1.The characteristic features of CT and MRI in the pancreatic neuroendocrine tumor

Objective Pancreatic neuroendocrine tumor( PNET) is a rare kind of tumor with slow growth for which surgery is the main treatment.The histological grading of PNET was closely related to the choice of operation mode and the comprehensive treat-ment after operation.Imaging examinations such as enhanced CT and MRI are of great value in the preoperative diagnosis of PNET. The aim of this study was to investigate the characteristic features of CT and MRI in the PNET. Methods Preoperative imaging data of 80 patients with PNET confirmed by pathology were reviewed, including CT and MRI, and the image features of each PNET were an-alyzed and the statistical analysis was performed. Results There were significant differences in the lesion morphology between the G1 and G2 levels of PNET.The majority of forms of the G1 stage PNET lesions presented round-like regular shape, but the G2 stage did not show regular shape.CT examination showed that regular morphological lesions accounted for 91.30%(21/23) in the G1 level, and 52.00%(13/25)in the G2 level,χ2 =8.857,P=0.003.MRI examination showed that regular morphological lesions accounted for 88.48%(22/27)in the G1 level, and 55%(11/20) in the G2 level, χ2 =3.862,P=0.050.The difference of scan density in PNET lesions was statistically significant.Low density lesions accounted for 73.91%(17/23)in the G1 level, and 32%(8/25)in the G2 level.Equidensity lesions accounted for 26.09%(6/23)in the G1 level, and 68.00%(17/25)in the G2 level,χ2 =8.842,P=0.004.The difference of bile pancreatic duct expansion in the G1 and G2 level lesions was statistically significant by MRI examination. Lesions with expanded pancreatic duct accounted for 7.41%(2/27)in the G1 level,and 40%(8/20) in the G2 level, χ2 =7.287, P =0.007.There was significant difference in the peak time after en-hanced scanning between the G1 and G2 levels.Lesion enhancement peak at the arterial phase by CT examination of accounted for 21.74%(5/23) in the G1 level,and 64%(16/25) in the G2 level,χ2 =8.694,P=0.003.Lesion enhancement peak at the arterial phase by MRI examination accounted for 29.63% (8/27) in the G1 level, and 70% (14/20) in the G2 level, χ2 =7.521,P=0.006. Conclusion The lesion morphology, density of CT plain scan, CT or MRI enhancement peak time, and expansion of the bile duct and pancreatic duct can provide reliable information to distinguish the G1 and G2 stage in PNET.
2.Comparison of accuracy of SVV, CVP and PAWP in monitoring changes in blood volume in patients undergoing renal transplantation

Objective To compare the accuracy of stroke volume variation (SVV),central venous pressure (CVP) and puhnonary arterial wedge pressure (PAWP) in monitoring the changes in blood volume in the patients undergoing renal transplantation.Methods Sixteen patients with chronic renal failure,of American Society of Anesthesiologists physical status Ⅱ or Ⅲ,aged 18-55 yr,scheduled for elective allograft renal transplantation under general anesthesia,were enrolled in the study.SVV was continuously monitored with the FloTrac/Vigileo monitor,and CVP,PAWP and stroke volume index (SVI) were continuously monitored with the volumetric pulmonary artery catheter during surgery.The parameters of hemodynamics were recorded at 30 min after induction of anesthesia,5 min before renal artery opening,5 and 30 min after renal artery opening,and at the end of surgery.Hydroxyethyl starch 130/0.4 electrolyte solution 6 ml/kg was infused over 15 min via the central venous catheter to perform fluid responsiveness starting from 30 min after induction of anesthesia.Positive fluid responsiveness was defined as the change in SVI ≥ 15%.The relationship between SVV and CVP and between SVV and PAWP was analyzed using the Pearson correlation analysis.The receiver operating characteristic curve for CVP,SVV and PAWP in monitoring the changes in blood volume was drawn,and the area under the curve was calculated.Results Compared with the value at 5 min before renal artery opening,SVV was significantly increased after renal artery opening (P<0.05),and no significant change was found in CVP and PAWP after renal artery opening (P>0.05).SVV was negatively correlated with CVP,and r=-0.82 (P<0.01);SVV was negatively correlated with PAWP,and r=-0.77 (P<0.01).The area under the curve of SVV in monitoring the changes in blood volume was 0.87,and of CVP and PAWP was 0.69 and 0.66,respectively.Conclusion SVV provides better accuracy than CVP and PAWP in monitoring the changes in blood volume in the patients undergoing renal transplantation.
3.Exercise intervention methods for senile sarcopenia

