1.Behavior Characteristics in Children with Benign Epilepsy Combined with Centro-Temporal Spikes

Objective To explore the behavior characteristics in children with benign epilepsy combined with centro-temporal spikes(BECTS).Methods Eighty-two children with BECTS aged 2.5-3.0(2.65?2.31)years old,51 male,31 female,who were free of mental retardation assessed with Gesell developmental schedules,untreated with antiepileptic drugs,and were investigated 15 days after the latest seizure.Eighty-two healthy children with sex and age matched to the cases,53 male,29 female,aged 2.5-3.0(2.6?0.4)years old.The behavior characteristics of infants in BECTS group and control group were assessed with CBCL,including 6 behavior factors which were sleep problem,social flinches,depression,physical aspect,attacking,act of sabotage and the infants-middle school student social ability scale.Results The total scores of behavior characteristics and the scores of depression,sleep problem,attacking and act of sabotage in BECTS group were all higher than those in control group,the differences were statistically significant.However,scores of social flinches,and physical aspect in BECTS group had no significant differences compared with those of control group.There were no significant difference of social adaptive component between the BECTS and control group.Conclusions Children with BECTS have behavior disorders to some extent,but their social adaptive capacity are the same as normal children.
2.Effect of Qingguang'an on elastic fiber, MMP- 7, TlMP - 1 in scarring area of filtration canal after glaucoma surgery

?AlM: To observe the effect of Qingguang'an on elastic fiber, MMP-7, TlMP-1 in scarring area of filtration canal after glaucoma surgery through the four Qingguang'an effective groups and Qingguang'an granules, to discuss and compare their mechanism of action on scarring area of filtration canal.?METHODS:Four effective components of Qingguang'an were used in groups D, E, F, G and H after glaucoma surgery, compared with group A ( blank ) , group B (model) and group C ( MMC) to observe the effect of elastic fiber, MMP-7, TlMP-1 in scarring filtration canal.?RESULTS:Compared with the preoperative basic lOP and 2d , 1, 2, 4wk postoperative lOP of groups C, E and H, the lOP of three group rose up slower than other groups, and kept the lowest data at 28d. There was significant difference compared with the rest of A, B, D, F, G groups (P<0. 05). The area and density of elastic fiber in surgery group were significantly different with that of black control group ( P<0. 05 ), but there were no statistical differences between groups C and H, groups C and F, groups H and E (P>0. 05). The difference was statistically significant among other groups (P<0. 01).?CONCLUSlON:The scarring area of filtration canal after glaucoma surgery is the major reason which lead to the failure of surgery. Qingguang'an effective group 2, Qingguang'an granules and MMC could reduced the scar tissue by restrained the elastic fiber, TlMP - 1 and increased the MMP-7. By observing the experimental results that both Qingguang'an effective group 2 and Qingguang'an granules could restrained the scarring area of filtration canal, the effects were unbiased, Qingguang'an granules group is better than effective group 2.
3.The false-negative reasons of positioning test in benign paroxysmal positional vertigo.

OBJECTIVE:
To analyze the false-negative proportion and reasons of positioning test in benign paroxysmal positional vertigo (BPPV).
METHOD:
Sixty-eight cases were reviewed and analyzed during February to April 2009. All patients had typical clinical history of BPPV and diagnosed by positioning test,and all were treated with PRM successfully. If positioning test at the first time were negative, examinations repeated after either head shaking or return visits.
RESULT:
Thirteen patients (19.12%) appeared false-negative in positoning test, and 11 cases showed typical nystagmus after head shaking,while 2 cases were diagnosed by return visitis the next day. There were no statistically difference between false-negative group and positive group in age (P > 0.05), gender (P > 0.05), course of disease (P > 0.05) and types of semi-circular canal (P > 0.05).
CONCLUSION
Re-examinations after head shaking or return visits are useful for the diagnosis in the suspected BPPV patients who have negative positioning test.

4.Efficacy comparison between endolymphatic sac surgery and semicircular canal occlusion in the treatment of stage 4 Ménière disease.

