2.Eosinophils, Lymphocytes and Mast Cells in the Nasal Polyps and the Mechanism of Polyp Formation

Objective To investigate the infiltration of inflammatory cells and the pathogenesis of nasal polyps. Methods the nasal polyps were obtained from 42 patients undergoing nasal operations. The tissues from each patient were stained with HE and Toluidine Blue for eosiophils, lymphocyte, plasma cell and mast cell and observed under the light microscope. A mean number of the above cells were counted. All data were analyzed with the Ttest. Results Eosinophils were found predominantly in 12 polyps(28.57%). Lymphocyte infiltration was shown in the other 30 polyps (71.43%). Furthermore, many mast cells with only a few eosinophils were found in the lymphocyte infiltration polyps; the number of mast cells in these polyps were much more than those in eosinophil accumulation polyps with a very significant difference(P<0.01). Conclusion The accumulation of eosinophils in the nasal polyps does not appear to be related to the presence of mast cells and allergy. The disorder of immunity may play an important role in the formation of nasal polyps.
3.Study on optineurin gene polymorphism in Chinese patients with primary open angle glaucoma

Background Primary open angle glaucoma(POAG) is a common type of glaucoma.It has been well known that a lot of factors are associated with the pathogenesis of POAG,but genetic factor plays a critical role.Optineurin (OPTN)gene is the second confirmed POAG-relevant gene,and screening its mutation in the population contribute to the deeply understanding of the pathogenesis of POAG.Objective The present study was to investigate the association between sequence variants of OPTN gene and POAG in Chinese patients.Methods DNA was isolated from peripheral blood of 100 POAG patients and 60 cataract individuals.The coding exons of OPTN gene were amplified by PCR.PCR products were then sequenced directly to assay the variants and contrasted to original sequence in GenBank.This study was approved by the Ethical Committee of Shenzhen Eye Hospital.All the subjects signed the written inform consent.Results A case-controlled study was designed.The mean intraocular pressure (IOP)of the POAG patients was (29.0±6.5)mmHg,and that of the cataract patients was (13.7 ±2.4)mmHg.Variant of synonymous coding T34T was found in 60 POAG patients.Genetic type frequencies of AA,GA and GG were 10%,50% and 40% in the POAG patients,and those of cataract patients were 0,25% and 75% respectively,showing significant difference between them (x2 =20.416,P =0.000).The allele frequencies of A and G were 35% and 65% in the POAG patients,and those of cataract patients were 12.5% and 87.5%,with a statistically significant difference (x2 = 19.464,P =0.000).The sequence changes of non-synonymous coding variants (M98K,691-692insA G,R545Q,H486R) were also found in both POAG and cataract patients,but no significant difference was seen in the genetype and allele frequencies between two groups (P>0.05).Conclusions No obvious association of OPTN gene variant with POAG is verified.The variant of T34T maybe increase the risk of POAG.
4.Correlation analysis on plasma D-dimer level with deep venous thrombosis after spinal surgery.

OBJECTIVETo analyze the relation of plasma D-dimer levels and incidence of deep venous thrombosis after spinal surgery.
METHODSThe clinical data of 63 patients underwent spinal surgery from October 2009 to October 2010 were retrospective analyzed. There were 40 males and 23 females with an average age of 48 years old(21 to 76) in operation. Operation levels of 15 cases were in cervical vertebrae, 4 cases were in thoracic vertebrae,and 44 cases were in lumbar vertebrae. Thirty patients with spinal fracture were caused by trauma and 33 patients without trauma, 11 patients combined with nerve injury. The patients were divided into two groups according to plasma D-dimer levels, more than or equal to 500 microg/L was D-dimer positive group and less than 500 microg/L was D-dimer negative group. Venous blood of all patients early morning with empty stomach were testd on admission, and at 2 h, 1 d, 2 d, 3 d, 4 d, 6 d, 8 d, 10 d, 15 d after operation,respectively.
RESULTSThere was no statistically significant differences in sex, operative segments, implants, operative posture, age, bleed volume, body weight, peroperative D-dimer levels between two groups. After operation, plasma D-dimer of 19 patients were more than or equal to 500 microg/L, with persistent or progressive increasing. Two cases occurred deep venous thrombosis in D-dimer positive group, they respectively were found at 3 days and 8 days after operation. Both of them underwent posterior decompression and internal fixation. However,no deep venous thrombosis was found in D-dimer negative group.
CONCLUSIONPostoperative D-dimer assay can effective predict deep venous thrombosis occurrence. D-dimer level more than or equal to 500 microg/L will be considered as a risk factor for deep venous thrombosis after spinal surgery.

7.Electrophysiological characterization of long QT syndrome associated mutations V630A and N633S.

