1.A case of hydroa vacciniforme-like primary cutaneous CD8-positive T-cell lymphoma

39℃)developed at the progressive stage of this disease.Physical examination showed variously sized,round or oval,atrophic and variola-like scars along with scattered erythematous patches,papules, necrosis and crusts on the face and extremities.The face was edematous,and there were some edematous and erythematous plaques with a necrotic center on the legs and arms.Histological examination revealed a massive infiltration with atypical CD8~+lymphocytes around the vessels and appendages in dermis.A diagnosis of CD8~+cutaneous T-cell lymphoma(CTCL)was made.Glucocorticoid and immunosuppressants were effective in controlling the condition.Up to the time of the writing,there has not been any definite evidence of systemic involvement.

3.The effects of compound CX09040 on the inhibition of PTP1B and protection of pancreatic β cells.

To investigate the effects of 2-(4-methoxycarbonyl-2-tetradecyloxyphenyl)carbamoylbenzoic acid (CX09040) on protecting pancreatic β cells, the β cell dysfunction model mice were induced by injection of alloxan into the caudal vein of ICR mice, and were treated with compound CX09040. Liraglutide was used as the positive control drug. The amount and the size of islets observed in pathological sections were calculated to evaluate the β cell mass; the glucose stimulated insulin secretion (GSIS) test was applied to estimate the β cell secretary function; the oral glucose tolerance test (OGTT) was taken to observe the glucose metabolism in mice; the expressions of protein in pancreas were detected by Western blotting. The effects on the target protein tyrosine phosphatase 1B (PTP1B) were assessed by the PTP1B activities of both recombinant protein and the intracellular enzyme, and by the PTP1B expression in the pancreas of mice, separately. As the results, with the treatment of CX09040 in alloxan-induced β cell dysfunction mice, the islet amount (P<0.05) and size (P<0.05) increased significantly, the changes of serum insulin in GSIS (P<0.01) and the values of acute insulin response (AIR, P<0.01) were enhanced, compared to those in model group; the impaired glucose tolerance was also ameliorated by CX09040 with the decrease of the values of area under curve (AUC, P<0.01). The activation of the signaling pathways related to β cell proliferation was enhanced by increasing the levels of p-Akt/Akt (P<0.01), p-FoxO1/FoxOl (P<0.001) and PDX-1 (P<0.01). The effects of CX09040 on PTP1B were observed by inhibiting the recombinant hPTP1B activity with IC50 value of 2.78x 10(-7) mol.L-1, reducing the intracellular PTP1B activity of 72.8% (P<0.001), suppressing the PTP1B expression (P<0.001) and up-regulating p-IRβ/IRβ (P<0.01) in pancreas of the β cell dysfunction mice, separately. In conclusion, compound CX09040 showed significant protection effects against the dysfunction of β cell of mice by enlarging the pancreatic β cell mass and increasing the glucose-induced insulin secretion; its major mechanism may be the inhibition on target PTP1B and the succedent up-regulation of β cell proliferation.

4.Influence of the VEGF antibody targeted vascular therapy on the expression of collagen type I in hyperplastic.

OBJECTIVETo investigate the influence of the vascular endothelial growth factor( VEGF) antibody targeted vascular therapy on the expression of human collagen type I in hyperplastic scar of nude mice.
METHODSThe hyperplastic scar from one female burn patient with 1% TBSA deep-partial thickness burns were implanted into subcutaneous skin of scapular region of 48 nude mice. Three weeks later, the nude mice were divide into large dose (LA) , medium dose (MD) , small dose (SD) and control groups, with 12 mice in each group. The mice in LA,MD and SD groups were injected with 200 microl of 15,10, 5 microg/ml VEGF monoclonal antibody diluted in 0.01 mol/L PBS, respectively in the scar twice a week for 3 weeks, while those in C group were injected with equal amount of 0. 01 mol/L PBS. The area and volume of the scar in each group were calculated and histological changes were observed, and the expression of collagen type I mRNA and its protein in each group were determined 3 days after treatment.
RESULTSThe volume of scar in LA, MD, SD and C groups were (55.3 +/-4.1, 67.9 +/-5.7, 78.9 +/-5.5, 85.0 +7.3) mm(3), respectively. Compared with that in C group, the volume of the scar were significantly decreased in AD and MD groups ( P <0.05). A few number of vessels and fibroblasts were observed in LD, MD groups, with decreased number of collagen fibers arranged in order. Compared with that in C group ,The expression of procollagen type I mRNA and its protein in C group was obviously higher than those in LD and MD groups ( P < 0. 05) , but it was similar to those in SD group.
CONCLUSIONVEGF targeted vascular therapy is beneficial for the inhibition of the angiopoietins of hyperplastic scar, the expression of collagen , and the growth of scar.

