1.Efficacy of rociverine tablets combined with imrecoxib tablets and comfortable nursing in the control of bladder spasm after transurethral laser resection of prostate

Objective To assess the efficacy of rociverine tablets combined with imrecoxib tablets and comfortable nursing in the control of bladder spasm after transurethral laser resection of prostate.Methods According to the digital table,120 patients who diagnosed as benign prostate hyperplasia (BPH) were randomly divided into four groups.The patients in combined drug group (group A) were administered rociverine tablets(10mg tid) and imrecoxib tablets (100mg bid) from the day of surgery to the day of catheter removal.The patients in rociverine tablets group (group B) were treated with rociverine tablets (10rag tid),and the patients in imrecoxib tablets group (group C) were administered imrecoxib (100mg bid) from the day of surgery to the day of catheter removal.The patients in group D were administered indometacin suppositories in anus when they felt pain.All patients were given comfortable nursing.The frequency and duration of bladder spasm,the time of bladder irrigation,time needed for washing solution to turn clear,and the time of catheter indwelling in the four groups were observed and compared.Results Compared with group D,group A,group B and group C all had statistically significant differences in the frequency and duration of bladder spasm,the time of bladder irrigation,time needed for washing solution to turn clear,and the time of catheter indwelling (Fa =9.469,Fb =13.443,Fo =12.157,Fd =26.609,Fe =33.717,Ff =30.524,Fs =100.836,Fh =89.985,P3 < 0.001).These parameters were lower in group A than those in group B and group C,and the differences were statistically significant (P1 =0.030,0.002,0.012,0.003,< 0.001,< 0.001,< 0.001,< 0.001;P2 =0.030,0.004,0.035,0.001,< 0.001,< 0.001,< 0.001,< 0.001).There were no statistically significant differences between group B and group C (all P4 > 0.05).Conclusion Rociverine tablets combinined with imrecoxib tablets for the prevention and treatment of bladder spasm after transurethral laser resection of prostate was effective,which was worthy of clinic application.
3.Ginsenoside rg3 reduces body weight by regulating fat content and browning in obese mice

Objective: To determine the effects of ginsenoside rg3 on the body weight of C57BL/6J obese mice and to investigate its underlying weight loss mechanisms with a focus on white fat browning-related factors. Methods: Eight-week-old C57BL/6J male mice were fed a high-fat diet for 12 successive weeks to construct the obese model. C57BL/6J male mice were fed a standard chow diet to construct normal control group. After 8 weeks of intervention with ginsenoside rg3, the food intake, body weight, body fat mass, blood sugar, and lipid profiles of the mice in each group were detected. Hematoxylin and eosin (HE) staining was used to observe the histological morphology of the adipose tissues. Real-time poly-merase chain reaction (RT-PCR) and Western blotting (WB) were applied to detect the gene and protein expression levels of peroxisome proliferators-activated receptor gama (PPARγ), Peroxisome proliferator-activated receptor-gamma coactivator -1alpha (PGC-1α), PR domain containing 16 (PRDM16), and uncoupling protein 1 (UCP-1).Results: Compared to normal control group mice, the body weight, food intake, body fat composition, and blood lipid levels of model group mice increased significantly. After 8 weeks of intervention with ginsenoside rg3, body weight, body fat composition, food intake, and blood lipid profiles decreased. HE staining showed that ginsenoside rg3 can improve white adipocyte hypertrophy to a certain extent. RT-PCR and WB demonstrated that ginsenoside rg3 can increase the mRNA and protein expression levels of PPARγ, PGC-1α, PRDM16, and UCP-1 in the adipose tissues of obese mice. Conclusion: The weight reduction effect of ginsenoside rg3 may be related to the promotion of white fat browning.
4.Loganin regulates glycolipid metabolism by influencing intestinal microbiota and AMPK signaling in obese mice

