1.Surgical treatment of acetabulum fracture with dislocation of hip joint

[Objective]To discuss the fractures classification,operative route,internal fixation and operative therapeutic effection of acetabular fractures.[Method]Thirty-four cases of acetabulum fractures and femoral head dislocation were involved from November 2001 to August 2006 in our hospital throught operative treatment.The classification according of AO system:type A fractures 24 cases,type B 10 cases.Applicated Kocher-Langenbeck operative route was applied in 20 cases,Gibson route 2cases,illioinguinal route 4 cases and anteriorcombined posterior route 4 cases.Thirty-two cases were adopted reconstruction plates and screws and 2 cases steel-wire cerclage to fix fracture.All patients were followed up.[Result]Acorrding to evaluation system of AAOS:excellent in 12 cases,good in 12 cases,fair in 4 cases,poor in 6 cases.Total exellence rate were 71%.[Conclusion]Combinate full-scale imageology information,correct classification of fracture according of AO,reasonable surgical approach and fixation,correct subtle technique during the operation and effective fracture resetting are keys to improve the effect of acetabular fractures.
2.Ti-alloy mesh and impaction morcellized cancellous allograft for acetabular deficiency in revision total hip arthroplasty

OBJECTIVE:To observe the effectiveness of using Ti-alloy mesh and morcellized cancellous allograft in treatment of acetabular deficiency in revision total hip arthroplasty. METHODS:A total of 31 patients with acetabular defects underwent impaction morsellized cancellous allografting combined with Ti-alloy mesh for the reconstruction of the defected acetabula at the Second Department of Orthopaedics,First Hospital,Jilin University from January 2003 to July 2008. There were 13 males and 18 females,aged from 56-77 years,averagely 65 years. The time from previous surgery to this revision surgery was 2.5-11.0 years,averagely 8.3 years. The defects were classified into various types according to AAOS:type Ⅱ in 14 hips,type Ⅲ in 17 hips. The average pre-operation Harris score were 37 points. Bone cement was used to fix the prosthesis in 31 patients,and clinical evaluation and X-ray assessment were conducted following surgery. RESULTS:Following-up was done in 31 patients for 3 months to 5 years. 24 hours following surgery,passive functional exercises were conducted in the lower limb of patients. At 3 weeks,20 patients could get out of bed,with incompletely loading. Eleven patients got out of bed at 5 weeks following surgery,and did normal activities at 3 months. Harris score elevated averagely 45 points half years following surgery compared with preoperation,including 20 excellent cases,9 good cases,and 2 average cases,resulting in the excellent and good rate of 94%. At 3 months following surgery,no infection,fracture around the prosthesis,dislocation,nerve vessel and pelvic cavity organ damages or embolism were detected. The wound was delayed union,with discontinuous effusion in 3 patients till 3 weeks following surgery. None of these patients needed to be revised again and 2 cases had radiolucent line in radiograph plain film,which indicated that the bone graft might be absorbed. CONCLUSION:The treatment of acetabular defects by Ti-alloy mesh and impaction allograft in hip revision operation is easy and effective method.
3.Effect of Cervical Stability Training on Cervical Vertigo

ObjectiveTo investigate the effects of cervical stability training on cervical vertigo. Methods67 patients were divided into 2 groups. The experiment group was treated with Neurac (neuromuscular activation) for cervical stability training, meanwhile, the control ones with manipulation. They were assessed with clinical assessment and fall index before and 14 d after treatment. ResultsThe scores of clinical assessment and value of fall index were significant improved more in experiment group than in controls (P<0.05). ConclusionSpecific cervical stability training is effective on cervical vertigo based on cervical instability and proprioceptive disorder.
4.The SWOT analysis on private hospitals in the background of new healthcare reform

Objective:This study aims to ensure the private hospitals development strategy in the background of new healthcare reform and put forward some scientific suggestions to help promote their sustainable development. Methods:The SWOT analysis method was adopted to analyze the private hospitals’ internal strength, weakness, exter-nal opportunity and threats. Results: The results showed a clear property right, advanced management system and other outstanding advantages in the private hospitals. Opportunities like the improved policy environment and people’s medical demand expansion are also very prominent. Meanwhile, the private hospitals development also faces some disadvantages and challenges such as illegal operation, disorder in the market competition, vague orientation, etc. Conclusions:It suggests the strengthening of the personnel and hospital culture construction, adoption of the differen-tiation strategy, improvement of the diagnosis and treatment ability, etc.
5.The application of the network investigative teaching in biochemistry teaching

Objective To explore teaching effectsof biochemistry by comparingthe traditional teaching with the network investigative teaching. Methods 425 five-year-program students of clinical medicine of grade 2012 were chosen as subjects, with 246 students from Classes 5-8 as experimental group in which network investigative teaching was implemented andwith 179 students of Classes 17-20 as the control group which wasgiven traditional teaching. After the teaching , the teaching effects were evaluated through a questionnaire and the test scores of the two groups were compared and statistically analyzed. The two groups were compared by using t test. Results More than 70% of students in the experimental groups were generally satisfied with several aspects of the network investigative teaching and gavea good evaluation. The experimental group students' total scores and every item scores were all higher than the control group students', in which the practise scores and the case analysis problem scores had significant differences by statistical analysis with P values being 0.000 and 0.002, respec-tively. Conclusion Network investigative teaching is better than traditional teaching in the teaching of biochemistry , which can enhance students' ability of problem analysis and their enthusiasm for learning.
6.Role of tripartite motif-containing 23 in the differentiation and maturation of dendritic cells