5.Comparison of clinical curative effect between open surgery and endovascular repair of abdominal aortic aneurysm in China.

OBJECTIVETo compare clinical curative effects of open surgery (OS) or endovascular repair (EVAR) for patients with abdominal aortic aneurysm (AAA) in China.
DATA SOURCESWe performed a comprehensive search of both English and Chinese literatures involving case studies on retrograde OS or EVAR of AAA in China from January 1976 to December 2010.
STUDY SELECTIONAccording to the inclusion criteria, 76 articles were finally analyzed to compare patient characteristics, clinical success, complications, and prognosis.
RESULTSWe analyzed a total of 2862 patients with 1757 undergoing OS (OS group) and 1105 undergoing EVAR (EVAR group). There was no significant difference in the success rate of the procedures. Operative time, length of ICU stay, fasting time, duration of total postoperative stay, blood loss, and blood transfusion requirements during the procedure were significantly lower in the EVAR group. A 30-day follow up revealed more cardiac, renal, pulmonary, and visceral complications in the OS group (P < 0.01). Low-limb ischemia, however, was more common in the EVAR group (P < 0.05). The 30-day mortality rate, including aorta-related and non-aorta related mortality, was significantly lower in the EVAR group (P < 0.01). In the follow-up period, there were more patients with occlusions of artificial vessel and late endoleak in the EVAR group (P < 0.01). The overall late mortality rate was higher in the OS group (P < 0.01), especially non-aorta-related late mortality and mortality during the fourth to the sixth year (P < 0.01).
CONCLUSIONSEVAR was safer and less invasive for AAA patients. Patients suffered fewer complications and recovered sooner. However, complications such as artificial vessel occlusion, low-limb ischemia, and endoleak were common in EVAR. Clinicians should carry out further research to solve these complications and improve the efficacy of EVAR.

6.The expression of glucagon-like peptide-1 receptor in skeletal muscle cells of diabetic rats

Objective To investigate the expression of glucagon-like peptide-1 receptor (GLP-1R) of skeletal muscle cells and the changes of skeletal muscle microstructure of diabetic rats, and to explore the relationship between the expressions of GLP-1R and insulin resistance in skeletal muscle. Methods Male Wistar rats were divided into two groups, normal glucose tolerance ( NT, n =25 ) , diabetes group ( DM group, n=30), the methods of creating diabetes model were used high-sugar high-fat diet and low-dose in-traperitoneal injection of streptozotocin. Immunohistochemical methods was used to detect the expression of GLP-1R in the muscle cells. The number of GLP-1R positive cells in the muscle of two groups was observed and compared. The relationship was analyzed between the homeostasis model assessment insulin resistance (HOMA-IR) and the number of GLP-1R positive cells. Results ⑴Compared to the NT group, the num-ber of GLP-1R positive cells in muscle of DM group was decreased (P<0. 05); ⑵ Compared to the NT group, HOMA-IR was increased in DM group (P<0. 05);⑶ Pearson correlation analysis showed that the percentage of GLP-1R positive cells expressed in rat skeletal muscle was negatively correlated with HOMA-IR (r= -0. 538, P<0. 01). Conclusions The expression of GLP-1R positive cells was decreased in the skeletal muscle of diabetic rats, insulin resistance increased. Therefore, the expression of GLP-1R on skele-tal muscle cells may promote the development of insulin resistance and diabetes mellitus.

8.Study on the micropermeability of resin-dentin bonding interfaces with ethanol-wet bonding technique

Objective To evaluate the micropermeability on bonding hydrophobic adhesive to dentin with ethanol-wet bonding under simulated pulp pressure.Methods Twenty-four intact human third molars were used in the study.After the enamel of occlusal surfaces was removed,the molars were randomly divided into six groups.Adper Scotchbond Multi-Purpose was used in the control group; in the experimental groups,the dentin surfaces were saturated with ethanol for 20 s ( group 1 ),1 min ( group 2 ),2 min ( group 3 ),3 min ( group 4 ) or with a series of increasing ethanol concentrations before application of hydrophobic adhesive ( group 5 ).All the bonding procedures were done under simulated pulp pressure.After 24 hours,micro-tensile bond strength test were performed on the specimens.Bonding interfaces were observed under laser scanning confocal microscope (LSCM) after the pulp chamber were filled with a water-soluble fluoroprobe rhodamine B for 3 hours.Results Compared with the control group[(38.14 ± 4.97 ) MPa],bond strengths in group 1 [(21.02 ± 7.23 ) MPa]and group 2 [( 29.64 ± 3.81 ) MPa]were statistically lower ( P > 0.05 ),while bond strngth in group 3 [( 38.40 ± 5.03 ) MPa],group 4 [( 37.26 ± 4.68 ) MPa]and group 5[(40.12 ±5.95) MPa]were similar to the control group (P<0.05).The images taken by LSCM showed that with extension of ethanol-wet time,the deposition of fluorescent dye in hybrid layer and along the dentinal tubules decreased gradually.Especially in group 5,only spare fluorescent dye deposition could be detected in the hybrid layer.Conclusions Dentin saturated with ethanol for more than 2 min before bonding hydrophobic adhesive to dentin could provide favorable bond strength and decreased the micropermeability of bonding interfaces under simulated pulp pressure.
9.Comparison of Measurement Methods of Exophthalmos and Its Forensic Significance

