1.Efficacy of bilateral facial muscle training combined with visual electromyography biofeedback in the treatment of idiopathic facial nerve palsy

Objective To explore the efficacy of bilateral facial muscle training combined with visual electromyography biofeedback on facial nerve function recovery in patients with idiopathic facial nerve palsy. Methods Patients with idiopathic facial nerve palsy admitted to Shanghai Fifth People’s Hospital, Fudan University from July 2022 to July 2024 were selected and randomly divided into a control group and an intervention group. The control group received conventional physical factor therapy, while the intervention group received bilateral facial muscle training combined with visual electromyography biofeedback therapy based on the control group’s regimen. After 20 treatment sessions, the total effective rate, the House-Brackmann (H-B) facial nerve grading system, the Sunnybrook Facial Grading System (SFGS) score, and the average value ratio of maximal amplitudes of bilateral frontalis and zygomaticus muscles were compared between the two groups. Results A total of 90 patients were included, 45 in each group. After 20 treatment sessions, the total effective rate was significantly higher in the intervention group than in the control group (84.4% vs 75.6%, P=0.003). Compared with the control group, the intervention group demonstrated a significantly lower H-B grade (P=0.003) and a higher SFGS score (P=0.001). The average value ratios of maximal amplitudes of the affected versus healthy side frontalis (P=0.013) and zygomatic (P=0.022) muscles were higher in the intervention group than in the control group. Conclusions Bilateral facial muscle training combined with visual electromyography biofeedback is an effective approach for treating idiopathic facial nerve palsy, effectively promoting the recovery of facial nerve function, and improving facial symmetry and facial muscle function.
2.Nursing care of a patient with right-sided aortic arch and thoracic aortic aneurysm complicated with acute coronary syndrome after thoracic endovascular aortic repair

This study summarized the nursing experience of a patient with right-sided aortic arch and kommerell's diverticulum and thoracic aortic aneurysm complicated with acute coronary syndrome after thoracic endovascular aortic repair.The nursing points are as follows:optimizing emergency procedures to improve the quality of rescue;monitoring the condition of the patient dynamically to prevent postoperative complications;developing personalized rehabilitation training to promote postoperative recovery;implementing"Internet+"follow-up management to improve treatment compliance.After the comprehensive management of a multidisciplinary team and careful nursing care,the patient was discharged 12 days after operation.During 18-month follow-up,the patient was in good condition.

4.Herbal Textual Research, Quality Evaluation and Phase Analysis of Halloysitum Rubrum

In this paper, we systematically sorted out and verified the name, origin, producing area change, quality, efficacy and processing of Halloysitum Rubrum by consulting materia medica, medical books, prescription books from past dynasties, and modern literature, and combined with the comprehensive analysis of trait identification, hygroscopicity measurement and X-ray diffraction(XRD) detection, in order to provide a basis for understanding the original source of the mineral medicine. The results indicated that Halloysitum Rubrum was first contained in the Shennong Bencaojing, it had been consistently named Chishizhi, there were also aliases such as Chifu, Hongxinshi and Chiyouzhi. The description of Halloysitum Rubrum in ancient materia medica aligns with modern findings, categorizing it under the silicate kaolinite family, specifically polyhydrous kaolinite, with associated minerals mainly being dickite, nacrite, hematite, gibbsite, mica, etc. Historically, the production area of Halloysitum Rubrum was mainly located in the Qinling Mountains, Shandong and Henan, and is now primarily distributed in most parts of China, including Henan, Hubei and other provinces. Quality evaluation in ancient times included descriptions like "fresh and greasy color", "delicately sticky tongue and lips" and "smooth as fat". Modern materia medica mostly evaluates its quality based on color, luster, texture and hygroscopicity, noting characteristics such as red color, smoothness, delicacy, softness and strong viscosity. Halloysitum Rubrum is sweet, sour and pungent in flavor, warm in nature, non-toxic, and belongs to the heart and large intestine meridians. It acts as an astringent and solidifying agent, particularly useful in the treatment of long-term dysentery and diarrhea, common processing methods include fire calcination, water flying, and vinegar quenching. Comprehensive analysis of the traits, XRD and humidity absorption of different batches of samples showed that the commercially available Halloysitum Rubrum is mainly bright red or brownish red, with a smooth surface like grease, soft and smooth texture, delicate cross-sectional texture, and some have waxy luster and strong water absorption. It is mainly composed of 10 Å(1 Å=0.1 nm) polyhydric kaolinite, and is often accompanied by 7 Å polyhydric kaolinite, nacrite, etc. Genuine products tend to have higher moisture absorption than counterfeit ones, which can be used as a key indicator to distinguish the authenticity. The quality evaluation of Halloysitum Rubrum aligns with historical materia medica, where "fresh color, delicate and greasy lips" could serve as a key feature for its quality evaluation.

