1.The role of arecoline on hepatic insulin resistance in type 2 diabetes rats.

OBJECTIVETo explore the effects of arecoline on hepatic insulin resistance in type 2 diabetes rats and to elucidate its possible mechanism.
METHODSForty five Wistar rats were fed with high fructose diet for 12 weeks to induce type 2 diabetic rat model. rats were randomly divided into 5 groups (n = 8): control group, model group and model group were treated with different dose (0, 0.5, 1, 5 mg/kg) of arecoline. After 4 weeks, the fasting blood glucose, blood lipid and insulin level measured , mRNA expression of liver constitutive androstane receptor (CAR), pregnane X receptor (PXR), glucose-6-phosphatase (G6Pase), phosphoenolpyruvate carboxykinase (PEPCK), interleukin-6 (IL-6) and tumor necrosis factor-alpha (TNF-alpha) were detected by reverse transcription polymerase chain reaction (RT-PCR), the protein expression of p-AKT and glucose transporter4 (GLUT4) were detected by Western blot.
RESULTS1.5 mg/kg arecoline could significantly decrease the level of fasting blood glucose, blood lipid, blood insulin level and liver G6Pase, PEPCK, IL-6, TNF-alpha mRNA level in type 2 diabetes rats. 1.5 mg/kg arecoline also could significantly increase CAR, PXR mRNA level and p-AKT and GLUT4 protein expression.
CONCLUSIONArecoline improved hepatic insulin resistance in type 2 diabetes rats by increasing the mRNA levels of CAR and PXR leading to the creased glucose metabolism and inflammation related genes expression.

2.MicroRNA-1246 Inhibits NFATc1 Phosphorylation and Regulates T Helper 17 Cell Activation in the Pathogenesis of Severe Alopecia Areata

Background:
We found microRNA (miR)-1246 to be significantly differentially expressed between severe active alopecia areata (AA) patients and healthy individuals.
Objective:
To explore the role and mechanism of miR-1246 in severe AA.
Methods:
Expression of miR-1246, dual-specific tyrosine phosphorylation-regulated kinase 1A (DYRK1A), and nuclear factor of activated T cells 1c (NFATc1) in peripheral CD4+ T cells and in scalp tissues of patients were detected using RT-qPCR, Western blot, and immunohistochemistry assays. Peripheral CD4+ T cells from the AA patients were transfected with lentiviral vectors overexpressing miR-1246. RT-qPCR and Western blot analysis were used to measure mRNA or protein expression of retinoic-acid-receptor-related orphan nuclear receptor gamma (ROR-γt), interleukin (IL)-17, DYRK1A, NFATc1, and phosphorylated NFATc1. Flow cytometry was used to assay the CD4+ IL-17+ cells proportion. ELISA was used to measure cytokine levels.
Results:
miR-1246 levels decreased and DYRK1A and NFATc1 mRNA levels significantly increased in the peripheral CD4+ T cells and scalp tissues of severe active AA samples.NFATc1 protein expression was also significantly increased in the peripheral CD4+ T cells but not in the scalp tissues. NFATc1 positive cells were mainly distributed among infiltrating inflammatory cells around hair follicles. In peripheral CD4+ T cells of severe active AA, overexpression of miR-1246 resulted in significant downregulation of DYRK1A, NFATc1, ROR-γt, and IL-17 mRNA and phosphorylated NFATc1 protein, as well as a decrease in the CD4+ IL-17+ cells proportion and the IL-17F level.
Conclusion
miR-1246 can inhibit NFAT signaling and Th17 cell activation, which may be beneficial in the severe AA treatment.
3.Rescuing Bombyx mori bidensovirus in BmN cells in vitro.

