1.THE INVESTIGATION OF THE ANESTHETIZATION OF FOREIGN BODY EXTRACTION IN RESPIRATORY TRACT OF INFANTS

Objective:To investigate the methods of the anesthetization of the foreign body extraction in respiratory tract of infants.Methods:This clinical study compared two anesthetic methods in foreign body extraction of respiratory tract of infants:Ketamine+Sodium r-hydroxybutyrate intravenous (group A) and Ketamine+propofol intravenous (group B) and analyzed the advantages and disadvantages of both methods.Results:Compared with group B,the extent of the changes of heart rate and blood oxygen saturation in patients of group A is littler,and the incidence rate of breath holding and asphyxiation is littler too.Conclusions:The anesthetic effect of Ketamine+Sodium r-hydroxybutyrate intravenous in better than that of Ketmaine+propcfol intravenous.
2.Juvenile dermatomyositis complicated by myelodysplastic syndrome:a case report

Objectives A rare case of juvenile dermatomyositis (JDM) complicated by myelodysplastic syndrome (MDS) was reported in order to improve the understanding of the disease in its clinical signs, differential diagnosis and treatment. Meth-ods The course of the disease, clinical characteristics and the process of diagnosis and treatment were analyzed. Specimens of bone marrow, lymph nodes, muscles were examined by histopathological or immunohistopathological means and lfow cytometry. Results The diagnosis of JDM complicated by MDS was made based on the clinical manifestations, related laboratory results and the accompanying changes in hematological and bone marrow examinations. Conclusions JDM complicated by MDS is very special and rarely seen in China and its pathological mechanism and strategy of diagnosis and treatment should be further explored.
3.Clinical study of Chinese medical chiropractic for the children with cough variant asthma and qi deficiency of spleen and lung syndrome

Objective:To evaluate the clinical effect of Chinese medical chiropractic in treating children with cough variant asthma syndrome (CVA) of Lung-Spleen Qi Deficiency.Methods:From October 2017 to October 2018, 60 CVA children with syndrome of lung-spleen qi deficiency were enrolled from pediatric outpatient department of Maoming Hospital of Traditional Chinese Medicine Affiliated to Guangzhou University of Traditional Chinese Medicine, and they were randomly divided into the control group and observation group by random number table method, with 30 in each group. The control group was given montelukast sodium granules orally on the basis of conventional treatment, and the observation group was given chiropractic treatment on the basis of the control group. Both groups were treated for 6 weeks and followed up for 2 months. The TCM syndrome scores were performed before and after treatment, and the changes of serum IgA, IgG, IgM and IgE were detected by ELISA. The recurrence was observed and recorded during the follow-up period, and the clinical effect rate was evaluated.Results:The total effect rates of the observation group and the control group were 86.7% (26/30) and 56.7% (17/30), respectively, with significant differences between the two groups ( χ2=6.648, P=0.008). After treatment, the scores of cough, sputum volume, oppression in chest, tongue coating, concomitant symptoms and total scores in the observation group were significantly lower than those in the control group ( t values were 2.639, 2.628, 2.633, 2.627, 2.725, 2.642, respectively, all Ps<0.05). After treatment, the IgA [(2.31 ± 1.20) g/L vs. (1.55 ± 0.72) g/L, t=2.635], IgG [(11.16 ± 3.39) g/L vs. (8.18 ± 3.27) g/L, t=2.751], IgM [(1.72 ± 0.34) g/L vs. (1.27 ± 0.36) g/L, t=2.660] in the observation group were significantly higher than those in the control group ( P<0.05), while the IgE [(51.32 ± 32.62) g/L vs. (74.58 ± 40.77) g/L, t=2.619] in the observation group were significantly lower than those in the control group ( P<0.05). During the follow-up, the recurrence rate was 19.2% (5/26) in the observation group and 70.6% (12/17) in the control group, and the difference between the two groups was statistically significant ( χ2=4.675, P=0.029). Conclusion:Chinese medicine chiropractic can improve the clinical symptoms and serum immune factor levels of children with cough variant asthma syndrome of lung and spleen qi deficiency, reduce the recurrence rate, and improve the clinical efficacy.
4.The progress of the research on the relationship between stanniocalcins and cancers

