Trichoepithelioma. so named by Jarisch(1894), is a rare benign neoplasm of the skin on face and eyelid. The solitary type which is the most usual form seen in the eyelid occurs mainly in adult, on the other hand multiple type begin to appear at adolescence or puberty as a dominantly inherited condition. A 21 year-old korean female was found to have trichoepithelioma in her left lower eyelid mangm.

2.Average Presbyopic Additions for Various Age Groups in Korean.

As a gradual loss of elasticity of lens with advancing age, the power of accomodation becomes progressively smaller, and eventually the near-point has receded beyond the distance which causes difficulty in seeing near object clearly. This condition is known as presbyopia. Presbyopia is treated by means of convex lenses added to the distance correction. The power of the lens required for clear vision for near work varies with an individual's habits, age, occupation, length of arms, and accustomed distance of doing near work. A rule of thumb method for prescribing presbyopic additions has been presented respectively by Lyle et al(May and Worth's manual of diseases of the eye), and by Sloane(Approximate average adds for various age groups). Average presbyopic additions had been investigated in 869 healthy Koreans with normal visual acuity over age 41 from November 1, 1969 to October 31, 1981. The results were as follows: 1. The rate of the patients for whom presbyopic additions were not ordered; between ages of 41 and 45: 30.4% of emmetropia, 51. 5% of myopic refractive error, 10.4% of hyperopic refractive error, 20.0% of others, between ages of 46 and 50: 5. 1 % of myopic refractive error, 2.1% of hyperopic refractive error. 2. The average presbyopic additions for various age groups based upon the 95% statistical reliability were as follows; 41 to 45: + 1.00D or + 1.25D, 46 to 50: + 1.50D or + 1.75D, 51 to 55: + 2.00D, 56 to 60: + 2.25D or + 2.50D, 61 to 65: + 2.50D 66 to 70: + 2.75D.

4.A Case of Acute Interstitial Keratitis with Congenital Syphilis.

Since the time of Jonathan Hutchinson(1858~63), it has been known that over 90% of cases of diffuse interstitial keratitis occur in syphilitic patients. Interstitial keratitis is due to acquired syphilis in 4% of cases and the other 6% of cases due to tuberculosis, sarcoid, trypanosomiasis, onchocerciasis and other rare infections. After the introduction of penicillin by Mahoney et al. in 1943 for syphilis therapy, syphilis epidemy was sharply decreased throughout the world. However, during the middle 1950's the incidence of syphilis began to increase throughout the world. In Korea. the syphilis incidence has been increased since around 1963-1965 in parallel with the world trends(Kim and Lew 1968). A 6 year old Korean girl has been found to have bilateral acute interstitial keratitis and a positive serological test for syphilis.

5.Atypical Pseudomonas Keratitis.

Pseudomonas aeruginosa is the most common and the most severe Gram-negative corneal pathogen. Pseudomonas keratitis often arises from the use of a contaminated fluorescein solution or from contaminated medications used either to examine or to treat a corneal abrasion or injury. Pseudomonas keratitis is an acute rapidly progressive corneal ulcer with greenish pus and hypopyon. A 49 year old Korean woman was found to have pseudomonas keratitis whose clinical features. were different from the classic pattern of pseudomonas keratitis.

6.Antimicrobial Prophylaxis Using a 2nd Generation Cephalosporin after Laparoscopic Colorectal Resection: A Randomized Trial of 1-day vs. 3-day.

PURPOSE: We performed this study to compare 1-day group using a 2nd generation cephalsporin with 3-day group for evaluating hospital acquired infection. METHODS: The patients underwent laparoscopic colorectal surgery at Korea University Medical Center Anam Hospital, from August, 2007 to June, 2008. They were randomly allocated to 2 groups: 1-day or 3-day group. A 2nd generation cephalosporin was administered within 1 hour before surgery with 12-hour intervals. In cases of suspected infection, further studies were done to identify infection. RESULTS: The study included 154 patients (1-day group - 78, vs. 3-day group - 76). No differences were noted between the 2 groups in age, body mass index, smoking, diabetes mellitus, corticosteroid-use were noted. Gender (P=0.011) and mean operative time (P=0.047) between the 2 groups were different. The preventive rates of infection were 87.18% in the 1-day group compared with 82.89% in the 3-day group (P=0.456). CONCLUSION: Our prospective randomized control study concluded that there were no differences between the 1-day and 3-day group in hospital acquired infection. We could come to the conclusion that 1-day antimicrobial agent in laparoscopic colorectal surgery would be adequate in preventing infection.