BACKGROUND:Sarcopenia refers to age-related progressive,systemic muscle mass reduction and/or muscle strength decline or muscle physiological function decline,which is related to the occurrence of a variety of adverse outcomes in older adults.Exercise is considered to be one of the main strategies for combating sarcopenia in older adults,but there is a lack of specific intervention methods of different exercise patterns to intervene in sarcopenia. OBJECTIVE:To elaborate the main influencing factors of sarcopenia and the research progress of different exercise methods to improve sarcopenia in older adults,providing reference and basis for combating sarcopenia in older adults. METHODS:Web of Science,PubMed,CNKI,VIP,WanFang databases were retrieved for relevant literature published from January 2000 to October 2023 using the keywords of"sarcopenia,sport,exercise intervention,resistant training,aerobic exercise,whole body vibration training,mixed training,physical performance,muscle strength,muscle mass"in Chinese and English,respectively.A total of 126 articles were included for review. RESULTS AND CONCLUSION:Resistance exercise is still the most effective way to prevent and treat senile sarcopenia,and the effect of high-intensity resistance exercise is more significant.However,in practical application,we should pay attention to the gradual increase of training load intensity.Aerobic exercise combined with resistance exercise is more effective to improve muscle mass and function in the elderly than a single exercise mode.It is suggested that older adults can carry out the transition of low-intensity aerobic exercise in the early stage and increase resistance exercise individually in the late stage.Whole body vibration training is a new treatment method for the prevention and treatment of senile sarcopenia,but particular attention should be paid to the effects of frequency,amplitude,and duration on patients during practical application.Multicomponent exercise combines different exercise modes,which can give full play to their respective advantages,so as to personalize exercise interventions.
4.The isochronous substitution effect of 24 hour activity behavior on physical health of college students

Objective:
An isochronous substitution model was established to explore the association and substitution effect between college students 24 hour activity behavior and physical health, so as to provide specific activity behavior suggestions for college students to improve their physical health.
Methods:
A stratified random cluster sampling method was used to conduct physical fitness tests and 24 hour activity behavior surveys among 2 794 college students in 12 colleges and universities in Tianjin.Time spent on sedentary behavior(SB), light intensity physical activity(LPA), moderate to vigorous physical activity(MVPA) and sleep(SLP) time. The isochronous method of components was used to explore the relationship between 24 h activity behavior and physical health.
Results:
Except for 50 m running, MVPA was negatively correlated with BMI Z ( β =-0.62, P <0.05), but positively correlated with other physical fitness indexes ( β =0.34~274.23, P <0.05). LPA was not associated with lung capacity, sitting forward flexion and 50 m running, and negatively correlated with other physical fitness indexes ( β =-14.30- -0.19, P <0.05). SB was negatively correlated with most physical fitness indexes ( β =-11.57- -0.33, P <0.05), but positively correlated with BMI Z ( β =0.45, P < 0.05 ). In addition to lung capacity, SLP was positively correlated with BMI Z , total physical fitness score,1 minute sit-ups, pull ups, 800/1 000 m running, sitting forward flexion, and 50 m running ( β =0.27-11.21, P <0.05), but negatively correlated with long jump ( β =-0.10, P <0.05). Isochronous substitution showed that the adverse effects of 30 min/d SB and LPA substitution of MVPA were much greater than the beneficial effects of MVPA substitution for corresponding behaviors (total physical score: SB, -0.58 vs 0.47 points; LPA, -0.50 vs 0.38 points).
Conclusion
MVPA and SLP have been found to have a positive effect on physical fitness among college students. Therefore, in the process of improving the physical health of college students, ensuring adequate sleep, improving MVPA and reducing SB as much as possible may be one of the effective methods.
5.The role of CD109 antigen in glioma

Leukocyte differentiation antigen CD109(CD109)is a glycosylphosphatidylinositol(GPI)-associated cell surface antigen,belonging to the α-2 macroglobulin/C3,C4 and C5 super family complex,and is a transfor-ming growth factor(TGF)-β/Smads co-receptor.CD109 regulates the occurrence and development of glioma and is closely related to drug resistance and recurrence of glioma.As a cell membrane protein,CD109 can be directly la-beled by specific antibodies or enzymes,and may also be recognized by targeted vectors for drug delivery,which is expected to become an effective diagnostic molecular marker and pharmacological therapeutic target.
6.Shuxuetong Inhibits Bim-dependent Apoptosis of Cerebellar Granule Neurons

[Objective]To investigate the effect and mechanism of Shuxuetong and its main component hirudin on the apoptosis of cerebellar granule neurons(CGNs)in Sprague-Dawley(SD)rats.[Methods]CGNs incubated in vitro for 7 days were divided into survival control group or 25 K group(cultured in medium containing 25 mmol/L KCL)and apopto-sis group or 5 K group(cultured in medium containing 5 mmol/L KCL).CGNs were separately treated with proportionally diluted and different concentrations of Shuxuetong(1/50,1/40,1/30,1/20 and 1/10)and the corresponding different con-centrations of hirudin(2,2.5,3.34,5 and 10 U/mL).Hoechst staining was performed to analyze the apoptosis.Western blot was used to detect the expression levels of Cleaved Caspase-3,Bim and VEGF.[Results]Hoechst staining showed that 5 K group had a higher apoptosis rate than 25 K group.In 25 K group,there was no significant change in the apoptosis rate between neurons treated with different concentrations of Shuxuetong and hirudin,but significant changes was found in 5 K group and the higher the concentration,the lower the apoptosis rate.Western blot results revealed that,compared with control neurons in 5 K group,Shuxuetong injection and hirudin treatments resulted in a decrease of Cleaved Caspase-3 and Bim expression,but an increase of VEGF protein.[Conclusions]Shuxuetong and its main component hirudin inhibits the apoptosis of CGNs through suppressing proapoptotic BH3-only protein Bim.
