1.Placement of Balloon-Expandable Intraluminal Stent in Recurrent Iliac Artery Stenosis.

BACKGROUND: Despite general agreement as to the safety and immediate success of percutaneous iliac angioplasty, its long-term efficacy remains subjects to debate and percutaneous transluminal angioplasty(PTA) induced dissection in the iliac artery may also lead to urgent surgery although frequency of this complication is not clearly stated in the literature. The relatively early result with the use of the Palmaz balloon-expandable intraluminal stent(BEIS) for treating atherosclerotic iliac artery and PTA-induced iliac artery dissections strongly suggest that Palmaz BEIS has its important role for limitations of PTA.It is generally accepted that indications for stent placement are (a) inadequate immediate postangioplasty response (b) restenosis after previous iliac balloon angioplasty, and (c) total iliac artery occlusion. METHODS: We report the successful implantation of Palmaz BEIS at right common iliac artery in a patients suffered from severe claudication. Previously the patients received PTA two times. First PTA resulted in intimal dissection and restenosis occurred after successful second PTA. RESULTS: After successful implantation of Palmaz BEIS, pressure gradients across the lesion and blood flow pattern were nearly normalized. The patients was discharged without symptom and was in good condition after 4 months follow-up. CONCLUSION: According to our this limited experience and other reports, stent placement in valuable adjunct in the management of iliac artery disease.

3.Desmoplastic Trichoepithelioma.

Desmoplastic trichoepithelioma, a tumor of hair follicle, is usually seen as solitary lesions. Its clinical configuration is hard and annular, asympromatic, white or yellowish and varied in size from 3 to 8 mm, but the appearance of these lesion is not so distinctive to hair follicle tumors that the diagnosis is estabilished by the histopathologic findings. Microscopical findings of desmoplastic trichoepithelioma consist of narrow strands of tumor cells, horn cyst, desmoplastic stroma. We present here a case of desmoplastic trichoepitheliorna, which develops as 6 mm sised papule on the left cheek in a 57-year-old woman.

4.Development and Evaluation of an Infection Prevention Self-care Application for Cancer Patients with Chemotherapy.

PURPOSE: The aim of this study is to develop and evaluate an infection prevention self-care application for cancer patients who have been discharged from hospital after receiving chemotherapy. METHODS: The app was developed through five stages: analysis, design, development, implementation and evaluation. The app's contents included infection prevention education, checking daily self-management, checking daily inquiry and information about the app. Cancer patients undergoing chemotherapy were asked to use the app and then a nonequivalent control group non-synchronized design was conducted to analyze the effect of the app on infection prevention self-care knowledge, self-care performance, infection occurrence and temperature. Twenty-two patients in the experimental group and twenty-four patients in the control group participated in this study. RESULTS: The self-care knowledge score (t=6.74, p<.001) and self-care performance score (t=13.44, p<.001) were statistically higher in the experimental group compared with the control group respectively. The infection occurrence was not different between the experimental and control groups. But temperature in the control group was statistically higher than in the experimental group (t=-2.39, p=.021). CONCLUSION: The infection prevention self-care application for cancer patients receiving chemotherapy was an effective intervention to decrease infections during home care.

5.A Case of Subcutaneous Fat Necrosis of the Newborn.

We report herein a case of subcutaneous fat necrosis in an 1-month-old abandoned female neonate who had subcutaneous nodule on the right cheek and linear cord-like subcutaneous nodule on the right upper arm which has been evident from the second post-delivery day. Hiatopathologic findings revealed massive subcutaneous fat necroais with infiltration of foreign body giant cells and inflammatory cells. Calcigm was also deposited but no needle shaped cleft was formed in the fat cells. The akin lesion resolved spontaneously after 2 months.

6.Solitary Keratoacanthoma Developing on an Acupuncture Site.

A 61-year-old woman, who had been treated by acupuncture on the glabellar region due to frontal headache ten days ago, had a rapidly growing tumor on that region. Histopathologically, the tumor was shown to be a central crater filled with eosinophilic keratin & a marginal but-tress formed by invagination of the epidermis. According to clinical & histopatholpgical findings, we can easily diagnose our case as solitary keratoacanthoma. As shown in our case, we think that acupuncture should be counted as one of the causative factors in the development of solitary keratoacanthoma.

8.The Significance of Palliative Nephrostomy.

Palliative open nephrostomy was done on 50 cases with advanced pelvic malignancy and benign conditions in the Department of Urology, Seoul National University Hospital during the period of March, 1979 to July, 1982. The results were as follows: 1. 20.5% (9/44) of our patients died without being discharged from the hospital after nephrostomy. 2. In advanced pelvic malignancy, only 52.6% of patients alive at 3 months and only 26.3% alive 6 months. 3. In advanced pelvic malignancy, the post-nephrostomy complication was 46.7% and the most common complication was various urinary problems. 4. The cause of death was multifactorical and impossible to define precisely. The nephrostomy complications were at least in part contributory to the deaths. 5. The terminal malignancy patients who might not expected to live six months and to lead a useful, relatively comfortable life outside the hospital after treatment could not have benefited from nephrostomy. Unavoidable cases, we recommended percutaneous nephrostomy and passage of ureteral catheters instead of open nephrostomy.

9.Two cases of extramammary Paget's disease: associated with adenocarcinoma with metastases.

We report two cases of extramammary Paget's disease associated with ad enocarcinoma with metascases. Histologically, Iaget cells in the epidermis and the hair follicles and adenocarcinoma in the der mis was observed in both cases. Of special interest was the demonstration of Eaget cells extending from the involved epidermis to the underlying dermis in case 2, There were metastatic lesioris of 3rd lumbar vertebra in case 1 and of the liver and lymph nodes in rase 2.

10.Cross - reactivity between pollens in patients sensitlzed to multiple pollens.

Objective : The aim of this study was to evaluate cross - reactivity between pollens in patients with strong skin reactivity to multiple pollens. METHODS: A pool of sera from 20 patients who showed strong skin responses( > or = 3+ ) to all three kinds of pollens ( tree, grass, and weed ) was used. Oak, ryegrass, mugwort, ragweed and hop Japanese pollens were chosen for the subject of study. The level and pattern of bound IgE to each pollen and cross - reactivity between pollens were investigated by ELISA and immunoblot experiments. RESULTS: In ELISA, the IgE of a pool of sera reacted to ryegrass and ragweed in 100%, to mugwort in 95%, to oak in 75%, and to hop Japanese in 65%, respectively. In inhibitory ELI - SA, more than 50% of bound IgE was mutually inhibited in oak vs ryegrass, oak vs mugwort, mugwort vs ryegrass and mugwort vs hop Japanese. In immunoblot, the IgE binding rate to major allergens of each pollen was 95% for hop Japanese, 75% for ragweed, 50% for ryegrass, 35% for mugwort, and 30% for oak, respectively. In inhibitory immunoblot, the mutually inhibited bound IgE could not be observed between major allergens of each pollen, but some minor allergens of oak pollen were mutually inhibited by ryegrass. CONCLUSION: Cross - reactivity between major allergens of oak, ryegrass, mugwort, ragweed and hop Japanese pollens was not observed at all, but some minor allergens of oak pollen cross-reacted with ryegrass and mugwort, and so did those of mugwort pollen with ryegrass and hop Japanese.
