To investigate the temporal variation of cell-mediated immunity in injured tissue of maxillofacial firearm trauma. Cells expressing messenger RNA(mRNA) for IL-2 were demonstrated by dot blotting and in situ hybrization in injured tissue of maxillofacial trauma inflicted by bullet, in conjunction with immunohistochemistry for IL-2 in normal and postinjury conditions. It was found that, apart from a transient eleratim in level within 24h post injury, both IL-2 production and IL-2 mRNA expression were consistently suppressed in cells at the site of the wound compared with the normal. The changes in the tissues of indirect injury by the projectile were significant. These results indicate that the cell-mediated immunity was suppressed in maxillofacial firearm trauma and the principal cellular abnormalities that resulted in altered T cell activation and IL-2 production postinjury was downregulation of mRNA before IL-2 gene transcription. It also suggested that the indirect injury by projectile contribute mainly to the depression of cell-mediated immunity.

Objective To investigate the clinical and laboratory profiles of a patient with pustular psoriasis-like skin lesion and cerebral palsy due to biotinidase deficiency. Methods A 5 year and 4 month-old boy with biotinidase deficiency was confirmed by urinary organic acid analysis with gas chromatography/mass spectrometry (GC/MS)and biotinidase activity assay of peripheral blood. His clinical features, laboratory findings, treatment and outcome were studied. Results The boy showed difficulty in taking food after birth, gradually eczema and pustules appeared at the age of 2 months, and generalized erythema and intractable pustular psoriasis-like lesion at the age of 8 months. His intellectual development was normal with retardation of locomotor system. He had muscular dystonia at the age of 6 months. Physical examination showed generalized pustular psoriasis-like lesion, generalized paralysis, hypertonic contracture of extremities, sparseness of scalp hair and severe malnutrition. Routine laboratory tests showed a mild anemia, metabolic acidosis and elevation of plasma creatine phosphokinase. Increased excretion of urinary lactate, pyruvate, 3-OH-propionate, propionylglycine, and 3-methylcrontonylglycine were observed. Biotinidase activity of his peripheral blood was below 0.1 pmol/min/3mm (normal 6.3-9.3 pmol/min/3mm). Biotin (10 mg/day) supplementation led to a dramatic recovery of the skin lesion. After the treatment of rehabilitation, his muscle power was also improved gradually. Conclusions Dermatological and neurological manifestations are the main features of biotinidase deficiency. Early diagnosis and biotin administration can greatly improve the clinical symptoms. Generalized pustular psoriasis-like lesion and cerebral palsy of this boy have improved after the supplementation of biotin, but he may be remained wheelchair-dependent because of delayed diagnosis.

OBJECTIVEThis study was to establish an animal model to imitate facial nerves injury by explosion.
METHODSThe impact was simulated by detonator blast in the distance of 5 cm, 10 cm and 15 cm over the face of dogs under anesthesia and the edge injury of masseters were made by rifle-shot steel ball to imitate segment injury in real explosion. The dogs were killed after different time of injury and the heart, lung, brain and facial nerve were taken to observe the pathological changes in order to evaluating the wound effect in different distances.
RESULTSThe animal was injured severely in the distance of 5 cm and could not survive for a long time because of severe damage to brain, heart and lung. The dogs injured at 10 cm could survive after emergency treatment, and there was diffusing hemorrhage in edematous nerve trunk. In the distance of 15 cm, the dogs were injured slightly and, as the distance to the explosive source increased, the local wound became slighter. Under light microscope, the breakage and necrosis of facial nerve fibers could be widely observed, degenerative and necrotic neurons with infiltrating inflammatory cells could be found in the facial nerve nuclei as well.
CONCLUSIONThe dog's traumatic model established by impact wave and segment in this experiment is an appropriate animal model for the research of explosive effect on facial nerves because of its controllable and repeatable injuring conditions.

Journal of International Oncology 2024;51(7):474-477
Acute myeloid leukemia (AML) is a common malignant disease of the hematological system, with high EVI1 expression accounting for 8%-10% of adult AML. Studies have shown that high EVI1 expression plays an important role in the treatment and prognosis of AML. In recent years, researchers have continuously revealed the structure and role of EVI1, but its mechanism of mediating AML has not been fully clarified. Therefore, systematically exploring the role of EVI1 in AML may provide a useful reference for the precise treatment of AML patients with high EVI1 expression.

