1.In vitro antimicrobial activity of different mouthwashes available in Korea.

OBJECTIVES: The purpose of this study was to compare the oral antimicrobial effects of seven different mouthwashes available in Korea. METHODS: To examine the antimicrobial effects of the seven mouthwashes, their minimal inhibitory concentration (MIC) and minimal bactericidal concentration (MBC) were determined using broth microdilution methods. Streptococcus mutans ATCC 25175, Lactobacillus acidophilus ATCC 4355, Candida albicans KCTC 7270, and Porphyromonas gingivalis ATCC 33277 were used in this experiment. S. mutans and P. gingivalis were examined using a scanning electron microscope (SEM), after treatment with the mouthwashes containing cetylpyridinium chloride (CPC). RESULTS: Mouthwashes containing CPC had lower MIC and MBC values against the four microorganisms. Their bactericidal effects were concentration-dependent. S. mutans and C. albicans were highly sensitive to the concentration of CPC in the mouthwashes. According to the SEM observation, the treatment of bacteria with mouthwashes containing CPC, changed the cell surface texture of S. mutans and P. gingivalis. CONCLUSIONS: Mouthwashes containing CPC showed relatively lower MIC and MBC values under the same conditions against the four microorganisms used in this study.

2.The Study for Limbal Epithelial Cell Movement according to the Position of Limbal Transplant in Rabbits.

PURPOSE: This study aimed to evaluate the limbal epithelial cell movement according to the position of limbal transplant in rabbits. METHODS: We performed autologous conjunctival limbal transplantation in rabbits, in situ (directed corneal end toward cornea and conjunctival end toward conjunctiva) or in versus (directed corneal end toward conjunctiva and conjunctival end toward cornea) manner. We performed 360o whole autologous conjunctival limbal transplantation in quadrant fashion. Twelve months later, we observed the gross cornea under a slit lamp, and then observed corneal tissue with PAS staining using light microscopy and immunohistochemical and immunofluorescence staining using monoclonal antibody. As mentioned above, the conjunctival limbal tissue had been transplanted in situ and in versus manner for confocal microscopy. However, before transplantation, we performed organ culture of the limbal epithelial cells with CellTracker(TM) Green CMFDA dye (Molecular Probes, USA). Two weeks later, we observed the corneal and limbal epithelial cells using the confocal microscope. RESULTS: The slit lamp examinations of the cornea showed no new vessels and clear in both specimens. There were no goblet cells and the corneal epithelial arrangement was normal under light microscopy. There were positive immunohistochemical stains for cornea-specific AK2 and AE5, and negative immunofluorescence stains for goblet cell-specific AM3 patterns in both in situ and in versus specimens. CellTracker(TM) Green CMFDA dye up-taken epithelial cells were not presented in the conjunctival epithelial side, but were in the corneal epithelial side in both specimens. CONCLUSIONS: We confirmed that the epithelial cells with characteristics of corneal epithelium moved toward the corneal center at any positions of limbal transplants in rabbits.

3.Augmentative and Alternative Communication Training Using Eye Blink Switch for Locked-in Syndrome Patient.

Locked-in Syndrome is a severe pontine stroke causing quadriplegia, lower cranial nerve paralysis, and mutism with preservation of only vertical gaze and upper eyelid movement in a conscious patient. We present a case of a Locked-in Syndrome patient who received communication training with augmentative and alternative communication equipment by using eye blinks. After 3 weeks of training, the patient was able to make an attempt to interact with other people, and associate a new word by Korean alphabet selection. Augmentative and alternative communication equipment which uses eye blinks might be considered to be beneficial in improving the communication skills of locked-in syndrome patients.

4.Changes of Ionic Flow through the Internal Limiting Membrane during the Visual Adaptation of Vertebrate Eye.

