Objective To analyze the occurrence of ventricular arrhythmia in old myocardial infarction (OMI) patients who combined with pathologic Q wave and fragmentation QRS (fQRS), and explore the relationship between QRS duration and ventricular arrhythmia. Methods Three hundred and thirty-five patients with OMI were examined with dynamic electrocardiogram, and they were divided into QRS duration≥110 ms group (M group, 57 cases) and QRS duration<110 ms group (N group, 278 cases). The incidence of ventricular arrhythmia was compared between 2 groups. The patients in M group and N group were divided into simple fQRS group (Ma, Na group), simple pathological Q wave group (Mb, Nb group), both group (Mc, Nc group) and none group (Md, Nd group). The incidences of ventricular arrhythmia among the subgroups were comparatively analyzed. Results The incidences of premature ventricular beats>720 beats/24 h, polymorphic ventricular extrasystoles, pairing ventricular extrasystoles, non-sustained ventricular tachycardia and ventricular escape in M group were significantly higher than those in N group: 38.60%(22/57) vs. 16.55% (46/278), 22.81% (13/57) vs. 7.55% (21/278), 43.86% (25/57) vs. 20.50%(57/278), 19.30% (11/57) vs. 3.96% (11/278) and 14.04% (8/57) vs. 3.96% (11/278), and there were statistical differences (P<0.01). In M group, the incidences of ventricular arrhythmia in Ma,Mb,Mc,and Md group had no significant differences (P>0.05). In N group, the incidences of premature ventricular beats>720 beats/24 h in Na, Nb, Nc and Nd group were 24.73%(23/93), 13.64%(6/44), 25.81%(8/31) and 8.18%(9/110);the incidences of pairing ventricular extrasystoles were 30.11% (28/93), 15.91% (7/44), 29.03% (9/31) and 11.82% (13/110), and there were significant differences ( P<0.01). The incidence of premature ventricular beats >720 beats/24 h of Nc group > Na group > Nb group > Nd group, and the incidence of pairing ventricular extrasystoles of Na group>Nc group>Nb group>Nd group. The incidences of polymorphic ventricular extrasystoles and non-sustained ventricular tachycardia in Ma group were significantly higher than those in Na group:34.78%(8/23) vs. 11.83%(11/93) and 30.43%(7/23) vs. 6.45%(6/93), and there were statistical differences (P <0.01). The incidences of premature ventricular beats >720 beats/24 h, polymorphic ventricular extrasystoles and pairing ventricular extrasystoles in Md group were significantly higher than those in Nd group:4/13 vs. 8.18%(9/110), 3/13 vs. 2.73%(3/110) and 6/13 vs. 11.82%(13/110), and there were statistical differences (P<0.05 or<0.01). Conclusions The longer QRS duration in OMI patients, the higher risk of ventricular arrhythmia. The ventricular arrhythmia risk is positively correlated with QRS duration in OMI patients with fQRS. There is still risk of ventricular arrhythmia in well recovered OMI patients (those without pathological Q wave and with fQRS), and it is positively correlated with QRS duration. There is a higher incidence of ventricular arrhythmia in OMI patients with QRS duration≥110 ms, but it is not significantly correlated with the pathological Q wave and fQRS. Simple fQRS and fQRS with pathologic Q wave in OMI patients with QRS duration<110 ms is significantly correlated with ventricular arrhythmia.

OBJECTIVE:To investigate the key point of pharmaceutical care supplied by traditional Chinese medicine(TCM) pharmacist in current period.METHODS:Questionnaire survey was applied to analyze blind spot and common concern on TCM statistically,taking outpatients of our hospital as example.RESULTS:Specific content of pharmaceutical care of TCM was put forward.CONCLUSION:Hospital pharmacy supplies timely and specific pharmaceutical care of TCM to improve the diagnosis and treatment of TCM.Pharmaceutical care of TCM are required with the development of TCM.

Objective To analyze the immunological feature of the multiple myeloma. Methods The serum of 117 cases were detected by serum protein electrophoresis and immunofixation electrophoresis;and analyzed quantitatively immune globulin(IgG、IgA、IgM) ,total protein and albumin. Results Monoclone protein peak was found in 67 patients out of 117 patients tested(57.3%) ,it located mainly at γ border. Immunoglobulin G was found in 62 patients out of 117 patients tested(53.0%). Immunoglobulin A was 19 cases(16.3%) ,Immunoglobulin M was 12 cases(10. 1%) ,DL was 2 cases(1.7%) ,free light chain was 22 cases(18.8%). Homotypic immunoglobulin level increased notably and non-correspond constituent was low in immunoglobulin test. The protein quantified displayed that total protein increased and albumin decreased in different level. Conclusion The seroimmunological index played a crucial role in diagnois,clinical stage and prognosis of MM.

