1.The changes of peripheral blood lymphocyte subsets in patients with idiopathic thrombocytopenic purpura

ObjectiveTo investigate the changes and clinical significance of peripheral blood lymphocyte subsets in patients with idiopathic thrombocytopenic purpura (ITP). MethodsPAIgG, PAIgM, PAIgA in platelets and CD3+ , CD4+ , CD8+, CD4+/CD8+ antigen expression in peripheral blood of 42 patients with ITP and 30 normal controls were detected by ELISA and FACS, respectively. And the correlation between the two indicators was evaluated.ResultsThe levels of PAIgG, PAIgA, PAIgM in platelets of patients with ITP were significantly higher than the normal controls( P < 0.01 ). Compared with the normal controls, the percentage of CD3+ and ratio of CD4+/CD8+ cell were significantly lower in ITP patients than that in normal controls, and the percentage of CD8+ cell in ITP patients was much higher than that in normal controls. The decreasing ratio of CD4+/CD8+ cell was significantly correlated with the levels of PAIgG,PAIgM, PAIgA. ConclusionTlymphocyte subsets showed abnormal function and ratio of lymphocyte subsets in ITP patients, and played an important role in pathological mechanism and diagnosis of ITP.
2.Changes of serum PCⅢ,Ⅳ-C,LN,TPO and EPO in patients with Leukemia

Objective To detect the levels of serum procollagen type Ⅲ (PCⅢ), Ⅳ collagen(Ⅳ-C) ,laminin ( LN ) , erythropoietin ( EPO ) and thrombopoietin ( TPO) in patients with acute and chronic leukemia. Methods PCⅢ , Ⅳ-Cand LN levels were determined by radioimmunoassay and TPO,EPO levels were determined by enzyme-linked immunosorbent assay,for 26 acute leukemia patients,16 chronic leukemia patients,and 16 normal controls. Results PC Ⅲ[(164.02 ± 118.53) μg/L], Ⅳ-C[(66.54 ± 19.48) μg/L] ,LN[(122.85 ±25.10)μg/L], EPO[(52.90±34.87)mU/ml]and TPO[(642.54±318.49)ng/L]levels of patients with acute and chronic leukemia were higher than that of the healthy control group before chemotherapy (P < 0. 05). EPO [( 72. 21 ± 52. 15 )mU/ml]and TPO[(642.54 ±318.49)ng/L]levels of acute leukemia patients after treatment were significantly lower than before treatment(P<0.05),while PC Ⅲ ,Ⅳ-C,LN levels had no significant difference(P >0.05). There was no significant difference before and after chemotherapy for PC Ⅲ, Ⅳ-C,LN,TPO and EPO levels of chronic leukemia patients (P> 0.05). Conclusion PC Ⅲ,Ⅳ-C,LN,TPO and EPO of patients with acute and chronic leukemia were abnormal, and dynamic monitoring of EPO and TPO levels could reflect the effect of AL chemotherapy, but conld not contribute to the prediction of CML blast crisis.
3.Expression of bcl-2 in Mareks disease tumor

AIM: Expression of bcl-2 in Marek's disease (MD) tumor was analyzed and relationship between bcl-2 gene and lymphomagensis clarified. METHODS: Model of MD tumor was reproduced on SPF White Leghorn chickens, collected tumor and normal tissues, conserved in liquid nitrogen. Total RNA in the tissues was obtained by AGPC method, detected their concentration and analyzed by 1% denatured agrose gel containing formaldehyde. A plasmid (pTTCB1) that contains chicken bcl-2 1.2 kb fragment was amplified, extracted and digested with restriction endonuclease (RE), the 1.2 kb fragment was recoveried and puarified with purigene kit, labeled probes with [α-32P]. Expression of bcl-2 was analyzed with Dot blot and Northern blot hybridization. RESULTS: (1) There are bcl-2 transcripts in MD tumor and relevent normal tissues; (2) Expression of bcl-2 in MD tumor is much higher than that in normal tissues. CONCLUSION: Our findings indicated that expressive products of chicken bcl-2 promotes lymphomagenesis of MD by blocking apoptotic cell death.
4.Clinical observation of pemetrexed contrast to the best supportive care in the patients with advanced non-small cell lung cancer

