3.Effects of Exhaustive Exercise on Inflammatory, Apoptotic, and Antioxidative Signaling Pathways in Human Peripheral Blood Mononuclear Cells

In the present study, we investigated the effects of exhaustive exercise and recovery on inflammatory, pro-apoptotic, and anti-oxidative responses in human peripheral blood mononuclear cells (PBMCs). Sixteen volunteers participated in a guided physical activity program in which they were subjected to progressive exercise on the treadmill until they were exhausted followed by an 1-hour recovery period. Isolated human PBMCs were collected before exercise, immediately after exercise, and after 1-hour recovery. Exhaustive exercise induced expression of heme oxygenase-1 and glutamate cysteine ligase catalytic subunit and activation of NF-κB and NF-E2 related factor 2 (Nrf2). Apoptosis, as measured by activity and cleavage of caspase-3 and its substrate PARP also significantly increased. However, induction of redox signaling and the pro-apoptotic response fully returned to the baseline level during the 1-hour recovery period. On the other hand, COX-2 expression was continuously elevated after exercise cessation throughout the 1-hour recovery period. Taking all these findings into account, we conclude that exhaustive exercise transiently induces Nrf2-mediated antioxidant gene expression and eliminates damaged cells through apoptosis as part of an adaptive cytoprotective response against oxidative and inflammatory stress.

5.A Case of Targetoid Hemosiderotic Hemangioma.

Targetoid hemosiderotic hemangioma is a benign vascular tumor with a peculiar clinical presentation and histologic features sometimes difficult to differentiate from the patch stage of Kaposi's sarcoma, progressive lymphangioma, eosinophilic hemangioma, retiform hemangioendothelioma. We report a case of targetoid hemosiderotic hemangioma on upper back and right upper arm of a 28-year-old male. He has a 5mm × 5mm sized slightly raised violaceous papule surrounded by an ecchymotic halo on upper back and right upper arm. We took a biopsy of the lesion and identified it as targetoid hemosiderotic hemangioma. This is a case of targetoid hemosiderotic hemangioma with typical clinical and histopathological findings.

6.The Outcome of Cardiac Surgery in Low Birth Weight Infants.

BACKGROUND: It is known that low birth weight is a risk factor for poor outcome in cardiac surgery for many cardiac defects.We reviewed our recent surgical experiences on congenital heart defect (other than patent ductus arteriosus)in low birth wei ght babi es.Material and METHOD: From September 1994 to February 2001,31 consecutive infants weighing 2500 g or less underwent cardiac surgery with (OHS group n=12)or without cardiopulmonary bypass (CHS group n=19).A retrospective study was carried out to evaluate short-and intermediate-term outcome.Mean gestational age and age at operation were 36.9 weeks(range,32.3-42weeks)and 32.1days (range,0-87days)respectively.Mean body weight at birth and operation were 1972g (range,1100-2500g)and 2105g (range,1450-2500 g)respectively.There was no difference between the two groups in age and body weight.Defects included ventricular septal defect (VSD)(n=3),VSD with arch anomaly (n=2),total anomalous pulmonary venous return (n=2),transposition of the great arteries (TGA)(n=2),truncus arteriosus (n=2),and univentricular heart with cor triatriatum (n=1)in OHS group,and coarctation of aorta (n=7),tetralogy of Fallot (TOF)(n=3),TOF with pulmonary atresia (n=3), multiple muscular VSDs (n=1),double outlet right ventricle (n=1),pulmonary atresia with intact ventricular septum (n=2),tricuspid atresia (n=1),and TGA with multiple VSD (n=1)in CHS group.13 patients (41.9%)were intubated pre-operatively. RESULT: There were 4 early deaths (<30 days);1 (8.3%)in OHS group and 3 (15.8%)in non-OHS group.All these early deaths were related to the pulmonary artery banding(PAB).There was no operative mortality in infants undergoing complete repair and palliative operations other than PAB.Delayed sternal closure was required in 3 patients.Prolonged postoperative mechanical ventilation (>7days)was required in 7 patients (58.3%)in OHS and 7 (38.8%)in CHS group.Late mortality occurred in 3 patients,two of which were non-cardiac.A patient in OHS group was documented to have neurologic sequelae.All the survivors except two are in NYHA class I. CONCLUSION: Complete repair and palliative operations other than PAB can be performed in low birth weight infants with low operative mortality and an acceptable intermediate-term result.However,about a half of the patients required long-term postoperative mechanical ventilation.

9.The Benefits and Adverse Effects of Weight Loss.

Obesity is an independent risk factor for cardiovascular disease, hypertension, and diabetes and is associated with several other medical and psychological disorders. Weight loss is known to reduce these risks and to improve or resolve these cormorbid disorders and it is generally assumed that weight loss will improve the health of large people. But there is little research to support this. And no randomized controlled trials have been published to demonstrate that intentional weight loss in the obese actually reduces morbidity or increases longevity. In fact, the large majority of published epidemiological studies show that persons who experience a net loss of weight over time have shorter life spans than those whose body weights remain relatively stable. In addition, many persons who have intentionally lost weight report that they ultimately gain back some or all of the weight they originally lost. For some persons, this pattern of weight loss followed by regain will be repeated over many years of dieting and be exposed to the risk for eating disorders. These findings suggest that our current attitude or assume about weight loss should be challenged or reevaluated. With a culturally determined esthetical ideal and well-documented salutary effects of intentional weight loss on physiological risk factors, dichotomous thinking pattern about weight loss that weight loss is good one but weight gain even weight maintenance above normal is bad one has been prevailing. Therefore, intentional weight loss has been prevalent and intentional weight loss and it's related physical and psychosocial problems may become one of the most common health related issue in Korea. However, the efforts for weight control have been mainly focused on the weight loss rather than weight maintenance without serious consideration about long-term consequences of weight loss. To provide more balanced informations for weight loss and weight control, the authors reviewed the literatures concerning physical and psychosocial effects of weight loss, and effects of weight loss on morbidity and mortality. And we also reviewed the literatures about possible problems that may be resulted by various methods of weight control and discussed what is more appropriate approaches to the weight control.

10.3 cases of xanthogranuloma in adults.

We herein report three caes of xanthogranuloma occurring in aduls representing solitary nodule or multiple papules. Blood lipid substances such as cholesterol and triglyceride were normal and extracutaneous manifestatioiis were not found in all cases. Histopathorogic findings commonly revealed a granulomatous infiltrate c(intaining foam cells, foreign body gianf. cells and Touton giant cells as well as histiocytes and lymphocytes.
