1.Two Cases of Penile Carcinoma Transformed from Giant Condyloma Acuminata.

The incidence of giant condyloma acuminata of the penis is extremely rare. Although they are considered clinically malignant, the real possibility of malignant changes from them are controversial even among the investigators but cannot be entirely overlooked. Therefore, attention must be focused on this fact, in our conclusion. Two cases of malignancy of the penis which have apparently been transformed from giant condyloma acuminata of the penis are herein presented with review of related literatures. Case I: Sixty nine year old male patient complained of a fungating mass on the sulcus coronaris of the penis with ulceration and bleeding from the mass. Biopsy confirmed giant condyloma acuminata and subsequently circumcision and electrocoagulation were performed. Pathological specimen taken after 4 months because of the recurrence of the mass on the same region showed squamous cell carcinoma of the penis and amputation of penis was performed. Case II: Sixty eight old male patient complained of a growing mass and pain on glans penis. This patient also underwent circumcision and electrocoagulation according to the biopsy finding of giant condyloma acuminata. But 1 year later with recurrence of the mass on the same region, the pathological finding revealed squamous cell carcinoma of penis.

2.Effect of intravenous injection of parecoxib before operation on efficacy of postoperative analgesia in patients undergoing thoracic surgery

Objective To investigate the effect of intravenous injection of parecoxib before operation on the efficacy of postoperative analgesia in patients undergoiag thoracic surgery.Methods Ninety ASA Ⅰ -Ⅲ patients,aged 38-76 yr,weighing 44-82 kg,scheduled for thoracic surgery under general anesthesia combined with thoracic epidural block,were randomly divided into 3 groups(n = 30 each): group A,B and C.Epidttral anesthesia was performed at the T6-7 interspace before general anesthesia. Anesthesia was induced with sufentanyl 0.4 μg/kg,vecuronium 0.12 mg/kg and propofol 1.5-2.0 mg/kg.Double-lumen bronchial tube was inserted and the patients were mechanically ventilated.Group A received iv injection of normal saline 2 ml 30 min before skin incision.Group B received iv parecoxib sodium 40 mg after extubation.Group C received iv parecoxib sodium 40 mg 30 min before skin incision.Anesthesia was maintained with propofol,vecuronium,sufentanyl and lidocaine.The patients received patient-controlled epidural analgesia(PCEA)with ropivacaine and 0.5 μg/ml sufentanil after surgery.The VAS score was maintained≤ 3.The comfort level was evaluated with Bruggrmann comfort scale(BCS)at 4,12,24 and 48 h after operation.The consumption of sufentanil during operation and within 48 h after operation was recorded.The adverse reactions were also recorded.Results Compared with group A,the consumption of sufentanil was significantly decreased in group B and C,the BCS score was significantly increased at 4 h after operation in group B,and the BCS score was significantly increased at each time point and the consumption of sufentanil during operation was significantly decreased in group C(P < 0.05).The BCS score was significantly higher,and the consumption of sufentanil during operation and within 48 h after operation was significantly lower in group C than in group B(P < 0.05).There was no significance difference in the adverse reactions among the 3 groups(P > 0.05).Conclusion Intravenous injection of parecoxib 40 mg before operation reduces the perioperative sufentanil consumption in patients undergoing thoracic surgery.
3.Individualized lung protective ventilation regimen based on pressure-volume curve for one lung ventilation in patients undergoing thoracic surgery

