1.The application effect of bedside blinding method of active indwelling of nasojejunal tube combined with the nasogastric tube gastrointestinal decompression for patients with severe stroke

Objective To investigate the effects of bedside blinding method of active indwelling of nasojejunal tube combined with the nasogastric tube gastrointestinal decompression for patients with severe stroke.Methods 50 patients with severe stroke were selected and divided into two groups by using random number tables,which are the observation group and the control group,with 25 cases in each group.The patients in the observation group were treated with bedside blinding method of active indwelling of nasojejunal tube combined with the nasogastric tube gastrointestinal decompression,while the patients in the control group simply received bedside indwelling of nasogastric tube.The enteral nutritional goal -rate of target feeding volume on the 7th day and the 14th day after admission and trace the incidence of gastric stasis,the reflux and aspiration,the aspiration pneumonia in the patients of the two groups within 14 days and the situation of the days of mechanical ventilation,the days in ICU and the 30 -day mortality of patients were compared in the two groups.Results The enteral nutritional goal -rate of target feeding volume on the 7th day and the 14th day in the observation group were superior to those of the control group[The goal -rate of target feeding volume on the 7th day:88% vs.64%,χ2 =3.947,P =0.047;the goal -rate of target feeding volume on the 14th day:80% vs.52%,χ2 =4.367,P =0.037].Meanwhile the incidence of gastric stasis,the reflux and aspiration,the aspiration pneumonia in the patients of the observation group within 14 days were significantly lower than those in the patients of the control group within 14 days[The gastric retention rate:8% vs.56%(14 /25 ),χ2 =10.784,P =0.001;the reflux rate:0% vs.24%(6 /25),χ2 =4.735,P =0.03;the aspiration rate:8% vs.32%,χ2 =4.500,P =0.034;the incidence of aspiration pneumonia:24% vs.68%,χ2 =9.742,P =0.002].The days of mechanical ventilation and the days in ICU of the patients in the observation group are far less than those of the patients in the control group[The days of mechanical ventilation:(11.16 ±4.86)d vs.(13.72 ±3.67)d,t =-2.101,P =0.041;the days in ICU:(15.36 ±5.66)d vs.(18.72 ±2.99)d,t =-2.625,P =0.012].While there was no significant difference between the two groups on the 30 -day mortality(24% vs.32%,χ2 =0.397,P =0.529).Conclusion The bedside blinding method of active indwelling of nasojejunal tube combined with the nasogastric tube gastrointestinal decompression can significantly improve the enteral nutritional goal -rate of target feeding volume for patients with severe stroke and greatly reduce the incidence of gastric stasis,the reflux and aspiration,the aspiration pneumonia,and limit the days of mechanical ventilation and the days in ICU.Accordingly,it has the value of popularization in the clinical application.
2.Influencing factors for early recurrence after curative pancreaticoduodenectomy of pancreatic head cancer