Cognitive dysfunction refers to dysfunction of individual perception,memory,understanding,learning,creation and other dysfunctions caused by abnormal brain function and structure.Based on the fact that the spleen can't regulate transportation and transformation,govern blood and send up essential substance,combined with the microbiota-gut-brain axis,this article discussed the etiology and pathogenesis of intestinal flora imbalance affecting cognitive dysfunction in TCM.It was proposed that the spleen in TCM and intestinal flora are connected in physiology and pathology:the spleen regulates spirit and governs cognition,when the spleen fails to function normally that it can't dominate transportation and transformation,govern blood and send up essential substance will cause that the brain spirit can not be nourished;intestinal flora is closely related to the spleen in TCM,and affects brain function through the nervous system,endocrine,immune and metabolic mechanisms.This article can provide explore new ideas for the clinical research and treatment of cognitive dysfunction of traditional Chinese and Western medicine.
6.Clinical effect of Kinesio Tape for treating lower limb function in children with spastic cerebral palsy accompanied by knee hyperextension

Objective To study the clinical effect of Kinesio Tape for treating the lower limb function in children with spastic cerebral palsy accompanied by knee hyperextension.Methods Sixty children with spastic cerebral palsy treated by rehabilitation therapy in this hospital from August 2017 to December 2018 were se-lected as the study subjects and divided into the experiment group and control group,30 cases in each group.The control group adopted the conventional rehabilitation therapy,while the experiment group was combined with Kinesio Tape on the basis of conventional rehabilitation therapy.The treatment course lasted for 3 months.Before and after treatment,the children patients conducted the Gross Motor Function Measure-88(GMFM-88)scoring,Modified Ashworth Scale(MAS)grading on the triceps surae muscle in the affected side,surface electromyography,measurement of dorsiflexion angle of foot in knee extension position and meas-urement of maximum knee extension angle of knee joint.Results The GMFM-88 score,triceps surae muscle MAS grade in the affected side,foot dorsiflexion angle in Knee extension position,maximum knee extension angle in the erect position in the two groups were improved compared with those before treatment(P<0.05).Furthermore,the above indicators in the experimental group were superior to those in the control group(P<0.05).The surface electromyographic value had no statistical difference between the two groups(P>0.05).No obvious adverse reactions occurred during the treatment process with good compliance.The chil-dren's parents in the experiment group filled in Kinesio Tape satisfaction questionnaire,and had 100%satis-faction.Conclusion Kinesio Tape combined with routine rehabilitation therapy could effectively improve the muscular tension,joint activity,knee excessive extension degree and exercise function in children with spastic cerebral palsy accompanied by knee hyperextension.
7.Effects of hypoxia preconditioning on the phosphorylation of NR2B and its tyrosine 1336 in the synaptic site and extrasynaptic site of HT22 cells and mouse hippocampus

