1.A case report of angiomatoid fibrous histiocytoma and the experience of treatment

AFH is a singular osculant tumor and difficult to be diagnosed. A case of AFH was treated in the author's department sucessfully. The author retrospected the course of the treatment and regarded that expanded excision is necessary and radiolagical therapy after operation is helpful.
2.THE STUDY OF IMPROVING THE ADHESION AND PROLIFERATION OF OSTEOBLASTS ON THE SURFACE OF CORAL

The marrow stromal osteoblasts from New Zealand rabbits were seeded on coral treated with L-polylysine, fibronectin and culture medium only, then the cells/coral composites were cultured in vitro. The cells/coral composites were observed for the process of cells growth and matrix formation at 7, 14 and 21 days after culture. With the aid of electron microscope,it was demonstrated that on the surface of the coral holes,which was treated with fibronectin, there were more adhering osteoblasts and matrix formation than those treated with both L-polylysine and culture medium after 7 and 14 days.Cell count in coral blocks was determined by doing a fluorimetric DNA assay. Although the samples treated with L-polylysine demonstrated higher cell count than the coral treated with culture medium after 7 days, there was no statistically difference between the two after 14, 21 days. At each time point, the samples treated with fibronectin showed higher cell count and alkaline phosphatase activity than the orals otherwise treated, and the cell count also increased with culture time. The study suggests that fibronectin has a significant effect on promoting the adhesion and proliferation of osteoblasts on the surface of coral.
3.A preliminary study of injectable tissue-engineered bone

Objective: To develop injectable tissue-engineered bone. Methods: Bone marrow cells isolated from lilac bone of New Zealand rabbits were cultured and induced to differentiate Into osteoblasts. The osteoblasts were mixed with 20 g/L alginate sodium solution to generate osteoblasts/alginate composite with final cellular density of 5 ? 10 6/ml. Calcium chloride was used as cross-linking agent. The osteoblasts/alginate composite was injected into the dorsal subcutaneous tissue of nude mice. The injected material with surounding tissue were examined with X-ray and histopathologic technique. Results: Four and eight weeks after injection, the hard knobbles were easily palpated under the dorsal skin of the animals. On X-ray photograph the knobbles showed calcified tissue image. In histological analysis, new bone formation was observed in the osteoblasts/alginate composite. The osteogenesis was in association with regenerated hematopoietic bone marrow. Conclusion: New bone tissue can be created through the injection of alginate sodium mixed with marrow stromal osteoblasts.
4.A topoanatomical study of the anterior perineal plane for the application of ultra-low anterior resection of the rectum

Objective To study the topoanatomy of anterior perineal plane and adjoining tissue structure in the preparation of ultra-low anterior resection of the rectum. Methods Dissection was performed on 16 male cadavers of semi-pelvis sectioned in the saggital plane. Eight indexes were measured and recorded. Results Anterior perineal plane was clearly found in all 16 cadavers. The median distance of rectum-urethra (R-U) was 14 mm (ranging 10 -17 mm). The contour of perineal body was trapezoid,which was narrow cranially and broad caudally. The median width of cranial perineal body was 8 mm (ranging 6 -9 mm), while the median width of caudal perineal body was 21 mm (ranging 18 -23 mm).The median numerus of thickness of perineal body (TPB), thickness of puborectalis (TPR), arrterior wall of rectum (aPR) -D, pPR-D and width of pelvic diaphragm (WPD) were 20. 5 mm ( ranging 17 - 23 mm),12 mm(ranging 10 -16 mm), 25 mm(ranging 21 -27 mm), 20 mm(ranging 16 -23 mm) and 8 mm (ranging 6 - 10 mm) respectively. Conclusions Anterior perineal plane clearly exists, through which about 20 mm more length of the distal rectum is available which could increase the sphincter-saving rate in cases of low rectal carcinoma.
5.Expression of bone sialoprotein in prostate cancer and its clinical significance

Objective To investigate the expression of bone sialoprotein (BSP) in prostate cancer and its clinical significance. Methods Prostate cancer tissues of different pathological grades (68 cases) and benign prostatic hyperplasia tissues (22 cases) were selected. SP method was used to detect the expression of BSP. Serum total prostate-specific antigen (tPSA) levels of prostate cancer were detected by electrochemiluminescence immunoassay before the operation. Results Compared with no or low expression in the adjacent normal glandular tissues, the detectable levels of BSP were examined in most of the prostate cancer tissues. The expression rate of BSP in prostate cancer tissues was higher than that in benign prostatic hyperplasia tissues [76.47%(52/68) vs 13.64%(3/22),χ2=27.614, P<0.001]. The expression rates of BSP in well differentiated, moderately differentiated and poorly differentiated tissues according to cell differentiating degree (Gleason system) were 75.0 % (12/16), 77.5 % (31/40) and 75.0 % (9/12) respectively. There was no significant difference in various pathological grading (χ2=0.057, P=0.972). The expression rates of BSP in pathological stage pT2, pT3 and pT4 tissues were 62.16%(23/37), 95.24%(20/21) and 90.0%(9/10) respectively. A statistically significant association was found between BSP expression and pathological stage (χ2=9.338, P=0.009). Serum tPSA level of prostate cancer group with BSP expression was higher than that with no BSP expression [(69.06±25.52)μg/L vs (38.00±21.64)μg/L, F=19.355, P<0.001]. Conclusion The high expression of BSP in prostate cancer has a relationship with pathological stage and serum tPSA level, it may play an important role in the biological behaviour of prostate cancer.
6.Effect of DNA methyltransferase 5-aza-2'-deoxycytidine on proliferation of human esophageal squamous cancer cell line Eca109 in vitro

