2.Species Identification Based on Morphological Variability of Femur.

OBJECTIVE:
To study the morphological characteristics of femurs of adult human and 11 kinds of adult animals from cattle, horses, pigs, goats, sheep, dogs, cats, rabbits, geese, ducks, chickens, and to establish an effective species identification method among various species.
METHODS:
The 4 cm mid-diaphyseal segment of the femur from adult human (older than 20 years old) at autopsy was obtained. Addi-tionally, the 4 cm ones from 11 kinds of adult animals were obtained. After decalcification, all femurs were made into slices, and then were observed by optical microscope. The 25 indexes were selected and analyzed by step discriminant analysis according to differences between human and mammal, human and poultry, and human and 11 kinds of animals.
RESULTS:
The histological structure of bone mineral density of middle part of femur had obvious characteristics among the species. And the morphology and number of osteon showed the trend of obvious biological evolution. There were 11 indexes with significant differences between human and 11 kinds of animals to establish some mathematical models to discriminate all species. The correct discrimination rate was 96.3% between human and mammal. The correct discrimination rate was up to 100% between human and poultry, and was 89.4% among human, mammal and poultry.
CONCLUSION
The mathematical models have good correct discrimination rate among human and the other animals, which could be applied in the practical species identification cases.

3.Cervical cancer mortality in 2004 - 2005 and changes during last 30 years in China.

OBJECTIVETo analyze the cervical cancer mortality and distribution profiles by areas and population in 2004 - 2005 and the changes in the last 30 years.
METHODSThe demographic information and cervical cancer death data came from the Third National Retrospective Sampling Survey of Death Causes which including 158 sampling areas (including 61 cities and 97 villages; 52 in the east region, 49 in the middle region and 57 in the west region) covered 31 province-level regions in the interior of China by multiple stage stratified clustering sampling. The total number of the samples were 142 660 482 person years (72 970 241 person years in male and 69 690 241 person years in female). The crude and age-specific death rate was calculated. The standardized death rate was calculated by using 5 years interval from the census data of 2000. In addition, the mortality data was compared with the two previous national death surveys in 1970's and 1990's, respectively.
RESULTSIn the sampling region of the third retrospective death causes survey from 2004 to 2005, the crude death rate of cervical cancer was 2.86/100 000 (1995/69 690 241), which accounting for 2.86% (1995/69 667) and ranking 9th of all cancer death causes in female population. The crude death rates of cervical cancer were 2.88/100 000 (1326/46 091 419) and 2.83/100 000 (669/23 598 822) for rural areas and urban areas, respectively, the Chinese population adjusted death rates of rural and urban areas were 2.01/100 000 and 1.67/100 000, respectively. The standardized death rates of cervical cancer in 1973 - 1975 and 1990 - 1992 were 11.10/100 000 and 3.25/100 000, respectively, which accounting for 17.91% and 4.86%, respectively; in 2004 - 2005, the crude death rates increased by age, rose remarkably from the age of 40 and arrived the peak of 20.83/100 000 (82/393 624) at the age of 85. The mortality among 35 - 44 year-old group was higher than those in 1990's. The crude death rates of the middle, west and east areas were 3.41/100 000 (827/24 225 738), 3.25/100 000 (636/19 563 647) and 2.05/100 000 (532/25 900 856), respectively, the Chinese population adjusted death rates of the middle and west areas were about twice that of the east (2.35/100 000 in the middle, 2.38/100 000 in the west, and 1.19/100 000 in the east).
CONCLUSIONThe mortality of cervical cancer was higher in rural areas than that in urban, as well as higher in the middle and west areas than that in east area in the interior of China in 2004 - 2005. Younger trend of cervical cancer death was observed; the mortality of cervical cancer and constituent ratio in the female tumor death continued to decline compared to those in 1973 - 1975 and 1990 - 1992.

4.Occult hepatitis B virus infection in chronic viral hepatitis patients with non-A to E hepatitis virus infection

Objective To observe the status of occuh hepatitis B virus infection in chronic viral hepatitis patients with non-A to E hepatitis virus infection and explore the diagnostic value of fluorescence quantitative polymerase chain reaction (FQ-PCR) technique for occult hepatitis B virus infection. Methods The amount of HBV-DNA in serum and liver tissue from 57 patients with non-A to E hepatitis virus infection who were diagnosed as chronic viral hepatitis by Menghini method liver biopsy were detected by using FQ-PCR technique,then the relation between the viral load of HBV DNA in liver tissue and hepatic inflammatory activity were analyzed.Results Thirteen (22.81% ),22 (38.60%) patients were positive for HBV DNA in serum and liver tissue,respectively.The positive rate and the level of HBV DNA quantity in liver tissue were significantly higher than those in serum;HBV DNA was found positive in both serum and liver tissue in 13 cases,negative in both serum and liver tissue in 35,positive in liver tissue but negative in serum in 9,and in none of the cases HBV DNA was positive in serum but negative in liver tissue ( P<0.01 ).The logarithmic value of HBV DNA from 13 patients in liver tissue and in serum was respectively:(6.62±1.21 ) copies/g vs.(4.03±1.06) copies/ml,P < 0.01.The hepatic lesions of all HBV DNA positive patients were active pathologic changes,but the level of HBV DNA in liver tissue was not significantly correlated with the grade of hepatic inflammation activity ( P0.05 ).Conclusion Occult HBV infection is the etiology of part of the chronic viral hepatitis patients with non-A-E hepatitis virus infection.Missed diagnosis will occur if diagnosis of hepatitis B is only hased on detection of serum HBV markers. It is useful for improvement of the diagnostic level of HBV infection via detection of HBV DNA quantitatively in serum especially in liver tissue of chronic viral hepatitis with non-A-E hepatitis virus infection by using FQ-PCR technique The chronic viral hepatitis patients occult HBV infection should be also given effective anti-viral therapy.
5.Initial establishment and reliability and validity testing for nurses occupational well-being questionnaire

