2.A survey of Chinese public knowledge about Traditional Chinese Medicine

Objective To understand Chinese public knowledge of Traditional Chinese Medicine ( TCM) and offer references for general education approaches. Methods Based on government guideline and pre-vious research, a questionnaire was designed and distributed randomly among Chinese residents in the country. Among the 1,800 questionnaires distributed, 1,486 was returned with 1,050 valid ones identified. The questionnaire consisted of seven demographic items including gender and ages;it also included seven-teen items of general TCM knowledge such as the nature of TCM discipline, advantages of TCM, and com-mon sense of health care in TCM. SPSS 20. 0 software was used to analyze the data obtained. Mean and standard deviation ( SD) was calculated for normal distribution and the homogeneity of variance test. One-
way ANOVA analysis was used to compare differences between groups. LSD test was conducted for homoge-neous variance and Tamhance’ T2 test for heterogeneous results. Results Chinese public knowledge about TCM was overall limited. Average score (x)of all 17 questions was 2. 07 (SD=0. 53). The best known item was precautions for decoction administration(x=2. 47)while the worst known one was about theory of five phases(x=1. 78). Comparison among groups indicated that students knew the most about TCM when compared with peasants(P <0. 001,Δaverage =6. 125); Difference was the most significant be-tween groups of different incomes (5 000-999 RMB/month vs. 1 000-4 999 RMB/month, P<0. 001,Δaverage=2. 870). Health status over the past year influenced the public’s knowledge as there was a significant difference between“I have been feeling better” and“I have been feeling worse” groups ( P<0. 001,Δaverage=7. 058) Conclusions Chinese public knowledge about TCM was generally limited. General education is in urgent need to provide systematic training to help the public know more about TCM philosophy and basic theories. Educators need to work from the grass to promote public’ s health care and benefit social awareness. The quantitative assessment of TCM general education also merits im-provement. This study proves to be a positive innovation.
3.Potential clinical value of detecting markers of bone metabolism in patients with ankylosing spondylitis

Objective:To investigate the clinical significance of bone metabolism indexes and human leukocyte antigen B27 (HLA-B27)in ankylosing spondylitis(AS)and analyze their diagnostic value. Methods:The subjects were 94 cases of AS patients,239 cases with other diseases and 80 healthy controls, and the results were retrospectively surveyed. Results:In the AS group, the concentration of the β-collagen specific sequence (β-CTX ) was higher ( P<0. 05 ) while the concentrations of osteocalcin ( OC ) , 25-hydroxyvitamin D[25-(OH)D] and parathyroid hormone(PTH)were lower than those in the control group. In the AS group,there was a positive correlation between the concentration of C-reactive protein(CRP)and β-CTX(P<0. 01),and the concentration of CRP was negatively correlated with the concentration of 25-(OH)D(P<0. 05). However,the concentrations of other bone metabolic indexes had no correlation with the concentration of CRP(P<0. 05). The positive rate of HLA-B27 in the AS group was significantly higher than that in other groups(P<0. 05),and HLA-B27 had highly sensitive and specific in the diagnosis of AS. In the AS group,the sensitivity of the concentration of 25-( OH) D was higher than β-CTX,while its specificity was lower than β-CTX. Conclusion:Bone metabolic indexes have great value in early screening and clinical diagnosis of AS,especially β-CTX and 25-( OH) D were more obvious.
4.Real-time quantitative PCR detecting minimal residual disease in multiple myeloma patients after autologous peripheral blood stem cell transplantation.

In order to explore the role of real-time PCR in detecting minimal residual disease in multiple myeloma and Waldenstrom's macroglobulinemia after autologous peripheral blood stem cell transplantation (APBSCT), real-time PCR was used to quantitate the IgH rearrangement in 8 patients with multiple myeloma (MM) and 1 case of Waldenstrom's macroglobulinemia before and after APBSCT. The results showed that the copies of IgH rearrangement pre- or post-APBSCT were 3108 +/- 1043 and 549 +/- 660 (P < 0.05) respectively. The number of IgH copies was positively correlated with the amount of plasmocytes in patient 's bone marrow and the M-protein in peripheral blood (r = 0.86, P < 0.05). Similar result was obtained in a case of relapsed Waldenstrom's macroglobulinemia. In conclusion, the quantitative analysis of IgH rearrangement by real-time PCR is a novel way to evaluate the therapeutic efficaciousness and predict the prognoses in MM patients.