OBJECTIVE:
To compare the vertigo controlling situation between the endolymphatic sac decompression(ELSD) and semicircular canal occlusion (SCO) in stage 4 Ménière disease.
METHOD:
Fourteen patients who underwent endolymphatic sac decompression and 9 patients who underwent semicircular canal occlusion from 2009 to 2013 were followed. All patients has complete preoperative examination and postoperative follow-up.
RESULT:
The vetigo control of the patients underwent endolymphatic sac decompression: completely control 35.7%; basic control 28.6%; partly control 14.3%; the vetigo control of the patients underwent semicircular canal occlusion: completely control 88.9%; basic control 11.1%.
CONCLUSION
ELSD and SCO are alternative methods for the patients of stage 4. SCO has a much highter complete vertigo control rate. For the patients without practical listening, SCO is a better choice.

5. Function of N-Methyl-D-Aspartic Acid Receptors in Cerebral Stroke

Cerebral stroke is caused by the interrupt of blood supply either by the blockage or by the rupture of the brain blood vessels. After the activation of the ischemic cascades, a series of neurochemical reactions might occur which involves the excess release of the excitatory amino acids. The ionotropic glutamate receptor of N-methyl-D-aspartic acid subtype (NMDA receptor) has been proven to play dual roles in the ischemic insults. On one hand, NMDA receptor has been proven to initiate the ischemic impairment, leading to the neuronal death. On the other hand, NMDA receptor is involved in the endo-neurogenesis process after the ischemic onset. Herein, we summarized the recent studies about the structures and functions of NMDA receptor in ischemic stroke pathogenesis.
6.Clinical value of metagenomic next-generation sequencing in complicated infectious diseases.

Infectious diseases are commonly seen in clinical practice, and pathogen diagnosis is the key link in diagnosis and treatment; however, conventional pathogen detection methods cannot meet clinical needs due to time-consuming operation and low positive rate. As a new pathogen detection method, metagenomic next-generation sequencing (mNGS) has a wide detection range and can detect bacteria, viruses, fungi, parasites, rare pathogens, and even unknown pathogens. The technique of mNGS is unbiased and can rapidly, efficiently, and accurately obtain all nucleic acid information in test samples, analyze pathogens, and guide clinical diagnosis and treatment, thereby playing an important role in complicated infectious diseases. This article reviews the diagnostic advantages and clinical value of mNGS in bacterial, fungal, viral, and parasitic infections.

7.Chemical constituents of bufadienolides in cinobufacino injection.

Cinobufacino injection is purified from water extraction of the skin of Bufo bufo gargarizans, which has been widely used for various cancers in clinic with significant anti-tumor effects. Bufadienolides were regarded as the main active constituents of cinobufacino injection in previous reports. In present study, 6 bufadienolides were isolated and purified from Cinobufacino injection. Their structures were identified as 3-epi-ψ-bufarenogin (1), ψ-bufarenogin (2), 3-epi-arenobufagin (3), arenobufagin (4), 3-epi-gamabufotalin (5), and 3-oxo-arenobufagin (6), separately. Among them, 1 and 3 were new compounds, 5 and 6 were new natural products. Compounds 1, 2 and compounds 3, 4 were two pairs configuration isomers at C-3, separately.

8.Antimicrobial resistance profiles of the imipenem-resistant Escherichia coli clinical isolates from 2011 to 2015

