ObjectiveTo investigate the risk factors of early rebleeding after ruptured intracranial aneurysm embolization surgery.MethodsForty-two patients with early postoperative rebleeding after ruptured intracranial aneurysm embolization surgery(study group) were enrolled,and 40 patients without early postoperative rebleeding after ruptured intracranial aneurysm embolization surgery at the same period was selected randomly as control group.Single factor and multiple factors were analyzed by non-condition Logistic regression model.ResultsThe single factor analysis result showed that age ≥50,history of hypertension,diabetes history,ruptured frequency number ≥2,combined with vasospasm,postoperative anticoagulation therapy,postoperative blood pressure fluctuation ≥ 30 mm Hg (1 mm Hg =0.133 kPa),pseudoaneurysm,fatal embolism and most embolism in study group [81.0％(34/42),76.2％(32/42),66.7％(28/42),76.2％ (32/42),81.0％ (34/42),76.2％ (32/42),66.7％ (28/42),66.7％ (28/42),66.7％ (28/42) ]had statistical differences compared with those in control group [57.5％ (23/40),50.0％ (20/40),37.5％( 15/40 ),50.0％ ( 20/40 ),57.5％ ( 23/40 ),47.5％ ( 19/40 ),37.5％ ( 15/40 ),37.5％ (15/40),37.5％ ( 15/40) ](P ＜0.05).Further multiple factors analysis showed that age ≥50,history of hypertension,ruptured frequency number ≥ 2,combined with vasospasm,postoperative anticoagulation therapy,postoperative blood pressure fluctuation ≥ 30 mm Hg were the independent risk factors of early postoperative rebleeding after ruptured intracranial aneurysm embolization surgery (P ＜0.05).ConclusionsAge ≥50,history of hypertension,ruptured frequency number ≥ 2,combined with vasospasm,postoperative anticoagulation therapy,postoperative blood pressure fluctuation ≥ 30 mm Hg are the independent risk factors of early postoperative rebleeding after ruptured intracranial aneurysm embolization surgery.The necessary interventions to these risk factors have important significance,and can lower the incidence of early postoperative ruptured rebleeding after ruptured intracranial aneurysm embolization surgery and improve the prognosis.

Objective To explore the relationship between work-family conflict and turnover inten-tions in nurses and find out the key points to reducing turnover intentions by releasing individuals' work-family conflict. Methods Cross-sectional study method was used in this investigation. By using cluster sampling method, 1287 nurses from some top-level and third-class general hospital were investigated. The questionnaire of work-family conflict and turnover intentions scale were used in this research and the re-suits underwent analysis. Results The score of turnover intentions in nurses was positively correlated with the level of their work-family conflict (r=0.233,P<0.01). Stress,behavior and time dimension were related factors of turnover intentions(r=0.243,0.191,0.176,P<0.01 ).After control of demographic factors, the turnover intentions deviation that can be explained increased by 7%. Conclusions The score of work-family conflict is one of the predicators of turnover intentions. A developing measure should be taken to reduce turnover intentions by releasing work-family conflict in nurses.

Objective To design the family support scale for adolescent patients with nephritic syndrome. Methods 192 adolescent patients with nephritic syndrome were selected in the pilot investigation, and then formed the first questionnaire. The reliability and the validity of the questionnaire were evaluated by the experts. Results The questionnaire involved 15 items and its reliability and the validity were satisfied. There was a positive correlation between the family support and the self-nursing degree. Conclusion The reliability and the validity of this questionnaire are both up to the demands, it can used to study the condition of family support for adolescent patients with nephrotic syndrome.

Background:Currently,chronic atrophic gastritis( CAG)is diagnosed by endoscopy combined with pathological examination. Narrow-band imaging magnifying endoscopy( NBI-ME)has been widely applied to diagnose CAG. However, it is a issue how to optimize the examination technology. Aims:To investigate the clinical value of NBI-ME in diagnosis of CAG,intestinal metaplasia and intraepithelial neoplasia. Methods:One hundred CAG patients reexamined gastroscopy were enrolled,sequential examination was performd,i. e. the extent of disease was observed under convention white light endoscopy( C-WLI ) followed by NBI-ME mode to observe locally,morphological changes of gastric mucosa pit was analyzed,endoscopic findings and histological results was compared. Results:Sensitivity of sequential examination for the diagnosis of CAG was 89. 7%,specificity was 63. 3%;sensitivity and specificity for the diagnosis of intestinal metaplasia were 89. 4%,89. 3%,respectively,for intraepithelial neoplasia were 84. 2%,95. 9%,respectively. Conclusions:Sequential examination can significantly improve the diagnostic accuracies of CAG,intestinal metaplasia and intraepithelial neoplasia,and effectively guide targeted biopsy,and easily to operate.

