1.Efficacy of laparoscopic gastric bypass on simple obesity patients and analysis of influence factors.

OBJECTIVETo evaluate the outcomes and factors related to weight loss after laparoscopic gastric bypass(LGBP) in obese patients.
METHODSForty-one obese patients who underwent LGBP from May 2010 to December 2011 in the First Affiliated Hospital of Nanjing Medical University were followed up. The operative time, intraoperative complications, postoperative complications, preoperative body mass index(BMI), postoperative BMI, and excess weight loss rate(EWL) were determined and their correlation with efficacy were analyzed.
RESULTSAll the surgeries were successful without conversions or perioperative deaths. The average operative time was (229±96)min, intraoperative blood loss was(15±3) ml, postoperative hospital stay was(5.7±1.7) d. Patients were followed up for 3-12 months. The average EWL at 1, 3, 6, 9, and 12 months after operation was 24.2%, 45.6%, 60.1%, 66.5% and 69.0%. The EWL was negatively correlated with preoperative BMI(P<0.01), but not correlated with age, gender, and waist-hip ratio(all P>0.05). Postoperative short-term EWL did not differ between central obesity patients and peripheral obesity patients, and before and after standardized treatment(both P>0.05). After standardization, however, operative time and postoperative hospital stay were significantly reduced(P<0.01).
CONCLUSIONSLGBP is an effective and feasible treatment for obesity patients. Short-term efficacy after surgery is negatively correlated with preoperative BMI. Standardization may reduce operative time and postoperative hospital stay, but not associated with improved short-term outcomes.

2. An analysis of the new "three tubes" method in the treatment of spontaneous esophageal rupture

To investigate the clinical efficacy, feasibility and safety of new "three tubes" method in the treatment of spontaneous esophageal rupture. A total of 22 patients with spontaneous esophageal rupture were retrospectively analyzed. Through the new "three tubes" method of treatment, patients achieved leak cured with reduced hospital stay, less medical expenses and early resumption of oral diet. The new "three tubes" method for spontaneous esophageal rupture has the advantages of easy handling, minimal invasion, few complication and exact curative effect.
3.Progress in Development of Targeted Therapeutic Drugs for Acute Myeloid Leukemia

Targeted therapeutic drugs for acute myeloid leukemia (AML) are showing immense development, thereby laying a solid foundation for the precise treatment of AML patients. The paper reviews four types of targeted drugs that have progressed rapidly for AML treatment (by targeting genes or signaling-pathway alterations, targeting apoptosis-related pathways, targeting cell-surface antigens, and targeting immune-related substances). We look forward to the future development directions of targeted drugs, providing references for hematologists and developers of new drugs for AML.
4.Relationship between Stat5b Expression in Cervical Tissues and Lesions

Objective To investigate the expression of signal transducer and activator of transcription 5 b (Stat5 b) and its phosphorylation (p-Stat5 b) in cervical lesions, its relationship with different degrees of cervical lesions, and its role in the pathogenesis and development of cervical lesions. Methods We obtained 80 specimens, including normal cervical tissues, low-grade squamous intraepithelial lesions (LSIL), high-grade squamous intraepithelial lesions (HSIL), and cervical cancer tissues. To analyze the correlation between Stat5 b and the degree of cervical lesions, immunohistochemical staining was performed to detect Stat5 b and p-Stat5 b expression. Results Increased Stat5 b expression had a positive correlation with the development of cervical lesions. The percentages of Stat5 b expressed in normal tissues, LSIL, HSIL, and cervical cancer tissues were 15.0%, 0.0%, 50.0%, and 80.0%, respectively, with significant differences among the groups (P < 0.05). Conversely, the expression of p-Stat5 b had a negative correlation with the development of cervical lesions.Expression of p-Stat5 b in normal tissues, LSIL, HSIL, and cervical cancer tissues was 80.0%, 45.0%, 5.0%, and 0.0%, respectively, with statistically significant differences among the groups (P < 0.05). Conclusion The expression of Stat5 b is significantly different in different degrees of cervical lesions, suggesting that Stat5 b signaling molecules may be involved in the development of cervical lesions.
5.Effects of camel milk on intestinal mucosal immune function in mice

Objective To investigate the effects of camel milk on immune cells in lamina propria (LP) of intestinal mucosa in mice. Methods Six male C57BL/6 mice(6-8 weeks) were randomly divided into two groups as follows: camel milk treatment group and double distilled water (DDW) control group. Samples of cells in LP of intestinal mucosa were collected. Cell counts and percentages of immune cells in LP were analyzed by flow cytometry. Levels of IL-4,IL-10,IL-17 and IFN-γ in the supernatants of cell cul-ture were measured by ELISA. Results Compared with the DDW control group, the camel milk treatment group showed increased percentage and absolute number of CD4+T cells as well as IFN-γ-secreting CD4+T cells in LP of intestinal mucosa(P<0.05). Moreover,significantly enhanced expression of IFN-γ and sup-pressed secretion of IL-4 were found in the camel milk treatment group (P<0.05). Conclusion Camel milk can promote the proliferation of CD4+T cells and enhance the secretion of IFN-γ,indicating that camel milk regulates the proliferation and cytokine secretion of immune cells in LP of intestinal mucosa in healthy mice.
6.Prevention and treatment of chronic atrophic gastritis in rats with isinglass.