Objective To investigate the distribution and antimicrobial resistance profiles of the imipenem-resistant Escherichia coli clinical isolates.Methods A total of 10428 Escherichia coli isolates were isolated from various specimens of patients at the first affiliated hospital of Wenzhou medical university from January 2011 to November 2015.The identification and antimicrobial susceptibility testing for Escherichia coli isolates were determined by VITEK Full Automated Microbiology Analyzer.Results Among 10428 Escherichia coli clinical isolates,including 62.69% ESBLs-producing isolates,84(0.81%) were resistant to imipenem.The rates of imipenem resistance among Escherichia coli isolates from 2011 to 2015 were 0.14% (2/1397 strains),0.47% (9/1898strains),0.55% (12/2170 strains),0.88% (22/2489 strains) and 1.58% (39/2474 strains).Among 84 imipenem-resistant isolates,50.00% (42/84 strains) were ESBLs-producing isolates;the non-susceptibility rates of all imipenem-resistant isolates to cefazolin,ampicillin,ampicillin/sulbactam,ertapenem and ceftriaxone were more than 90.00%.However,the susceptibility rates of these isolates to nitrofurantion and amikacin were more than 50.00% (50.00% and 62.50%).The susceptibility rates of imipenem-resistant isolates to other types of antibiotics were less than 30.00%.Conclusion Imipenem has fine antimicrobial activity in vitro against Escherichia coli isolates.However,the prevalence of imipenem resistance among Escherichia coli clinical isolates is increasing year by year.
9.Application of iliac cancellous and chondral bone to repair the cleft of alveolar process and the nasal deformity at the same time.

OBJECTIVETo study a new method of simultaneous repair of alveolar cleft and the deformity of the nose and lip.
METHODS42 patients with cleft of alveolar process were included in this study. We filled the cleft of alveolar process with iliac cancellous bone and covered the depression of piriform aperture and the nasal side with iliac cartilage. Simultaneously, we corrected the nasal deformity using V-Y plasty of a mucosa-cartilage compound flap at the nasal vestibule.
RESULTSIn the patients aged 9 approximately 11 years, the mean height of the repaired alveolar process was 86.7%; the mean thickness was 89.6%; the contour and function of the alveolar process was grade I or II. The canine erupted at the place of bone grafting. The depression of the piriform aperture and alae nasi was repaired. The contour of the nares was symmetrical. In the patients over 12 years, the mean height of the alveolar process was 70.1%; the mean thickness was 71.7%; the contour and function of the alveolar process was grade II or III. The deflexion of nasal dorsum and nasal dome was not ameliorated in 2 patients of 18 years old.
CONCLUSIONSThe application of iliac cancellous and chondral bone to simultaneously repair cleft of alveolar process and the nasal deformity is an ideal plastic method. It can recover the height of alveolar process and the continuity of dental arch, ameliorate the nasal deformity, and redress the deflexion of nasal dorsum and nasal end. It can also eliminate the psychological obstacle of the patients at an early stage.

Objective To construct the Neisseria surface protein A (NspA) DNA vaccine of Neisseria gonorrhoeae and evaluate the humoral and cellular immune responses induced by this vaccine in mice model.Methods The recombinant expression vector pcDNA3.1 (+)/NspA was constructed by inserting NspA gene into the eukaryotic expression vector pcDNA3.1 (+) and confirmed by poly merase chain reaction (PCR),restriction enzymes HindⅢ,XbaⅠand DNA sequencing.NspA mR- NA in transfected RAW264.7 cells and NspA protein expression in transfected COS-7 cells were de- tected by reverse transcription-polymerase chain reaction (RT-PCR) and immunohistochemical stai- ning,respectively.Forty-five male BALB/c mice were immunized with pcDNA3.1 (+)/NspA recom binant plasmid.The level of serum anti-Neisseria gonorrhoeae antibody of the immunized mice was detected by tube agglutination test,and the level of interieron (IFN)-?was assayed by enzyme-linked immunosorbent assay (ELISA).The proliferation of splenocytes was determined by methyl thiazolyl tetrazolium (MTT) colormetry.The NspA gene in BALB/c mice was identified by PCR with the total DNA extracted from quadriceps femoris in immunized sites.Results Restriction enzymes digestion a- nalysis and DNA sequencing results revealed that the pcDNA3.1 (+)/NspA had been constructed successfully.NspA gene had been transcripted and expressed in mammalian cells.The peak titer of specific antibody was 1:640 in pcDNA3.1(+)/NspA immunized group and there was no specific an- tibody detected in both pcDNA3.1 (+) immunized group and PBS group.The IFN-?level in pcD NA3.1 (+) immunized group was (23.79?11.85)pg/mL and that in pcDNA3.1 (+)/NspA immu- nized group was(169.71?30.52)pg/mL (P