Bombyx mori bidensovirus (BmBDV) has been identified as causing chronic densonucleosis in Bombyx mori specifically. The replication mechanism of BmBDV remains unknown. Its genome comprises two single stands DNA (VD1 and VD2). In order to rescue infectious virions in vitro, we obtained the total viral DNA extracted from the BmBDV-infected larvae midguts, subsequently cloned the full-length sequence of BmBDV genome fragments by PCR and constructed recombinant plasmids pMD18T-VD1 and pUC-VD2. The linear genome fragments were obtained by digesting recombinant plasmids with corresponding restriction enzymes, and then collectively transfected BmN cells by the method of liposome-embedding. We determined the replication of the virus gene by PCR with the template of demethylated total DNA extracted from the post-transfect BmN cells. Meanwhile, we collected the total proteins from the post-transfect BmN cells and the larvae midgut of feeding the post-transfect BmN cells to perform Western blotting analysis, and detected the expression of viral genes. Here we firstly confirm that infectious virions can be rescued in BmN cells by linear co-transfect method.

4.Study on time limit of ACP and sperm positive-detected in mixed spot.

OBJECTIVE:
The study was carried out on time limit of ACP and sperm positive-detected in mixed spot.
METHODS:
600 vaginal swabs from living body after sexual intercourse were tested by routine and three-blindness methods.
RESULTS:
The longest time limit of ACP which was positively detected is 255 h, average time limit is 52 h, 41.3% of those samples was ACP-detected in positive degree (++). The longest time limit of sperm positively detected is 132 h, average time limit is 29 h. The number of sperms observed in 34.3% of those samples with positive result, is from 6 to 10/HP.
CONCLUSION
This study is helpful to forensic identification of sex cases.

5.The role of low-back soft tissue after vertebroplasty and kyphoplasty for spinal osteoporotic fractures

Objective To investigate the effect of soft tissue on the posterior lumbar spine after percutaneous vertebroplasty (PVP) and percutaneous kyphoplasty (PKP) for spinal osteoporotic fractures. Methods From January 2015 to December 2016, 52 patients with thoracolumbar osteoporotic fracture in our hospital underwent PKP or PVP. According to their fat suppression sequence (FSE) on Magnetic Resonance Imaging (MRI), they were divided into FSE positive and FSE negative groups. The FSE negative group had 38 patients, 9 males and 29 females with a mean age of 75.1 ± 6.0 years; the FSE positive group had 14 pa-tients, 4 males and 10 females with a mean age of 73.9 ± 5.8 years. The 2 groups were compared in terms of visual analogue scale (VAS) for lying on bed and standing at the 1, 3 and 7 days after surgery and painless ambulation at the 3 time points as well. Results There were no significant differences between the 2 groups in age, gender or responsible vertebrae distributions. When the patients were lying on bed, their VAS scores at the 1, 3 and 7 days after surgery were 4.1 ± 1.7, 3.0 ± 0.9 and 1.0 ± 0.8 for the FSE negative group, and 4.8 ± 1.0, 3.0 ± 0.7 and 1.3 ± 0.9 for the FSE positive group; when the patients were standing, their VAS scores at the 1, 3 and 7 days after surgery were 4.2 ± 1.1, 3.2 ± 0.7 and 1.9 ± 0.7 for the FSE negative group, and 5.4 ± 1.1, 4.0 ± 1.0 and 2.3 ± 0.5 for the FSE positive group. The VAS scores for standing at the 1 and 3 days after surgery for the FSE negative group were significantly lower than those for the FSE positive group (P <0.05). There were no significant differences between the 2 groups in the VAS scores for lying on bed at the 1, 3 or 7 days after surgery and for standing at the 7 days after surgery (P > 0.05). At the 1, 3 and 7 days after surgery, there were respectively 22, 37 and 38 patients in the FSE negative group who accomplished painless ambulation and there were respectively 3, 9 and 14 patients in the FSE positive group who accomplished painless ambulation. A significantly larger proportion of the patients in the FSE negative group accomplished painless ambulation than in the FSE positive group (P <0.05). At 7 days after surgery, all the patients in the 2 group accomplished painless ambulation. Conclusions The soft tissue of the lower back may play an important role after PKP and PVP, for fine soft tissue can reduce time for lying on bed and foster surgical efficiency. The VAS scores for standing may be more significant than those for lying on bed in prediction of ambulant time.
6.HPV Genotyping and Clinical Studies of Women in Yan'an Area