Objective To compare the differences between Hertel exophthalmometry and CT on exophthalmos measurement,and explore its practical application value in forensic medicine.Methods Fifty-six normal individuals without eye injuries or diseases were selected as the control group.The absolute values of exophthalmos were measured in the standardized CT image workstation.The difference of binocular exophthalmos was compared in normal group.Forty-seven cases with unilateral orbital fracture were divided into injured eye group and normal eye group according to the presence of binocular orbital fractures.The differences of the absolute values of eophthalmos from the same eye and the relative values of eophthalmos between two eyes for same person measured by Hertel exophthalmometry and CT were analyzed.Results There was no statistical difference of exophthalmos between the normal eyes measured by CT method.In the normal eye group,the absolute value of exophthalmos measured by CT method was (16.66±5.41) rm,which was (16.16±4.45)mm when measured by the Hertel exophthalmometry method.There was no statistical difference between two groups (P>0.05).In the injured eye group,there was statistical difference between the absolute values of exophthalmos measured by two measurement methods (P<0.05).There was no statistical difference of the relative values of exophthalmos between the injured eyes and the normal eyes by two measurement methods (P>0.05).Conclusion CT method has a good consistency with Hertel exophthalmometry,which can be applied into the practice of medicolegal expertise.
10.A 10-day sequential therapy for eradication of Helicobacter pylori infection in children.

OBJECTIVETo evaluate the clinical effect of a 10-day sequential therapy which was made up of omeprazole, clarithromycin, amoxicillin-clavulanate and metronidazole for the eradication of Helicobacter pylori (Hp) infection in children.
METHODA total of 214 children with abdominal pain, who were confirmed to have Hp infection through endoscopy, biopsy, and Hp culture. The 214 cases were randomly divided into four groups. A 10-day sequential therapy group accepted omeprazole 0.8 - 1.0 mg/(kg·d) plus amoxicillin-clavulanate 50 mg/(kg·d) for five days and omeprazole 0.8 - 1.0 mg/(kg·d), clarithromycin 20 mg/(kg·d) and metronidazole 20 mg/(kg·d) for the remaining five days. The 7-day triple therapy group, 10-day triple therapy group and 14-day triple therapy group received omeprazole 0.8 - 1.0 mg/(kg·d), amoxicillin-clavulanate 50 mg/(kg·d) and clarithromycin 20 mg/(kg·d) for 7 days,10 days,14 days, respectively. All drugs were given twice daily. All these patients received (13)C urea breath test ((13)C-UBT) four weeks after the treatment.
RESULTFinally, 199 patients were followed up, and the total rate of loss to follow-up was 7.0% (15/214). Hp eradication rate was 85.2% and 90.2% in the 10-day sequential therapy group on intention to treat (ITT) and per protocol (PP) analyses, 66.0% and 71.4% in the 7-day triple therapy group on ITT and PP analyses; 60.0% and 67.3% in 10-day triple therapy group on ITT and PP analyses, and 78.8% and 82.0% in patients who received the 10-day sequential regimen on ITT and PP analyses, respectively. By ITT analysis, there was significantly difference between the 10-day sequential therapy group and 7-day or 10-day triple therapy group (P < 0.05), while no significant difference was found between the 10-day sequential therapy group and 14-day triple therapy group (P > 0.05). The results of the ITT analysis and the PP analysis were the same. The four groups had neither significant difference in abdominal pain relief (P > 0.05) nor in incidence of adverse reactions (P > 0.05).
CONCLUSIONThe 10-day sequential regimen was significantly more effective than both 7-day triple regimen and 10-day triple regimen, while had the same eradication rate compared with the 14-day sequential therapy. But 10-day triple regimen to eradicate Hp infection in children had the advantages such as short course of treatment and better compliance.
