4.Inclusion Body Myositis: A case report.

In 1971 inclusion body myositis was reported by Yunis and Samaha. This disease is similar with chronic multiple myositis clinically. Pathologically, inclusion body myositis is characterized by intracytoplasmic vacuole with degenerating fibers and accompanied with inclusion body in internal nucleus and cytoplasm. Since then 240 cases of inclusion body myositis have been reported in the world including 3 cases in Korea. A 27 years-old lady had inclusion body myositis, which show slowly progressive muscular weakness. We confirmed this with clinical symptom, muscle biopsy, and electrophysiologic study. We report the typical manifestation of inclusion body myositis in a 27 years-old lady with the brief review of literature.

5.Correcting unfolded helix in a 22-year-old male patient: a case report

An unfolded helix is an auricular deformity that has garnered relatively little research focus. This study introduces a surgical technique designed to correct flat helix deformities. A 22-year-old male with bilateral unfolded helixes and prominent ears underwent this procedure under local anesthesia. The surgery employed a posterior auricular approach, with incisions extending from the upper pole of the ear to the concha. Along the helical rim, four radiating incisions were made, followed by oblique cuts in the cartilage and the creation of triangular flaps through advancement. The free edges of the cartilage were then overlapped and sutured together. Post-surgery, the patient experienced a successful recovery, evidenced by the curling of the helix and the absence of complications. This surgical technique is not only effective for correcting unfolded helixes but can also be efficiently performed under local anesthesia.

9.Teicoplanin Dosing Strategy for Treatment of Staphylococcus aureus in Korean Patients with Neutropenic Fever.

PURPOSE: The present study was conducted to determine and compare the target attainment rate (TAR) between microorganism-nonspecific (Ctrough) and microorganism-specific (AUC24/MIC) targets over two weeks of teicoplanin administration according to several dose regimens for the treatment of Staphylococcus aureus in Korean patients with neutropenic fever. MATERIALS AND METHODS: One thousand virtual concentrations were obtained for each dose using the population pharmacokinetic parameters of teicoplanin adopted from a published study. Simulation of 1,000 virtual MICs was performed using the MICs of 78 clinical isolates of S. aureus collected from a hospital in Korea. Thereafter, these simulated MICs were randomly allocated to 1,000 virtual patients in whom the TARs for AUC24/MIC >125 [or 345] and Ctrough >10 [or 20] mg/L were determined. The relationship of the maintenance dose with the steady-state TAR was predicted with respect to the AUC24/MIC >125 [or 345] using logistic analysis. RESULTS: The standard dose regimen of teicoplanin showed TARs of about 70% [or 33%] and 70% [or 20%] at steady-state in cases with AUC24/MIC >125 [or 345] and Ctrough >10 [or 20] mg/L, respectively. CONCLUSION: The current standard dose regimen was predicted to be insufficient to adequately treat S. aureus in Korean patients with neutropenic fever. To assure at least an 80% TAR in this population, dose adjustment of teicoplanin should be considered.

A case of Kaposi's sarcoma involving the small bowel two years after receiving a renal transplant is described. Immunosuppression had been achieved using cyclosporine A and prednisolone. Lesions extended from the duodenum to the ileum ; radiologically, they were demonstrated on small bowel follow-through study and computed tomography as multiple small nodular intraluminal masses with or without central umbilication, and endoscopically, were seen as intramural mucosal elevations with a central crater-like ulceration.