It already has been known that various ions were participated in phototransduction mechanism of the vertebrate photoreceptor during the visual adaptation. The vitreous humor(VH)contains certain amount of Na+, K+ and Ca++ as well. However, the roles of these ions are unknown except functioning of metabolic process. Our preliminary electroretinogram(ERG) experiments suggested that the permeability of these ions has light dependent activity during the light illumination. We have supported this suggestion more tangibly by trying out various experiments with bullfrog eyes as a reference of visual adaptation. The results are summarized as follows: 1)According to our atomic absorption spectroscopic measurements, the concentration of Na+, K+ and Ca++ in light adapted VH was higher than that of dark adapted VH. This result indicated that the light-dependention transport systems which affect the ionic movements might be in internal limiting membrane(ILM). 2)The amplitude of ERG b-wave was reduced by concentration decrement of Na+ and concentration increment of K+ respectively. On the other hand, the elimination of Ca++ within ringer solution resulted in b-wave amplitude increment. 3)Treatment of Na+ hannel blockers(TTX, STX)and K+ channel blockers(Ba++, Cd++, Cs+, 4-AP and TEA)in the vitreous humor side induced the increment and the decrement of b-wave response respectively. Taken together, these results suggest that the light dependent Na+ K+ channels exist in the ILM of vertebrate retina which participates visual adaptation.

5.Prediction of Amnionicity Using the Number of Yolk Sacs in Monochorionic Multifetal Pregnancy.

The purpose of this article was to evaluate the accuracy of predicting amnionicity using the number of yolk sacs by diagnostic ultrasound examination in monochorionic (MC) multifetal pregnancies between 7 + 0 and 9 + 6 gestational weeks. A total of 97 patients with MC multifetal pregnancies underwent early ultrasound examination from 2004 to 2014 at Cheil General Hospital and Women's Healthcare Center. All patients for whom the number of yolk sacs was reported were included in this study. We compared the number of yolk sacs with amnionicity confirmed by an intertwine membrane. Overall, there was a 9.3% (9 cases) discrepancy in number of yolk sacs and amnionicity (4.3% for monochorionic diamniotic, 36.4% for monochorionic monoamniotic, and 33% for monochorionic triamniotic). Among the 9 cases with discrepancies, 4 cases with 2 yolk sacs were confirmed as monoamniotic pregnancies and 4 MC twin pregnancies showing a single yolk sac were diagnosed as diamniotic twin pregnancies. One case with 2 yolk sacs was identified as a triamniotic triplet pregnancy. In 9.3% of MC gestations, the number of yolk sacs was not correlated with the number of amnions in our study. To determine amnionicity in MC multifetal pregnancies, we recommend careful evaluation not of the number of yolk sacs but the presence or absence of intertwine dividing membrane after 8 gestational weeks.

6.Stress distribution in esthetic orthodontic brackets: An analysis using the finite element method.

The aim of this study were to measure and compare the stress level on three type brackets and each other material (stainless steel, ceramic) with tipping and torquing forces by using the finite element analysis and to design bioniechanically favorable brackets. For this study, three kinds of brackets were selected(A:Transcend-RMO, B:Signature-Unitek, C:PAW plain archwire appliance-applied for a patent in Yonsei University). The slot size of bracket was 0.022inch and the size of archwire was 0.0175x0.025inch and taper shaped archwire was used in PAW. Loading force in tipping was 4.27N and torquing force was 32.858N applied by archwire torsion with 19.7degree and 113 degree in C type bracket. The conclusions were that (1) The finite element method proved to be a useful tool in the stress analysis of orthodontic bracket subjected to various forces. (2) With tipping, the stresses were concentrated at the gingival wall of the wire slot where it meets the mesial bracket surface and the incisal wall of the wire slot where it meets the distal bracket surface and with torquing, the stresses were concentrated at the junction of the gingival or incisal wall and base of the slot. (3) The maximum stress value was higher in torquing force than tipping force and therefore it is desirable to design on the basis of torquing force. (4) It was considered that the change in material might be affect on the diminish of stress value in the place of stess concentration. (5) The maximum stress value was highest on PAW bracket when the tipping and torquing force was applied and therefore it would be desirable to use mechanically favorable material on PAW bracket.

7.Electromechanical Gait Trainer for Gait Rehabilitation for Patients with Stroke.

