1.Effect of argatroban on C-reactive protein and therapeutic efficacy in patients with malignant tumor complicated by acute lower limbs deep venous thrombosis

Objective To investigate the effect of argatroban on C-reactive protein (CRP) and therapeutic efficacy in patients of malignant tumor complicated by acute lower limbs deep venous thrombosis (DVT).Methods 56 patients of malignant tumor complicated by acute lower limbs DVT were randomly divided into two groups.In observation group (n =28),argatroban was given intravenously as anticoagulation.Urokinase as thrombolysis and warfarin as anticoagulation were used in the control group (n =28).The concentrations of serum CRP were detected before treatment and on the 14th day of post-treatment in observation group and control group.Recanalization and therapeutic effect of DVT were compared.Results Before treatment,there was no significant differences in the serum CRP concentrations between observation group and control group [(46.44±28.82) mg/L vs (45.96±29.40) rmg/L,F =0.004,P =0.951].On the 14th day of post-treatment,the CRP concentrations in observation group and control group were lower than those of pretreatment (F =25.996,P < 0.05,F =4.828,P < 0.05).Compared with the 14th day of post-treatment,the CRP concentration of observation group was lower than that of control group [(16.97±10.22) mg/L vs (31.63± 18.07) mg/L,F =13.965,P < 0.01].Recanalization rate of observation group on the 14th day of post-treatment (96.43 %,27/28) was higher than that in control group (67.86 %,19/28) (x2 =8.204,P =0.017).Therapeutic effect rate of observation group on the 14th day of post-treatment (96.43 %,27/28) was higher than that in control group (75.00 %,21/28) (x2 =6.205,P =0.045).Conclusion Argatroban can decrease serum level of CRP in patient of malignant tumor complicated by acute lower limbs DVT,which is favorable for improvement of clinical effectiveness.
2.Prognostic value of fluid-attenuated inversion recovery hyperintense vessel in acute middle cerebral artery occlusion

Objective To evaluate the prognostic value of hyperintense vessel (HV) in patients with acute middle cerebral artery (MCA) occlusion.Methods Seventy-four consecutive patients with first ever stroke(48 male and 26 female,the mean age was (60.7 ± 15.3) years) in the territory of MCA,retrieved from Nanjing Stroke Registry Program between May 2009 and February 2011,were enrolled assubjects.All subjects completed brain MRI,and MRA or DSA indicated proximal MCA occlusion.According to the location and extent of HV,all subjects were classified into 3 groups:without HV,proximal HV and distal HV.Clinical data were obtained and compared among patients with different grades of HV.Logistic regression analysis was employed to confirm the relevant factors of prognosis 90 days after index stroke.Results HV was observed in 49 (66.2% ) of the 74 enrolled patients.Among patients with HV,7 (9.4% ) were classified as proximal HV and 42 ( 56.8% ) as distal HV.Initial NIHSS score ( 11 ( 1 -22) ),10-day NIHSS score ( 13.5(4-25) ),infarction size ( >2/3:5 cases(6.8% ) ),and 90-day mRSscore (3-6 scores:12 cases( 16.2% )) were significantly lower in patients with distal HV than those without (15(6-25),Z=-3.544;7(0-22),Z=-4.461;20 cases(27.0%),x2 =20.916;27 cases (36.5%),x2 =22.689;all P<0.01).The NIHSS score decreased from baseline to that on 10 days and the mRS score decreased from 10 days to that on 90 days in patients with distal HV was more than that in patients without distal HV. Multivariate analysis revealed that patients with older age ( OR =1.111,95% CI 1.036-1.191,P=0.003),high infarction size (OR=3.679,95% CI 1.35-10.025,P=0.011) worsened outcome,whereas distal HV (P =0.012,OR =0.131,95% CI 0.027-0.638)improved outcome.Conclusion Distal HV on FLAIR may predict a favorable outcome in patients with acute middle cerebral artery occlusion.
3.Impact of early lactate clearance rate on damage control surgery in patients with severe trauma