Three compounds of metalloporphyrins were studied using electrospray ionization mass spectrometry.The bonding power between substitutional phenyl and porphyrin cycle and the coordinate conditions of metalloporphyrins with imidazole were disscused. The experimental result indicated that the bonding power between substitutional phenyl and porphyrin cycle in metalloporphyrins became weak from Mn,Fe to Co.The complexes abundances formed by metallophorphyrin with imidazole were stronger with the increase of the ligand concentration.At the same ligand concentration,the abundance of the complexes was intensified gradually and the stability of the ligands was become stronger from Mn,Fe to Co.

To compare the characteristics of the traditional Chinese medicine (TCM) syndromes of gallbladder heat attacking the stomach and stagnant heat of the liver and stomach in patients with reflux esophagitis (RE), in terms of clinical symptoms, combination of gallbladder conditions, esophageal mucosal inflammation, gastric bile reflux under endoscopy and helicobacter pylori (HP) infection.

Objective To analyze the changes of WBC classification,sugar and protein in cerebro-spinal fluid(CSF)and pathogenic bacteria of neonatal purulent meningitis.Methods Thirty-one neonates with bacterial meningitis in our department of neonatology from June 201 1 to June 2013 were enrolled,and the clinical features,pathogenic bacteria,laboratory examination of CSF were analyzed.Results Fever (90.3%),convulsions(67.7%)and changed consciousness(58.1 %)were common clinical symptoms.The incidences of other nervous system abnormal signs such as gastrointestinal dysfunction(25.8%),abnormal breathing(16.1 %),cervical resistance(16.1 %),bulging fontanel(9.7%)were lower.The Gram -negative bacteria was more than Gram -positive in both blood and CSF culture.The escherichia coli was the most common bacteria,with positive rate of 38.1 %(8 /21 )in blood culture and 55.5%(5 /9)in CSF culture.The germiculture positive rate in CSF was lower than in blood culture (29.0% vs.67.7%).Polymorphonuclear leukocyte(PMN)[(79.61 ±12.06)%]was the most predominant cell of the leukocyte classification in CSF within 1 week in all cases,PMN was still predominant in 1 to 2 weeks in 7 cases,while only 2 cases in 2 to 3 weeks still dominated by PMN,PMN was not the predominant cell 3 weeks later.Conclusion In the typi-cal neonatal purulent meningitis,PMN was the predominant cell in CSF within the first week,but the propor-tion of monocyte gradually increased and was dominant later.Escherichia coli was a common bacteria caused by this disease.

ObjectiveTo explore the relationship of the fragmented QRS (fQRS) and the fQRS time limit with ventricular arrhythmia in old myocardial infarction (OMI) patients through contrasting the incidence of ventricular arrhythmia in OMI patients whether fQRS or not and ventricular arrhythmia in different fQRS time limit.MethodsAccording to the routine electrocardiogram,321 OMI patients were divided into group A (fQRS appearance,167 cases) and group B(fQRS non-appearance,154 cases).The lead with fQRS extense was ehosen and traced another 50 mm/s electrocardiogram,and 3 consecutive fQRS time limit were measured and them average was taken in group A.According to the fQRS time limit,the patients in group A were divided into 3 groups: group X( ≤0.100 s,96 cases),group Y (0.101-0.119 s,54 cases) and group Z( ≥0.120 s,17 cases).All the patients were continuously monitored with 24 hours dynamic electrocardiogram,and the incidence of ventricular arrhythmia was analyzed.ResultsThe incidence of ventricular arrhythmia in group A [ 78.4％ ( 131/167 ) ] was higher than that in group B [ 63.6％(98/154) ] (P＜ 0.01 ).The incidence of premature ventricular contraction(PVC) ＞ 720/24 hours in group A [ 28.7％(48/167 ) ] was higher than that in group B[ 17.5％(27/154) ] (P ＜ 0.05 ).The incidence of multifocal PVC,coupled PVC,nonsustained ventricular tachycardia and Lown 3-5 grades PVC was 16.2％ (27/167),33.5％ (56/167),12.0％ (20/167),34.1％ (57/167) in group A,7.8％ (12/154),21.4％ (33/154),4.5％(7/154),23.4％(36/154) in group B,there were significant differences between two groups (P＜ 0.05 ).The incidence of ventricular arrhythmia in group Z [ 100.0％( 17/17 ) ] was significantly higher than that in group Y [79.6％(43/54)] and group X [74.0％(71/96)](P＜ 0.05).The incidence of Lown 3-5 grades PVC in group Z[ 70.6％( 12/17 )] was significantly higher than that in group Y[ 42.6％(23/54)] and group X [ 22.9％(22/96) ],and the incidence of Lown 3-5 grades PVC in group Y was significantly higher than that in group X (P＜ 0.05).ConclusionsOMI patients with fQRS have higher incidence and severe degree in ventricular arrlhythmia than those without fQRS.With the fQRS time limit widened,PVC and Iown 3-5 grades PVC significantly increased.So fQRS is a new predicting index of OMI,and fQRS time limit has definite value in predicting the heart event for OMI patients.