5.Research progress of traditional Chinese medicine in treatment of vascular cognitive impairment

With aging of Chinese population,prevalence of vascular cognitive impairment(VCI)is increasing year by year.At present,drugs for VCI treatment at home and abroad still lack large samples of evidence-based medicine evidence.Compared with western medicine,Chinese medicine has characteristics of multi-component and multi-target.The present article makes a review on research progress of traditional Chinese medicine in treatment of VCI,aiming at providing new thinking for VCI treatment.
6.Clinical trial of cipofol combined with afentanil in the treatment of patients undergoing bronchoscopy

Objective To compare the clinical efficacy and safety of cyclopofol injection and propofol injection combined with afentanil injection in patients undergoing tracheoscopy under laryngeal mask ventilation under general anesthesia.Methods The patients to undergo tracheoscopy were randomly divided into treatment group and control group.Induction of anesthesia:treatment group received 20 μg·kg-1 afentanil,0.4 mg·kg-1 ciprofol and 0.2 mg·kg-1 cisatracurium;control group received 20 μg·kg-1afentanil,2 mg·kg-1 propofol and 0.2 mg·kg-1 cisatracurium.Two groups were given laryngeal mask ventilation for general anesthesia.The treatment group received 0.8 mg·kg-1·h-1 cypofol and 0.5-1.0 μg·kg-1·min-1 afentanil to perform the anesthesia maintenance;the control group was received 8 mg·kg-1·h-1propofol and 0.5-1.0 μg·kg-1·min-1 afentanil to perform the anesthesia maintenance.The vital signs,induction and recovery time,dosage of afentanil during anesthesia and safety were compared between the two groups.Results Treatment group were enrolled 70 cases,10 cases dropped out,and 60 cases were finally included in the statistical analysis.Control group were enrolled 70 cases,10 cases dropped out,and ultimately 60 cases were finally included in the statistical analysis.Three minutes after induction of anesthesia(T1),the mean arterial pressure(MAP)of treatment group and control group were(79.32±5.73)and(73.15±6.20)mmHg,the heart rate(HR)were(70.53±8.20)and(65.77±7.75)beat·min-1,respectively.At insert the bronchoscope(T2),MAP of treatment group and control group were(82.52±5.81)and(75.99±6.09)mmHg,HR were(70.27±7.94)and(65.42±7.73)beat·min-1,respectively.The MAP and HR of treatment group at T1 and T2 were significantly higher than those of control group at the same time,the differences were statistically significant(all P<0.05).The induction time of treatment group and control group was(76.23±6.51)and(66.93±6.26)s,and the difference was statistically significant(P<0.05).The eye opening time during anesthesia recovery of treatment group and control group was(8.42±1.94)and(8.48±2.13)min,the intraoperative dosage of fentanyl was(3 456.67±608.51)and(3 515.00±619.41)μg,respectively,the differences of above indexes in two groups were not statistical significance(all P>0.05).The incidences of injection pain during induction period in treatment group and control group were 3.33%and 30.00%,the incidences of hypotension in treatment group and control group were 18.33%and 40.00%,the incidences of intraoperative bradycardia in treatment group and control group were 3.33%and 13.33%,respectively,the differences were statistically significant(all P<0.05).Conclusion Compared with propofol injection combined with afentanil injection,cipofol injection combined with afentanil injection can better maintain hemodynamic stability during anesthesia induction and maintenance in patients undergoing tracheoscopy under general anesthesia with laryngeal mask ventilation,and has better safety.
7.Chemical constituents from Codonopsis pilosula in Shanxi and their anti-inflammatory activities

AIM To study the chemical constituents from Codonopsis pilosula(Franch.)Nannf in Shanxi and their anti-inflammatory activities.METHODS The 70% ethanol extract from C.pilosula in Shanxi was isolated and purified by silica gel,ODS and preparative HPLC,then the structures of obtained compounds were identified by physicochemical properties and spectral data.Their in vitro anti-inflammatory activities were evaluated by RAW264.7 model.RESULTS Sixteen compounds were isolated and identified as ethylsyringin(1),7-O-ethyltangshenoside Ⅱ(2),triandrin(3),trans-isoconiferin(4),methylsyringin(5),9-acetoxy syringin(6),cordifolioidyne B(7),codonopiloenynenoside A(8),codonopilodiynoside F(9),pratialin B(10),lobetyolinin(11),lariciresinol-4-O-β-D-glucoside(12),dihydrodehydrodiconiferyl alcohol 4′-O-β-D-glucoside(13),atractylenolid Ⅲ(14),baimantuoluoamide B(15),benzyl primeveroside(16).Compounds 1-2,5,7-11 and 13-15 had certain anti-inflammatory activities,among which compounds 11,14-15 had higher activities,whose IC50 values were(18.23±4.18),(17.73±3.12),(14.89±2.47)μmol/L,respectively.CONCLUSION Compounds 3,6,13,16 are first isolated from Campanulaceae,2,5,15 are first found from this plant.Compounds 11,14 and 15 have good anti-inflammatory activities.
8.Prognostic value of Montreal Cognitive Assessment in heart failure patients.