ObjectiveTo monitor the early blood lactic acid concentration of patients with severe trauma who have been experienced routine surgery or damage control surgery,and explore the influence of surgical methods for the early lactate clearance rate.MethodsSelected 40 patients with severe trauma,they were divided into two groups with 20 cases each in accordance with the adopted operation mode,reference group by damage control surgery,and control group by routine surgery.Recorded acute physiology and chronic health evaluation Ⅱ (APACHE Ⅱ ) in patients after admission,blood lactic acid concentration at 6 h after admission and admission,calculated the early lactate clearance rate.ResultsIn reference group,blood lactic acid concentration at 6 h after admission was significantly lower than that in control group[ (3.5 ± 1.1 )mmol/L vs.(4.2 ± 1.4) mmol/L,P< 0.05 ],early lactate clearance rate was higher than that in control group [ (24.6 ± 6.3 )% vs.( 11.4 ± 5.3 )%,P< 0.05 ].Conclusions Damage control surgery in patients with severe trauma in favour of the early removal of lactic acid,maintaining the homeostasis of the organism,is the key to improve the achievement ratio of treatment with severe trauma.
4.Changes of endogenous cystathionine-β-synthase/hydrogen sulfide system in PC12 cells injury induced by rotenone

Aim To research dynamically the changes of endogenous cystathionine- β-synthase/hydrogen sul-fide system in PC12 cells injury induced by rotenone. Methods Rotenone-induced injury in PC12 cells ( characteristic of dopaminergic neurons) was used as a PD cell model. The expression of CBS was evaluated by Western blot. Intracellular CBS activity and H2 S production were detected by Methylene blue spectro-phot-ometric method. The viability of PC12 cells was measured by CCK-8 assay. GSH detection kit was used to detect the intracellular GSH content. Results In the groups of 6 and 12 hours, the expression and activ-ity of CBS were elevated, and the production of H2 S was increased. In the groups of 24 and 48 hours, CBS expression and activity were significantly decreased, and the amount of H2 S was significantly reduced. Ap-plication of 1. 5 μmol·L-1 rotenone for different time (6-48h) could decrease the cell viability and intra-cellular GSH contents in a time-dependent manner. Conclusions The expression and activity of endoge-nous CBS, stimulated by rotenone, are elevated firstly and then decreased. The generation of H2 S, stimulated by rotenone, is increased and then reduced significant-ly, which may be related to PC12 cells against oxida-tive stress damage induced by rotenone.
5.Effect of polydatin on neuropathic pain in rats

Objective To evaluate the effect of polydatin on neuropathic pain in rats.Methods Forty male Sprague-Dawley rats,aged 6-8 weeks,weighing 200-230 g,were randomly divided into 5 groups (n =8 each) using a random number table:sham operation group (group S),neuropathic pain group (group NP),polydatin 5 mg/kg group (group P1),polydatin 10 mg/kg group (group P2),and polydatin 20 mg/kg group (group P3).Neuropathic pain was induced by chronic constriction injury in NP and P1-3 groups.In group S,the sciatic nerve was only exposed but not ligated.In S and NP groups,normal saline 0.1 ml was injected intraperitoneally immediately after operation and at 1,3,5 and 7 days after operation (T1-4).In P1-3 groups,polydatin 5,10 and 20 mg/kg (in normal saline 0.1 ml) were injected intraperitoneally immediately after operation and at T1-4.At 1 day before operation (T0) and T1-4,the mechanical paw withdrawal threshold (MWT) and thermal paw withdrawal latency (TWL) were measured.After measurement of pain threshold at T4,the rats were sacrificed,and L4-6 segments of the spinal cords were removed for determination of the expression of high-mobility group box 1 (HMGB1),Toll-like receptor 4 (TLR4),interleukin-1beta (IL-1β),tumor necrosis factor-alpha (TNF-α) and monocyte chemotactic protein-1 (MCP-1) by Western blot.Results Compared with group S,the MWT was significantly decreased,and the TWL was significantly shortened at T1-4 in group NP,the MWT was significantly decreased at T1-4,and the TWL was significantly shortened at T2-4 in group P1,the MWT was significantly decreased at T1-4,and the TWL was significantly shortened at T3.4 in group P2,the MWT was significantly decreased at T1-4 in group P3,and the expression of HMGB1,TLR4,IL-1β,TNF-α and MCP-1 was significantly up-regulated in NP,P1 and P2 groups (P<0.05).Compared with group NP,the MWT was significantly increased at Tt-4,and the TWL was significantly prolonged at T1,2 in group P2,the MWT was significantly increased,and the TWL was significantly prolonged at T1-4 in group P3,the expression of HMGB1,TLR4,IL-1β,TNF-α and MCP-1 was significantly down-regulated in P2 and P3 groups (P<0.05),and no significant change was found in the parameters mentioned above in group P1 (P>0.05).Compared with group P1,the MWT was significantly increased at T4 in group P2,and the MWT was significantly increased at T14,the TWL was significantly prolonged at T3,4,and the expression of HMGB1,TLR4,IL-1β,TNF-α and MCP-1 was significantly down-regulated in group P3 (P<0.05).Compared with group P2,the MWT was significantly increased at T3,4,and the expression of TLR4,IL-1β,TNF-α and MCP-1 was significantly down-regulated in group P3 (P<0.05).Conclusion Polydatin can alleviate neuropathic pain through inhibiting inflammatory responses in the spinal cord of rats.
6.Expression of bone sialoprotein in prostate cancer and its clinical significance

