Early life adversities like parental loss during childhood, physical
abuse, sexual abuse and emotional harassment may have deleterious
consequences in an individual’s life, which can manifest under the form of
various externalizing or internalizing behaviors. This case study aimed to
highlight the impact of unusual early life adversities in a young woman’s mental
health and related management issues. Methods: We reported a case of a young
lady presenting with anxiety, low mood, disturbed sleep and appetite for more
than six months. She also had episodes of dissociative stupor following any
stressful event for approximately 13 years. She was hospitalized, evaluated
clinically as well as by psychometric assessment. Relevant pharmacological and
psychological interventions were performed. Results: She was diagnosed with
Major depressive disorder with dissociative disorder and borderline personality
disorder. The patient had multiple stressors during childhood like - loss of
parents, emotional & physical abuse, which had an impact on her mental wellbeing.
Conclusion: Early life adversities are detrimental to the mental health of
an individual. The clinical outcome depended upon on the nature of trauma to
the mental well-being, mode of intervention done and available psychosocial

4.Post-living donor liver transplant biliary strictures: prevalence, predictors, and long-term outcomes in a retrospective study

Background:
Post-liver transplant biliary strictures are a common cause of morbidity among patients who have undergone living donor liver transplantation (LDLT). Limited data are available concerning the response rates to various treatment modalities and the long-term outcomes for these individuals.
Methods:
This study was a retrospective analysis of a prospectively collected database, including adult patients aged 18 years or older who underwent LDLT between 2006 and 2022.
Results:
Between 2006 and 2022, a total of 3,550 patients underwent liver transplantation. After applying exclusion criteria, 2,956 patients were included in the analysis.During the study period, 344 patients (11.6%) developed biliary strictures. Of these, 77.0% underwent endoscopic retrograde cholangiopancreatography as the primary treatment for biliary strictures, while the remainder received percutaneous transhepatic biliary drainage. Identified risk factors for post-liver transplant biliary strictures included the presence of multiple biliary anastomoses, bile leak, and older donor and recipient ages. The overall graft and patient survival rates were comparable between patients with and without biliary strictures, at both 1 year (93.0% vs. 96.3%) and 5 years (82.3% vs. 79.2%).
Conclusions
Biliary strictures are observed in approximately 11% of recipients following LDLT. While the presence of biliary strictures is associated with increased morbidity, it does not significantly impact patient survival.

8.Comparison of propofol alone and in combination with ketamine or fentanyl for sedation in endoscopic ultrasonography

BACKGROUND: We evaluated whether the addition of a small dose of ketamine or fentanyl would lead to a reduction in the total dose of propofol consumed without compromising the safety and recovery of patients having endoscopic ultrasonography (EUS). METHODS: A total of 210 adult patients undergoing elective EUS under sedation were included in the study. Patients were randomized into three groups. Patients were premedicated intravenously with normal saline in group 1, 50 µg fentanyl in group 2, and 0.5 mg/kg ketamine in group 3. All patients received intravenous propofol for sedation. Propofol consumption in mg/kg/h was noted. The incidence of hypotension, bradycardia, desaturation, and coughing was noted. The time to achieve a Post Anesthesia Discharge Score (PADS) of 10 was also noted. RESULTS: There were 68 patients in group 1, 70 in group 2, and 72 in group 3. The amount of propofol consumed was significantly higher in group 1 (9.25 [7.3–13.2]) than in group 2 (8.8 [6.8–12.2]) and group 3 (7.6 [5.7–9.8]). Patient hemodynamics and oxygenation were well maintained and comparable in all groups. The time to achieve a PADS of 10 was significantly higher in group 3 compared to the other two groups. CONCLUSIONS: The use of 50 µg fentanyl or 0.5 mg/kg ketamine in a single dose during EUS reduces the dose of propofol required for sedation. However, unlike the addition of fentanyl, the addition of ketamine increased the time to recovery. Thus, 50 µg fentanyl is a good additive to propofol infusion for sedation during EUS to reduce the requirement for propofol without affecting the time to recovery.
