1. The treatment of chronic eczema in mice by matrine hydrogels

Objective: To prepare and characterize the matrine-loaded poloxamer hydrogels and explore their therapeutic effect on chronic eczema in mice. Methods: The matrine hydrogels were prepared with poloxamer 407 and 188 as matrix. The viscoelasticity of the matrine hydrogels was characterized by rheometer. The ear swelling and weight difference were tested, and the levels of tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)were detected by ELISA method. The histopathological change of mouse ear tissues was examined via HE-staining, and the immunohistochemical examination was performed to detect the protease-activated re-ceptor-2(PAR-2)expression in mouse ear. These assays were performed to investigate the therapeutic effect of matrine hydrogels on chronic eczema in mice. Results: The matrine hydrogels had good fluidity and spreadability, and rapidly formed a transparent, viscous uniform hydrogel at the skin temperature. Compared with the normal group, the ear weight difference and swelling degree were significantly increased(P<0.05)at the 3, 5 and 10 days after the establishment of the chronic eczema model. Compared with the model group, the ear weight difference and swelling degree were reduced significantly in the martrine hydrogel group(P<0.01), and the serum TNF-α and IL-6 level(P<0.01)and the PAR-2 expression in the chronic eczema tissue(P<0.01, P<0.05)were both significantly decreased in the martrine hydrogel group. Conclusion: Martrine hydrogels could inhibit the inflammatory infiltration of dermal layer and the epidermal layer damage caused by chronic eczema, which are expected to be a good topical gel preparation for treating chronic eczema of skin.
2. Preparation and characterization of armodafinil nanocrystals

Objective: To prepare and characterize armodafinil nanocrystals, and investigate its release in vitro. Methods: Anti-solvent precipitation technology was used to prepare the armodafinil nanocrystals. The formulation and preparation process of the armodafinil nanosuspension were optimized by the orthogonal design experiment with the average particle diameter as the evaluation index. The suspension was prepared into nanocrystals by freeze-drying technology. The particle size and the polydispersity coefficient of the armodafinil nanocrystals were measured. The X-ray powder diffraction method was used to investigate the crystal form transition of the armodafinil. The dissolution behavior of the armodafinil nanocrystals and raw substances was investigated and compared with each other by the slurry method. Results: The prepared amodafinil nanocrystals had a quite uniformly distributed particle size around 100 nm. The nanocrystals appeared to be in irregular form. After amodafinil was made into nanocrystals, the solubility and dissolution were significantly improved. The crystal form of amodafinil significantly changed after nanocrystallization. Conclusion: The preparation of a small and uniformly distributed particle size of amodafinil nanocrystals could significantly improve the dissolution performance of amodafinil, which is beneficial to improving the bioavailability of insoluble drugs.
3.The role of arecoline on hepatic insulin resistance in type 2 diabetes rats.

OBJECTIVETo explore the effects of arecoline on hepatic insulin resistance in type 2 diabetes rats and to elucidate its possible mechanism.
METHODSForty five Wistar rats were fed with high fructose diet for 12 weeks to induce type 2 diabetic rat model. rats were randomly divided into 5 groups (n = 8): control group, model group and model group were treated with different dose (0, 0.5, 1, 5 mg/kg) of arecoline. After 4 weeks, the fasting blood glucose, blood lipid and insulin level measured , mRNA expression of liver constitutive androstane receptor (CAR), pregnane X receptor (PXR), glucose-6-phosphatase (G6Pase), phosphoenolpyruvate carboxykinase (PEPCK), interleukin-6 (IL-6) and tumor necrosis factor-alpha (TNF-alpha) were detected by reverse transcription polymerase chain reaction (RT-PCR), the protein expression of p-AKT and glucose transporter4 (GLUT4) were detected by Western blot.
RESULTS1.5 mg/kg arecoline could significantly decrease the level of fasting blood glucose, blood lipid, blood insulin level and liver G6Pase, PEPCK, IL-6, TNF-alpha mRNA level in type 2 diabetes rats. 1.5 mg/kg arecoline also could significantly increase CAR, PXR mRNA level and p-AKT and GLUT4 protein expression.
CONCLUSIONArecoline improved hepatic insulin resistance in type 2 diabetes rats by increasing the mRNA levels of CAR and PXR leading to the creased glucose metabolism and inflammation related genes expression.

Through the examples of explanatory and pragmatic trials on acupuncture for migraine and the comparison between research objectives, experiment environment and trial designs, the design of current clinical trial should focus on: (1) Making a point of continuum research on efficacy-effectiveness at trial design type; (2) Thinking highly of therapeutic effect criteria that could have a better show of trial purpose; (3) More use of qualitative research embedded in the randomized controlled trials. With these improvements, the effects of acupuncture could be evaluated more scientifically, objectively and comprehensively.

6.CuZn-SOD and MDA in the serum and EPS of chronic prostatitis patients: quantitative determination and analysis.

OBJECTIVEChronic prostatitis (CP) is a common disease in adult males. Oxidative stress injury has been found to play a significant role in the pathogenesis of CP in recent studies. This study aimed to determine the contents of CuZn-super oxide dismutase (CuZn-SOD) and malondialdehyde (MDA) in the serum and EPS in CP patients and healthy men, and investigate their significance in the diagnosis and treatment of the CP.
METHODSA total of 120 out-patients with confirmed CP were equally divided into a type II, a type IIIA and a type IIIB group, and another 40 healthy males were included as controls. We determined the contents of CuZn-SOD and MDA in the serum and EPS of each group and compared their differences.
RESULTSNo significant differences were found in the serum CuZn-SOD content among the four groups (P > 0.05). The MDA contents were markedly higher in the CP groups than in the control (P < 0.01), but with no significant differences among the three CP groups (P > 0.05). The CuZn-SOD contents in EPS were remarkably lower in the type II and type III A than in the type III B and control groups (P < 0.01), but with no significant differences between the type II and type III A as well as between the type III B and control groups (P > 0.05). The MDA contents in EPS were markedly higher in the type II and type III A than in the type III B and control groups (P < 0.01), but with no significant differences between the type II and type III A as well as the type III B and control groups (P > 0.05).
CONCLUSIONOxidative stress is stronger in type II and type III A CP patients than in healthy men, but has no significant difference between type III B patients and non-CP males. Determining the contents of CuZn-SOD and MDA in the serum and EPS could be very valuable for the diagnosis and assessment of chronic prostatitis.

