1. Diagnosis and surgical treatment of small peripheral pulmonary nodules: A report of 122 cases

Objective To investigate the diagnostic method and surgical therapeutic outcome of small peripheral pulmonary nodules (diameter ���2cm). Methods The data of 122 patients with small peripheral pulmonary nodules who were admitted from Jan. 2001 to Oct. 2010 and diagnosed by pathology postoperatively were retrospectively analyzed. The nodule diameter of nodules in 101 cases were 1-2 cm, and they were less than 1 cm in 21 cases. All the patients were examined by multislice spiral computed tomographic angiography (MSCTA), while 52 patients were examined by positron emission tomography/computed tomography (PET/CT), and 49 patients were subjected to CT-guided transthoracic needle biopsy (CTNB). The sensitivity, specificity and diagnostic accuracy were compared between the three methods for the diagnosis of lung cancer. Results Confirmed by postoperative pathological examination, 29 patients were suffering from benign lesions (23.8%), and 93 were with malignant lesions (76.2%). In the 85 patients with non-small cell lung cancer, only 56 (65.9%) were in early stage (Ia), and in 12(14.1%) hilar and mediastinal lymph node metastases were found. The sensitivity, specificity and diagnostic accuracy was 80.3%, 81.7% and 75.9% by MSCTA, 84.6%, 86.5% and 80.1% by PET/CT, and 91.8%, 88.6% and 100% by CTNB, respectively. The specificity of CTNB was better than that of MSCTA (P=0.045), and no significant difference was found in other indexes between MSCTA, PET/CT and CTNB (P>0.05). Conclusions MSCTA can serve as the first choice for the non-invasive diagnosis of small peripheral pulmonary nodules. Surgical operation is necessary for suspected malignant lesions, and CTNB can be selectively performed in patients who are considered to have benign lesions. In order to improve the speed and accuracy in locating the pulmonary nodules during the operation, the CT imaging should be carefully reviewed before operation in order to identify the location of the lesion, and CTguided transthoracic needle biopsy could be done when necessary.

4.Spontaneous Cervical Epidural Hematomas in Mild Cervical Spondylotic Myelopathy Patients: An Analysis of 8 Cases

Spontaneous cervical epidural hematoms (SCEH) complicated with mild cervical spondylotic myelopathy (CSM) is a rare but emerging condition.Early diagnosis and treatment are important for good outcomes.This study aimed to investigate the clinical characteristics of this condition and to discuss the optimal treatment.The clinical data from 8 patients with SCEH plus CSM who were divided into two groups by treatment methods were retrospectively analyzed.The neurological function of the patients was assessed by Japanese Orthopedic Association (JOA) score before and after the surgical operations.Other factors were reviewed with medical records.Among them,4 out of the 8 patients underwent emergency surgery,and the rest 3 patients experienced an initial conservative treatment and ultimately received a laminectomy.We found that the Frankel Scale scores in most of the surgical patients were increased after surgery (6/7,85.7%).However,the JOA scores at the 6th month after onset were even lower than those before onset in 3 of the operative cases,and those in the patients who were given conservative treatment showed no significant change.It was concluded that some patients with SCEH and CSM treated with a timely operation may obtain relief from their previous CSM symptoms.However,the final neurological deficits of these patients were closely related to the progressive interval which refers to the hours between the initial onset and the occurrence of new neurological deficits or mild CSM deterioration,no matter whether they accept the operation.We found the crucial progressive interval may be in 9 h.Early MRIand prompt neurosurgical intervention are also important to improve the neurological deficits.
6.Determination of benzyl glucosinolate in Lepidium meyenii from different regions by HPLC.

The content of benzyl isothiocyanate (BITC) which as the enzymatic hydrolysis product of benzyl glucosinolate through thioglucosidase was determined by HPLC. The content of benzyl isothiocyanate (BITC) which as the enzymatic hydrolysis product of benzyl glucosinolate through thioglucosidase was determined by HPLC. The chromatography condition was as follows: Kaseisorb LC ODS 2000 (4.6 mm x 150 mm, 5 min) column with the mobile phase of acetonitrile(A)-water( B) under gradient elution (0-5 min, 3%-8% A; 5-9 min, 8%-48% A; 9-23 min, 48%-62% A; 23-28 min, 62%-99% A); the flow rate was 1.0 mL x min(-1) with 10 microL injection volume; detection wavelength was 246 nm and temperature of column was 40 degrees C. The content of benzyl glucosinolate was in the range of 10.76-17.91 g x L(-1). The method is simple, accurate and good reproducibility which can be used for the determination of benzyl glucosinolate in Lepidium meyenii, effectively.

Objective:To explore the traits of gray matter volume (GMV) and regional homogeneity (ReHo) in patients with schizophrenia and obsessive-compulsive disorder (OCD) by magnetic resonance imaging technique (MRI).Methods:Twenty-two patients with schizophrenia,20 patients with OCD and 23 normal controls were recruited in this study.All of the patients satisfied diagnostic criteria from the Diagnostic and Statistical Manual of Mental Disorders,Fourth Edition (DSM-Ⅳ),patients and the normal controls were well matched for gender,age and years of education.All subjects received structural magnetic resonance imaging (MRI) scans and resting functional MRI scans.The ANOVA and post hoe anolysis were used to compoare the GMV and Relto differences among the subjects.Results:Compared with the normal controls,patients with schizophrenia and OCD had common GMV loss in the right anterior cingulate (P < 0.001,uncorrected),while lower ReHo in the left cuneus(P < 0.001,uncorrected) and higher ReHo in the left upper medial frontal gyrus(P < 0.001,uncorrected) respectively.Conclusion:It suggests that patients with schizophrenia and OCD share common structural changes and functional alterations,which could attribute so many similarities between the two diseases.

PBL teaching method is a new mode of teaching which is originated from the West and implemented into China in recent years with an expectation that it would mainly develop the students’ self-learning ability,and enhance their skills of comprehensive thinking and solving actual problems.The author summarizes the practical experience of using PBL teaching methods in the theory teaching in Department of Medical Microbiology and Human Parasitology,China Medical University in the past three years,and then proved this method is very helpful to improving the students’integrated thinking by analysis of sample.At the same time the results also suggested that the students showed high enthusiasm in discussing the cases.By this way,the students showed great subjective intiative in their studies.