Objective To contrast HPV genotyping and clinical characteristics of women in Yan'an and provide data support for cervical cancer screening and prevention.Methods The genotyping of HPV infection was carried out by the method of PCR amplification and reverse hybridization.To statistical analysis clinical infection distribution data.Results Found 16.48% (332 of 2014) were positive for HPV DNA in these women.HPV-16,52 and 53 were in the top three HPV high-risk types.HPV-81,6 and 44 were in the top three HPV low-risk types.There were 219 cases (65.96%) with pure high-risk type infection,74 cases (22.29%) with pure low-risk type infection and 39 cases (11.75%) with mixed infection in positive specimens.The HPV infection rate had showed first increasing then decreasing with age,and there were significant differences among the each groups (x2=34.238,P<0.01).There were also differences in HPV positive detection rates in different occupations and there were significant differences among the each groups (x2=50.35,P<0.01).There were also differences in HPV positive detection rates in different regions,and there were significant differences among the each groups (x2 =12.084,P<0.05).Single infection was highest in HPV infection.Conclusion HPV infection was different in different populations,regions and ages,and it is of great significance to carry out HPV screening for early prevention and treatment of cervical cancer.
7.miRNA expression change of differentiation of mice marrow mesenchymal stem cells into adipocytes.

OBJECTIVETo explore miRNA expression change of differentiation of mice marrow mesenchymal stem cells (MSCs) into adipocytes, which lay the foundation for further studies on molecular mechanism of miRNA regulating the differentiation of MSCs into adipocytes.
METHODSC57BL/6 mice MSCs were isolated, cultured through the whole bone marrow method, amplified by the differential adherent method. Cell growth was observed by morphology and the expression of superficial antigen CD29, CD44, CD34 were detected through immunohistochemistry. MSCs was induced to differentiation into adipocytes with adipocyte differentiation medium, and adipogenic differentiation of MSCs was analyzed by oil Red O staining. MicroRNA microarray was used to investigate the differentially expressed miRNAs in MSCs and adipocytes.
RESULTS(1) The fifth passage of MSCs had high purity under an inverted m icroscope. Immunohistochemistry staining showed that CD29, CD44 were positive and CD34 was negative in more than 90% MSCs. There were a large number of lipid droplets in cytoplasm after MSCs were induced with adipocyte differentiation medium, Oil O staining was positive. (2) The microarray experiment showed that 75 differentially expressed miRNAs were obtained in adipocytes compared with MSCs, 20 up-regulated and 55 down-regulated miRNAs were observed among them.
CONCLUSIONThere was a expression change of miRNA of differentiation of MSCs into adipocytes, some miRNAs might play important roles in MSCs adipogenic differentiation.

8.Analysis of application effect of orthopedic medical device intelligent acceptance equipment combined with UDI code in the management of orthopedic medical devices

Objective:To analyze the application effect of intelligent acceptance equipment for orthopedic medical equipment combined with medical device unique device identifier(UDI)code in the management of orthopedic medical devices.Methods:Based on the UDI code,the UDI application process of orthopedic medical devices was formulated,and all operations such as inspection and accurate billing of orthopedic medical devices were completed through the identification and comparison of intelligent acceptance equipment for orthopedic medical devices.A total of 8 317 orthopedic instruments used in orthopedic surgery in Wangjing Hospital of China Academy of Chinese Medical Sciences from September to December 2023 were selected.According to the different time of the implementation of intelligent acceptance equipment for orthopedic medical devices,the 4 072 orthopedic devices used from September to October 2023 was marked as before implementation and adopted the traditional management mode,the 4 245 orthopedic devices used from November 2023 to December 2023 were marked as before implementation and adopted the intelligent acceptance management mode.The management effect and incidence of adverse events were compared before and after the implementation of intelligent acceptance equipment for orthopedic medical devices.Results:The total effective rate of management after the implementation of intelligent acceptance equipment for orthopedic medical instruments was 98.78%,which was significantly higher than that before the implementation,the difference was statistically significant(x2=272.03,P<0.01).The total incidence of adverse events after the implementation of the intelligent acceptance equipment for orthopedic medical devices was 8.83%,which was significantly lower than that before the implementation,the difference was statistically significant(x2=281.38,P<0.01).Conclusion:The intelligent acceptance device combined with UDI code for orthopedic medical devices can achieve efficient distribution,accurate acceptance and billing,reduce the occurrence of adverse events,meet the requirements of the whole process traceability management of one thing and one code,and improve the management level orthopedic medical devices.
9.Genetic susceptibility to ulcerative colitis in the Chinese Han ethnic population: association with TNF polymorphisms.

