Medicine is intended for human beings and the crowd.The constant variation of the record of diseases and the continuous emergence of the technical means of diagnosis and intervening measures to promote health make it urgent to have the idea of life-long study and cultivating the creative talents of medicine.Therefore the medical higher learning must renew the idea of running schools,change the model and goals of cultivation,push the teaching reform forward including paying attention to pushing the curriculum system,teaching methods and the reform of teaching management system,emphasize creative education to create the condition for cultivation of the creative talents.
3.Clinic and microbial characteristic of fever along with agranulocytosis in patients with malignant hematop-athy

Objective To investigate the clinic and microbial characteristic of fever along with agranulocy-tosis in patients with malignant hematopathy. Methods Retrospective study was carried out on patients with malig-nant hematopathy who developed agranulocytosis during hospitalization. Results A total of 512 febrile events occurred. Duration of agranulocytosis,severe agranulocytosis,poor physical condition,previous exposure to broad-spectium antibiotics and central venous catheterization were correlated with higher incidence of febrile event. There are 328 cases(64.06%)with definite infected sites and 184 cases(35.94%)with fever of unknown origin(FUO). Gram-negative bacterium was most common(64.35%),followed by Gram-positive bacterium(23.61%)and fungus (12.04%). 57 cases resistant bacteria were detected. Mortality incidence resulting from pulmonary infection was higher than that from FUO(P < 0.005). No significance was found between pulmonary infection and blood stream infection(P>0.25)or between blood stream infection and FUO(P>0.1). Conclusions Many factors were corre-lated with incidence of fever along with agranulocytosis in patients with malignant hematopathy. Gram-negative bacte-rium was most common pathogenic microorganism,followed by Gram-positive bacterium,all of which manifested high drug resistance rate. Understanding the epidemiological trend will be beneficial for clinic treatment.
4.Changes of ICAM-1 expression of myocardial ischemic reperfusion injury in old rats and cardiac protective effect of Esmolol

ObjectiveTo investigate the changes of expression of intercellular adhesion molecule 1 (ICAM-1) on myocardial ischemic reperfusion injury(IRI) in old rats and the cardiac protective effect of Esmolol(ES). Methods116 rats were divided into three groups: IR group, IR+ES group and Sham group. The ischemic samples were observed in ischemia and 3,6,12,24 hours after IR. The myocardial levels of expression of ICAM-1 mRNA were evaluated by method of IN Situ Hybridization and the protein were evaluated by immunocytochemistry. The content of infiltration of polymorphonuclear neutrophils(PMNs), malomdialdehyde(MDA), superoxides dismutase(SOD) and the myocardial infarction area were measured too. ResultsAfter IR, myocardial levels of expression of ICAM-1 mRNA, protein,MDA and PMNs were increased significantly; SOD was decreased significantly. Between the levels of expression of ICAM-1 protein and PMNs, infarction area of myocardium, a close correlation were observed(P<0.05). In IR+ES group, all of the indicators were increased after IR, but the levels of increase in IR+ES group were more significantly modification as compared with IR group(P<0.05-0.01).Conclusions The findings indicate that PMNs could induce myocardial IRI after IR, which result from the ICAM-1 mediated PMNs adhesion.ES is able to decrease myocardial IRI by blocking the expression of ICAM-1 partially.
5.Research on effect of static elastography on the elastic modulus of normal breast tissue

Objective:To investigate the effects of static ultrasound elastography on the elastic modulus of normal breast tissue.Methods: (1)Fifteen cases of normal breasts was tested by static ultrasound elastography at the same locations. Intercepted two pictures each time with one stress-free and the other compressed the maximum amount. For these two cases we measured the breast thickness and calculated the minimum stretch ratios(λ). (2)Fifty breasts with no breast nodules were prospectively enrolled in the study. Intercepted two pictures each time with one stress-free and the other compressed to the minimum stretch ratios(λ). For these two cases we measured the Elastic modulus (E0, Em, kPa). Calculate the elastic modulus change ratio (Rm/0).Results: (1)The minimum stretch ratio caused by static ultrasound elastography was 0.74±0.07. (2)The elastic modulus change ratio of static ultrasound elastography was 4.38±1.72. Conclusion: The normal operation of static ultrasound elastography caused 74 percent deformation in normal breast tissue. There was 4.38 times elastic modular increasing caused by 74 percent deformation in normal breast tissue.
6.THE EFFECT OF POLYACTIA B ON TUMOR-INFILTRATING IMMUNOCOMPETENT CELLS WITIHN MURINE S_(180) SARCOMA