Objective:To investigate the effect of tripartite motif-containing 23 (Trim23) on the differentiation and maturation of dendritic cells and the possible mechanism.Methods:Mouse bone marrow-derived dendritic cells (BMDCs) were prepared from bone marrow cells of C57BL/6 mice with the presence of Flt3L. Real-time quantitative PCR and Western blot were used to detect the expression of Trim23 in BMDCs after LPS stimulation. An overexpression vector for full-length Trim23 (Trim23 OE) was constructed and transfected into BMDCs, and the pcDNA3.1 empty vector was used as control. Flow cytometry was used to detect the expression of CD80, CD86, CD40 and MHCⅡ on the surface of vector-transfected BMDCs after LPS stimulation and ELISA was used to detect the secretion of IL-12p40, TNF-α, IL-6 and IL-10 by these cells. CD8 + and CD4 + T cells were isolated from spleen and lymph nodes of OT-Ⅰ and OT-Ⅱ mice by magnetic beads and co-cultured with LPS-treated BMDCs in the presence of ovalbumin (OVA). Flow cytometry was used to detect the proliferation and differentiation of CD8 + and CD4 + T cells. Western blot was performed to analyze the phosphorylation of p38, ERK1/2 and AKT in BMDCs. Two overexpression vectors for Trim23 mutants lacking RING or ARF domain (Trim23 ΔRING and Trim23 ΔARF) were constructed and transfected into BMDCs. Then flow cytometry and ELISA were used to detect the expression of surface molecules and cytokines. Results:The expression of Trim23 in BMDCs was significantly down-regulated after LPS stimulation. The expression of MHCⅡ, CD86 and CD80 and the secretion of TNF-α and IL-6 decreased significantly in BMDCs overexpressing Trim23. Furthermore, overexpression of Trim23 inhibited the ability of BMDCs to induce the proliferation and differentiation of CD4 + T cells and the proliferation of CD8 + T cells. Western blot showed that the phosphorylation of p38 and ERK1/2 decreased significantly in Trim23-overexpressing BMDCs. Compared with wildtype Trim23, overexpression of Trim23 ΔRING had no significant influence on the expression of surface molecules (MHCⅡ and CD86) and the secretion of cytokines (TNF-α and IL-6) in BMDCs stimulated by LPS. Conclusions:Trim23 overexpression inhibited the maturation and immune activation of BMDCs via MAPK signal pathway and its RING domain. This study provided reference for targeting Trim23 to improve the immune response of dendritic cell-based tumor vaccines.
7.Inhibitory effect of silencing HMGB1 gene on growth of human epithelial ovarian cancer xenografts in nude mice

[Abstract] Objective: To investigate the effect of HMGB1 gene on the growth of human epithelial ovarian cancer xenografts in nude mice, and to lay a foundation for finding new targets for the treatment of ovarian cancer. Methods: Human epithelial ovarian cancer SKOV3 cells in logarithmic growth phase were selected to establish a human epithelial ovarian cancer xenograft model in nude mice. Nude mice with successful model establishment were randomly divided into control group and HMGB1-siRNA group. On the 7th, 9th, 11th, 14th, and 16th days after cell inoculation, the same amount of saline and HMGB1-siRNA were respectively injected into two groups of mice under the armpit.After 3 weeks, the nude mice were sacrificed by cervical dislocation, the tumor tissues were separated, and the volume of the tumor was measured. The apoptosis of transplanted tumor cells was detected by Tunnel staining. The expressions of HMGB1, STAT3 and p-STAT3 were detected by Western blotting. The expression of vascular endothelial growth factorA(VEGF-A) and microvascularization were detected by immunohistochemistry. Results: Compared with the control group, the growth of tumor volume slowed down in HMGB1 siRNA group, and on the 21st day, the tumor volume of HMGB1-siRNA group was significantly smaller than that of the control group (P<0.05). HMGB1-siRNA successfully knocked down the expression of HMGB1 mRNA in transplanted
tumor tissue. The apoptosis rate of tissue cells in HMGB1-siRNA group was significantly increased ([34±8]% vs [6±2]%, P=0.04), and the expressions of HMGB1 and p-STAT3 were significantly reduced (P<0.05). The expression of VEGF-Aand the number of microvessels were significantly lower than those of the control group (both P<0.05). Conclusion: Knockdown of HMGB1 gene reduces the expression of VEGF-A and microvessel formation possibly by inhibiting the HMGB1/STAT3 signaling pathway, thereby promoting the apoptosis of tumor tissues and slowing the growth of xenografts.