Objective To initially establish an occupational well-being questionnaire for Chinese clinical nursing staffs.Methods The draft questionnaire,including open-ended questions and semi-structured interview,was established according to literature review and revised after expert′s consultation and pilot survey. Then,the questionnaire was tested by 460 nurses who randomly selected from a Class Ⅲ Grade Ⅰ hospital in Wuhan province from September 201 4 to June 201 5.The reliability and validity of the questionnaire were examined by exploratory factor principal component analysis,confirmatory factor analysis of maximum likelihood estimation method,and the Cronbach′s αanalysis.Results The questionnaire contained 1 9 items belonging to 5 dimensions that included welfare,value,interpersonal relationship,leaders and characteristics.The Cronbach′sαcoefficient for the questionnaire and each dimensions were 0.91 4 and 0.753-0.862.The correlation coefficients between each items and the total score as well as its related dimension were 0.487-0.729 and 0.709-0.883.The accumulative contribution rate of the five factors was 71 .1 05%.Conclusions The nurses′occupational well-being questionnaire is a reliable and valid instrument to assess the occupational well-being for clinical nursing staffs.
6.Endoscopic Submucosal Dissection versus Surgical Treatment for Early Esophagogastric Junction Adenocarcinoma:Propensity Score Matching Analysis

Objective To compare the clinical efficacy and prognosis of endoscopic submucosal dissection(ESD)and surgical methods in the treatment of early esophagogastric junction adenocarcinoma(AEG),and to analyze factors influencing prognosis.Methods Hospitalized patients with early AEG who underwent ESD or surgical treatment at Anhui Provincial Hospital from January 2010 to December 2022 were collected.Among them,186 patients underwent ESD and 364 patients underwent surgical treatment.Propensity score matching was used with a ratio of 1∶1,with 164 patients in each group.Clinical outcomes,survival outcomes,and postoperative complications were compared before and after matching.Factors influencing mortality and recurrence in EGJ patients were analyzed.Results 1.Before and after matching,the ESD group had lower surgical time,hospital stay,hospital costs,intraoperative bleeding volume,and adverse events compared to the surgical group(P<0.001).2.The matched ESD group had 1-,3-,and 5-year overall survival rates of 99.5%,94.5%,and 90.2%,respectively,while the surgical group had rates of 100%,99.4%,and 97.5%for the same periods.The 1-,3-,and 5-year recurrence-free survival rates in the matched ESD group were 99.5%,96.3%,and 93.4%,respectively,compared to 100%,98.6%,and 92.5%in the surgical group.Kaplan-Meier survival analysis before and after matching showed no significant difference in overall survival and recurrence-free survival between the two groups(P>0.05).3.Age,poor differentiation,and vascular invasion were independent risk factors for OS;age and tumor size were independent risk factors for RFS.Conclusion Patients with early AEG undergoing ESD or surgical treatment have consistent clinical outcomes.ESD can be considered an effective and safe treatment for early AEG.
7.Construction of COL1A1 short hairpin RNA vector and its effect on cell proliferation and migration of gastric cancer cells.

OBJECTIVETo construct COL1A1-targeted short hairpin RNA (shRNA) vector with pSilencer 4.1-CMV neo siRNA expression vector and to evaluate its effect on proliferation and migration of gastric cancer BGC-823 cells in vitro.
METHODSThree COL1A1-shRNA plasmids (COL1A1-shRNA-1, COL1A1-shRNA-2, COL1A1-shRNA-3), targeting different sites of COL1A1 gene, were constructed using pSilencer 4.1-CMV neo siRNA expression vector and transfected into gastric cancer BGC-823 cells. Real time quantitative RT-PCR and Western blot were performed to detect expression levels of COL1A1. MTT and Transwell migration assays were employed to evaluate the effects of COL1A1 gene silence on cell proliferation and migration.
RESULTThree recombinant plasmids targeting COL1A1 were constructed successfully. The expressions of COL1A1 in BGC-823 cells, including mRNA and protein levels, were significantly inhibited by the COL1A1-shRNA transfectants, which resulted in a clear reduction of cell proliferation and migration capacity.
CONCLUSIONThe COL1A1-shRNA can effectively knock down gene expression and inhibit proliferation and migration of gastric cancer BGC-823 cells.

