Renal blood flow was measured by single injection technique of HippuranI13 in 13 patients with chronic glomerulonephritis and 11 control subjects. There was a significant decrease in renal blood flow in chronic glomerulonephritis particulary in those with elevated blood urea nitrogen. Renal blood flow was inversely proportionate to renal vascular resistance but no correlation was noted between renal blood flow and creatinine clearance.

Respiratory effects of acute coronary embolism were studied in mongrel dogs. Coronary embolism was produced by Agress's method and respirtory effects were observed for two hours. In the experimental group, in which coronary embolism was produced, there were relative decreases in respiratory rate and arterial oxygen saturation along with relative increases in tidal volume, minute ventilation, ventilation equivalent and arteriovenous oxygen difference compared to those in the control group. However, differences in these various measurements between tow groups, those in tidal volume and arterial oxygen saturation, have greatly diminished in two hours. These facts suggest that changes in respiratory rate, minute ventilation, ventilation equivalent and arteriovenous oxygen difference in the experimental group are early but short-lasting responses to acute coronary embolism, whereas changes in tidal volume and arterial oxygen saturation occur early and last longer.

Erythromelalgia(Erythermalgia) is a syndrome of red, warm, congested, and burning painful extremities. Presently, there are three distinct clinicopathological entities: erythromelalgia, primary erythermalgia, and secondary erythermalgia. Secondary erythermalgia usually arises at an adult age either in association with a detectable underlying disorder or with the use of vasoactive drugs. Typically, it responds to proper treatment of the underlying disorder or discontinuation of the causative drugs. A seven-year-old boy presented with persisting burning pain, erythema, and warmth on both of his feet and lower legs, along with hypertension. He had had this condition for 20 months. Laboratory studies of the patient revealed increased urinary catecholamines and plasma renin activity. Various treatment modalities including aspirin, captopril, sodium nitroprusside, labetalol, epidural block, and stellate ganglion block yielded unfavorable results. Herein, we report an intractable case of secondary erythermalgia associated with hypertension in a child.

To investigate the effect of Kawasaki syndrome on left ventricular function, we studied 52 patients with Kawasaki syndrome at initial visit and after 3 months (36 patients). Using Pulsed Doppler echocardiogram,we obtained aortic velocity (peak and mean), acceleration time(AT),ejection time(ET), ratio of AT to ET(AT/ET), acceleration (peak and mean) and velocity time integral and mitral velocity of E and A waves(peak and mean) and velocity time integral. Mitral time for peak velocity time integral. Mitral time for peak velocity was significantly prolonged in Kawasaki syndrome,being a mean(+/-SD) of 66.2(+/-14) msec in the control group, 79.2(+/-13)msec at initial vist(p<0.05) and 79.4(+/-13) msec after 3 months (p<0.05). Aortic peak acceleration was significantly decreased in Kawasaki syndrome being a mean(+/-SD) of 2590(+/-785) cm/sec2 after 3 months (P<0.05).Aortic mean acceleration was also significantly decreased in Kawasaki syndrome being a mean(+/-SD) of 1575( +/-542)cm/sec2 in the control group, 1198(+/-351)cm/sec2at initial visit(p<0.05)and 1124 +/-275cm/sec2 after 3 months(p<0.01). Aortic acceleration time was significantly prolonged in Kawasaki svndrome being a mean(+/-SD) of 62(+/-13) msec in the control group, 72(+/-13) msec at initial visit(p<0.05) and 76(+/-16) msec in the control group, 72(+/-13) msec at initial visit(p<0.05) and 76(+/-16) msec after 3 months (p<0.01). We conclude that early abnormalities of left ventricular function, as assessed by echocardiograpy,gencrally persist after 3 months of onset.

Several symptoms of dry eye in the patients who underwent cataract surgery are related to the stability of precorneal tear film layer. We studied the change of the lipid layer pattern in tear film after cataract surgery. Ninety-eight eyes of 87 patients were evaluated for the lipid layer patterns before operation and at two months after operation. Thirty-two eyes(32.7%) among 98 eyes had the change of the pattern of lipid layer. The change was more marked in the eyes of color fringe pattern preoperatively than others(p=0.000). The rate of change was not significantly different between the superior limbal incision group and temporal clear corneal incision group, and between the groups according to the number of sutures(p>0.05). The eyes which had had the dry eye symptoms developed more change of the lipid layer pattern(p=0.006). After cataract surgery, the change of the lipid layer pattern got more marked especially in the eyes which had had color fringe pattern before surgery. In the eyes which had had the dry eye symptoms, however, no difference was shown by the incisional site and by the number of sutures.

Limb salvage operations for osteosarcoma of the extremity usually consist of wide excision and skeletal reconstruction. Most osteosarcoma patients are anemic prior to the surgery as majority of them undergo preoperative neo-adjuvant chemotherapy; thus, it is necessary to treat anemia before and after the surgery since limb salvage operation tends to accompany significant blood loss. Despite the fact that blood transfusion has bad influence on prognosis, complication, and postoperative outcome of cancer patients, it is still considered as a standard management to fix anemia for limb salvage operations. We would like to present a case report in which the authors succeeded in performing limb salvage operations on patients with distal femur osteosarcoma without transfusion.