We developed an electromechanical gait trainer for rehabilitation of the persons with disabilities, which was designed to simulate normal gait with partial body weight support. The purpose of this study was to apply it to stroke patients and to evaluate its usability. Four nonambulatory stroke patients had 20 sessions of gait training with newly developed gait trainer for 5 to 6 weeks. Knee and ankle joint angles were measured by electrogoniometer during training. Main outcome was assessed by functional ambu lation category (level 0~5). Knee and ankle joint angles during exercise in the gait trainer showed repetitive pattern of gait-like movement similar to normal gait, though it was variable among the subjects. Functional ambulation category levels after training were 2 in 3 patients and 1 in 1 patient. The electromechanical gait trainer was a useful device for gait retraining in stroke patients with severe neurologic impairments.

8.A Case of High-grade Endometrial Stromal Sarcoma with Metastasis to the Stomach.

To our knowledge, this is the first report of case of high-grade endometrial stromal sarcoma metastasized to the stomach. Endometrial stromal sarcoma is very rare, accounting for 0.2% of female genital tract malignancies. High-grade endometrial stromal sarcoma demonstrates more frequent infiltration into the muscle layer of the uterus or metastasis to other organs, having a poorer prognosis than low grade disease. During its clinical course, endometrial stromal sarcoma may involve ovary, fallopian tube and pelvic cavity. Distant metastasis to lung, liver, bladder, breast, heart, brain and bones have been reported. However, to the best of our knowledge, high-grade endometrial stromal sarcoma presenting with gastric metastasis has never been reported. We now report a case of a 52-year-old woman with previously diagnosed lung metastasis having stomach metastasis from endometrial stromal sarcoma of the uterus.

9.Fatal Neutropenic Enterocolitis during Pegylated Interferon and Ribavirin Combination Therapy for Chronic Hepatitis C Virus Infection.

It is known that neutropenia caused by combination pegylated interferon plus ribavirin therapy for hepatitis C virus (HCV) infection is well tolerated and carries a negligible risk of infection. Neutropenic enterocolitis is encountered most frequently in patients with hemato-oncologic diseases who are undergoing intensive chemotherapy. However, little information exists regarding this life-threatening event in the setting of HCV therapy. We present here an unusual case of fatal neutropenic enterocolitis in a cirrhotic patient receiving combination therapy for HCV infection. This is the first report of a death from neutropenic enterocolitis associated with treatment for chronic HCV infection. The present case suggests that caution should be exercised when continuing HCV therapy in neutropenic patients with advanced fibrosis, and the decision to maintain such therapy should be balanced against the potential for serious adverse events.

10.Development of a Screening Kit for Early Diagnosis and Prevention of Wilson's Disease.

PURPOSE: Wilson's disease is an autosomal recessive disorder characterized by copper accumulation in the liver, brain, and other organs due to defected copper metabolism. The incidence of Wilson's disease is approximately one in 30,000 population in the world, more common than phenylketonuria in Korea. The early diagnosis or presymptomatic diagnosis of Wilson's disease is critical in order for them to live a normal life. However, unfortunately, there are no commercial kits available for Wilson's disease screening in the world yet. METHODS: We developed a mass-screening kit for the purpose of early diagnosis and prevention of Wilson's disease using sandwich ELISA method. This kit can handle a large number of samples at the same time by using filter paper as in newborn screening. Using the polyclonal or monoclonal anti-ceruloplasmin antibodies, this kit determines the plasma ceruloplasmin levels-one of the main markers for Wilson's disease. RESULTS: The plasma levels of the ceruloplasmin were considerably lower in the Wilson's disease (4.5+/-1.6 mg/dL) group compared to normal controls(22.1+/-1.4 mg/dL), sufficient to be used for mass screening. In addition, the results using this screening kit showed 100% positive and negative concordance rates with the test results obtained from immuno-turbidimetry analysis which is the currently used in most test centers for ceruloplasmin measurement in the serum or plasma after centrifugation. CONCLUSION: Taken together, we successfully developed a screening kit which is very effective for the early diagnosis and prevention of Wilson's disease. By using simple filter paper method for sample collection, this kit provides suitable mass screening. We suggest the screening for Wilson's disease at the age of 3-5 years.