Objective:To investigate the influencing factors for early recurrence after curative pancreaticoduodenectomy of pancreatic head cancer.Methods:The retrospective case-control study was conducted. The clinicopathological data of 104 patients with pancreatic head cancer who underwent radical resection in Fudan University Shanghai Cancer Center from May 2014 to May 2015 were collected. There were 62 males and 42 females, aged (61±10)years. Patients underwent carative pancreaticoduodenectomy. Observation indicators: (1) surgical situations; (2) follow-up; (3) influencing factors for early recurrence after curative pancreaticoduodenectomy of pancreatic head cancer. Follow-up was conducted using telephone interview to detect recurrence of patients up to postoperative 1 year. Measurement data with normal distribution were represented as Mean±SD. Count data were described as absolutes numbers, and comparison between groups was analyzed using the chi-square test. Multivariate analysis was analyzed using the Logistic regression model. Results:(1) Surgical situations: 104 patients underwent curative pancreaticoduodenec-tomy successfully. The volume of intraoperative blood loss was (474±280)mL and the number of lymph node dissection was 21±10. (2) Follow-up: 104 patients received postoperative follow-up, 44 of whom had early recurrence. Of the 44 patients with early recurrence, 42 cases had intraperitoneal recurrence including 23 cases with liver metastasis, 7 cases with metastasis in surgical site, 7 cases with retroperitoneal lymph node metastasis, 5 cases with omentum metastasis, 2 cases had extraperitoneal recurrence including 1 case with pleural metastasis and 1 case with pulmonary metastasis. (3) Influencing factors for early recurrence after curative pancreaticoduodenectomy of pancreatic head cancer: results of univariate analysis showed levels of preoperative CA19-9, levels of postoperative CA19-9, the number of lymph node dissection were related factors for early recurrence after curative pancreaticoduodenectomy of pancreatic head cancer ( χ2=5.833, 9.276, 4.261, P<0.05). Results of multivariate analysis showed that postoperative CA19-9 >37 U/mL was an independent risk factor for early recurrence after curative pancreaticoduodenectomy of pancreatic head cancer ( odd ratio=3.599,95% confidence interval as 1.551-8.347, P<0.05). Conclusion:Postoperative CA19-9>37 U/mL is an independent risk factor for early recurrence after curative pancreaticoduodenectomy of pancreatic head cancer.
3.Clinical efficacy observation of acupoint thread-embedding in treating obese patients with food addiction

Objective: To observe the effects of acupoint thread-embedding therapy and low-carbohydrate diet therapy on obese patients with food addiction. Methods: Sixty-five eligible patients were randomized into a thread-embedding group of 33 cases and a diet group of 32 cases to respectively receive 12-week treatment. Before treatment, after treatment and at 6-month follow-up, the two groups were observed and compared in terms of body mass (BM), waist circumference (WC), hip circumference (HC), waist-to-hip ratio (WHR), body mass index (BMI), body fat rate (BFR), basal metabolic rate (BMR) and Yale food addiction scale version 2.0 (YFAS 2.0). Results: At the end of treatment, there were no significant differences in the general efficacy, and the improvements in BM, BMI, WC, HC, WHR and BFR between the thread-embedding group and diet group (all P>0.05). At follow-up, the thread-embedding group showed more significant improvements in all the aforementioned indicators compared with the diet group except HC (all P<0.05). At the end of treatment and follow-up, BMR and YFSA 2.0 had more significant improvements in the thread-embedding group than in the diet group (all P<0.05). Conclusion: Acupoint thread-embedding therapy can produce significant efficacy in treating obese patients with food addiction; it can improve the food addiction state and work better in maintaining the efficacy compared with low-carbohydrate diet therapy.
4.Analysis on the characteristics of injuries from the Chinese National Injury Surveillance System

Objective To analyze the injury characteristics, using data from the National Injury Surveillance System (NISS), 2006 to 2008, to develop a injury prevention strategy. Methods 129 hospitals from 43 surveillance points in NISS were selected according to the DSP sample framework, also considering the capacity of local CDC. Results Relative information on 1 318 739cases were analyzed. The number of male cases were more than the females. The top three causes of injuries were as follows: falls (28.02%,29.36%,30.12%), traffic-related (23.07%,21.61%,21.44%)and blunt injuries (21.46%, 21.17%, 20.59% ). The major places where injuries occurred were: road/street (29.80%, 27.73%, 28.37% ) and at home (21.51%, 22.80%, 24.02% ). Recreation activities (35.63%, 41.93%, 42.13% ) and at work (24.43%, 22.60%, 21.44% ) were the two major activities when injuries took place. Most injury cases were unintentional (83.81%, 86.32%, 86.71% ) and mild (70.42%, 74.79%, 73.90% ). Rush hours that caused injuries to happen were between 7 to 10 AM.Conclusion Male injury cases were more than females. Falls, road traffic-related and blunt injuries were the major causes, with most of them were unintentional and minor.
5.Analysis on injuries among the elderly from Chinese Injury Surveillance System, 2006 - 2008