Objective To investigate the expression of bone sialoprotein (BSP) in prostate cancer and its clinical significance. Methods Prostate cancer tissues of different pathological grades (68 cases) and benign prostatic hyperplasia tissues (22 cases) were selected. SP method was used to detect the expression of BSP. Serum total prostate-specific antigen (tPSA) levels of prostate cancer were detected by electrochemiluminescence immunoassay before the operation. Results Compared with no or low expression in the adjacent normal glandular tissues, the detectable levels of BSP were examined in most of the prostate cancer tissues. The expression rate of BSP in prostate cancer tissues was higher than that in benign prostatic hyperplasia tissues [76.47%(52/68) vs 13.64%(3/22),χ2=27.614, P<0.001]. The expression rates of BSP in well differentiated, moderately differentiated and poorly differentiated tissues according to cell differentiating degree (Gleason system) were 75.0 % (12/16), 77.5 % (31/40) and 75.0 % (9/12) respectively. There was no significant difference in various pathological grading (χ2=0.057, P=0.972). The expression rates of BSP in pathological stage pT2, pT3 and pT4 tissues were 62.16%(23/37), 95.24%(20/21) and 90.0%(9/10) respectively. A statistically significant association was found between BSP expression and pathological stage (χ2=9.338, P=0.009). Serum tPSA level of prostate cancer group with BSP expression was higher than that with no BSP expression [(69.06±25.52)μg/L vs (38.00±21.64)μg/L, F=19.355, P<0.001]. Conclusion The high expression of BSP in prostate cancer has a relationship with pathological stage and serum tPSA level, it may play an important role in the biological behaviour of prostate cancer.
7.Determination of militarine in Rhizoma Bletillae by HPLC.

OBJECTIVETo develop an HPLC method for determination of militarine in Rhizoma Bletillae.
METHODAnalysis was carried out on a Symmetry C18 analytical column, and a C18 guard column eluted with mobile phase consisted of acetonitrile and water (37:63) militarine the detection wavelength was set at 244 nm.
RESULTThe contents of in 12 samples of Rhizoma Bletillae in market were in the ranges of 1.45%-3.32%.
CONCLUSIONThe method is simple, accurate and reproducible, and is suitable for the quality control of Rhizoma Bletillae.

8.Effect of atorvastatin on blood lipid and carotid atherosclerosis in patients with hyperlipidemia and cerebral infarction

