1.Analysis of intraocular lens power calculation for cataract patients with high myopia and posterior scleral staphyloma

Background The accurate calculation of intraocular lens (IOL) power is essential for attaining the desired refractive outcome after cataract surgery,especially for patients with high myopia and posterior scleral staphyloma.Objective This study was to evaluate the clinical feasibility of IOL Master compared with contact A-scan in cataract patients with high myopia and posterior scleral staphyloma,then compare the accuracy of different IOL power calculation formulas.Methods This was a prospective case control clinical research.Fourty-one eyes with age-related cataract of 28 patients underwent phacoemulsification with monofocal foldable IOL implantation in Tianjin Medical University Eye Hospital were involved,who were all high myopia with posterior scleral staphyloma.Preoperative measurement was measured with IOL Master as well as with contact A-scan and manual keratometry.IOL power was calculated according to the SRK-Ⅱ,SRK-T,Haigis,Hoffer Q,Holladay 1 formulas.The refractive outcome was followed-up 3 months after operation.Results The difference was significant between the 2 methods in axial length (AL) and anterior chamber depth (ACD) measurement (P =0.005,0.000) ; In corneal curvature measurement,there was no significant difference between them (P =0.398).When mean absolute refractive error (MAE) was divided by ±1.00 D,The SRK/T and Haigis formula performed better than other formulas measured by IOL Master;The Holladay 1,Hoffer Q and Haigis formula performed better than other formulas measured by contact A-scan combined with manual keratometry,respectively.Conclusions For cataract patients with high myopia and posterior scleral staphyloma,SRK/T and Haigis formula were recommended when employing IOL Master; whereas when using contact A-scan combined with manual keratometry,we prefer Holladay 1,Hoffer Q or Haigis formula.
2.The effect of music exposure in juvenile stage on anxiety-like behavior and fear extinction in adult rat

ObjectiveTo study the effect of music exposure in juvenile stage on anxiety-like behavior and fear extinction in adult rats.Methods4 broods of two-week-old healthy Sprague-Dawleyda rats were separated into two groups randomly.The music groups were exposed to music from 20:00 to 22:00 for 21 consecutive days.When the rats were 5 weeks old,they were caged by sex,with 8 male rats and 6 female rats in each group.When the rats were 8 weeks old,the two groups were assessed in the elevated-plus maze test,the open-field test,trained and assessed by the conditional fear training and fear extinction training.ResultsIn the elevated-plus maze test:Prior to the foot shock,the percent of time spent in the open arms in the music group (female(7.07 ± 1.14)%,male (5.12 ± 1.95 ) % ) exhibited no significant difference (P> 0.05 ) comparing with the rats in the control group (female (4.65 ± 0.86 ) %,male ( 4.86 ± 1.95 ) % ).After the foot shock,the percent of time spent in the open arms in the music group ( female ( 8.63 ± 3.35 ) %,male ( 7.79 ± 2.49 ) % ) increased comparing to the control group (female(1.48 ±0.11)%,male(4.29 ± 1.68)%) (P<0.01). In the open-field test:prior to the foot shock,the percent of time spent in the center zone of the music group ( female (6.16 ± 2.17 ) %,male (6.25 ±3.47) % ) exhibited no significant difference (P> 0.05) comparing with the rats in control group (female(5.27 ±1.95 )%,male (6.22 ± 3.13 )% ).After the foot shock,the percent of time spent in the center zone in the music group (female(8.52 ± 1.93) %,male (6.95 ± 2.46 ) % ) was larger than the control group ( female ( 3.47 ±0.93 ) %,male (4.36 ± 2.22 ) % ) (P < 0.05 ).The fear extinction training showed that the percent of freezing time exhibited no significant difference between the male and female rats of the music group and control group in the first block of the first extinction training day and the percentage of freezing time of female rats in music group was significantly lower than that of female rats in control group on the third extinction training day.The percentage of freezing time of male rats in the music group was significantly lower than that of male rats in control group on the second and third training day.ConclusionMusic exposure in juvenile stage decreases the anxiety level after foot shock,and promotes the long-term fear extinction,while has no effect on the level of fear memory.
3.Causes analysis of early postoperative high intraocular pressure of angle - closure glaucoma underwent compound trabeculectomy