Objective To explore the effect of the DNA methyltransferase 5-aza-2'-deoxycytidine (5-aza-CdR)on human esophageal squamous cancer Ecal09 cells.nethods Human esophageal squamous cell cancer(ESCC)Eca109 cells were treated by 5-aza-CdR with 10-7,10-6,1O-5,10-4,0 mol/L. Respectively.Consequently,the growth rate of the cells was detected by MTT assay and morphological structure Was observed.Meanwhile,cell apoptosis and cell cycle were analyzed by flow cytometry method. (FCM).ResulIs The proliferation of Eea109 cells Was inhibited by 5-aza-CdR from 10-7 to 10-4 mol/L Moreover,the inhibition rate showed time-and-concentration-dependent manner(24~96 h,F=160.06,P=0.000,10-7~10-4 mol/L,F:60.95,P:0.000).The maximum rate of inhibitory was reached up to(15.70±0.75)% in the group treated by 10-4 mol/L 5-aza-CdR after 96 hours.An apoptosis peak appeared before diploid peak.The proportion of Go/G1 phase cells Was significantly increased(F=6479.46, P=0.000),especially up to(89.70±0.91)% in the group treated by 10-4 mo/L 5-aza-CdR after 96 h. However,the proportion of S phase cells Was obviously decreased(F=4222.26,P=0.000),especially down to(9.10±0.48)% in the group treated bv 10-4mol/L 5-aza-CdR after 96 h.Conclusions The proliferation of Eca109 cells is inhibited bv 5-aza-CdR in a time-and-concentration-dependent manner.Moreover,the 5-aza-CdR can inhibit cell growth by regulation of DNA cycle and apoptosis.
7.Effect of polydatin on neuropathic pain in rats

Objective To evaluate the effect of polydatin on neuropathic pain in rats.Methods Forty male Sprague-Dawley rats,aged 6-8 weeks,weighing 200-230 g,were randomly divided into 5 groups (n =8 each) using a random number table:sham operation group (group S),neuropathic pain group (group NP),polydatin 5 mg/kg group (group P1),polydatin 10 mg/kg group (group P2),and polydatin 20 mg/kg group (group P3).Neuropathic pain was induced by chronic constriction injury in NP and P1-3 groups.In group S,the sciatic nerve was only exposed but not ligated.In S and NP groups,normal saline 0.1 ml was injected intraperitoneally immediately after operation and at 1,3,5 and 7 days after operation (T1-4).In P1-3 groups,polydatin 5,10 and 20 mg/kg (in normal saline 0.1 ml) were injected intraperitoneally immediately after operation and at T1-4.At 1 day before operation (T0) and T1-4,the mechanical paw withdrawal threshold (MWT) and thermal paw withdrawal latency (TWL) were measured.After measurement of pain threshold at T4,the rats were sacrificed,and L4-6 segments of the spinal cords were removed for determination of the expression of high-mobility group box 1 (HMGB1),Toll-like receptor 4 (TLR4),interleukin-1beta (IL-1β),tumor necrosis factor-alpha (TNF-α) and monocyte chemotactic protein-1 (MCP-1) by Western blot.Results Compared with group S,the MWT was significantly decreased,and the TWL was significantly shortened at T1-4 in group NP,the MWT was significantly decreased at T1-4,and the TWL was significantly shortened at T2-4 in group P1,the MWT was significantly decreased at T1-4,and the TWL was significantly shortened at T3.4 in group P2,the MWT was significantly decreased at T1-4 in group P3,and the expression of HMGB1,TLR4,IL-1β,TNF-α and MCP-1 was significantly up-regulated in NP,P1 and P2 groups (P<0.05).Compared with group NP,the MWT was significantly increased at Tt-4,and the TWL was significantly prolonged at T1,2 in group P2,the MWT was significantly increased,and the TWL was significantly prolonged at T1-4 in group P3,the expression of HMGB1,TLR4,IL-1β,TNF-α and MCP-1 was significantly down-regulated in P2 and P3 groups (P<0.05),and no significant change was found in the parameters mentioned above in group P1 (P>0.05).Compared with group P1,the MWT was significantly increased at T4 in group P2,and the MWT was significantly increased at T14,the TWL was significantly prolonged at T3,4,and the expression of HMGB1,TLR4,IL-1β,TNF-α and MCP-1 was significantly down-regulated in group P3 (P<0.05).Compared with group P2,the MWT was significantly increased at T3,4,and the expression of TLR4,IL-1β,TNF-α and MCP-1 was significantly down-regulated in group P3 (P<0.05).Conclusion Polydatin can alleviate neuropathic pain through inhibiting inflammatory responses in the spinal cord of rats.
8.Application of lateral arm flap in the repair of oral and maxillofacial defects

