Objective To evaluate application of quality care in prevention of general surgery wound infection in elderly patients. Methods 116 cases of elderly patients undertook general surgery from October 2013 to October 2014 were selected in alphabetical order by pinyin of last name as the research objects, who were divided into the observation group (58 cases) and control group (58 cases) by lottery. The control group received routine care, the observation group received quality care. The occurrence of wound infection and the hospitalization time were compared. Results The incidence of wound infection of the observation group was 6.9%(4/58), which was lower than that of the control group [24.1%(14/58),χ2=6.576, P<0.05]. The hospitalization time of the observation group was lower than that of control group [(22.6±3.2) d vs.(28.6±3.8) d, t=5.401, P<0.05]. Conclusions Quality care contributes to reduce the incidence of wound infection on general surgery incisions of elderly patients, and promotes the recovery of patients.

Objective: To observe the clinical efficacy of electroacupuncture (EA) plus long-snake moxibustion for rheumatoid arthritis (RA) due to kidney deficiency and cold coagulation. Methods: A total of 60 patients in active stage of RA were randomly divided into an observation group and a control group, with 30 cases in each group. Patients in the control group took methotrexate tablets and nimesulide dispersible tablets orally for treatment, while those in the observation group received additional treatment of EA plus long-snake moxibustion. Both groups were treated for 12 weeks. The post-treatment changes in symptomatic grading and quantifying score, disease activity score with 28 joint counts (DAS28), and inflammatory indicators erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) were observed, and the efficacy was evaluated. Results: The total effective rate in the observation group was 89.9％, versus 76.6％ in the control group, and there was a significant difference in the total effective rate between the two groups (P<0.05). After treatment, the symptomatic grading and quantifying score and DAS28 in both groups were significantly improved (all P<0.05), and the improvements in the observation group were more significant than those in the control group (both P<0.05). Besides, there were 11 cases with low disease activity index (DAI) and in remission in the observation group versus 5 cases in the control group, showing a statistically significant between-group difference (P<0.05). The ESR and CRP scores in both groups were significantly reduced after treatment (all P<0.05), also showing statistically significant between-group differences (both P<0.05). Conclusion: The efficacy of additional EA plus long-snake moxibustion based on oral medications for RA due to kidney deficiency and cold coagulation is better than that of the oral medications alone.

Objective To investigate the correlation between caspase-3 and left ventricular function at different phases after ischemia-reperfusion. Methods Totally 36 wistar rats were randomly divided into Sham operation group and ischemia-reperfusion(IR)group. The rat ischemic cardiac region of IR group was taken after 1, 3, 6, 12 and 24-hour reperfusion followed to 30-minute ischemia and detected for the activity fragment of caspase-3 P20, the activity of caspase-3, myocardial apoptosis index, left ventricular function (LVF) and creatine kinase-MB (CK-MB) respectively. Results With reperfusion time prolonging, the activity fragment of caspase-3 P20 increased, the activity of caspase-3, myocardial apoptosis index, left ventricular function and CK-MB all changed. The activity fragment of caspase-3 P20 and CK-MB peaked at 6 h, and the activity of caspase-3 and myocardial apoptosis index at 12 h, and then maintained platform; LVSP and ?dp/dt_max increased while LVEDP decreased gradually with the reperfusion time prolonging. Objective To investigate the effective predictors of response in HBeAg-positive patients treated with adefovir dipivoxil(ADV)and to provide evidence for individualized treatment.Methods Patients administered with ADV for 48 weeks in a randomized,placebo-controlled,multicenter trial were studied.Statistical analyses,such as Backward stepwise logistic regression and 2?2 method were used for predictors analysis at week 48.Results The baseline serum ALT levels,HBV DNA levels,and undetectable serum HBV DNA by PCR at week 24 were predictors for HBV DNA negativity at week 48.The median of serum ALT levels and HBV DNA levels prior to treatment were 134.5 U/L and 6.57 lg copies/mL,respectively.Patients with baseline ALT levels higher than the median,HBV DNA levels lower than the median,and serum HBV DNA undectectable by PCR at week 24 had greater rate of HBV DNA negativity(93.3%),HBeAg loss(60%)and HBeAg seroconversion(40%)at week 48 than the others.47.8% of patients whose HBV DNA levels were positive at week 24 also achieved HBV DNA negativity at week 48,and 8.6% achieved HBeAg seroconversion.Conclusion Better response at week 48 has significantly higher serum ALT levels and lower HBV DNA levels prior to treatment and HBV DNA negativity at week 24 compared with non-response.Patients whose HBV DNA levels ware still positive at week 24 should continue therapy.

In recent years, it was found that heart was significantly protected against ischemia-reperfusion injury if it was first preconditioned by brief ischemia or by administering a potassium channel opener. Both of these preconditioning strategies were found to require opening of a K(ATP) channel, we showed that this pivotal role was mediated by the mitochondrial ATP-sensitive K+ channel (mitoK(ATP)). As the mechanism was not clarified yet, this paper reviewed the evidence showing that the mechanism and the characters of mito K(ATP) and its cardioprotection against ischemia-reperfusion injury

ObjectiveTo evaluate the feasibility,effectiveness and safety of laparoscopic splenectomy combined with pericardial devascularization for the treatment of portal hypertension in cirrhotic patients. MethodsIn this study 16 cases underwent initial totally laparoscopic splenectomy and paraesophagogastric devascularization,with 10 cases undergoing upfront open surgery as controls by one treatment group.Clinical data including operative time,intraoperative blood loss,post-operative complications and hospitalization were compared between the two groups.ResultsIn the laparoscopic group the procedure was successfully performed in 12 cases.4 cases were converted to open surgery.The operative time in laparoscopic group and the open group was respectively (315 ± 77 ) min and (291± 31) min (P =0.384).The mean intro-operative blood loss was (409 ±216) ml and (980 ±402) ml (P ＜0.01).The post-operative stay was ( 10 ± 3 ) d and ( 17 ± 8 ) d ( P ＜ 0.01).Differences in post-operative complication rate between the two groups was not statistically significant ( 17％ vs.30％,P =0.525 ).ConclusionsLaparoscopic splenectomy and pericardial devascularization is feasible,effective and safe for cirrhotic patients with portal hypertension,it has the advantage of less intra-operative blood loss,less pain and shorter hospitalization than open surgery.

The paper retrieved the literature information on the artificial bone and composite artificial bone in CNKI, Wanfang data and foreign databases such as Pubmed and SCI-E from 2009 to 2016, and summarized the characteristics and deficiency of all kinds of artificial bone materials.On this basis, it briefly described the functional application and the principles of the composite artificial bone repair materials, and introduced the application of tissue engineering and 3D printing technology in the field, which could provide reference for the exploration of new types of composite artificial bone repair materials.