8.Multifactor dimensionality reduction analysis of the correlation of Chinese medicine syndrome evolvement and cardiovascular events in patients with stable coronary heart disease.

OBJECTIVETo analyze the correlation of Chinese medicine syndrome evolvement and cardiovascular: events in patients with stable coronary heart disease (CHD).
METHODSThis prospective cohort study investigated and: collected the clinical information of patients with stable CHD and observed the syndrome type at the baseline and 6-month at follow-up, as well as the cardiovascular events during the 6-month and 12-month follow-up. The patients were divided into the event group and the non-event group. The interaction and the impact of syndrome evolvement on cardiovascular events were examined through multifactor dimensionality reduction (MDR) analysis and the results were verified by Chi-square test.
RESULTSTotally 1,333 of 1,503 stable CHD patients enrolled met the inclusion criteria: of MDR analysis. Among them, 959 (71.9%) cases were males and 374 (28.1%) cases were females. Thirty seven cases had cardiovascular events during 6 to 12 months after the study began. The results of the MDR analysis and verification using Chi-square test showed that the development of cardiovascular events was positively correlated with interaction between blood stasis and toxic syndrome at the baseline, blood stasis at the baseline and qi deficiency at the 6-month follow-up, toxic syndrome at the baseline and qi deficiency at the 6-month follow-up, toxic syndrome at the base line and blood stasis at the 6-month follow-up, qi deficiency and blood stasis at the 6-month follow-up (P<0.05 for all).
CONCLUSIONSBlood stasis, toxic syndrome and qi deficiency are important factors of stable CHD. There: are positive correlation between cardiovascular events and syndrome evolution from blood stasis to qi deficiency, from toxic syndrome to qi deficiency and from toxic syndrome to blood stasis, indicating the pathogenesis of toxin consuming qi, toxin leading to blood-stasis in stable CHD patients prone to recurrent cardiovascular events.

9.MRI findings of uterine cervical cancer and value of MRI in preoperative staging.

OBJECTIVETo evaluate the value of magnetic resonance imaging (MRI) in diagnosis and preoperative staging of uterine cervical cancer.
METHODSMRI findings and staging in 72 patients with cervical carcinoma were retrospectively analyzed, and the size, location, signal intensity and invasion of the tumor were observed. MRI sequence included SE T1WI, (TSE)T2WI, T2WI/SPIR and contrast-enhanced T1WI.
RESULTSMRI identified uterus cervical cancer in all cases with the exception of only 1 case of IA stage. The tumor was represented by hypointensity and isointensity on T1WI, heterogeneous and homogeneous hyperintensity on T2WI, mildly heterogeneous enhancement after bolus intravenous GD-DTPA injection. MRI had an accuracy of 86% in localization of the tumor, but its accuracy in clinical staging was only 64% (chi2=6.453, P<0.05). The tumor volume measured by MRI was similar with that by pathological measurement (1.94-/+1.15 vs 1.94-/+1.11, P>0.05).
CONCLUSIONMRI can accurately describe the size and invasion of uterine cervical cancer, especially useful in detecting parametrial invasion, but for diagnosis of IA uterine cervical cancer, MRI findings are not sufficient without considerations of clinical findings and cellular examination.

10.Value of anti-neutrophil cytoplasmic antibody in assessing the severity of bronchiolitis obliterans in children.

OBJECTIVE:
To study the value of anti-neutrophil cytoplasmic antibody (ANCA) in assessing the severity of bronchiolitis obliterans (BO) in children.
METHODS:
A prospective analysis was performed on 59 children who were diagnosed with BO from June 2009 to October 2014. ELISA was used to measure the concentrations of myeloperoxidase (MPO)-ANCA and proteinase 3 (PR3)-ANCA in serum. According to the results of ELISA, the children were divided into three groups: double-negative ANCA (n=22), single-positive ANCA (n=17), and double-positive ANCA (n=20). The three groups were compared in terms of the scores of BO risk factors, clinical symptoms, chest high-resolution computed tomography (HRCT), and lung pathology on admission, as well as the changes in the expression level of ANCA and the scores of clinical symptoms and chest HRCT over time.
RESULTS:
Compared with the double-negative ANCA group, the double-positive ANCA group had a significantly higher score of BO risk factors (P<0.05), and the single-positive ANCA group and the double-positive ANCA group had significantly higher scores of clinical symptoms, chest HRCT, and lung pathology (P<0.05). The children were followed up for 6 months after discharge, and there were significant reductions in MPO-ANCA and PR3-ANCA titers from admission and discharge to the end of follow-up (P<0.05), as well as a significant reduction in the score of clinical symptoms from admission to the end of follow-up (P<0.05), while there was no significant change in the score of chest HRCT from admission to the end of follow-up (P>0.05). The single-positive ANCA and double-positive ANCA groups still had a significantly higher score of clinical symptoms than the double-negative ANCA group (P<0.05).
CONCLUSIONS
The expression level of ANCA is correlated with the severity of BO in children and thus has certain clinical significance in disease evaluation.