7.Culturing at Low Cell Density Delays Cellular Senescence of Human Bone Marrow-Derived Mesenchymal Stem Cells in Long-Term Cultures

Background and Objectives:
Mesenchymal stem cells (MSCs) have immense therapeutic potential for treating intractable and immune diseases. They also have applications in regenerative medicine in which distinct treatments do not exist. Thus, MSCs are gaining attention as important raw materials in the field of cell therapy. Importantly, the number of MSCs in the bone marrow is limited and they are present only in small quantities. Therefore, mass production of MSCs through long-term culture is necessary to use them in cell therapy. However, MSCs undergo cellular senescence through repeated passages during mass production. In this study, we explored methods to prolong the limited lifetime of MSCs by culturing them with different seeding densities.
Methods:
and Results: We observed that in long-term cultures, low-density (LD, 50 cells/cm2) MSCs showed higher population doubling level, leading to greater fold increase, than high-density (HD, 4,000 cells/cm2) MSCs. LD-MSCs suppressed the expression of aging-related genes. We also showed that reactive oxygen species (ROS) were decreased in LD-MSCs compared to that in HD-MSCs. Further, proliferation potential increased when ROS were inhibited in HD-MSCs.
Conclusions
The results in this study suggest that MSC senescence can be delayed and that life span can be extended by controlling cell density in vitro. These results can be used as important data for the mass production of stem cell therapeutic products.
8.Plate and Screw Removal after Orthognathic Surgery, under Intravenous Sedation with Dexmedetomidine and Pethidine

Compared with the preoperative anxiety score, the intraoperative anxiety score was decreased.CONCLUSION: In this study, we found cardiovascular and respiratory stability in intravenous sedation using dexmedetomidine with pethidine, in plate and screw removal, after orthognathic surgery. Furthemore, intravenous sedation using dexmedetomidine with pethidine shows adequate analgesic and sedative effects.]]>

9.Relationship between Degree of Aortic Regurgitation Graded by 2-D Color Doppler Echocardiography and Diastolic Fluttering of Anterior Mitral Leaflet.

In 25 aortic regurgitation patients relationship between degree of aortic regurgitation graded by 2-D color Doppler echocardiography and diastolic fluttering of anterior mitral leaflet on M-mode echocardiography was evaluated. The results were that all 13 aortic regurgitation patients, not associated with mitral stenosis, showed diastolic fluttering of anterior mitral leaflet; but only 3 patients among 12 patients (25%), associated with mitral stenosis, had characteristic mitral fluttering. On the whole, regardless of associated mitral stenosis, the sensitivity was 64%. In severe aortic regurgitation (Grade 3, 4 group) 12/13 patients revealed diastolic fluttering of anterior mitral leaflet (sensitivity of 92%); in mild to moderate aortic regurgitation (Grade 1, 2 group), only 4/12 patients (sensitivity of 33%). In conclusion, 2-D color Doppler echocardiography is a very useful method for detection of aortic regurgitation without difficulty. And in aortic regurgitation, not associated with mitral stenosis, fine fluttering of anterior mitral leaflet is a very sensitive sign of existence of aortic regurgitation, though there may be some false positive findings in normal individuals.

10.Chondroid Syringoma: Clinical, Pathological, and Immunohistochemical Study of 23 Cases.

BACKGROUND: Chondroid syringoma is a rare benign tumor composed of epithelial and stromal elements. Studies of chondroid syringoma are lacking in Korea and controversy still exists regarding the stromal cell origins. OBJECTIVE: To investigate the clinicopathological and immunohistochemical characteristics of chondroid syringoma in Korea. METHODS: We included 23 patients who were diagnosed with chondroid syringoma. RESULTS: There was a predilection for males (M : F=2.3 : 1), and the mean patient age was 50.4+/-15.4 years (range 22~78 years), peaking in the forties. Of the 23 cases, 22 (95.7%) had it on the head and neck. There were 21 apocrine type cases and 2 eccrine type cases. Apocrine type cases showed decapitation secretion as well as follicular and sebaceous differentiations. Myxohyaline matrix was present in all cases, and in some cases, chondroid, lipomatous, and osseous metaplasias were also observed. In apocrine type cases, inner cell layers displaying tubuloglandular structures expressed cytokeratin, carcino-embryonic antigen, and epithelial membrane antigen, while outer cell layers expressed vimentin, S-100 protein, neuron-specific enolase, and smooth muscle actin. Stromal cell immunophenotypes were similar to those of the outer layer. CONCLUSION: Chondroid syringoma was most commonly observed on the head and neck in middle-aged males. The apocrine type was far more common than the eccrine type and showed various differentiating characteristics. The matrix may contain myxohyaline stroma with alterations, including chondroid, lipomatous, and osseous metaplasias. Our immunohistochemical study suggests that myoepithelial cells play an important role in the histogenesis of chondroid syringoma and matrix production.