OBJECTIVETo evaluate the effect of isinglass on the prevention and treatment of chronic atrophic gastritis in rats.
METHODAnimal model of SD rats with CAG was established in accordance with the previous experience of combined administration of 60% ethanol, 20 mmol x L(-1) sodium deoxycholate and 0.1% ammonia water. In prevention groups, sucralfate and isinglass were used as preventive therapy while we were establishing CAG rat model. In the reverse groups, sucralfate and isinglass were used to treat rats after establishment of CAG rat model. Finally all the rats were executed and pathologic changes of the gastric mucosa were studied by gross appearance and microscopy.
RESULTIn isinglass prevention groups and reverse groups, inflammation grades of gastric antrum were less than these in model control group (P < 0.01) while the means of ratios of the thickness of gastric mucosal gland and muscularis mucos (L1/L2), the number of gastric glands in 1-mm lengths of mucosal layer were much better than those in model control group (P < 0.01). They were very close to normal control group (P > 0.05).
CONCLUSIONIsinglass can prevent the gastric mucosal atrophy in the CAG model and can improve and cure the gastric mucosal atrophy of the SD rats with GAG.

7.Clinical analysis of peripheral blood stem cell mobilization regimens in autologous transplantation for treating non-Hodgkin's lymphoma.

The purpose of this study was to compare the efficacy of CEP plus G-CSF and CVP plus G-CSF regimens in the mobilization and collection of peripheral blood hematopoietic stem cells (PBHSC), and in the hematopoietic recovery. 57 patients with non-Hodgkin's lymphoma (NHL) underwent autologous PBHSC transplantation were analyzed retrospectively. The PBHSC were mobilized and collected by using CEP plus G-CSF and CVP plus G-CSF respectively, and were retransfused into these NHL patients after preconditioning, then the mobilization efficacy, adverse reactions and hematopoietic recovery were analyzed. The results showed that the WBC count decreased to ≤ 1.0 × 10(9)/L, platelet amount dropped to ≤ 40 × 10(9)/L during peripheral blood stem cell mobilization of all patients, which indicated successful collection of PBHSC. The mean value of (4.38 ± 3.40) × 10(8)/kg mononuclear cells (MNC) containing (2.79 ± 2.53) × 10(6)/kg CD34(+) cells were collected in CEP plus G-CSF group, while the mean value of (3.31 ± 1.23) × 10(8)/kg MNC containing (2.02 ± 0.87) × 10(6)/kg CD34(+) cells were collected in CVP plus G-CSF group. The efficacy of mobilization in CEP plus G-CSF group was significantly higher than that in CVP plus G-CSF group (p < 0.05). After preconditioning, bone marrow was suppressed in all patients. The average time of WBC count recovery to ≥ 1.0 × 10(9)/L was 11.4 days in CEP plus G-CSF group and 12.3 days in CVP plus G-CSF group; the average time of platelet amount recovery to ≥ 50 × 10(9)/L was 18.6 days in CEP plus G-CSF group and 19.3 days in CVP plus G-CSF group. The statistical analysis showed no significant difference in the average time of hematopoietic recovery between 2 groups. It is concluded that autologous PBHSC transplantation shows significant effect for treatment of patients with NHL. Either modified CEP or CVP plus G-CSF regimen is safe and effective in PBHSC mobilization. The CEP plus G-CSF regimen is better than CVP plus G-CSF regimen.

METHODAnimal models of SD rats with CAG were made in accordance with the previous experience of combined administration of 60% ethanol, 20 mmol x L(-1) sodium deoxycholate and 0.1% ammonia water. In prevention groups, sucralfate and isinglass were used as preventive therapy while CAG rat model was being made. In the reverse groups, sucralfate and isinglass were used to treat rats after establishment of CAG rat model. Finally all the rats were executed. Then the length of the proliferation zone of the gastric mucosa and serum epidermal growth factors (EGF) and growth hormones (GH)level were studied.
RESULTIn isinglass prevention groups and high dose isinglass reverse group, the length of the proliferation zone of the gastric mucosa was very close to that in normal control group (P > 0.05), much better than model control group (P < 0.01). In low dose isinglass reverse group, it was lower than that in normal control group (P < 0.01), but much better than model control group (P < 0.01). In both prevention and reverse groups, serum EGF level was higher than that in normal (P < 0.01) and model control group (P < 0.05). Serum GH level was the same in every group (P > 0.05).
CONCLUSIONThe mechanism of isinglass in the prevention and treatment of CAG rats lies in revitalizing and proliferating gastric mucosal cells by stimulating endogenous EGF secretion.

10. A single-center retrospective study on axillary evaluation in 1 557 breast ductal carcinoma in situ patients between 2006 and 2016

Objectives:
To examine the influence factors on axillary evaluation in ductal carcinoma in situ (DCIS) patients, and the prognosis of different choices of axillary evaluation in a single-center retrospective study.
Methods:
Totally 1 557 DCIS patients admitted in Department of Breast Surgery, Fudan University Shanghai Cancer Center from January 2006 to November 2016 were retrospectively enrolled. All patients were female. The median age was 49 years (range: 21 to 85 years). Surgical methods included modified radical mastectomy, simple mastectomy (with or without axillary evaluation) and breast conservation surgery (with or without axillary evaluation). Axillary evaluation included axillary lymph node dissection (ALND) and sentinel lymph node biopsy (SLNB).