OBJECTIVE:To provide reference for the establishment and improvement of work system for clinical pharmacist in PIVAS of our province even the country. METHODS:Questionnaire was conducted to investigate the PIVAS directors in the 65 third-grade class A hospitals in China,and the data was statistically analyzed. RESULTS:Totally 65 questionnaires were sent out, 63 were effectively received,with effective rate of 96.9%. 98.4% respondents thought PIVAS should equip clinical pharmacist,but the actual situation in respondent's hospital was not ideal. 100% thought clinical pharmacist in PIVAS should have the ability of prescription checking and prescription evaluation;more than 95% thought clinical pharmacist in PIVAS should assume the medi-cine publicity and education and need to have good communication with clinic;only 42.9% thought clinical pharmacist in PIVAS need to take part in the routine drug dispensing. CONCLUSIONS:It is necessary to equip clinical pharmacist with professional training in PIVAS. Clinical pharmacist in PIVAS should focus their ability on prescription checking and evaluation,medicine public-ity and education,communication with clinic.

Objective To investigate treatment of meditation training for elderly patients with cogni-tive disorder recovering from operation for valvular heart disease( VHD) . Method 67 elderly patients who were up to the inclusion criteria were divided into experiment group( n=34)and control group( n=33),by dropping coins according to the admission time.Both of the two groups accepted regular health education and mental nursing.Besides,the experiment group accepted meditation training 5 times a week. Montreal Cogni-tive Assessment (MoCA) was used to evaluate cognitive function before and after the treatment. Results ( 1) Ridit analysis showed the intervention effect of the experimental group was better than that of the control group( R=2.12, t=-2.48, u=0.02),and the difference was statistically significant(P<0,05).The total effec-tive rate of experiment group is 90.32%,while the rate of control group is 62.89%. (2)The score of MoCA for the experiment group(25.79±1.23)was higher than that of the control group(23.85±1.82)after 6 weeks intervention,and the difference was statistically significant( t=5.14,P<0.01).(3) The score of MoCA of the experiment group before the intervention (23.91±2.22)was lower than after 6 weeks intervention(25.79±1. 23),and the difference was statistically significant( t=-4.30,P<0.01).Conclusion Meditation training can effectively improve cognitive function of elderly patients recovering from operation for VHD.

Objective To investigate the clinical and pathologic features of small gastrointestinal stromal tumors (small GISTs,d ＜ 2.0 cm).Methods Medical records of 95 patients undergoing surgery (endoscopic surgery,thoracoscopic/laparoscropic surgery and open surgery)and diagnosed as having GISTs by pathology and immunohistochemistry in Nanfang hospital from October 2003 to June 2014 were retrospectively analyzed.Based on clinical and pathological results,correlation analyses between risk factors for endoscopic ultrasonography(EUS) and Mitotic count(MI),clinicopathologic parameter and NIH risk classification were performed.Results Among 95 cases (104 lesions),88 were single,while 7 were multiple;81.7％ (85/104) small GISTs arose from stomach,including 87.1％ (74/85)in middle-upper stomach;5 cases (5.3％) presented calcification of different degrees,3 cases(3.2％) presented local necrosis and 2 cases (2.1％) with arrangement of epithelioid cells;88 cases (92.6％) were very low grade of NIH risk classification,6 cases (6.3％) were intermediate risk and 1 case(1.1％) was high risk.Positive rates of CD34 and CD117 were 95.8％ (91/95) and 96.8％ (92/95) respectively.The risk factors (border,mucosal surface,echo and heterogeneity) of EUS had no correlation with mitotic count(P＞0.05).The correlation analysis between clinicopathologic features and NIH risk classification revealed tumors more than 1.5 cm had a striking correlation with NIH risk classification (P＜ 0.05).Conclusion Most small GISTs,single or multiple,located at middle-upper stomach,were of very low or low risk,and have a favorable prognosis.But it has worse biological behavior and a higher grade risk when the diameter is more than 1.5 cm,intervention should be recommended.