Background and purpose:Lung cancer is the one with the highest incidence tumors, is mainly appeared to be non-small cell lung cancer(NSCLC). The main treatment method is chemotherapy. However, it is controversial whether using chemotherapy in the patients of advanced NSCLC. The present study aimed to observe the efifcacy and toxicity of pemetrexed contrast to the best supportive care in the NSCLC patients of ECOG=2 and older than 70.Methods:A total number of 84 patients of ECOG=2 and older than 70 with advanced NSCLC were randomly assigned into two groups. The study group accepted pemetrexed, 500 mg/m2,every 3 weeks as a cycle, and the control group was given 200 g glucose supply and protein according to patients’ weight every day. Evaluation of the study group was performed at end of 2 cycles and the control group was evaluated after 2 months.Results:There was no CR patient in 2 groups. In the study group there were 6 patients of PR (14.3%), 21 patients of SD (50%) and 13 patients of PD (30.9%), the effective rate was 14.3%, and the disease control rate was 64.3%. There were 1 patient of PR (2.4%), 15 patients of SD (35.7%), 25 patients of PD (60.9%), 1 patient of death, the effective rate was 2.4% and the disease control rate was 39.0% in the control group (P<0.05).Conclusion:The study group has better treatment effect and adverse reaction in patients of ECOG=2 and older than 70 with advanced non-small-cell lung cancer and the toxicity could be tolerated.
5.Observation of Clinical Efficacy of Dong’s Pushing Epiglottis Cartilage Method for the Treatment of Children Food Retention

Objective To observe the clinical efficacy of Dong’s Pushing Epiglottis Cartilage Method for the treatment of children food retention through randomized, parallel and controlled clinical trial.Methods Totally 72 children diagnosed with children food retention were randomly divided into treatment group and control group, 36 cases for each group. Children of the treatment group were treated with Dong’s Pushing Epiglottis Cartilage Method, while children of the control group were treated with acupuncture Four Seam. The clinical symptoms, gastric half-emptying time and antral contraction frequency of the two groups before and after the treatment were compared. Results After treatment, integral of clinical symptom in both groups were obviously improved compared with before treatment (P<0.05), but there was no significant difference between the two groups (P>0.05). After treatment, gastric half-emptying time and antral contraction frequency of the both groups were obviously improved, with significant difference (P<0.05), but there was no significant difference between the two groups (P>0.05). The total effectiveness of the treatment group was 97.2% (35/36) and the control group was 94.4% (34/36), with no significantly difference (P>0.05).Conclusion Dong’s Pushing Epiglottis Cartilage Method has relatively good clinical efficacy for the treatment of children food retention.
6.The effect of glial cell line derived neurotrophic factor on facial nerve regeneration following injury

?Objective: To study the effect of glial cell line derived neurotrophic factor (GDNF) on facial nerve regeneration after injury.Methods:Defect of 5 mm was made in each facial nerve on both sides in 32 rabbits.Silicon tubes were used to bridge the defects.Pure GDNF (15 ?g in 30 ?l silane) was injected into silicon tube on one side and saline (30 ?l) on the other side as the control.Nerve regeneration was studied by morphological observation with HE and Bielschowsky stainning,counting of nerve neurons and axon and measuring of motor nerve conduction velocity.The rest 3 rabbits was as blank contol. 7.T-helper (Th) 17 cell quantity and related cytokine expressions in skin lesions and peripheral blood of patients with psoriasis vulgaris

