1.A Study on the Cell Surface Antigens of Bladder Carcinoma by Means of Monoclonal Antibodies.

This study was carried out to investigate the localization and change of the cell surface antigens, such as URO-9(Om5) and URO-10 (T43), in transitional cell carcinomas of bladder. The subjects used were normal bladder mucosae of 10 autopsy cases of fetus and 10 biopsy cases of child or adult patients with disease of other organs as controls, and neoplastic tissue of 15 cases of patient with transitional cell in carcinoma of bladder. Avidin-Biotin complex(ABC) kits(Cambridge Research Lab.) and URO-9 and URO-10 monoclonal antibodies were used and the data observed were analyzed. The results obtained were summarized as follows: 1. URO-9 expression was none of 10 fetal bladder mucosae and 3 or 10 normal bladder mucosae and URO-10 expression was none showed. 2. Of 15 cases with transitional cell carcinoma of bladder, URO-9 expression was 5, of which 1 was Ta grade II, 1 T1 grade II and the rest T2 grade III. URO-10 expression was 10, of which I was T1 grade III, 3 T2 grade III, 3 T3 grade III and 3 T4 grade III, of 15 cases of transitional cell carcinoma of bladder. As above results, the transitional cell carcinoma of bladder in low grade and low stage have tendency to express URO-9 monoclonal antibody but not URO-10 monoclonal antibody, whereas the transitional cell carcinoma of bladder in high grade and high stage were inclined to express URO-10 monoclonal antibody with variable expression of URO-9 monoclonal antibody.

2.Metastatic Adenocarcinoma of Bladder from Stomach: A Case Report.

Recently, imaging diagnosis is valuable in detecting of primary malignancy and secondary malignancy. When urologic symptoms newly developed to the patients who had malignancy of digestive organs, metastatic malignancy of urogenital organs should be considered. Here, we report a case of metastatic bladder tumor from gastric cancer which occurred 9 months after subtotal gastrectomy.

4.A Case of Alagille's Syndrome.

This is a case report of a 5-month-old male who was brought in to hospital for evaluation of jaundice from birth. The baby had a history of ileal atresia operated 2 days after birth. At the age of one month, Tc-99m DISIDA hepatobiliary scintigraphy was performed at other hospital and reported to show good hepatic uptake of the tracer but no uptake in the billiary tree, gall bladder, or intestine for 24 hours post injection. He was judged to have biliary atresia. However, subsequent exploratory laparotomy revealed that the hepatobiliary tree appeared intact and that there was a gall bladder. Additionally, the patient had central aorto-pulmonary shunt for the right ventricular septal defect with pulmonary stenosis of a peripheral type at the age of 4 months. The second hepatobiliary scintigraphy was performed on admission at the age of 5 months, showing a gall bladder but no intestinal uptake up to 24 hours. Retrospectively, the histological specimen of the liver obtained at the exploratory laparotomy was re-evaluated, and by the histological findings coupled with clinical data, arteriohepatic dysplasia (Alagille's syndrome) was diagnosed. In this report, we emphasize the diagnostic limitation of hepatobiliary scintigraphy and the importance of overall clinical and histologic evaluation in a case of Alagille's syndrome. 5.Hypospadias: The Surgical Repair and its Results.

Two hundred cases of surgically repaired hypospadias at Pusan University Hospital in the past sixteen half years were surveyed and analyzed. Of 200 patients, 99(49.5%) were corrected in their preschool age. The number of patients with the distal type and proximal type of hypospadias performed in 134(67%) of 200 and the success rate was 84.3 per cent(113 of 134). One stage repair was performed in 66(33.0%) of 200 and the success rate are as follows : King, 100%(7 of 7), MAGPI, 100.0% (4 of 4), Mathieu, 100.0%(5 of 5), Hodgson ( I , III), 75%(12 of 16), and preputial island flap including Duckett, Hodgson XX and double faced technique, 67.6% (23 of 34). The most common postoperative complication was fistula.

6.A Case Report of Anesthetic Management in a Patient with Complete Left Bundle Braneh Block .

Among the intraventricluar block, bundle branch block is the most common type and left bundle branch block(LBBB) may progress to a more serious condition of complete heart block. Anesthetic methods such as nitrous oxide-oxygen-relaxant sequence, neuroleptanesthesia, and nitrous oxide-oxygen-minimal halothane(or penthrane) are generally accepted by the patient with heart disease. In the U.S., opioid anesthesia is also applied frequently. The authors successfully twice performed of operations with balanced anesthesia and spinal anesthesia respectively in a 39-year-old male patient who showed complete left bundle branch block pattern in a preoperative electrocardiographic tracing without any subjective symptoms.

8.Respiratory Arrest following Intrathecal Administration of Morphine .

Epidural and subarachnoid narcotics have raised new possibilities for selective blockade of pain transmission at the spinal cord level. However, it must still be regarded as an experimental technique until detailed pharmacological and physiological data are available, since many reports have treated the development of respiratory arrest which may be related to the dynamics fo CSF flow. We experienced 2 cases of respiratory arrest after intrathecal injection of 2mg morphine. One patient developed respiratory arrest at approximately 5 1/2 hours after intrathecal morphine and the other at approximately 12 1/2 hours. Those respiratory arrests were completely reversed with naloxone hydrochloride without interfering with the analgesic effect of the drug.

9.Caudal Morphine for Postoperative Pain Control after Abdominal Surgery .

Caudal narcotic analgesia was assessed after the injection of 3mg morphine diluted in 30ml(physiologic) saline into the sacral canal in 15 patients after upper abdominal surgery, in 20 patients after lower abdominal surgery under general anesthesia, and in 20 patients after perianal surgery under caudal block. Pain relief was evaluated by the subsequent need for systemic analgesics. All cases had considerable relief from pain and the morphine was effective for 12 or more hours. There were no significant differences between pain relief of the upper abdominal and lower abdominal surgery group, upper abdomianl and perianal surgery group, and lower abdominal and perianal surgery group (p>0.05, p>0.05, p>0.05). It is suggested that the morphine, which was administered into the sacral, cannal, reached the subarachnoid space and produced it's effect by direct action on the specific opiate receptors in the substantia gelatinosa of the posterior horn cell of the spinal cord. Consequently, whether analgesia from epidural narcotics appears to be segmental in distribution or not is still in controveray.