Stanniocalcin (STC) was ifrst found as a calcium- and phosphate-regulating hormone produced in bony ifsh by the corpuscles of Stannius. In mammals, the homolog STC-1 displays a relative high amino acid sequence identity (nearly 50%) with ifsh STC, and STC-2 has a lower identity (nearly 35%) with STC-1 and ifsh STC. Both STC-1 and STC-2 are expressed in a variety of tissues. The functions of STC have not been understood. But some ifndings have been reported on their cellular localization, gene structure, and expression in different physiological and pathological conditions, which will be clues in elucidating the functions of STC in mammals. Moreover, STC-1 and STC-2 are expressed in many tumor cell lines, suggesting other biological functions of STC in mammals other than mineral metabolism.
5.Effects of repeated sevoflurane anesthesia on long-term cognitive function in lactating rats

Objective To evaluate the effects of repeated sevoflurane anesthesia on long-term cognitive function in lactating rats.Methods Twenty-four healthy Spragne-Dawley rats,aged 7 days,weighing 14-17 g,were randomly divided into 3 groups (n =8 each):control group (group C),2.6 % sevoflurane group (group S1),and 1.5% sevoflurane group (group S2).At 7,14 and 21 days after birth,2.6% and 1.5% sevoflurane and carrier gas were inhaled for 1 h in groups S1,S2 and C,respectively.Visible plafform trial was carried out on 28 and 29 days after birth,and the swimming speed of the rats was recorded.Place navigation test was performed on 32-36 days after birth,and the escape latency was recorded.Spatial probe test was carried out on 36 days after birth,and the time spent in the platform quadrant,swimming distance and the number of times crossing the platform quadrant was recorded.Results Compared with group C,the escape latency was significantly prolonged in groups S1 and S2 (P < 0.05).The escape latency was significantly longer in group S1 than in group S2 (P <0.05).There were no significant differences between the three groups in the swimming speed,time spent in the platform zone,swimming distance and the number of times crossing the platform quadrant (P > 0.05).Conclusion Repeated inhalation of sevoflurane during lactation can impair the long-term declarative memory,which is concentration-related,while it has no effects on the associative learning in rats.
6.Epidemiological characteristics of rotavirus and adenovirus in children with diarrhea

Objective To investigate the infection and epidemiological characteristics of group A rotavirus (RV-A)and adenovirus in children with diarrhea in Guangzhou. Methods The colloidal gold technique was used to detect RV.A and adenovirus antigen in 2,171 stool samples from children with diarrhea in Guangzhou Women and Children′s Medical Center from January to December 2015,and the data were statistically analyzed. Results Among the 2,171 patients,the positive rate of RV-A infection was 17.96%and that of adenovirus infection 8.66%, and the co-infection rate of both virus was 3.45%. The positive rates between different genders were not significantly different(P > 0.05);the infectious time peak of RV-A was January(40.78%),followed by December(39.24%) and February(32.61%)and that of adenovirus infection was July(15.89%)and May(15.79%). The infectious peak of RV-A and adenovirus was December(7.29%),followed by January(7.01%). The peak age of infection ranged from 1y to 3y. Conclusion RV-A and adenovirus are the main pathogens of children diarrhea ,and the onset of virus infection has obvious seasonal change.
7.Relationship between illness perceptions and health behavior of patients with lower extremity arterial disease

Objectives To explore the relationship between illness perceptions and health behavior in the subjects. Methods Revised illness perception questionnaire and health promoting lifestyle profileⅡ (HPLPⅡ) were used to measure 101 patients′illness perception and health behavior between the groups. Results The HPLPⅡscore of LEAD was(2.17 ± 0.29) with the score of dimension. The course of disease, prognosis and perception of emotion were negatively related to health behaviour; personal control and primaty congtion of drsease were postively related to health behaviour (all P<0.01). Conclusion Medical personnel should pay attention to the relationship between illness perceptions and health behavior in patients with LEAD and effective measures should be taken to improve the patients′illness perceptions, promote their choices and persistence in health behavior and improve the quality of life.
8.Effect and Mechanism of Extracts of Nidus Collocaliae on Resisting Avian Influenza A H5N1 Virus

