Subperiosteal Ganglion is a rare cystic lession, although ganglions are frequently found in close proximities to joints and tendon sheaths. It is characterized by a thin-walled cyst containing tenacious mucoid fluid and by the pedicle attached to periosteum. This report describes a middle-aged man with a subperiosteal ganglion which occured beneath the periosteam of the tibia. Presenting symptoms were pain and circumscribed swelling over the anteromedial aspect of the right proximal tibia.

Ankle injuries include not only disruption of bony architecture of ankle joint but also significant ligamentous and soft tissue injuries as well. Since ligamentous injury frequently has more serious implication than fracture, it is important that be recognized and included in the plan of treatment. The goal of treatment of ankle fracture is the establishment of the normal ankle joint. To obtain the this goal, the fracture should be reduced as accurately as possible (by open or closed), the fracture fragment must be retained in position until healing is secure, ligamentous structure should be repaired when indicated, and joint motion should be restored. The following clinical results shown by analysls of 108 cases of ankle injuries experienced in the Department of Orthopaedic Surgery, Capital Armed Forces General Hospital, from Jan. 1977 to June 1979. 1. The main cause of the injuries was falling from a height and the other causes were direct violence, traffic accident, and injuries in order. 2. All but one of the fractures could be classified in the manner described by Lauge-Hansen and the type of supination-external rotation was most common type in this series. 3. Ninty-two (85.1%) patients were treated by open reduction and internal fixation of both malleoli and sixteen by closed method. 4. The average period of cast immobilization was 8 weeks after closed reduction and that of open reduction was 6 weeks. 5. The good clinical results were achieved in 95 petients and the better results were obtained in fracturers treated in operative interventions.

Hemangioma*

Targetoid hemosiderotic hemangioma is a benign vascular tumor with a peculiar clinical presentation and histologic features sometimes difficult to differentiate from the patch stage of Kaposi's sarcoma, progressive lymphangioma, eosinophilic hemangioma, retiform hemangioendothelioma. We report a case of targetoid hemosiderotic hemangioma on upper back and right upper arm of a 28-year-old male. He has a 5mm × 5mm sized slightly raised violaceous papule surrounded by an ecchymotic halo on upper back and right upper arm. We took a biopsy of the lesion and identified it as targetoid hemosiderotic hemangioma. This is a case of targetoid hemosiderotic hemangioma with typical clinical and histopathological findings.

Meningiomas causing intracranial hemorrhage are rare, and hemorrhage from a lateral ventricular meningioma seems to be even rarer. We report a case of trigonal meningioma in a 43-year-old woman who presented with intraventricular hemorrhage, and describe the CT, MRI and angiographic findings.

The purpose of this study was to elucidate the epidemiology and the clinical characteristics of herpes zoster and chicken pox. 703 patients with herpes zoster and 237 patients with chicken pox who were seen at the Department of Dermatology, St. Marys Hospital during 8 years were taken for this retrospective study. Both groups of the patients were evaluated in regard to annual, seasonal and monthly incidence, age incidence and sex ratio. Patients with herpes zoster were further assessed concerning associated conditions, dermatomic distributions and complications. The results were as follows, 1. The annual rate for chicken pox ranged from 0.03% to 0.18% (mean 0.13%) and revealed no epidemics, however, its incidence was higher in early summer and winter. There was no sexual difference. Chicken pox was seen most. frequently in the 0-9 year age group (91.1%). 2. The annual rate for herpes zoster ranged from 0.23% to 0.52% (mean 0.38%). There were no significant annual, seasonal and monthly variations, and there was a. equal sex ratio. Herpes zoster was seen most frequently in the 50-59 year age group (19.1%) 3. The frequencies of the associated conditions in herpes zoster, in descending order, were infectious disease (2. 7%), diabetes mellitus (2.3%), malignant neoplasm(1. 4%), immunosuppressive drug (l. 4%), autoimmune disease (0.6%), and radiation therapy (0.4%). 4. The dermatomic involvement of herpes zoster was most frequent on thoracic dermatome (53.5%) and others were cervical (16.5%), lumber (10.1%), ophthalmic branch of trigeminal nerve (8.8%), sacral (1.6%), maxillary branch of trigemenal nerve (1.3%), mandibular branch of trigemical nerve (0.6%) and facial nerve dermatome (0.1%). The incidence of involving two different dermatomes was 2.9% and was most common on cervicothoracic dermatome (1.8%). However, one case of cervico thoracic dermatome showed bilateral involvement. 5. The most common complication of herpes zoster was postherpetic neuralgia (6.5%) and others were secondary bacterial infection (2.6%), keratoconjunctivitis (0.6%), keloid formation (0.3%), generalized varicelliform eruption (0.1%), Ramsay-Hunt syndrome (0.1%) and recurrent herpes zoster (0.1%). The mean age of incidence for postherpetic neuralgia was 58.4 years and its incidence was the highest over 70 years (39.5%). Postherpetic neuralgia was most frequently seen in the involvement of ophtalmic branch of trigeminal nerve.
Incidence

An experimental study was performed for the observation of the effect of the shock wave on the renal parenchyme of the white rat. Sixty Sprague-Dawley rats were subjected to this study. The experimental rats were divided into four groups by the intensity and amount of shock wave and also each group was subdivided into three small groups by the time of sacrifice. The following results of gross and microscopic examination were obtained. After expulsion of shock wave, various injuries such as subcapsular hematoma, parenchymal hematoma and laceration were seen and the degree of injuries was increased according to the intensity and amount of shock wave macroscopically. On the 1st day of sacrifice, the ischemic necrosis of parenchyme was observed at directly exposed site, the hemorrhage, congestion and arteriolar damages at transitional zone. On the 7th day, the renal capsular thickening by fibroblast proliferation and focal fibrosis was appeared at direct zone. At transitional zone, moderate lymphocyte infiltration was observed. On the 28th day, renal parenchyme was replaced by fibrosis at direct zone, severe lymphocyte infiltration and thyroidaization were observed at transitional zone. The degree of injuries was not proportional to the amount and intensity of shock wave microscopically and the reasons will be discussed later. Conclusively, the shock wave caused the partial functional loss and pathologic sequela of renal parenchyme.

The effects on renal function following percutaneous nephrolithotomy(PNL) were investigated on 37 patients. The overall success rate was 76%. The success rate on renal stone(79% ) was better than on upper ureter stone(67% ). The complication rate was much lower on later cases according to the increasing experiences and improved technique. The renal function was evaluated on 15 cases using DMSA renal scan and separate creatinine clearance rate pre-and post-operatively. Fourteen cases out of fifteen cases showed no significant renal functional change statistically. However, only one case who had massive renal bleeding for eight days postoperatively showed about 18% reduction of renal function and which case was considered due to major renal vascular damage during PCN tract creation. Conclusively, PNL seemed to be safer and has lower morbidity rate than surgical removal of stone if carried out by experienced hands.

Aspergillosis is a rare disease of fungal infection produced by the genus Aspergillus in immunocompromised patients. It mainly involves the lung and may disseminate by invasion of biood vessels into the gastrointestinal tract. Brain and kidney. We report a case of the disseminated aspergillosis in malignant lymphoma affecting the lung. Brain and thyroid gland.