To analyze the causes and treatments of high intraocular pressure ( > 21mmHg ) of angle - closure glaucoma underwent compound trabeculectomy 1mo after surgery.
●METHODS: This was a retrospective study of our hospital, from March 2010 to March 2013. Thirty-four (38 eyes) of angle-closure glaucoma patients were collected, who underwent compound trabeculectomy with high intraocular pressure ( > 21mmHg) 1mo after operation. We analyzed the causes and summarized the treatments.
●RESULTS:The causes which lead to early postoperative high intraocular pressure included malignant glaucoma (9 eyes of 8 cases), blood clot and connective tissue block under the scleral flap (15 eyes of 13 cases), hyphema after surgery(5 eyes of 5 cases), sclera flap incision was incarcerated with iris tissue ( 3 eyes of 3 cases ), preoperative high intraocular pressure for a long time(5 eyes of 4 cases), 1 eye of 1 case for unknown reason. After proper treatments, intraocular pressures of all patients were bellowed 21mmHg.
● CONCLUSlON: Early postoperative high intraocular pressure of angle - closure glaucoma underwent compound trabeculectomy is caused by various factors, and the early prevention and timely treatment are key points of a successful operation.
4.Efficacy of internal limiting membrane insertion in the treatment of refractory macular holes evaluated by SD-OCT

AIM: To evaluate the efficacy of pars plana vitrectomy combined with internal limiting membrane(ILM)insertion for the treatment of refractory macular holes(MH)by spectral-domain optical coherence tomography(SD-OCT).
METHODS: Retrospective analysis was conducted in 17 patients(17 eyes)with refractory MH from July 2019 to December 2020. All patients were treated with pars plana vitrectomy combined with ILM insertion. Preoperative and postoperative best corrected visual acuity(BCVA)were compared. Quantified evaluation of the postoperative macular restoration was performed by SD-OCT.
RESULTS: The widths of ellipsoid zone disruption were 839.00-1577.50μm, with an average of 1182.90±226.68μm. During the follow-up period(3-12mo), 17 cases achieved successful closure(100%), and no recurrence was seen. Postoperative BCVA was significantly improved compared with preoperative(P<0.01). The widths of external limiting membrane disruption and ellipsoid zone disruption were smaller gradually at 1wk and 3mo after surgery compared with preoperative(all P<0.01). But the continuity was still not restored. The thickness of the central fovea of macula(within 1mm diameter range)at 1wk and 3mo after surgery decreased significantly compared with preoperative(all P<0.01).
CONCLUSION: Pars plana vitrectomy combined with ILM insertion proves to be effective to achieve anatomical and functional improvement in the treatment of refractory MH. SD-OCT has important clinical value in the diagnosis and postoperative follow-up evaluation of refractory MH.
5.Correlation between resting heart rate and blood glucose level in elderly patients with coronary heart disease and diabetes mellitus.

OBJECTIVETo explore the correlation between resting heart rate (RHR) and blood glucose level in elderly patients with coronary heart disease (CHD) complicated by diabetes mellitus.
METHODSBetween April and July, 2011, a total of 1336 outpatients over 60 years of age recruited from 165 hospitals were asked to complete a questionnaire and received blood glucose and RHR examination. According to baseline RHR, the patients were divided into 3 groups with HRH <70 min-1 (group I, 372 cases), between 70 and 79 min(-1) (group II, 533 cases), and ≥80 min(-1) (group III, 431cases) for analysis of the relationships of RHR with blood glucose control rate.
RESULTSHbA1c levels in the total, male and female patients differed significantly among the 3 groups (F=15.436, 15.436, and 24.270, respectively, P<0.05), and increased in the order from group I to group III. Blood glucose control rate in the total, male and female patients also differed significantly among the 3 groups (χ(2)=13.471, 6.752, and 6.522, respectively, P<0.05), and was significantly lower in group III than in group I (P<0.05). RHR was found to positively correlate with FPG, 2 hPG and HbA1c by Pearson correlation analysis (r=0.058, 0.085, and 0.058, respectively; P<0.05) and multiple linear regression analysis (β=0.075, 0.075, and 0.018, respectively; P<0.05). Multivariable logistic regression equation showed that compared with patients with RHR <70 min-1, the total, male and female patients with RHR ≥80 min(-1) had OR values of blood glucose control failure of 1.99 (95% CI: 1.23-2.37, P<0.05), 1.81 (95% CI: 1.17-2.77, P<0.05), and 2.18 (95% CI: 1.12-3.74, P<0.05), respectively.
CONCLUSIONRHR in elderly CHD patients with MD is positively correlated with their blood glucose level, and an increased RHR is associated with an increased risk of poor blood glucose control. Rigorous RHR control in such high-risk patients may prove beneficial for both blood glucose control and secondary prevention of CHD.

