The Journal of Practical Medicine 2017;33(14):2345-2348
Objective To investigate the clinic and microbial characteristic of fever along with agranulocy-tosis in patients with malignant hematopathy. Methods Retrospective study was carried out on patients with malig-nant hematopathy who developed agranulocytosis during hospitalization. Results A total of 512 febrile events occurred. Duration of agranulocytosis, severe agranulocytosis, poor physical condition, previous exposure to broad-spectium antibiotics and central venous catheterization were correlated with higher incidence of febrile event. There are 328 cases(64.06%)with definite infected sites and 184 cases(35.94%)with fever of unknown origin(FUO). Gram-negative bacterium was most common(64.35%),followed by Gram-positive bacterium(23.61%)and fungus (12.04%). 57 cases resistant bacteria were detected. Mortality incidence resulting from pulmonary infection was higher than that from FUO(P < 0.005). No significance was found between pulmonary infection and blood stream infection(P>0.25)or between blood stream infection and FUO(P>0.1). Conclusions Many factors were corre-lated with incidence of fever along with agranulocytosis in patients with malignant hematopathy. Gram-negative bacte-rium was most common pathogenic microorganism, followed by Gram-positive bacterium, all of which manifested high drug resistance rate. Understanding the epidemiological trend will be beneficial for clinic treatment.

Objective To explore the effects of perinatal exposure to low dose tributyltin (TBT) on development and estrogen level of female offspring KM mice. Methods The CL healthy adult pregnant KM mice were randomly divided into 4 groups,6 in each group, they were given doses of TBT (100,10 and 1 ?g/kg) by gavage from days 6 of gestation to the end of lactation,1 time each day,at a volume of 1 ml/kg.On postnatal day (PND) 49,10 female offspring mice were randomly selected in each group and killed after weighed and the blood was collected. The uterus and ovary were weighed for account of viscera coefficient. The concentrations of estrogen in serum were measured with radioimmunoassay. Results Compared with the control group the time of eye opening was significantly delayed and the vagina opening was significantly ahead in treatment groups (P0.05).The weight of ovary and its coefficient increased significantly in 10 ?g/kg group (P

Objective To provide the anatomical basis for detecting distal outflow tract in late atherosclerosis obliteration in lower extremities. Methods Ten lower extremities that were amputated above knees because of late atherosclerosis obliteration were used in this experiment. The blood vessels in the residual bodies were perfused to run blood vessel cast mould to observe the anatomical and pathological change of the popliteal artery, the anterior and posterior tibial arteries and their collateral vessels. The number and distribution of those collateral vessels were also observed. Results The popliteal artery, anterior and posterior tibial arteries were all occluded due to atherosclerosis. However, there were three types of those collateral arteries:① Atheromatous plaque in bole stretched into collateral arteries and led to occlusion.② Obliteration was only observed at the initial segment, with no obstruction at the distal end but extenuated.③ The collateral arteries originated from the bole artery symmetrically, keeping communicative with each other through punctiform interspaces. The last two types were mainly distributed at the inferior segment of popliteal artery, the superior segment of anterior and posterior tibial arteries, forming vascular anastomosing network in the whole cnemis muscle group. Conclusion Un-obstructed collateral arteries in certain places can be still found, though atherosclerosis obliteration is formed in popliteal artery, anterior and posterior tibial arteries in lower extremities. Therefore,it may be possible to construct collateral outflow tracts if endo-membrane stripping operation is performed.

Objective To discuss the nursing intervention of Rey syndrome in children patients.so as to search effective nursing measures. Methods 12 children patients with Rey syndrome received comprehensive nursing treatment with reducing intracranial pressure and were under continuous close observation.Slightest changes were feeded back timely, then effective nursing measures were taken to stabilize their illness, patients also received dietary therapy, medication nursing and rehabilitation training, etc. Results All patients ameliorated after 3 to 7 days, and achieved clinical recovery after 20 to 30 days without sequelae. Conclusions Early diagnosis and correct effective nursing care can improve curative rate and avoid the occurrence of complications and sequelae.

Objective To make contrast analysis of sleeve lobectomy and wedge resection in lung cancer treatment by observing the clinical curative effect. Methods 100 patients with lung cancer using random table method, all the patients were divided into the observation group ( 50 cases, treated with sleeve resection ) , control group (50 cases, treated with wedge resection),follow-up after treatment lasted respectively for 3 years. Results In the two groups, anastomotic fistula incidence and mortality rates of the observation group were 4%,6%;6% and 8% in the control group, there were no statistical difference (χ2 =0.00,0.00,all P>0.05);the positive rate of stump cancer cell bronchial in the observation group was 4%,it was obviously less than 16% in the control group, and the differ-ence was obvious (χ2 =4.00,P<0.05) .The anastomotic scar tissue hyperplasia and recurrence rate of the observa-tion group during the follow-up period were 2%,2%;which were lower than 16%,18% of the control group (χ2 =4.40,7.11,all P<0.05).The 1,3 year survival rates of the observation group were 92%,64%,which were higher than 62%and 42%of the control group (χ2 =12.70,4.86,all P<0.05).Conclusion Taking sleeve resection for bronchogenic carcinoma can effectively control the positive rate of bronchial stump with cancer cells, inhibit anasto-motic scar tissue hyperplasia, reduce the recurrence rate, and the curative effect is very good, which is worth of clinical application.

Objective To measure the expressions of IL-10, IL-23 and CD86 in lesions of epidermodysplasia verruciformis (EV), and to explore the relationship between cellular immune abnormality and EV pathogenesis. Methods Immunohistochemistry was used to measure the expressions of IL-10, IL-23 and CD86 in tissue samples from 10 patients with EV and 10 normal human controls. Results Three cytokines were observed in all the samples of EV, with the expression score ranging from 3 to 6 and expression intensity from moderate to high. However, of the control specimens, only 1 was positive for IL-10 with the expression score being 3, and expression intensity being moderate. Conclusion The pathogenesis of epidermodysplasia verruciformis may be correlated with the expression abnormality of some cytokines secreted by keratinocytes.

13C metabolic flux analysis(13CMFA)have been the research hotspots of metabolic engineering internationally due to its accuracy and applicability. It is vital that the measurement of 13C labelling pattern of proteinogenic amino acids for 13C metabolic flux analysis. To acquire 13C-labelling proteinogenic amino acids,Pseudomonas denitrifican which products Vitamin B12 was firstly fed with mimimal culture medium contained 20% U-13C and 80% natural glucose, after the culture reached a steady state, then about 20 mg biomass was hydrolyzed by 1 ml of 6 mol/L hydrochloric acid for 24h at 110℃.Then amino acids was separated, concentrated, evaporated in a vacuum, and derivatized with MBDSTFA,TBDMS-derivatized amino acids can be observed by GC-MS last we get 13C labelling pattern of fifteen aminio acids through mass spectrum. The experimental methods and sample preparation offers referential value for the development of 13C metabolic flux analysis in our courtry.