Objective:To investigate the value of the lateral arm free flap (LAFF)in the repair of oral and maxillofacial defects af-ter resection of malignant tumor.Methods:22 cases received LAFF from January 2012 to December 2012 were studied.The flap size,preparation time,vessel diameter,anastomosis time,survival situation,success rate and postoperative results were observed. Results:The flap ranged from 14 cm ×8 cm to 6 cm ×4 cm.All flaps survived well without vascular crisis.Repair results were satis-factory.Conclusion:LAFF,with constant anatomy and simple preparation,can be applied in the repair of oral and maxillofacial de-fects after tumor resection.
9.The study of establishing the ideal median sagittal plane of mandible based on multipleanatomic landmarks

Objective:To apply multiple mandibular anatomical landmark points to establish the mandibular median sagittal plane to find and establish the ideal median sagittal plane of the mandible that is suitable for clinical diagnosis and treatment.Methods:The maxillofacial spiral CT data of 30 normal maxillofacial patients in the Western Theater General Hospital were collected, and the 3D cranial model was reconstructed by using Mimics Research 21.0 software to select different anatomical landmarks to establish multiple mandibular MSPs, and with the whole cranial MSP as a reference, the different MSPs were respectively used as the mirror planes to establish a mirror model. The overlap of the original model and the mirror model were compared qualitatively and quantitatively by the overlap three-dimensional color illustration of 3-Matic software and Friedman′s test, respectively, to evaluate the application effect of different planes as mandibular MSPs.Results:There was a statistically significant degree of overlap between the mandibular mirror model and the original model with the whole head MSP and the different mandibular MSPs as mirror planes, respectively (χ 2=12.34, P<0.05). The mirror model of plane Mf-Go-Sim had the smallest overlap with the original model 0.07 (0.04, 0.18) mm; plane N-ANS-PNS was the second smallest 0.14 (0.09, 0.58) mm; the difference in overlap of plane ANS-SBWG-Mp 0.12 (0.04, 0.40) mm; the difference in overlap of plane Me-B-Sim 0.19 (0.09, 0.44) mm; the difference in overlap of plane Po-B-Go 0.18 (0.06, 0.45) mm. Conclusions:Whole-head MSP cannot completely replace mandibular MSP; plane Mf-Go-Sim can be the ideal MSP for the mandible in clinical applications.
10.Efficacy of CyberKnife combined with temozolomide in treatment of brain metastasis of non-small cell lung cancer

Objective To analyze the efficacy and safety of CyberKnife combined with temozolomide (TMZ) in treatment of brain metastasis of non-small cell lung cancer (NSCLC). Methods From March 2013 to March 2016, 62 NSCLC patients with brain metastases in department of oncology of the 187th Hospital of PLA were divided into two groups according to the random number table method, the CyberKnife combined with TMZ group (CyberKnife + TMZ group, 31 cases) and simple CyberKnife group (CyberKnife group, 31 cases). Hypofractionated radiation of CyberKnife was given 18-36 Gy in 1-5 fractions of 5-25 Gy. CyberKnife+ TMZ group was given temozolomide 150 mg·m-2·d-1 for 5 days in first cycle, then every 28 days they received temozolomide therapy from the second to the sixth cycles: 200 mg·m-2·d-1 for 5 days. The clinical symptom remission rate after the treatment of CyberKnife in one week, the effective rate after CyberKnife in 3 months, the median intracranial progression-free survival time, overall survival, and the incidences of adverse reaction were comparatively analyzed. Results The clinical symptom remission rates of CyberKnife+TMZ group and CyberKnife group after the treatment of CyberKnife in one week were 93.6 % (29/31) and 96.8 % (30/31). There was no significant difference in the clinical symptom remission rates (χ2= 1.207, P=0.547). The effective rates of the two groups after CyberKnife in 3 months were 93.6 % (29/31) and 90.3 %(28/31). There was no significant difference in the effective rates (χ2 = 0.695, P= 0.706). The median intracranial progression-free survival time in CyberKnife + TMZ group (14.0 months) was significantly higher than that in the CyberKnife group (9 months) (χ2=8.977, P=0.003), and the median overall survival time in CyberKnife + TMZ group (15.0 months) was also significantly higher than that in the CyberKnife group (12.0 months) (χ2 = 5.190, P= 0.023). There was no significant difference in the adverse reaction of the central nervous system between the two groups (χ2=0.746, P=0.689), but the adverse reactions of the digestive system (χ2 = 6.062, P= 0.014) and the hematologic system (χ2 = 6.613, P= 0.010) in CyberKnife + TMZ group were significantly higher than those in the CyberKnife group. Systemic adverse reactions of the two groups were tolerated by most patients. Conclusions CyberKnife combined with TMZ is a feasible therapeutic option for NSCLC patients with brain metastases. This therapy can improve the median survival time to cerebral progression of the disease and the median overall survival time.