6.Short-term efficacy of intense pulsed light for the comprehensive treatment of blepharokeratoconjunctivitis

AIM: To investigate the short-term efficacy of intense pulsed light(IPL)for the comprehensive treatment of blepharokeratoconjunctivitis(BKC).METHODS: A total of 66 patients(66 eyes)diagnosed with BKC in Cangzhou Aier Eye Hospital from December 2020 to December 2021 were selected(All selected the more severe eye for research). They were divided into two groups according to the different clinical treatment, with 30 cases(30 eyes)in control group and 36 cases(36 eyes)in treatment group. Both groups were administrated for 1wk(0.1% fluorometholone eye drops, 0.3% sodium hyaluronate eye drops and tobramycin dexamethasone eye ointment). Ocular surface state was taken as the baseline level. The control group continued to receive drugs(0.1% fluorometholone eye drops and 0.3% sodium hyaluronate eye drops), while the treatment group is required to undergo regular IPL in the hospital on the basis of drug treatment(atomization fumigation+IPL+meibomian gland massage + meibomian margin cleaning+cold compress), once every 2wk, for a total of 4 times. The patients in both groups were followed up at 1wk after drug treatment, before the first IPL treatment(V1), before the third treatment(V2)of IPL, and at 2wk after the fourth treatment(V3)of IPL, respectively. Data including ocular surface disease index(OSDI), the morphology of meibomian margin under the slit lamp, the nature of meibomian gland(MG)secretion, the difficulty of excretion of MG secretion, non-invasive tear film break-up time(NIBUT), red eye index and corneal fluorescent staining were collected. Moreover, the best corrected visual acuity(BCVA), intraocular pressure, anterior segment and fundus were examined to observe the occurrence of complications.RESULTS: There was no statistically significant difference in the indexes of the patients in both groups before treatment, including OSDI, morphology of meibomian margin, nature of MG secretion, the difficulty of excretion of MG secretion, NIBUT, ocular surface hyperemia and corneal fluorescein sodium staining(P>0.05). In V2 and V3, the indexes of the patients in both groups, including OSDI, morphology of meibomian margin, nature of MG secretion, the difficulty of excretion of MG secretion, ocular surface hyperemia and corneal fluorescein sodium staining, were lower than those before treatment(V1), while the NIBUT was longer than that before treatment, the differences were statistically significant(all P<0.05). There was difference between the groups(P<0.05). Those indexes improved more obvious in the treatment group. No obvious complications occurred in all patients.CONCLUSION: IPL comprehensive treatment have better effect on reducing the inflammation of ocular surface, and improving the function of MG, and it can be used as a new option for physical therapy of BKC.
7.Macular microstructural changes in patients with rhegmatogenous retinal detachment after silicone oil tamponade evaluated by SD-OCT