Objective N-methyl-D-aspartate(NMDA)receptor subunit 2B(NR2B)and its phosphorylation are involved in cerebral ischemia/hypoxic neural injury.Hypoxic preconditioning(HPC)can serve as an endogenous protective intervention to protect the brain from ischemic/hypoxic injury.This study intended to explore the effect of HPC on NR2B and the phosphorylation of its two tyrosine sites(1252 and 1336)in hippocampal cells through in vivo and in vitro experiments and thus determine the role of NR2B in HPC neuroprotection.Methods 6~8 weeks-old male SPF-grade ICR mice and the mouse hippocampal neuron cell line HT22 were repeatedly exposed to hypoxia to replicate HPC animal and cell models.Western blot and immunofluorescence were applied to detect the levels of NR2B and the phosphorylation levels of its tyrosine 1336(pY1336NR2B)and 1252(pY1252NR2B)residues in the hippocampus of mice and HT22 cells.The distributions of NR2B,pY1336NR2B,and pY1252NR2B in the synaptic site(TxP)and extrasynaptic site(TxS)were analyzed by Western blot.The levels of cleaved caspase-3 and α-spectrin,which indicate cell apoptosis,were also detected.Results HPC downregulated the levels of NR2B and pY1336NR2B in the mouse hippocampus and HT22 cells.Changes in NR2B and pY1336NR2B levels in the TxS of the mouse hippocampus were similar to those in hippocampus and HT22 cells,whereas changes in the TxP showed the opposite trend.Conclusions Downregulation of NR2B and pY1336NR2B may be involved in HPC-induced neuroprotection,and their localization at synapses and extrasynapses may play different roles in neuroprotection.
8.Analysis of influencing factors and pathways of postoperative self-efficacy in patients with fragility fracture

Objective To investigate the relationship between symptom burden and rehabilitation self-efficacy of patients after fragility fracture surgery,and to study the chain mediating role of social support and perceived stress in the influence of symptom burden on rehabilitation self-efficacy of such patients.Methods Approved by the Ethics Committee of the hospital,168 patients who met the admission criteria after fragility fracture surgery admitted to the Second Affiliated Hospital of Inner Mongolia Medical University from July 2020 to December 2023 were included in the study by convenient sampling method,and general data of the patients were collected.M.D.Anderson Symptom Inventor(MDASI),Self-Efficacy for Managing Chronic Disease 6-Item Scale(SECD6),Social Support Rating Scale(SSRS)and Perceived Stress Scale(CPSS)were used to investigate the above patients,and multiple sets of quantitative data were analyzed for attribution association profile and mediation effect.Results After removing invalid questionnaires,a total of 160 valid questionnaires were collected,with an effective response rate of 95.24%.Correlation analysis showed that symptom burden was negatively correlated with rehabilitation self-efficacy(r=-0.405,P<0.05);social support was significantly positively correlated with rehabilitation self-efficacy(r=0.558,P<0.05);perceived stress was negatively correlated with rehabilitation self-efficacy(r=-0.330,P<0.05);symptom burden was negatively correlated with social support(r=-0.191,P<0.05);social support was negatively correlated with perceived stress(r=-0.280,P<0.05);symptom burden was positively correlated with perceived stress(r=0.376,P<0.05).Mediating effect test showed that the symptom burden of patients after fragility fracture surgery had a direct impact on rehabilitation self-efficacy(β=-0.402,t=0.148,P<0.05),and had an impact on rehabilitation self-efficacy through 3 mediating pathways:first,the mediating effect of social support alone(β=-0.098,95%CI:-0.112~-0.074);the second was the mediating effect of stress perception alone(β=-0.081,95%CI:-0.104~-0.061);the third is the chain mediating effect of social support and stress perception(β=-0.056,95%CI:-0.074~-0.030).Conclusion The burden of symptoms after fragility fracture surgery has a direct effect on rehabilitation self-efficacy.Symptom burden influences postoperative self-efficacy of patients with fragility fractures through the chain mediation of social support and perceived stress.Early and accurate assessment of symptom burden,stress perception and social support should be conducted.According to needs,the multidisciplinary medical team provides individualized and diversified health education support to actively help and guide the self-regulation of patients'stress perception,so as to reduce patients'symptom burden and promote the improvement of their self-efficacy in the process of rehabilitation.