Objective To describe the distribution of injuries among the elderly through hospital based National Injury Surveillance System (NISS), and to provide basic information for prevention of injury among the elderly. Methods Data on injuries among the elderly aged 60 years and above was descriptively analyzed from the Chinese NISS from 2006 to 2008. Using the data of NISS from 2006 to 2008, information was collected on those cases who were diagnosed as injuries for the first-time in the site hospitals. Information included demographic characteristics, features of the injury events, and clinical characteristics of injuries. SPSS 16.0 software was used for data analysis.Results The leading causes of injuries among the elderly were: falls, road traffic injuries, and blunt injuries, which were in total accounting for more than 70% of all the injuries among the elderly. Most of the injuries occurred at home (46.25 %, 44.44%, 46.23 % ), on the roads ( 30.95%, 28.51%, 28.61% )and in the public residences (8.89%, 11.57%, 11.25% ). Recreation activates (42.14%, 48.09%,48.59%) ,housework/study (21.15%, 18.21%, 18.15%) and driving (13.67%, 13.80%, 13.56%) were the three major activities when injuries took place. Most injuries among the elderly were unintentional (91.34%, 92.84%, 93.09% ). The leading causes of suicide of the elderly were poisoning and sharp injury. The most common natures of injuries were bruise (36.07%, 38.18%, 38.50% ), fractures (25.65%, 22.47%, 23.77% ), and sharp/bite/open injuries ( 17.19%, 18.21%, 17.46% ) while the most injured part of bodies were head (26.32%, 25.59%, 25.90%), lower limbs (25.74%, 25.70%, 25.26%)and upper limbs (21.01%,21.14%,21.37%). Most of the injuries were minor ones (60.53%,65.09%,63.58% ), and patients mostly went home after treatment (64.11%, 68.53%, 67.99% ). Conclusion According to NISS, the proportion of injuries among the elderly in all injuries kept increasing from 2006 to 2008, while distribution of injuries among the elderly being stable. Falls was the most common injury type for the elderly which also brought along bad outcome. Falls was the priority for injury prevention among the elderly.
6.Analysis of road traffic injuries from Chinese National Injury Surveillance System, 2006-2008

Objective To describe the distribution of road traffic injuries through hospital based National Injury Surveillance System (NISS). Methods Data of road traffic injuries was descriptively analyzed from Chinese NISS from 2006 to 2008. Results In 2006-2008, road traffic injury was the second leading cause from NISS among attendants in ERs or clinics of the hospitals,with males (64.63%, 64.07%, 64.38% ) more than females (35.37%, 35.93%, 35.62%). People aged 30-44 (36.04%, 34.82%, 34.28% ), 15-29 (30.74%, 31.57%, 30.13%), 45-64 (20.28%, 20.70%,22.80% ) years were seen more than other age groups. The majority of road traffic injuries were unintentional (98.34%, 99.07%, 99.07% ), and mostly injured in head (35.21%, 33.74%, 35.77% )and lower limbs (24.08%, 24.54%, 23.95%) which mainly as bruise (56.47%, 57.92%, 58.89%) and fractures (17.70%, 15.84%, 15.88% ). The severities of injuries were mainly minor ones (63.69%,67.24%, 65.68% ), and mostly went home right after treatments (59.43%, 63.76%, 62.80% ).Conclusion The distribution of road traffic injuries from NISS kept stable from 2006 to 2008. Young and middle aged men were the focus population for road traffic injuries intervention. Further improvement ofNISS, multi-sectional collaboration-based advocacies and education programs as well as the enforcement of road safety law seemed the good practices for road traffic injury prevention.
7.Effects of enantiomers(R,R)-XY and(S,S)-XY on ocular blood flow in rabbits