AIM:To evaluate the macular microstructural changes in patients with rhegmatogenous retinal detachment(RRD)after silicone oil tamponade by spectral-domain optical coherence tomography(SD-OCT).METHODS:From November 2019 to July 2021, 27 patients with 27 eyes in RRD who underwent vitrectomy combined with silicone oil tamponade in Cangzhou Aier Eye Hospital were enrolled in this study as the observation group, other 30 healthy volunteers with 30 eyes were included in the control group. The best corrected visual acuity(BCVA)of patients before and after operation were observed, and quantified evaluation of the postoperative macular microstructural changes were performed by SD-OCT.RESULTS: The BCVA(LogMAR)of the observation group at 1wk and 3mo after operation(0.61±0.23, 0.69±0.34)were improved compared with those before operation(1.43±0.77)(all P<0.01). The cube volume and average cube thickness in the macular area at 3mo after operation in the observation group were lower than those at 1wk and 1mo after operation in the control group(all P<0.05). There were no differences in the average ganglion cell-inner plexiform layer(GCIPL)thickness, minimum GCIPL thickness, average macular retinal nerve fiber layer(mRNFL)thickness and minimum mRNFL thickness at 1wk, 1 and 3mo after operation in the observation group, but all decreased compared with the control group(all P<0.01). There were 9 eyes with subretinal fluid(SRF)in the observation group during postoperative follow-up, SRF had a tendency to be gradually absorbed, but 1 eye had a secondary macular hole; 3 eyes had ellipsoid zone disruption, which had a tendency to be gradually repaired; 2 eyes had submacular perfluorocarbon liquid; 2 eyes had macular edema.CONCLUSION: SD-OCT can show the microstructure and morphological changes very well in macular area in patients with RRD after silicone oil tamponade, and has important clinical value for the preoperative and postoperative follow-up evaluation of RRD.
8.Different Energy Spectrum CT Findings between Anterior Mediastinal Lymphoma and Thymic Carcinoma.

To investigate the differences in energy spectrum CT findings between anterior mediastinal lymphoma and thymic carcinoma. Twenty-two cases of anterior mediastinal lymphoma and 28 cases of thymic carcinoma confirmed by biopsy in Tangshan People's Hospital were selected.The CT values and changes of iodine content and water content in lesion sites were measured by energy spectrum analysis software.The differences between anterior mediastinal lymphoma and thymic carcinoma were compared. The single-energy CT value of 40-80 keV in thymus carcinoma was higher than that in anterior mediastinal lymphoma(=0.001,=0.037,=0.042,=0.034,=0.002;=0.016,=0.013,=0.018,=0.024,=0.012).The difference in the single-energy CT value of 90-110 keV between anterior mediastinal lymphoma and thymic carcinoma showed no statistical significance(all >0.05).The concentrations of water in the arterial and venous stages of thymic carcinoma were significantly lower than those in the anterior mediastinal lymphoma(=0.030,=0.037),whereas the iodine concentrations were significantly higher(=0.026,=0.000). Anterior mediastinal lymphoma and thymic carcinoma have remarkably different 40-80 keV single energy CT value and iodine concentration in arterial and venous phases,which may be helpful for the differential diagnosis of these two malignancies.

9.Differences of Energy Spectrum CT Findings between Small Cell Lung Cancer with Mediastinal Lymph Node Metastasis and Mediastinal Sarcoidosis.

Objective To compare the differences of energy spectrum CT between small cell lung cancer(SCLC)with mediastinal lymph node metastasis and mediastinal sarcoidosis.Methods Twenty-five SCLC patients with mediastinal lymph node metastasis(SCLC group)and 26 patients with mediastinal sarcoidosis(sarcoidosis group)confirmed by bronchoscopy and biopsy in Tangshan People's Hospital from January 2018 to June 2019 were selected as the research objects.The CT value,iodine concentration,water concentration and energy spectrum curve slope under different single energy levels were compared between SCLC group and sarcoidosis group.Results The single-energy CT values of 40-80 keV segments in the arterial phase of the SCLC group were significantly higher than those in the sarcoidosis group(all P <0.05).The single-energy CT values of 90-140 keV segments were not significantly different from those in the sarcoidosis group(all P >0.05).The single-energy CT values of 40-90 keV segments in venous phase of the SCLC group were significantly higher than those of the sarcoidosis group(all P <0.05),and the single-energy CT values of 100-140 keV segments were not significantly different from those of the sarcoidosis group(all P >0.05).The concentrations of iodine in the arterial phase and venous phase of the SCLC group were(11.56±4.06)μg/cm