OBJECTIVETo identify the electrophysiological properties of long-QT syndrome (LQTS) associated missense mutations in the outer mouth of the HERG potassium channel in vitro.
METHODSMutations V630A and N633S were constructed by Megaprimer PCR method and cRNA were produced by T7 RNA polymerase. The electrophysiological properties of the mutation were investigated in the Xenopus oocyte heterologous expression system.
RESULTSCoexpression of mutant and wild-type HERG subunits caused a dominant-negative effect, and the currents were significantly decreased. Compared with wild-type HERG channels, V630A and N633S mutations were related to decreased time constants for inactivation for V630A/WT and N633S/WT at all potentials, reduced slope conductance and the voltage dependence of steady-state inactivation was shifted to negative potentials for V630A/WT and N633S/WT.
CONCLUSIONPresent study shows that LQTS associated missense mutations located in the outer mouth of HERG cause a dominant-negative effect and alterations in steady-state voltage dependence of channel gating of heteromultimeric channels suggesting a reduction in expressional current might be one of the pathophysiologic mechanisms of LQTS.

8.A meta-analysis on clinical effect of acupuncture for Parkinson disease

Objective To evaluate the clinical effect of acupuncture on Parkison disease (PD) with systematic review in view of evidence-based medicine (EBM).Methods Articles on clinical effect of acupuncture on Parkinson disease published from the database-established year to May of 2017 we searched from China National Knowledge Internet (CNKI),Wanfang,VIP,Chinese Biomedical Literature Database (CBM),PubMed,and Cochrane Library databases without language limitation.Quality evaluation and data extraction were carried out according to the tool for assessing risk of bias provided by the Cochrane Handbook for Systematic Reviews of Interventions (Version 5.1.0).All statistical analyses were performed with Reviewer Manager Software (Version 5.3;Cochrane Collaboration,Oxford,UK).Assessments were performed with the total effective rate,the scores of each scale (the unified Parkinson disease rating scales-UPDRS and the Webster scale),and the improvement of different clinical manifestation.Results In all,12 randomized controlled trials (RCTs) met our inclusion criterion,a total of 892 patients,including 468 cases in the experimental group (acupunctuer with or without medicine) and 424 cases in the control group (medicine only).Meta-analysis showed favorable results for the experimental group compared to control group in the total effective rate,the total scales of UPDRS and the modified Webster scale [OR =2.16,95% CI (1.57,2.97),P<0.01;OR =7.20,95% CI(4.02,10.37),P<0.01;OR=3.33,95% CI(2.13,4.53),P <0.01].The experimental group was effective in relieving partial symptoms of PD such as rigidity,postural,gait,bradykinesia compared to the control group,while there was no significant difference in tremor at rest and sit-stand up movements (P > 0.05).Conclusions Acupuncture had certain clinical effect on Parkinson disease,it can relieve the clinical symptoms of Parkinson disease to some extent,and postpone the progression of PD,which improves the quality of life of PD patients.Acupuncture can be recommended as a combination treatment for Parkinson disease.

The delta-opioid receptor (DOR),which is a classic analgesic drug widely existed in the central system and peripheral system,is a kind of inhibitory G-protein coupled receptor with seven transmembrane regions.In addition to pain modulation,the opioid receptors are involved in various physiological and pathological activities through gene or cytokines.This review addresses the influence and possible mechanisms of the delta-opioid receptor in ischemic brain injury,analgesia,anti-anxiety and depression,learning and memory,and Parkinson's disease.
10.Safety and efficacy comparison of myocardial contrast enhancement-guided and angio-pressure-guided transcoronary ablation of septal hypertrophy for patients with hypertrophic obstructive cardiomyopathy.

OBJECTIVETo compare the safety and efficacy of myocardial contrast enhancement (MCE)-guided and angio-pressure (AP)-guided transcoronary ablation of septal hypertrophy (TASH) for patients with hypertrophic obstructive cardiomyopathy (HOCM).
METHODSTASH was performed under MCE-guide (n = 47, group I) or AP-guide (n = 25, group II) for drug-refractory patients with HOCM. Myocardial perfusion imaging (MPI) data as well as other clinical data were compared.
RESULTSTASH both under MCE-guide or AP-guide resulted in similar and significant reduction of left ventricular outflow tract gradient (PG) and associated with significant symptom improvement (all P < 0.001). Dosage of ethanol use, peak-level of CK-MB and ablated myocardial area and incidence of arrhythmia were also similar between the two groups.Similar left ventricular/atrial dimension changes post TASH were observed in the 2 groups during follow-up. However, the first selected septal vessels were changed under MCE in 6 patients.
CONCLUSIONSOur data demonstrated that the MCE-guided TASH was not superior to AP-guided TASH in safety and efficacy. However, MCE-guided TASH can avoid the misplace of ethanol to avoid innocent myocardial ablation.
