal of the retrieved studies , a quantitative systemic review(Meta-analysis)was conducted by RevMan 5.0 software.Results Four RCTs and ten CCTs were included with the literature quality of B .Meta-analysis showed that auricular acupressure after the surgery could significantly shorten the time to first flatus (WMD=-14.16,95%CI=-18.40 -9.93,P<0.01),time to first bowel motion(WMD =-10.33,95%CI =-12.98 -7.69,P <0.01),time to first passage of feces(WMD=-19.19,95%CI=-27.56 -10.81,P<0.01),and reduced the rate of abdominal distension (OR=0.17,95%CI:0.10-0.29,P<0.01).Conclusions Auricular acupressure after surgery is beneficial in promoting the recovery of gastrointestinal peristaltic function .However ,more high quality RCTs on the basic research of action mechanism , and standardized clinical research are still required to verify the effects .
9.Analysis of risk factors for contact dermatitis in patients with PICC catheterization

Objective To investigate the risk factors for contact dermatitis in patients with peripherally inserted central catheter (PICC) catheterization.Methods A total of 244 patients with PICC catheterization admitted in the Venous Catheter Nursing Outpatient of Affiliated Hospital of Nanjing University of TCM from June to August 2016 were selected using convenience sampling. A prospective study was performed on those patients for general information, PICC catheter information, PICC care information and contact dermatitis development information. SPSS 21.0 was used to analyze the date.Results A number of 24 patients were observed to have contact dermatitis with an incidence of 9.84%. Single factor analysis showed that gender, age, allergy, dressing type and catheter material correlated to the development of contact dermatitis with PICC. The differences were statistically significant (P<0.05). Multiple factor logistic regression analysis showed that the gender[OR=2.721, 95%CI (1.059-6.994),P=0.038], allergy[OR=5.846, 95%CI (2.030-16.833),P=0.001] and dressing type[OR=4.055, 95%CI (1.472-11.169),P=0.007]were risk factors of contact dermatitis in patients with PICC catheterization.Conclusions For PICC catheterization patients, effective preventive measures aiming at risk factors can reduce the occurrence of contact dermatitis with PICC.
10.Effect of WeChat combined with 5A nursing model on the uncertainty in illness and the quality of life in patients with inflammatory bowel disease

Objective To explore the effect of WeChat combined with 5A management model on the uncertainty in illness and the quality of life in patients with inflammatory bowel disease(IBD). Methods By convenience sampling, totally 64 patients with IBD hospitalized in Department of Gastroenterology, East Wing of the Second People's Hospital of Lianyungang in Jiangsu Province from April to December of 2018 were selected as research objects, and divided into the experimental group and control group, with 32 cases in each group according to their enrolling time. Both of the two groups received routine nursing care and health education, on the basis of that, the experimental group was implemented by the WeChat combined 5A nursing model. The two groups were compared in terms of the uncertainty in illness and quality of life before and after intervention. Results Finally, 31 valid samples were achieved in the experimental group and 30 in the control group. After intervention, each dimensional scores of sense of uncertainty in illness of the experimental group were lower than the control group with statistical significances(P<0.01). After intervention, the total score of quality of life in the experimental group as well as the dimensional scores of "intestinal symptoms", "emotional symptom", and "social function" were all higher than the control group with statistical significances(P<0.05), there was no statistical difference in the scores of "constitutional symptoms" dimension between the two groups (P> 0.05). Conclusions Wechat combined 5A nursing model can improve the uncertainty in illness of the IBD patients and improve their quality of life.