ObjectiveTo evaluate the effectiveness of setting PEEP and tidal volume (VT ) according to pressure-volume (P-V) curve during one lung ventilation (OLV) in patients undergoing thoracic surgery.Methods Twenty-five ASA Ⅰ or Ⅱ patients of both sexes aged 44-64 yr weighing 57-75 kg undergoing lobectomy under general anesthesia were enrolled in this study.Double-lumen tube was inserted.Correct positioning was verified by flberoptic bronchoscopy.The patients were mechanically ventilated.P-V curve was measured by SSS system during OLV.Lower inflection point (LIP)and upper inflection point (UIP) were determined.The pressure at LIP (PLIP) and volume at UIP (VUIP) were measured.Bilateral lungs were ventilated for 30 min (T0) at first before OLV was started.PEEP was set at PLIP + 0.196 kPa and VT was set at VUIP,and the patients were ventilated for 30 min (T1).VT was then reduced to 80% of VUIP.OLV was performed for another 30 min (T2).VT was then further reduced to 60% of VUIP and the patients were ventilated for 30 min (T3).PEr CO2 was maintained at 4.67-6.00 kPa.Arterial blood and central venous samples were taken at T0-3.Blood gas analysis was performed.Qs/Qt was calculated.MAP,HR,CVP,peak airway pressure (Peak),airway resistance (Rsw) and lung compliance (CL) were measured and recorded at T0-3.ResultsHR,Ppeakk,Rsw and Qs/Qt were significantly increased while CL and PaO2 decreased at T1-3,CVP was significantly increased at T1.2 and MAP and PaCO2 were increased at T3 as compared with the baseline values at T0.Ppeak and Rsw were significantly decreased at T2.3 and PaO2 was significantly increased while Qs/Qt decreased at T2,CVP was decreased,MAP and PaO2 were increased at T3 as compared with the values at T1.ConclusionsMechanical ventilation with VT set at 80% of VUIPandPEEPatPUIP+0.196kPa provides best ventilatory efficacy for OLV in terms of PaO2 and hemodynamics.

Objective To study the chemical constituents of the South China Sea black coral Amtipathes dichotoma. Methods Compounds were isolated and purified by repeated column chromatography on silica gel and their structures were identified by spectral analyses and compared with literatures. Results Ten compounds were obtained from A. dichotoma and characterized as stigma-7,22-dien-3?,5?,6?-triol (Ⅰ),5,8-epidioxycampesta-6,22-dien-3-ol (Ⅱ),cholesterol (Ⅲ),(2S,3R,4E,8E)-2-N-(2'-hydroxyhexadecanoyl) hexadecasphinga-4,8-diene (Ⅳ),(2S,3R,4E)-2-N-(2'-hydroxytetracosanoyl) octadecasphinga-4-ene (Ⅴ),batyl alcohol (Ⅵ),thymine (Ⅶ),thymidine (Ⅷ),uracil (Ⅸ),and guanine (Ⅹ). Conclusion All compounds are obtained from Amtipathes dichotoma. for the first time.

6.Wuling capsule played an assistant role in primary prevention of post-stroke depression: a clinical research.

OBJECTIVETo observe the primary prevention role of Wuling Capsule (WC) on poststroke depression (PSD) patients.
METHODSAcute stroke patients were recruited and randomized into 2 groups by stratification, 55 in each group. All patients received same routine treatment of cardiovascular diseases. Patients in the experimental group additionally took WC (0.33 g each pill), 3 pills per day, three times per day; while those in the control group additionally took placebos, 3 pills per day, three times per day. Two weeks consisted of one therapeutic course. The diagnosis of PSD was performed once every other week. Those in accordance with PSD diagnosis discontinued any drug therapy. Those not in accordance with PSD diagnosis continued the drug therapy for 1-12 therapeutic course(s) (in total of 6 months). If they were still not in accordance with PSD diagnosis, then they discontinued the drug therapy. The morbidity of PSD, the average time of depression occurrence, Hamilton depression rating scale (HAMD) score, and adverse reactions were observed.
RESULTSThe 1-, 3-, and 6-month morbidity of PSD was 8%, 16%, and 34% in the experimental group, while they were 19.6%, 29.4%, and 54.9% in the control group. The occurrence rate was lower in the experimental group than in the control group. Besides, there was statistical difference in the 6-month occurrence rate between the two groups (chi2 = 4.465, P < 0.05). The average time of PSD occurrence was longer in the experimental group than in the control group (14.96 +/- 8.31 weeks vs. 9.36 +/- 6.06 weeks; t=6.762, P < 0.05). The HAMD score at the PSD occurrence was 11.96 +/- 2.14 in the experimental group, lower than that of the control group (14.57 +/- 4.24), showing statistical difference (t=5.641, P < 0.05).
CONCLUSIONWC was superior to the placebos in lowering the incidence of PSD, delaying the occurrence time of PSD, attenuating the depression degree of PSD, and had certain preventive effect on the incidence of PSD.