·AIM: To evaluate the effects of two series of enantiomers [(R, R)-XY-1 through (R, R)-XY-12 and (S,S)-XY-1 through (S, S)-XY-12] on ocular blood flow in rabbits.·METHODS; Colored microsphere technique was used for in vivo experiments to determine the ocular blood flow in various tissues of ocular hypertensive (40mmHg) rabbit eyes.·RESULTS; Of the twelve compounds of ( R, R)-XY series examined, four increased choroidal blood flow at 10g/L, 50uL instilled into eyes. All compounds of (S, S)-XY series were not effective on ocular blood flow.·CONCLUSION; Some compounds of (R, R)-XY series increased the ocular blood flow, which might be useful for the prevention and treatment of ocular blood flow related eye diseases. 8.Effects of ZX-5 and its optical isomers on ocular blood flow in rabbits and retinal function recovery in rats

AIM: The effects of ZX-5, as nitric oxide (NO) donor, on ocular blood flow has been investigated using colored microsphere technique in previous study. The relationship between the production of NO by ZX-5 and ocular blood flow has been evaluated. ZX-5 has been shown to have strong positive effect on increasing choroidal blood flow. However,the effect of ZX-5 on retinal function recovery, the effects of its optical isomers, (R, R)-ZX-5 and (S, S)-ZX-5, on choroidal blood flow and retinal function recovery have not been studied and merit investigation.METHODS: Colored microsphere technique was used for in vivo experiments to determine choroidal blood flow of ocular hypertension (40mmHg) in rabbit eyes. Electroretinography was used to measure the b-wave recovery as an indication of retinal function recovery.RESULTS: (R, R)-ZX-5 increased choroidal blood flow at 10g/L, 50μL instillation into eyes at all time points (P<0.05).(S, S)-ZX-5 was not effective in increasing choroidal blood flow. ZX-5 and (R, R)-ZX-5 showed significant effects in retinal function recovery after ischemia of the retina at all time points (P<0.05); whereas (S, S)-ZX-5 did not show significant effect on recovery of b-wave after ischemia at most time points except at 120 and 240 minutes.CONCLUSION: ZX-5 and (R, R)-ZX-5 have high potency in increasing the choroidal blood flow and improving the retinal function recovery. It is hoped that they could be used for the prevention/treatment of ocular blood flow related eye diseases.

10.Preliminary study on treatment of partial androgen deficiency in aging males with Jingui Shenqi Pill.

OBJECTIVETo observe the efficacy and safety of Jingui Shengqi Pill in treating partial androgen deficiency in aging males (PADAM), and to explore the new approach in improving the quality of life in PADAM patients.
METHODSForty patients with PADAM were treated with JSP, the efficacy was evaluated with international index of erectile function (IIEF) scoring, PADAM questionnaire scoring, hormone, prostatic specific antigen (PSA), etc., and the data before treatment were compared with those after treatment in the same group.
RESULTSAfter 3 months of treatment, PADAM scoring and IIEF scoring were all significantly improved. Symptoms regarding physical ability, vasomotion, and psychical and mental condition all got improved more markedly than symptoms regarding sexual hypofunction. The serum level of testosterone was 3.85 +/- 0.36 before treatment and 5.02 +/- 0.83 after treatment (P < 0.05); luteinizing hormone of 7.33 +/- 2.14 and 4.84 +/- 1.43 (P < 0.01), follicle-stimulating hormone of 10.22 +/- 4.48 and 6.47 +/- 3.28 (P < 0.01), respectively. The level of PSA failed to change significantly (1.94 +/- 0.55 and 2.06 +/- 0.47, P > 0.05).
CONCLUSIONJSP is effective and safe in treating PADAM, the mechanism of it is different from supplementing extrinsic androgen. It may have produced the effect by means of favorably regulating the condition of sex hormone to improve the balance of pituitary-sex gland axis, so it has more extensive clinical application.