Objective: To explore the occurrence of cognitive impairment in Chinese heart failure (HF) patients and it's impact on prognosis. Methods: In this prospective observational study, a total of 990 HF patients were enrolled from 24 hospitals in China during December 2012 to November 2014. All patients were administrated with the interview-format Montreal Cognitive Assessment (MoCA), according to which they were divided into MoCA<26 (with cognitive impairment) group and MoCA≥26 (without cognitive impairment) group. Baseline data were collected and a 1-year follow up was carried out. Univariate and multivariate logistic or Cox regression were performed for 1-year outcomes. Results: Cognitive impairment was evidenced in 628 patients (63.4%) and they were more likely to be older, female, and with higher proportion of New York Heart Association(NYHA) class Ⅲ-Ⅳ, chronic obstructive pulmonary disease (COPD), ischemic heart disease, while body mass index (BMI), education level, and medical insurance rate were lower (all P<0.05) as compared to patients in MoCA≥26 group. The rate of percutaneous intervention, device implantation, cardiac surgery and evidence-based medications were significantly lower in MoCA<26 group than in MoCA≥26 group (all P<0.05). During the 1-year follow up, patients in the MoCA<26 group had higher all-cause mortality (10.2%(64/628) vs. 2.2%(8/362), P<0.01), cardiovascular mortality (5.9%(37/628) vs. 0.8%(3/362), P<0.01) and major adverse cardiac and cerebrovascular events (MACCE) (9.6%(60/628) vs. 2.5%(8/362), P<0.01) than patients in the MoCA≥26 group. In univariate regression, MoCA<26 was associated with increased all-cause mortality (HR(95%CI):4.739(2.272-9.885), P<0.01), cardiovascular mortality (HR(95%CI):7.258(2.237-23.548), P=0.001) and MACCE (OR(95%CI):4.143(2.031-8.453), P<0.01). After adjustment by multivariate regression, MoCA<26 was indicated as an independent risk factor for all-cause mortality (HR(95%CI): 6.387(2.533-16.104), P<0.01), cardiovascular mortality (HR(95%CI): 10.848(2.586-45.506), P=0.001) and MACCE (OR(95%CI): 4.081(1.299-12.816), P=0.016), while not for re-hospitalization for HF (OR(95%CI):1.010(0.700-1.457), P=0.957). Conclusions: Cognitive impairment is common in HF patients,and it is an independent prognostic factor for 1-year outcomes. Routine cognitive function assessment and active intervention are thus recommended for HF patients.

9.Salidroside Ameliorates Lung Injury Induced by PM2.5 by Regulating SIRT1-PGC-1α in Mice

Objective This study aimed to clarify the intervention effect of salidroside(SAL)on lung injury caused by PM2.5 in mice and illuminate the function of SIRT1-PGC-1ɑ axis. Methods Specific pathogen-free(SPF)grade male C57BL/6 mice were randomly assigned to the following groups:control group,SAL group,PM2.5 group,SAL+PM2.5 group.On the first day,SAL was given by gavage,and on the second day,PM2.5 suspension was given by intratracheal instillation.The whole experiment consist of a total of 10 cycles,lasting 20 days.At the end of treatment,blood samples and lung tissues were collected and analyzed.Observation of pathological changes in lung tissue using inverted microscopy and transmission electron microscopy.The expression of inflammatory,antioxidants,apoptosis,and SIRT1-PGC-1ɑ proteins were detected by Western blotting. Results Exposure to PM2.5 leads to obvious morphological and pathologica changes in the lung of mice.PM2.5 caused a decline in levels of antioxidant-related enzymes and protein expressions of HO-1,Nrf2,SOD2,SIRT1 and PGC-1ɑ,and an increase in the protein expressions of IL-6,IL-1β,Bax,caspase-9 and cleaved caspase-3.However,SAL reversed the aforementioned changes caused by PM2.5 by activating the SIRT1-PGC-1α pathway. Conclusion SAL can activate SIRT1-PGC-1ɑ to ameliorate PM2.5-induced lung injury.
10.Analysis of Chinese citizens' traditional Chinese medicine health culture literacy level and its influence factors in 2017.

To analyze the TCM health culture level and influence factors of Chinese citizens in 2017. PPS sampling combined with random sampling was used to select the residents aged between 15-69 years old in 30 provinces as the respondents,and a questionnaire study was conducted to investigate their TCM health culture level. In 2017,there were 87 287 valid questionnaires for Chinese citizens' TCM health culture level,including 48. 25% male and 51. 75% female,with a sex ratio of 1 ∶ 1. 073. In 2017,the overall TCM health culture level was 13. 39%,specifically 18. 77% for the urban areas and 10. 51% for the rural areas. Compared with people who were illiterate or less literate,people with an educational background of elementary school,junior high school,high school/vocational/technical school and junior college/university had a higher TCM health culture level,and the OR values were 1. 584( 95% CI[1. 166,2. 152]),2. 827( 95%CI[1. 839,4. 345]),5. 651( 95%CI[3. 637,8. 781]),9. 785( 95%CI[6. 187,15. 477]) in order. With civil servants as the reference,medical workers had a higher TCM health culture level( OR = 1. 829,95%CI[1. 279,2. 616]),while farmers had the lowest TCM health culture level( OR = 0. 493,95% CI[0. 349,0. 697]). Compared with people with the annual household income per capita of 20 000 yuan and below,people with the annual household income per capita between 20 000-50 000,50 000-80 000,80 000 yuan or above had a higher TCM health culture level,and the OR values were 1. 176( 95% CI[0. 963,1. 437]),1. 458( 95%CI[1. 168,1. 820]) and 1. 930( 95%CI[1. 509,2. 469]). Based on the differences between urban and rural areas,the influence factors of citizens' TCM health culture level include education,occupation and income.
