1.The effects of tetrandrine on metabolism of prostaglandin and lipids in patients with essential hypertension

The effects of tetrandrine on serum TXB2, 6 - Keto - PGF1?, LPO and lipids in patients(n= 40)with essential hypertension has been studied. The results showed that: (1) The levels of serum 6 - Keto -PGF1?,apoA1 in EHT were lower and the level of serum TXB2,LPO and apoB100 were higher than those in control. (2)Patients with essential hypertension treated with tetrandrine (0.1 ~ 0. 2g,tid,po)for 3 mon revealed that thelevels of TXB2, LPO and apoB100 were decreased, and the levels of apoA1 and 6 - Keto - PGF1? were increased. In conslusion,tetrandrine stimulated the synthesis of PGI2 and inhibited the release of TXA2 and normalized the metabolism of lipids in patients with essential hypertension.
2.Investigation on the Pollution of Cryptosporidiumparvum and Giardia lamblia Stiles in Drinking Water and Wastewater in Shenzhen

Objective To investigate the pollution of parasitic protozoa in drinking water and wastewater in Shenzhen in or-der to present evidence to establish the national hygienic standard for parasitic protozoa in drinking water.Methods Water samples of resource water,fin ished wa ter from3water plants,and post-treated wastewater samples from3wastewater treatment plants in Shenzhen city were collected.Para sitic pro tozoa Giardia lamblia Stiles and Cryptosporidiumparvum were detected by following steps:filtering,washing,magnetic isolation and staining.Results Giardia lamblia Stiles and Cryptosporidiumparvum were not found in samples from raw water and finished water,while they were found in post-treated wastewater samples from2wastewater treatment plants.Conclusion The results indicated that the prob lem of Giardia lamblia Stiles and Cryptosporidi -umparvum pollu tion in the water supplying systems didnt exist at present,but Giardia lamblia Stiles and Cryptosporidi umparvum in the post-treated wastewater were the potential pollution source to surface water.
3.A long-term follow up of surgical treatment for a child with melorheostosis: a case report and literature review

Melorheostosis is a rare disorder of osteopathia. The clinical characteristics of melorheostosis in children is totally different from that in adults. The radiographic features of melorheostosis include the hyperostosis in endosteal on the one side of the cavum medullare and formed streakiness. The soft tissue contracture of the limb and joint deformities are the symptoms of melorheostosis. Here, the authors reported a melorheostosis case of 6 years old girl who suffered from severe valgus deformity of the right knee with permanent patellar dislocation. A surgical stabilization was applied with lateral soft tissue release, medial soft tissue stabilization and transferred the vastus medialis laterally (kinetic stability). After 19 years follow up postoperatively, the limb developed well in satisfied alignment with good function of knee joints, even participated in some sports activities. Other authors reported a similar case of melorheostosis with surgical treatment and achieved good outcomes in limb realignment and reduction. The further suggested that the surgical treatment should be produced before epiphyseal closure.
4.The discovery, application and enlightenment of Lanwei (EX-LE 7).

The discovery and application of Lanwei (EX-LE 7) was reviewed to elaborate its enlightenment on the inheritance and innovation of acupuncture. Literature published before April of 2017 was searched in CNKI and VIP database with"Lanwei (EX-LE 7)"and"acupuncture"as key words. Related references and publications were retrieved. According to the purpose, literature was screened and key information was extracted to perform systematic review and analysis. The results indicated Lanwei (EX-LE 7), reflection of relationship between organ and body surface under the guidance of acupuncture and meridians theory, was not a point, but an area. The discovery and application of Lanwei (EX- LE 7) was the product of western and Chinese combination. The best indication was simple appendicitis, but operation should be applied once adhesion, purulent and perforation occurred. It is indicated to improve clinical efficacy and avoid accident, the reasoned core of theory system should be insisted and the communication with modern medicine should be reinforced.
5.Advances in the oncological safety of laparoscopic surgery for stage T4 colon cancer

Stage T4 colon cancer is divided into two categories:tumor penetrating the visceral peritoneum(T4a)and tumor directly invading or attaching to adjacent organs or structures(T4b).Treatment of T4 colon cancer requires technically demanding surgical procedures,including total resection of adjacent infiltrating organs or structures,and is characterized by a high incidence of postoperative complications and a high rate of positive surgical margin microscopy.It has been found that taking laparoscopic surgery for T4 colon cancer may decrease long-term survival and increase the rate of peritoneal metastasis,but taking laparoscopic surgery for colon cancer also has the potential advantages in increasing perioperative benefits and improving the prognosis.With the upgrade of laparoscopic equipment and the improvement of surgeons'surgical skills,more and more surgeons have adopted laparoscopic surgery to treat T4 colon cancer.Currently,the oncologic safety of laparoscopic surgery for T4 colon cancer has not been effectively evaluated.Therefore,this article reviews the advantages,risks,and the choice of treatment strategies for laparoscopic surgery for T4 colon cancer,taking into account the current status and progress of domestic and international studies.
6.Comparative study of different large language models and medical professionals of different levels responding to ophthalmology questions

AIM: To evaluate the performance of three distinct large language models(LLM), including GPT-3.5, GPT-4, and PaLM2, in responding to queries within the field of ophthalmology, and to compare their performance with three different levels of medical professionals: medical undergraduates, master of medicine, and attending physicians.METHODS: A total of 100 ophthalmic multiple-choice tests, which covered ophthalmic basic knowledge, clinical knowledge, ophthalmic examination and diagnostic methods, and treatment for ocular disease, were conducted on three different kinds of LLM and three different levels of medical professionals(9 undergraduates, 6 postgraduates and 3 attending physicians), respectively. The performance of LLM was comprehensively evaluated from the aspects of mean scores, consistency and confidence of response, and it was compared with human.RESULTS: Notably, each LLM surpassed the average performance of undergraduate medical students(GPT-4:56, GPT-3.5:42, PaLM2:47, undergraduate students:40). Specifically, performance of GPT-3.5 and PaLM2 was slightly lower than those of master's students(51), while GPT-4 exhibited a performance comparable to attending physicians(62). Furthermore, GPT-4 showed significantly higher response consistency and self-confidence compared with GPT-3.5 and PaLM2.CONCLUSION: LLM represented by GPT-4 performs well in the field of ophthalmology, and the LLM model can provide clinical decision-making and teaching aids for clinicians and medical education.

ABSTRACT:
Meningiomas are the most common primary intracranial neoplasm with diverse pathological types and complicated clinical manifestations. The fifth edition of the WHO Classification of Tumors of the Central Nervous System (WHO CNS5), published in 2021, introduces major changes that advance the role of molecular diagnostics in meningiomas. To follow the revision of WHO CNS5, this expert consensus statement was formed jointly by the Group of Neuro-Oncology, Society of Neurosurgery, Chinese Medical Association together with neuropathologists and evidence-based experts. The consensus provides reference points to integrate key biomarkers into stratification and clinical decision making for meningioma patients.