Objective To observe the inhibitory effect of the extracts of Nidus Collocaliae on avian influenza A H5N1 virus in vitro. Methods Nidus Collocaliae water extract, artificial gastric juice digestion products of Nidus Collocaliae water extract, and artifitial intestinal juice digestion products of Nidus Collocaliae water extract were prepared for the experimental study. 293T cells transfection in vitro was carried out. The effects of 3 kinds of Nidus Collocaliae extracts on H5N1 pseudovirus and VSV-G pseudovirus were determined by luciferase detection kit. Blood clotting response to erythrocyte hemagglutinin subtypes H5, H7, H9 antigens and the inhibitory effects of 3 kinds of Nidus Collocaliae extracts were observed. The effects of 3 kinds of Nidus Collocaliae extracts on neuraminidase activity were determined by neuraminidase inhibitor screen kit. Results The 3 kinds of Nidus Collocaliae extracts had inhibitory effects on H5N1 avian influenza pseudovirus, the effects being enhanced with the increase of the concentrations of Nidus Collocaliae extracts. Of the 3 extracts, artificial intestinal digestion products had the strongest inhibitory effect, while Nidus Collocaliae water extract had the weakest effect. However, Nidus Collocaliae extracts had no obvious effect on VSV-G pseudovirus. The concentration of H5, H7 antigen for positive blood clotting response was 1 ∶ 128, and that of H9 antigen was 1 ∶ 256. The 3 kinds of Nidus Collocaliae extracts at certain concentrations could inhibit blood clotting response to H5, H7, H9 antigen, but had no obvious effect on neuraminidase. Conclusion The anti-H5N1 virus effect of Nidus Collocaliae extracts has been achieved probably through resisting hemagglutinin.
9.The study of left ventricular pacing site and clinical benefit in heart failure patients with cardiac resynchronization therapy

Objective To investigate the relationship between different left ventricular pacing sites and clinical benefit in heart failure patients treated with cardiac resynchronization therapy (CRT).Methods Clinical data of 52 patients of CRT-P/D (pacing and defibrillation) implantation were collected.According to the left ventricular lead implantation sites,52 cases were divided into anterior wall (10 cases),lateral wall (15 cases),posterior wall (16 cases),and posterior base group (11 cases).The efficacy of CRT was evaluated by Minnesota life quality score,left ventricular function and remodeling index.Results In addition to the anterior wall group,Minnesota life quality score of the other groups were significantly lower than preoperative group (P < 0.05).Compared to pre-CRT implantation,left ventricular end diastolic diameter (LVEDD) of the anterior wall and posterior basal group 3 months after CRT implantation had no statistical significance (P > 0.05);the rest groups were lower than preoperative group (P < 0.05).Left ventricular ejection fraction (LVEF) in posterior wall group was increased after CRT implantation 3 months compared to the preoperative group (P <0.05).LVEF in anterior wall group was increased only in the 12 months after CRT implantation (P < 0.05).LVEF in the rest groups was increased comparing to the preoperation at 6 and 12 months postoperation (P <0.05).CRT non-response rate in anterior wall group was significantly higher than that in the other groups (P < 0.05).CRT response ratio was significantly increased in side,posterior and posterior basal wall compared to the anterior Wall group (P < 0.05).And there were no statistical significance among side wall,posterior wall and basal wall group (P > 0.05).△ QRSd was higher in side wall,posterior wall and posterior basal group after CRT implantation 3 months than anterior wall group (P < 0.05),△ QRSd in posterior basal group was lower than the other two groups (P <0.05).Conclusions Left ventricular electrode should be implanted at the side wall and posterior wall firstly,secondly at the posterior basal wall,and avoid at the anterior wall of the left ventricle.
10.Clinical value of endoscopic submucosal dissection for early cancer and precancerous lesions in the pylorus (with video)

Clinicopathological data of 15 patients with pyloric early cancer and precancerous lesions, who received endoscopic submucosal dissection (ESD) in Zhejiang Cancer Hospital from March 2011 to January 2020 were retrospectively analyzed. Postoperative pathology showed 7 cases of low-grade intraepithelial neoplasia, 3 cases of high-grade intraepithelial neoplasia, and 5 cases of early gastric cancer. R0 complete resection was achieved in all patients. The mean operation time was 55.2 min (35-78 min). One patient had delayed postoperative bleeding, and no other complications such as bleeding, perforation or abdominal pain occurred in other 14 patients. No recurrence, metastasis or pyloric stenosis was found during the follow-up of 31.3 months (1-106 months). ESD is safe and effective for early cancer and precancerous lesions in the pylorus.