Objective To investigate the effect of atorvastatin on blood lipid and carotid atherosclerosis in patients with hyperlipidemia and cerebral infarction.Methods 98 patients with hyperlipidemia and cerebral in-farction in our hospital from September 2014 to February 2017 were selected and divided into observation group(49 cases)and control group(49 cases)according to the random number table method.The control group was treated with conventional treatment,the observation group was treated with atorvastatin on the ba-sis of the control group.The two groups were treated for 4 weeks.The ADL score,NIHSS score,blood lipid index,carotid IM T and carotid plaque area were compared between the two groups before and after treatment. Results ADL score of the patients in the observation group after treatment higher was than the control group (t=12.7783,P<0.05),NIHSS score was lower than the control group(t=10.941,P<0.05);LDL-C,TC and TG levels after treatment in the observation group were than in the control group(t= 17.239,9.216, 9.800,P<0.05),while the HDL-C level was higher than the control group(t=7.683.P<0.05);the patients in the observation group were treated by carotid artery IM T and carotid plaque area was higher than that of the control group(t=8.919,6.344,P<0.05).Conclusion Atorvastatin can improve hyperlipidemia in pa-tients with cerebral infarction and carotid atherosclerosis,dyslipidemia,has the important research signifi-cance.
9.The study of establishing the ideal median sagittal plane of mandible based on multipleanatomic landmarks

Objective:To apply multiple mandibular anatomical landmark points to establish the mandibular median sagittal plane to find and establish the ideal median sagittal plane of the mandible that is suitable for clinical diagnosis and treatment.Methods:The maxillofacial spiral CT data of 30 normal maxillofacial patients in the Western Theater General Hospital were collected, and the 3D cranial model was reconstructed by using Mimics Research 21.0 software to select different anatomical landmarks to establish multiple mandibular MSPs, and with the whole cranial MSP as a reference, the different MSPs were respectively used as the mirror planes to establish a mirror model. The overlap of the original model and the mirror model were compared qualitatively and quantitatively by the overlap three-dimensional color illustration of 3-Matic software and Friedman′s test, respectively, to evaluate the application effect of different planes as mandibular MSPs.Results:There was a statistically significant degree of overlap between the mandibular mirror model and the original model with the whole head MSP and the different mandibular MSPs as mirror planes, respectively (χ 2=12.34, P<0.05). The mirror model of plane Mf-Go-Sim had the smallest overlap with the original model 0.07 (0.04, 0.18) mm; plane N-ANS-PNS was the second smallest 0.14 (0.09, 0.58) mm; the difference in overlap of plane ANS-SBWG-Mp 0.12 (0.04, 0.40) mm; the difference in overlap of plane Me-B-Sim 0.19 (0.09, 0.44) mm; the difference in overlap of plane Po-B-Go 0.18 (0.06, 0.45) mm. Conclusions:Whole-head MSP cannot completely replace mandibular MSP; plane Mf-Go-Sim can be the ideal MSP for the mandible in clinical applications.
10.Lgr5 and CD44 expressions in different types of intestinal polyps and colorectal cancer.

OBJECTIVETo study the expression of tumorigenesis-related stem cell markers Lgr5 and CD44 in different pathological types of intestinal polyps and their clinical significance in predicting tumorigenesis.
METHODSA total of 145 cases of colorectal polyps, adenomas and cancer tissues were obtained by colonoscopy biopsy. Immunohistochemistry was employed to detect the expression of Lgr5 and CD44 to analyze their relationship with the occurrence and prognosis of colon and rectal cancer.
RESULTSThe expression of CD44 in colon cancer tissue was 95.65%, significantly higher than that in normal mucosa (5%), inflammatory hyperplastic polyps (22.58%), tubular adenomatous polyps (55.26%) and villous polyps (75.76%) (P<0.05). The expression of Lgr5 in colorectal cancer was up to 95.65% while negative in normal colorectal tissue and was 16.12% in inflammatory hyperplastic tissues (P<0.05). The expression rate of Lgr5 was 86.84% in tubular adenoma and 93.94% in villous polyps, both comparable with that in colon cancer (P>0.05). Correlation analysis indicated that the expression of CD44 and Lgr5 were positively correlated with the progression of intestinal polyp tumorigenesis (rs=0.69377, P<0.0001; rs=0.81637, P<0.0001).
CONCLUSIONLgr5 and CD44 are highly expressed in colorectal cancer tissues in close correlation with the clinical and pathological features. The expression profiles of Lgr5 and CD44 represent a distinct feature to differentiate colorectal cancer from normal intestinal mucosa. Lgr5 is more closely correlated with tumor progression of polyps than CD44. This means detecting of the expression of Lgr 5 together with CD44 is important and necessary in clinical diagnosis of patients with early stage colorectal diseases such as polyps and their canceration.