Objective To detect the quantity of Th17 cells and expressions of related cytokines including interleukin (IL)-17 and IL-22, in peripheral blood and skin lesions of patients with psoriasis vulgaris, and to analyze their correlation with disease severity and clinical course. Methods Peripheral blood was obtained from 44 patients with progressive psoriasis vulgaris and 28 normal human controls. Three-color flow cytometry was carried out to detect the quantity of Th17 cells, and ELISA to examine the levels of serum IL-17 and -22.Skin samples were obtained from 20 patients with psoriasis vulgaris and 8 normal human controls, and a quantum dot-based double labled immumofluorescence method was used to determine the quantity of Th17 cells.Results The percentage of peripheral blood Th17 cells was higher in patients with psoriasis vulgaris than in normal human controls (4.71% ± 2.55% vs. 0.55% ± 0.39%, P < 0.01 ). Elevated expressions of IL-17 and IL-22 were noted in the patients compared with the normal human controls (24.02 ± 12.31 ng/L vs. 7.16 ±4.04 ng/L, P < 0.05; 18.32 ± 8.14 ng/L vs. 6.52 ± 4.15 ng/L, P < 0.01 ). The percentage of peripheral blood Th17 cells and serum levels of IL-17 and IL-22 were positively correlated with psoriasis area and severity index (r= 0.53, 0.47, 0.53, respectively, all P < 0.01 ), but unrelated to the clinical course of psoriasis (r = 0.09,0.03, 0.19, respectively, all P > 0.05). There was an infiltrate of Th17 cells in psoriatic lesions, which was mainly distributed around the blood vessels in superficial dermis, whereas there were only a small number of CD4+ T cells in the normal control skin with the absence of Th17 cells. Conclusions Th17 cells are involved in the development of psoriasis, and Th17 cell-secreted cytokines, such as IL-17 and IL-22, may serve as a new therapeutic target for psoriasis.
8.Manifestations of18F-FDG PET/CT in Primary Tracheal Cancer

Purpose To improve the understanding of features and diagnostic value of PET/CT imaging in primary tracheal cancer as it is a rare disease.Materials and Methods The18F-FDG PET/CT image data of 11 patients with primary tracheal cancer confirmed pathologically were retrospectively analyzed. The contrast-enhanced CT scan was performed in 9 patients. The PET/CT and contrast-enhanced CT imaging data were analyzed.Results Out of the 11 cases, 5 were adenoid cystic carcinoma, 4 were squamous cell carcinoma and 2 were adenocarcinoma. Endotracheal round or elongated uptake lesions were found in PET imaging. The maximum standardized uptake value of adenoid cystic carcinoma, squamous cell carcinoma and adenocarcinoma were 4.5±1.2, 8.1±1.7 and 4.5±2.4, respectively. The accuracy of diagnosis of PET/CT was 100%(11/11). Soft tissue density nodule or mass in trachea was found in CT scan. The accuracy of CT diagnosis was 81.8% (9/11). Among the 9 patients who underwent enhanced CT scan, 3 had significantly enhanced lesions and 6 had moderate enhanced ones. Mediastinal lymph node metastasis was found in 1 patient. There was no distant metastasis in all the 11 patients.Conclusion Primary tracheal cancer has features of PET/CT imaging such as soft tissue nodules or mass in tracheal lumen or on its walls accompanied by FDG increase. The utilization of metabolic and anatomic imaging of PET/CT has great significance in clinical diagnosis and therapy of primary tracheal cancer.
9.Reliability and validity of Chinese version of the Client-Centred Care Questionnaire

Objective To translate the English version of the Client-Centred Care Questionnaire (CCCQ) into Chinese and to test the reliability and validity of the Chinese version of CCCQ.Methods The English version of CCCQ was translated into Chinese by the method of double translation-translation.Totally 320 chronic patients were recruited and they were investigated by the Chinese version of CCCQ.Results The Cronbach coefficient of the Chinese version of CCCQ was 0.925,P<0.01.The retest reliability was 0.807,P<0.01.The split-half reliability with r=0.883,P<0.01.The content validity index (CVI) was 0.956,and every purpose CVI values were above 0.8.Factor analysis got one factor,which explained 52.549% of the total variance,and each objective factor loading was above 0.4.Conclusions The Chinese version of CCCQ has been proved to be reliable and valid.It can be used as a valid tool for the chronic patients to measure their perceptions of the quality of nursing care.
10.Clustering intervention on insulin injection for patients with type 2 diabetes

Objective To know the effect of using clustering intervention on insulin injection in the patients with type 2 diabetes.Methods 123 patients with type 2 diabetes were selected in the research.Clustering intervention was carried out in these patients with a series of effective measures to develop and integrate the application.Compared the controlling condition of blood sugar and the condition of insulin injection in patients by self control.Results The fasting blood glucose and the postprandial blood glucose after the intervention was (5.91±0.41) mmol/L and (8.27±0.53)mmol/L respectively,which was significant lower than that of before the intervention [(8.94±1.32) mmol/L and (11.15±1.34)mmol/L respectively,P<0.01].Conclusion Clustering intervention can regulate insulin injections,improve patients' ability of self-management and the treatment compliance,effectively control blood sugar and delay or reduce complications after the implementation of cluster intervention.