Objective:We aimed to observe the effects of loganin(Log)on serum glycolipid levels and probe the mechanisms focusing on intestinal flora and AMP-activated protein kinase(AMPK)signaling in obese mice.Methods:A high-fat diet was given for 12 consecutive weeks to generate the obesity model in institute of cancer research(ICR)mice.Body weight was measured weekly and fasting blood glucose(FBG)was determined every 2 weeks.Both the oral glucose tolerance test and the intraperitoneal insulin tolerance test were performed.The serum levels of total cholesterol(TC),triglyceride,high-density lipoprotein-cholesterol,low-density lipoprotein-cholesterol(LDL-C),and free fatty acids(FFA)were measured.The expression of key proteins in the AMPK signaling pathway in skeletal muscle tissue was detected by immunoblotting,and gut microbiota were characterized using 16S rDNA sequencing.Results:Log significantly decreased the body weight and the FBG in obese mice(P<.05),and it could restore FBG to normal levels.The total cholesterol,LDL-C,and FFA levels were significantly reduced by Log compared with the obese controls(TC:P=.0020;LDL-C:P=.0233;FFA:P=.0127),and the glucose tolerance of animals was significantly improved(P=.0477).The western blot results showed that Log could upregulate the protein expression of Adenosine 5'-monophosphate(AMP)-activated protein kinase(AMPKα),Sirtuin 1(SIRT1),and peroxisome proliferator-activated receptor-gamma coactivator-1 alpha(PGC1α)in skeletal muscle tissue of obese mice.16S rDNA sequencing indicated that Log reduced the diversity of the gut flora in feces and altered the floral composition of obese mice.Conclusions:Log was effective in reducing body weight and improving glucolipid metabolism in obese mice,probably through activating AMPK signaling and regulating intestinal microbial diversity.
5.Relationship between left ventricular mass index and left ventricular myocardial work index in patients with end -stage renal disease with preserved ejection fraction

Objective :
To quantitatively evaluate the left ventricular myocardial work index (MWI) of patients with end-stage renal disease (ESRD) with preserved ejection fraction using left ventricular pressure-strain loop (PSL) , and group them according to left ventricular mass index( LVMI) ,in order to compare the effect of different LVMI levels on left ventricular MWI.
Methods:
81 patients with stage 5 chronic kidney disease,including 43 males and 38 females,were divided into left ventricular hypertrophy group ( LVH group) ( n = 44) and non-left ventricular hypertrophy group (NLVH group) ( n = 37) according to LVMI,and 37 healthy people matched in age and sex with the ESRD groups were randomly selected as the control group.The general clinical data,conventional echocardiographic parameters,global longitudinal strain ( GLS) ,and MWI namely global work index ( GWI) ,global constructive work ( GCW) ,global wasted work ( GWW) and global work efficiency ( GWE) of the three groups were compared.The correlation between MWI and various parameters were analyzed,and the independent predictors of MWI were analyzed too.
Result :
The results showed that there were no significant differences in age,gender,height,weight and other general clinical data as well as left ventricular ejection fraction ( LVEF) among the three groups.In two ESRD groups,GWW was higher than that in control group,GWE was lower than that in control group,and were more significant in LVH group (P<0. 05) .LVMI was positively correlated with GWW,and negatively correlated with GWE ,respectively ,with specific independent correlation.
Conclusion
GWW and GWE can detect early impairment of left ventricular systolic function in ESED patients,and myocardial function impairment in LVH group is more serious than that in NLVH group.LVMI is an independent positive factor of GWW and an independent negative factor of GWE.

Since December 2019, a novel coronavirus (2019-nCoV, SARS-CoV-2) pneumonia (COVID-19) outbreak has occurred in Wuhan, Hubei Province, and the epidemic situation has continued to spread. Such cases have also been found in other parts of the country. The spread of the novel coronavirus pneumonia epidemic has brought great challenges to the clinical practice of thoracic surgery. Outpatient clinics need to strengthen the differential diagnosis of ground glass opacity and pulmonary plaque shadows. During the epidemic, surgical indications are strictly controlled, and selective surgery is postponed. Patients planning to undergo a limited period of surgery should be quarantined for 2 weeks and have a nucleic acid test when necessary before surgery. For patients who are planning to undergo emergency surgery, nucleic acid testing should be carried out before surgery, and three-level protection should be performed during surgery. Patients who are planning to undergo emergency surgery in the epidemic area should be confirmed with or without novel coronavirus pneumonia before operation, and perform nucleic acid test if necessary. Surgical disinfection and isolation measures should be strictly carried out. Among postoperative patients, cases with new coronavirus infection were actively investigated. For the rescue of patients with novel coronavirus infection, attention needs to be paid to prevention and treatment and related complications, including mechanical ventilation-related pneumothorax or mediastinal emphysema, and injury after tracheal intubation.