Objective To explore the correlation between thyroid peroxidase antibody (TPOAb) and outcomes during pregnancy and the effects of treatment on outcomes. Methods PubMed, Cochrane Library, Science Direct, Embase, Chinese Biomedicine, and Wanfangdata had been searched. Case-control and cohort studies about TPOAb and pregnancy outcomes were searched according to the inclusion and exclusion criteria. Fifty studies were finally recruited (all of cohort-studies, 10 for English and 5 for Chinese). Review Manager 5.3 were used to test the heterogeneity of the results among the different studies and amalgamate the effect size using fixed or random effect models. Results Meta-analysis showed TPOAb (+)with normal thyroid function increase the risks of miscarriage,and premature delivery, OR calculated were 2.02(95%CI:1.13-3.62, P=0.001)and 1.39(95%CI:1.11-1.76, P=0.005), while showed no relative risk to hypertensive disease,placental abruption in pregnancy and fetal growth restriction, OR calculated were 1.29(95%CI:1.00-1.67, P=0.080),0.42(95%CI:0.12-1.43, P=0.210)and 1.61(95%CI:0.23-11.12, P=0.100). TPOAb(+)with normal thyroid function increase miscarriage in in vitro fertilization and embryo transfer (IVF-ET), OR calculated were 2.14(95%CI:1.43-3.21, P=0.000). Levothyroxine (LT4) for patients of TPOAb(+)with normal thyroid dysfunction decrease adverse obstetric outcomes, OR calculated were 0.43(95%CI:0.22-0.85, P=0.020). Conclusions TPOAb(+)with normal thyroid function increase the risks of miscarriage,and premature delivery. TPOAb(+) with normal thyroid function increase miscarriage in IVF-ET. LT4 for patients of TPOAb(+)with normal thyroid dysfunction decrease adverse obstetric outcomes.

Objective To research the relationship between the expression of PAI-1 and the clinical characteristics of the endometrial carcinoma.Methods We detected the level of the serum PAI-1 by ELISA in the patients with endometrial carcinoma,the patients with endometrial hyperplasia and the patients with normal endometrium.The expression of PAI-1 in endometrial carcinoma and normal endometrial tissues was observed by immunohistochemistry.Results The concentration of serum PAI-1 in patients with endometrial carcinoma was 18.64 ± 6.22 μtg/L,significantly higher than those of patients with endometrial hyperplasia (6.94 ± 2.87) μg/L and patients with normal endometrium (6.68 ± 2.13)μg/L (P=0.00).The expression rate of PAI-1 was 68.2％ (15/22) in endometrial carcinoma tissue,and 8.3％ (2/24) in normal endometrial tissue (P=0.00).Compared with the early endometrial carcinoma,the expression rate of PAI-1 in the advanced endometrial carcinoma was significantly higher (6/6 w 9/16,P=0.03).Conclusions The expression level of PAI-1 may be related to the stage of the endometrial carcinoma.

Objective To explore the clinical head nurses'effective competence through interviewing nursing supervisors by in depth interview method and to provide reference for constructing training content for clinical head nurses in Macau and Guangzhou.Methods In depth interview was carried out among 8 nursing supervisors from 2 hospitals in Macau and 9 nursing supervisors from 2 tertiary hospitals in Guangzhou.Content analysis was utilized through organizing records and then forming written documentations.Results The themes were drawn after the interview,which were good moral character accomplishment,good communication skill,prospective and creative thinking,favorable executive ability.Forwardlooking and innovative were significantly different in Macau and Guangzhou.Conclusions The effective clinical head nurses' competence is multi-faceted,the results can provide reference for selection,evaluation and training for head nurses.

