1.Comment of Pattern Identification in Traditional Chinese Medicine on Eczema

The article reviews the viewpoints and thinking toward eczema with pattern identification in Traditional Chinese Medicine in recent five years,including professional standard given by the government and program of pattern identification which is summarized by experts of TCM from their clinical practice.The pattern identification emphasis on different stages,Zang-organs,Fu-organs,lesions and different meridians,with the principals for treatment as cleaning heat,resolving dampness,dispelling wind and arresting itching,promoting blood circulation and resolving blood stasis,it thinks highly of factors of mind in its treatment,so that to approach better thinking to normalize the treatment of eczema in TCM.
2.Effects of desflurane and sevoflurane on postoperative cognitive function in elderly patients

ve To evaluate the effects of desflurane and sevoflurane on postoperative cognitive function in the elderly to see if there is any difference. Methods Seventy ASA Ⅰ-Ⅱ patients agedl≥65yr undergoing total knee or hip replacement were randomly assigned to one of the two anesthetic groups of thirty-five patients each:desflurane group and sevoflurane group. All patients were premedicated with midazolam 1 mg iv. Anesthesia was induced with fentanyl 1.0-1.5?g?kg-1, propofol 1.0-2.0 mg?kg-1 and succinylcholine 1 mg?kg-1 and maintained with either 2%-4% desflurane or 1.0%-1.5% sevoflurane combined with N2 O in O2 (1:1) and intermittent iv boluses of vecuronium. MAP, HR, ECG, SpO2, PETCO2 and end-tidal concentration of desflurane or sevoflurane were monitored continuously during operation. After tracheal intubation the patients were mechanically ventilated and PETCO2 was maintained at 30-40 mm Hg. The Mini-Mental State (MMS) test was used to assess cognitive function before and at 1, 3, 6 and 24h after surgery. Results Compared with the baseline values the MMS scores decreased significantly at 1h after operation in both groups. However the MMS scores returned to preoperative baseline levels within 6h after surgery in both groups. There were no significant differences in MMS scores between the two groups before and after operation. At 1h after operation 51% of the patients in desflurane and 57% in sevoflurane group experienced cognitive impairment. But at 3h after surgery the percentage of the patients with cognitive dysfunction decreased to 11% in desflurane group and 9% in sevoflurane group. Conclusions In the elderly the postoperative cognitive dysfunction is temporary lasting only a few hours and the effects of desflurane and sevoflurane are similar.
3.Dose-effect relationship between Q-switch ruby laser and skin pigmentation in Guinea pigs

Objective To investigate the dose-effect relationship between Q-switch ruby laser and skin pigmentation in Guinea pigs. Methods Ten Guinea pigs were included in this study, and the back of each pig was divided into 5 areas, negative control area receiving no UVB irradiation or laser treatment, positive control area receiving UVB irradiation but no laser treatment, low-, middle- and high-dose areas receiving UVB irradiation followed by the treatment with laser at an energy density of 1.5, 3.0 and 6.0 J/cm2 for 4, 2, and 1 session, respectively, with an interval of 1 week. Melanin content index (MCI) was determined, and confocal laser scanning microscopy (CLSM) and histopathology were performed to observe melanin granules and melanocytes in the skin tissue from Guinea pigs before the initial laser treatment, 2 and 4 weeks after the final laser treatment. Results The best response was observed in the low-dose areas, and there was a complete clearance of hyperpigmentation in these areas, which maintained 8 weeks without recurrence, while different degrees of skin pigmentation recurred in the middle- and high-dose areas. Moreover, the low-dose areas showed a significantly lower melanin content index (MCI) compared with the middle- and high-dose areas (F = 4.2,P < 0.05), together with a marked reduction in the extent of pigment deposition, melanin granules, melanocyte number and density. Conclusions The efficacy of Q-switched ruby laser on skin pigmentation in Guinea pigs is correlated with the energy density and treatment frequency, and, low intensity and high frequency are often associated with a better treatment outcome.
4.Analysis of transepidermal water loss and skin hydration in patients with facial steroid dermatitis

Objective To analyze the skin barrier function in patients with facial steroid dermatitis.Methods Ninety-five patients with facial steroid dermatitis were enrolled into this study along with 25 healthy volunteers as controls.Transepidermal water loss(TEWL)and skin hydration were measured in 8 facial regions.including lesional skin in forehead,nasal tip,submaxilla,both cheeks and angulus oris,and non-lesional skin in the left posteior ear.Results Compared with the healthy group,the patients with steroid dermatitis had a significant higher value of TEWL on lower mandible,both angulus oris and cheeks(t:4.90,2.60,2.57,2.54,3.77,respectively,P<0.01),while a significant lower level of skin hydration was noted on both angulus otis,right cheek and forehead(t=3.27,3.81,2.02,2.78,respectively,P<0.05).Among the 8 test sites in both patients and controls,TEWL value decreased in the following order:submaxilla and both angulus oris>both cheeks and forehead>non lesional skin on the posteroir ear,and the decrease in skin water content was highest in submaxilla and both angnlus oris,followed by cheeks and forehead,and finally by left posterior ear.Conclusions In patients with facial steroid dermatitis,skin barrier function is disturbed on submaxilla,both angnlus oris and cheeks,and TEWL value is found to be higher in perioral area,while cheeks have a lower level of skin hydration.
5.Lowering Inventory Cost of Chinese Patent Drugs by Means of Zero-Inventory-Ordering System