AIM:To establish a project management tool to measure the working ability of research coordinators and the workload of clinical trial projects, and optimize medical institutions or clinical trial site management organization (SMO) allocation basis of clinical trial items by collecting the work stress and personal ability values of clinical coordinators. METHODS: Different words in the five major databases were searched and information on the work capacity and workload in the Subei People' s Hospital was collected. In addition, the paper analyzes the characteristics of coordinators working pressure source by SPSS26.0 statistical software and stability coefficient method. RESULTS: The study established the clinical research coordinator's personal ability - assessment tool (CRCPA-AT) and clinical trial project workload - assessment tool (CTPW-AT) to assess the coordinator's working ability and clinical trial workload. The two tools in this study were tested retrospectively in 61 coordinators and 144 clinical trials, 39 (66.1%) coordinators' projects were reasonably allocated, 18 (30.5%) coordinators were in the state of theoretical pressure and 12 (20.3%) coordinators had high actual pressure, which matched with the theoretical pressure. CONCLUSION: In this study, two scoring scales were established to quantify the working ability of the coordinators and the workload of clinical trial projects. Through the verification of the 61 coordinators and its management projects, the Likert5 point scoring method was used to analyze, more than 86.4% of the actual pressure of coordinators was consistent with the theoretical pressure quantified by the tool.

Journal of Environmental and Occupational Medicine 2023;40(9):1052-1057
Background Few studies have been conducted on the associations of dietary habits with the risks of high normal blood pressure and hypertension in employees of chemical companies in northwest China. Objective To understand the excess intakes of red meat, offal, and salt, the current status of high normal blood pressure and hypertension detection among employees of a large chemical company in Gansu, China, and to analyze the relationships between different dietary habits and the risks of high normal blood pressure and hypertension. Methods A large state-owned chemical enterprise in Gansu Province was selected and 914 employees were recruited by age-stratified random sampling. A set of questionnaires and health examination were adopted for the survey. χ2 test was used to compare dietary habits. A logistic regression model was used to analyze the relationships between different dietary habits and the risks of high normal blood pressure and hypertension in the chemical enterprise employees after including the factors significantly different in the χ2 test. Results A total of 914 workers completed the survey. Significant differences were found in the distribution of normal blood pressure, high normal blood pressure, and hypertension among the participants grouped by gender, age, body mass index (BMI), and family history of hypertension (P < 0.001). The excess intake rates of red meat, offal, and salt were 25.4%, 29.0%, and 62.1%, respectively. Significant differences were found in the distribution of normal blood pressure, high normal blood pressure, and hypertension among the participants grouped by intakes of meat, offal, and salt (P < 0.001). The highest positive rate of high normal blood pressure was detected in the workers with excess intakes of offal and salt (P < 0.001). The prevalence rate of hypertension increased with higher intakes of red meat, offal, and salt (P < 0.001). After adjusting for gender, age, BMI, family history of hypertension, and other factors, the results of logistic regression showed that the risks of high normal blood pressure and hypertension in workers with excess red meat intake were 1.55 (95%CI: 1.00, 2.38) times and 2.88 (95%CI: 1.63, 5.07) times higher than those in workers with low red meat intake respectively. The risks of high normal blood pressure and hypertension in workers with excess offal intake were 1.75 (95%CI: 1.12, 2.72) times and 1.87 (95%CI: 1.01, 3.44) times higher than those in workers with no offal intake respectively. The risks of high normal blood pressure and hypertension in workers with excess salt intake were 1.80 (95%CI: 1.22, 2.65) times and 2.59 (95%CI: 1.63, 4.11) times higher than those in workers with moderate salt intake, respectively. Conclusion Excessive intakes of red meat, offal or salt are common among the chemical enterprise workers. Excess intakes of red meat, offal, and salt are risk factors for both high normal blood pressure and hypertension. The northwest chemical enterprise should further advocate a healthy and reasonable diet among employees, and propose to reduce the intakes of red meat, offal, and salt.