Objective To evaluate the feasibility and clinical value of identifying sentinel lymph node (SLN) and to assess possible factors associated with detection rate in both cervical cancer and endometrial cancer. Methods Retrospective study of 76 cases (39 with cervical cancer and 37 with endometrial cancer) were conducted in Peking University People′s Hospital. All patients underwent SLN biopsy with tracers of indocyanine green (ICG) and (or) carbon nanoparticles. All mapped SLN was resected and followed by procedures that systematic pelvic lymphadenectomy and hysterectomy according to National Comprehensive Cancer Network (NCCN) guidelines. All the lymph nodes were examined postoperatively for the routine paraffin section of hematoxylin and eosin (HE) staining. Detection rate, sensitivity and negative predictive value of SLN were calculated and factors associated with the detection rate were analyzed. Results The overall detection rate was 95%(72/76), with 74%(56/76) positive bilaterally. The bilateral detection rate of SLN with combined technique was significantly higher than that with single technique (P<0.05). The difference of SLN detection rate between cervical and endometrial cancer patients were not significant (P>0.05). SLN were mostly recognized in obturator (32.1%, 114/355) and external iliac areas (32.4%, 115/355) in cervical cancer, and in external iliac (41.2%, 91/221) and obturator areas (39.4%, 87/221) in endometrial cancer. Among 55 patients underwent systematic pelvic lymphadenectomy, the sensitivity of SLN detection was 75% and the negative predictive value was 96%. The sensitivity and negative predictive value were both 100% in patients with successfully bilateral mapped of SLN. Conclusions The overall detection rate of SLN in cervical and endometrial cancer is the highest with the combined technique of ICG and carbon nanoparticles. The detection rate and located regions of SLN are similar between cervical and endometrial cancer,and SLN are mostly recognized in the external iliac and obturator areas. The sensitivity and negative predictive value of SLN detection are high, especially when SLN are bilateral mapped.

Objective To investigate the clinical value of serum gastric function(pepsinogen I, II and PGR, gastrin-17), Helicobacter pylori (Hp) infection combined with narrow-band imaging magnifying endoscopy for the diagnosis of gastric ulcer. Methods From October 2015 to December 2016, we selected 113 patients diagnosed as gastric ulcer and gastric cancer by narrow-band imaging endoscopy (NBI-ME) and diagnosed by pathological examination. 65 patients were divided into gastric ulcer group. 48 patients were divided into gastric cancer group, 65 healthy subjects who had passed the physical examination were selected as the control group. Enzyme-linked immunosorbent assay was used to measure serum gastric function parameters such as serum pepsinogen I / Ⅱ, PGR, and gastrin-17, and C14 breath test was used to detect Hp. Results The Hp infection rate in the gastric ulcer group and the gastric cancer group was significantly higher than that in the control group, the difference was statistically significant (P＜0. 05). Compared with the control group, serum PG I and PGR levels in the gastric ulcer group were significantly increased (P＜0. 05); serum PG I and PGR levels in the gastric cancer group were lower than those in the control group(P＜0. 05). The PGI and PGR levels in the gastric ulcer group were significantly higher than those in the gastric cancer group, the difference was statistically significant (P＜0. 05). Compared with the control group, the serum G-17 concentration in the gastric ulcer group and the gastric cancer group was higher than that in the control group(P＜0. 05). Conclusion PG I, PG Ⅱ, G-17 and other serum gastric functional parameters combined with Hp indicators have been further measured by NBI-ME in gastric ulcer and gastric cancer diagnosis and prognosis has a high reference value.

Present study aimed to investigate the eff ect of curcumin-pretreatment on intestinal I/R injury and on intestinal mucosa barrier. Thirty Wistar rats were randomly divided into: sham, I/R, and curcumin groups (n=10). Animals in curcumin group were pretreated with curcumin by gastric gavage (200 mg/kg) for 2 days before I/R. Small intestine tissues were prepared for Haematoxylin & Eosin (H&E) staining. Serum diamine oxidase (DAO) and tumor necrosis factor (TNF)-alpha levels were measured. Expression of intestinal TNF-alpha and tight junction protein (ZO-1) proteins was detected by Western blot and/or immunohistochemistry. Serum DAO level and serum and intestinal TNF-alpha leves were signifi cantly increased after I/R, and the values were markedly reduced by curcumin pretreatment although still higher than that of sham group (p<0.05 or p<0.001). H&E staining showed the significant injury to intestinal mucosa following I/R, and curcumin pretreatment signifi cantly improved the histological structure of intestinal mucosa. I/R insult also induced significantly down-regulated expression of ZO-1, and the eff ect was dramatically attenuated by curcumin-pretreatment. Curcumin may protect the intestine from I/R injury through restoration of the epithelial structure, promotion of the recovery of intestinal permeability, as well as enhancement of ZO-1 protein expression, and this eff ect may be partly attributed to the TNF-alpha related pathway.