OBJECTIVE:To explore an effective,economical drug purchasing mode for hospital drug warehouse.METHODS:The inventory costs and stock turnover rates for the zero-inventory-ordering system and fixed-interval-ordering system once applied in the Chinese patent drug stockroom in our hospital were analyzed using the analytical method of stock turnover rate.RESULTS:The average inventory costs of 2 policies were 155 600 yuan and 103 800 yuan,respectively,and their average stock turnover rates were 199.23% and 301.16%,respectively.CONCLUSION:The zero-inventory-ordering system is more conducive to the reduction of inventory cost of stock drugs.
6.Papular mycosis fungoides: a case report

A 27-year-old man presented with a 3-year history of persistent asymptomatic papules on the left chest and axirlary fossa. Multiple skin biopsies were performed and histopathology revealed mild acanthosis, extension of the dermal papilla, lichenoid lymphoid infiltrates in upper dermis. Some lymphoid cells migrated into the epidermis and formed Pautrier's microabscesses. Immunohistochemistry revealed that the infiltrating cells were positive for LCA, CD45RO, CD3, CD4 and CD8 (scattered), but negative for CD68 or CD30. Cutaneous laser confocal microscopy showed the shadow of scattered, oval or round, slightly refractive cells measuring 4-8 pm in diameter. A diagnosis of papular mycosis fungoides was made. The papules were softened with the lightening of lesional color after treatment with narrow-band ultraviolet B, topical fluticasone propionate cream and isotretinoin gel.
7.Prevention and treatment of oral lesion in AIDS by Chinese herb gargle

Objective To study the effects of self-made Chinese sherbal gargle on AIDS related oral lesion.Methods 353 hospitalized AIDS patients from June 2007 to December 2009 were divided randomly into the experimental group(179 patients)and the control group(174 patients).ALL the patients were treated with systemic anti-viral therapy while the experimental group was mouthwashed by self-made herbal gargle and the control group with normal saline solution.The incidence of new oral lesion and the changes of the originM lesion were observed.Results The incidence of new oral lesion in the experimental group was obviously lower than that of the control group.The cure rate and effective rate of original oral lesion were much higher than the control group.Conclusions Self-made herbal gargle shows good effect in preventing and treating the AIDS related oral lesion.
8.Influenc of electroacupuncture on urinary retention after abdominal operation

Objective To investigate the effect of prevention and treatment of urinary retention for patients after abdominal surgery using electroacupuncture unit model 6805.Methods 180 cases of ab-dominal surgery patients were randomly divided into three groups, the treatment group(64 patients)were treated with electroacupuneture at the points of sanyinjiao, zhongji, pangguangyu, the control group 1(57patients)Was treated with moxibustion and foment were used in the control group 2(259 patients).The incidence rate of urinary retention Was compared between the three groups. Results The rate of spontaneously emiction was 56.25%in 30 minutes and 35.94%in 40 minutes in the treatment group, the results in the control group 1 and 2 were 22.81% and 15.79%,23.73% and 13.56% respectively. There were significant differences between three groups. Conclusions Electroacupuncture Can effectively prevent and treat urinary retention after abdominal operation.
9.Clinical efficacy of large-spot and low-energy Q switched Nd:YAG laser in the treatment of melasma

ObjectiveTo evaluate the effect of large-spot and low-energy Q switched Nd:YAG laser on melasma,and to observe the changes of melasma lesions with confocal laser scanning microscopy (CLSM) before and after the laser treatment.MethodsTotally,45 patients aged from 24 to 48 years and diagnosed with facial melasma were included in this study,and treated with large-spot and low-energy Q switched Nd:YAG once a week for 10 or more sessions.CLSM was used to estimate the melanin content in melasma lesions before each irradiation and after the last irradiation.ResultsAmong the 45 patients,8 ( 17.78% ) were nearly cured,25 (55.56%) markedly improved,11 (24.44%) improved,and only 1 (2.22%) unimproved after the laser irradiation.The total response rate was 73.33%.As CLSM showed,there was an increment in melanin granules in melasma lesions compared with the normal skin surrounding melasma lesions,but a reduction in melanin granules was induced by the laser treatment in melasma lesions.ConclusionsLarge-spot and lowenergy Q switched Nd∶YAG laser is substantially effective and highly safe for the treatment of melasma,and CLSM can be used to evaluate the therapeutic effect of laser on melasma.
10.Relationship between Cytochrome P450 1A2 Gene Polymorphism and Susceptibility to Chronic Obstructive Pulmonary Disease

Objective To investigate the relationship between cytochrome P450 1A2 (CYP1A2) gene polymorphism and susceptibility to chronic obstructive pulmonary disease (COPD). Methods CYP1A2 gene polymorphisms in 100 COPD cases and 100 healthy controls were tested with polymerase chain reaction- restriction fragment length polymorphism (PCR- RELP). Results Genotype frequencies of 4 SNPs in both the COPD and control groups were in accordance with Hardy-Weinberg equilibrium (P>0.05). There was significant difference between the COPD and control groups in genotype and allele frequencies of 1D and 1F (P<0.05), but not of 1C and 1E (P>0.05). Conclusion CYP1A2*1D and CYP1A2*1F polymorphisms may play an important role in the development of COPD.