7.Studies on furmaric acid and isofraxidin content in Sarcandra glabra of different provenances.

OBJECTIVETo study the content variation of furmarid acid and isofraxidin in Sarcandra glabra from 21 different provenances and provide the basis for resource utilization and quality optimization of S. glabra.
METHODHPLC method was developed to determine the contents of furmarid acid and isofraxidin in 330 samples of S. glabra which were collected respectively from 21 different provenances.
RESULTThere were significant differences in the contents of isofraxidin and furmarid acid in S. glabra from different provenances. The contents of isofraxidin and furmarid acid dropped off from low altitude to high altitude, which were also close with longitude and latitude. The content of isofraxidin in S. glabra at central area of natural distribution was the highest. The different parts of the plant had different results, the influence on the contents of the acitive components in stem were more obvious than the leaf.
CONCLUSIONThis simple, accurate and reproducible method could be use to determine the contents of furmarid acid and isofraxidin in S. glabra. The results represented the status of medicines quality and difference of Chinese S. glabra. These agreed with the traditional views that the medicines quality of Sarcandra glabra in Jiangxi, Fujian, Zhejiang was better. These provenances were considered as important areas of medicines breeding and bases building on S. glabra in future.

8.Comparison of 3 microsurgical approaches to the treatment of varicocele: report of 120 cases.

OBJECTIVETo compare the outcomes of the three microsurgical strategies, inguinal high ligation (IHL), retroperitoneal high ligation (RHL) and low ligation (LL) of internal spermatic veins, in the treatment of varicocele.
METHODSWe retrospectively analyzed 120 cases of varicocele, which were equally divided into groups I , II and III to be treated by IHL, RHL and LL of internal spermatic veins, respectively. We compared the operation times, post-operative complications, recurrence rates and results of pre- and post-operation semen analysis among the three groups.
RESULTSThe mean operation time was significantly longer in group III ( [55 +/- 6 ] min) than in I ([35 +/- 10] min) and II ([42 +/- 12] min) (P<0.05), while the rate of post-operative complications remarkably higher in group I (4 cases of hydrocele [10% ] and 3 cases of epididymitis [7.5%]) than in II (2 cases of hydrocele [5%] and 2 cases of epididymitis [5%]) and III (1 case of hydrocele [2.5%] and 1 case of epididymitis [2.5%]) (P<0.05). Six to 12 (mean 9) months of follow-up visit found 2 cases (5% ) of recurrence in group I, 1 case (2.5%) in group II and none in group III, with no statistically significant difference among the three groups (P>0.05). At 12 months after surgery, group III showed significantly higher sperm concentration, grade a +b sperm and the sperm motility than the other two (P<0.05), but no significant differences were observed in these parameters among the three groups preoperatively.
CONCLUSIONAs a microsurgical approach to the treatment of varicocele, LL is better than IHL and RHL of internal spermatic veins in improving the seminal parameters of the patients.

9.The assessment of vulnerability to floods in Guangdong province at district level.

OBJECTIVETo evaluate the vulnerability to floods in Guangdong province at district level.
METHODSData were collected from the sixth census, the 2010 Statistical Yearbook of Guangdong, the 2010 Health Statistics Yearbook of Guangdong and China Disease Prevention and Control information systems, etc. The weight of each indicator was determined based on subjective method and objective method respectively; and finally the results of the two methods were compared.
RESULTS13 indicators were selected for the assessment of vulnerability to floods, including 6 sensitivity indicators, 5 adaptability indicators and 2 exposure indicators. Indicators with large weight (subjective weight/objective weight) were the proportion of population older than 65 years old (0.31/0.30), the proportion of population older than 65 years old (0.16/0.23), infant mortality rate (0.18/0.20), the total Gross Domestic Product (GDP) per capita (0.33/0.21), the proportion of illiterate in the population older than 15 years old (0.19/0.28), history frequency of floods (0.75/0.75). The mean vulnerability index (VI) calculated by subjective method was 0.35 with the standard deviation of 0.10; the mean vulnerability index calculated by objective method was 0.31 with the standard deviation of 0.08. The two weighting methods showed consistent results of vulnerability index (ICC = 0.975, P < 0.01). VI of most districts dropped in the interval of 0.30 - 0.39. Districts with subjective VI > 0.50 or objective VI > 0.40 should pay more attention to floods, including parts of the coastal areas, Beijiang River Basin, the eastern tributary area of Dongjiang River and the northern part of Pearl River Delta. Dapu district of Meizhou (0.55/0.45), Dianbai district and Maogang district of Maoming (0.54/0.48) were most vulnerable. Districts of Heyuan, Dongguan, Zhaoqing and Huizhou were less vulnerable, Yuancheng district of Heyuan showed least vulnerable to floods (0.15/0.12) followed by Dongguan (0.18/0.16), Duanzhou district (0.18/0.16) and Guangning (0.17/0.15) district of Zhaoqing. The score of indicators differed among different level of vulnerability (P < 0.05).
CONCLUSIONDifferent regions of Guangdong province showed different vulnerability to floods, vulnerable areas should be priority in the prevention and control of floods.