Polyactin B (Pb) has been proved to have the effect of considerable tumor suppression. Recently,we used murine S_(180) Sarcoma as a model and observed the effect of Pb on the tumor—infiltrating immunocompetent cells, and also the infiltrating neutrophils and the alteration of small blood vessels within the tumor tissue. Compared with control: in Pb—treated group, there were more L_3T_4~+and Lyt_2~+ lymphocytes infiltrating in the periphery of the tumor ,and also within the tumor. In addition, the tumors had more prominant hyperemia, micro—thrombosis and neutrophil infiltration around the necrotic areas. The present findings suggest that the tumor—suppression effect of Pb might be mediated through TNF produced by immunocompetent cells.
7.Diagnosis and treatment of hydatidosis of retroperitoneum and bladder (Report of 9 cases)

Objective To investigate the diagnosis and treatment of Hydatidosis of post-peritoneum and bladder.Methods Nine cases were reported. Among them,one was epinephros-hydatid cyst,two renal hydated cyst,two hydatidosis of post-peritoneum in ileopelvic, and four bladder hydrated cyst. All cases had history of contacting with dog or sheep.Results All of the nine cases were cured by operation. Conclusions In percussion of the lower abdominal mass, hydrated cyst vibration, a specific sign, can usually be felt; and the mass do not disappear after urination. Besides, E-immune Test for echinococcus, B-ultrasonography, and CT are very useful diagnostic methods. Therefore, excision of the cyst still is the only effective treatment for hydatidosis.
8.The Reliability,Validity of PANSS and its Implication

Objective:To study the reliability, validity of PANSS(Chinese version) and its implication.Methods:105 schizophrenic patients were recruited and were evaluated using PANSS. The principle component analysis and reliability analysis were used to study the factor structure and internal reliability, homogeneity among items.Results:Factor analysis resulted in a five-factor: negative, positive, excitement-hostile, anxiety/depression, cognitive defect. The internal consistency reliability was 0.87 (Cronbach ?). The internal consistency reliability of the 5 dimensions ranged from 0.74~0.90.Conclusion:The structure, validity and reliability of PANSS (Chinese version) are acceptable. PANSS can be used to assess the symptoms of schizophrenia in Chinese patients.
10.Analysis of intraocular lens power calculation for cataract patients with high myopia and posterior scleral staphyloma

Background The accurate calculation of intraocular lens (IOL) power is essential for attaining the desired refractive outcome after cataract surgery,especially for patients with high myopia and posterior scleral staphyloma.Objective This study was to evaluate the clinical feasibility of IOL Master compared with contact A-scan in cataract patients with high myopia and posterior scleral staphyloma,then compare the accuracy of different IOL power calculation formulas.Methods This was a prospective case control clinical research.Fourty-one eyes with age-related cataract of 28 patients underwent phacoemulsification with monofocal foldable IOL implantation in Tianjin Medical University Eye Hospital were involved,who were all high myopia with posterior scleral staphyloma.Preoperative measurement was measured with IOL Master as well as with contact A-scan and manual keratometry.IOL power was calculated according to the SRK-Ⅱ,SRK-T,Haigis,Hoffer Q,Holladay 1 formulas.The refractive outcome was followed-up 3 months after operation.Results The difference was significant between the 2 methods in axial length (AL) and anterior chamber depth (ACD) measurement (P =0.005,0.000) ; In corneal curvature measurement,there was no significant difference between them (P =0.398).When mean absolute refractive error (MAE) was divided by ±1.00 D,The SRK/T and Haigis formula performed better than other formulas measured by IOL Master;The Holladay 1,Hoffer Q and Haigis formula performed better than other formulas measured by contact A-scan combined with manual keratometry,respectively.Conclusions For cataract patients with high myopia and posterior scleral staphyloma,SRK/T and Haigis formula were recommended when employing IOL Master; whereas when using contact A-scan combined with manual keratometry,we prefer Holladay 1,Hoffer Q or Haigis formula.