BACKGROUNDTumor necrosis factor alpha (TNFalpha) is an important proinflammatory cytokine that has been implicated in the pathogenesis of inflammatory bowel disease (IBD). Recent studies have evaluated the role of TNF promoter polymorphisms in IBD, whereas the data are inconsistent. Trans-racial mapping in an ethnically distinct but homogenous population may help clarify these associations. We investigate the association between TNF promoter polymorphisms and susceptibility to ulcerative colitis (UC) in the Chinese Han ethnic population.
METHODSWe studied 110 unrelated UC patients and 292 healthy controls from Zhejiang Province, China. Genotyping for 6 common TNF promoter polymorphisms (TNF -1031T/C, -863C/A, -857C/T, -380G/A, -308G/A, -238G/A) was carried out by polymerase chain reaction sequence-specific primers (PCR-SSP).
RESULTSTNF -308A was associated with disease (allele frequency patients 14.6% vs controls 8.9%, P = 0.02). TNF -857T was increased in patients but without statistical significance (allele frequency 17.3% vs 12.2%, P = 0.06). Haplotype analysis revealed 6 haplotypes including two (H5 and H3), which contained TNF -308A. H5 was associated with disease (haplotype frequency patients -12.3% vs controls 7.5%, P = 0.03). Of note the rare haplotype H3 has not previously been identified in Caucasian populations. Homozygosity for the haplotype H4 comprising the common alleles at each TNF promoter single-nucleotide polymorphism (SNP) was negatively associated with disease (patients vs controls 24.5% vs 34.9%, P < 0.05).
CONCLUSIONSWe report the association with TNF -308A polymorphisms in Chinese patients with ulcerative colitis. The functional study in Chinese Han ethnic population is now required.

10.An analysis of randomized controlled trials published in the Chinese Journal of Gastrointestinal Surgery between 2003-2009.

OBJECTIVETo study the academic level of randomized controlled trials (RCT) published in the Chinese Journal of Gastrointestinal Surgery between 2003 and 2009.
METHODSPublished RCTs in the 42 issues of the Chinese Journal of Gastrointestinal Surgery was searched for relevant articles published between 2003 and 2009. Data extracted for analysis included the time at manuscript received, publication time, total number of citations, number of citations in Chinese, number of citations in English, author's affiliations, single- or multi- center study, positive conclusions from RCTs, number of patients recruited in RCTs, research funding source, the start time, the finish time and the number of authors in RCTs.
RESULTSDuring the past seven years, a total of 80 clinical RCTs were published in the Chinese Journal of Gastrointestinal Surgery, accounting for 12% of all the clinical studies published in the journal, and the average number of RCTs in each issue was 1.6. The average delay time before publication was 208 days. The total number of citations and the total number of patients in RCTs were 685 and 9402. The average number of citations, the average number of patients recruited in each RCT, and the average research period in RCTs were 8.6, 118 and 29.2. There were 7 multi-center studies, and the number of single-center study was 73. All the RCT studies had significant conclusions, and 17 (21.3%) RCT studies were funded. Nanjing general hospital of Nanjing military command had the largest number of RCTs (n=6).
CONCLUSIONThe Chinese Journal of Gastrointestinal Surgery puts emphasis on clinical studies of high evidence level such as RCT, which provide evidence for making the clinical guidelines in the specialty of gastrointestinal surgery.
