2.Observation and nursing of irinotecan in combination with fluorouracil for advanced colorectal cancer

Objective To observe the major adverse reaction of irinotecan in combination with fluorouracil for advanced colorectal cancer,and sum up the nursing experience of acetylcholine syndrome and acute delayed diarrhea.Methods Pathologically confirmed 45 cases of patients with advanced colorectal cancer were treated with modified FOLFIRI regimen for total of 190 cycles.Results Acetylcholine syndrome rate was 28.9%,the incidence of acute delayed diarrhea was 40.0% ,more than 3 grade diarrhea was 6.7%.Conclusion The incidence of acute delayed diarrhea in Chinese patients is lower than in caucasians,and reasonable care measures can reduce the occurrence of adverse reactions,irinotecan in combination with fluorouracil is a safe and effective regimen for Chinese patients.
3.Pharmacokinetic Effects of Baicalin on Cerebral Ischemiareperfusion after iv Administration in Rats

ObjectiveTo investigate the pharmacokinetic effects of baicalin on cerebral ischemia-reperfusion (I/R) after iv administration in rats.MethodsThe cerebral I/R rats were induced by occluding the bilateral carotid arteries of normal rats for 2 h,followed by reperfusion.The resultant animals were immediately iv administrated with baicalin (90 mg/kg),whilst the same dose of baicalin was injected to the normal rats.Plasma samples were collected at different time to construct pharmacokinetic profiles by plotting drug concentration vs time.Quantification of baicalin in rat plasma was achieved using a simple and rapid HPLC method.ResultsIn normal rats,the major parameters of distribution half-life,elimination half-life,area under the plasma concentration-time (AUC),apparent volume of distribution (Vd),and clearance (CL),estimated by an open two-compartmental model,were 0.8868 min,26.0968 min,149.6204 mg/min·L,4.765 L/kg,and 0.5776 L/kg·min),respectively.However,in I/R rats,the corresponding parameters were 2.084 min,34.4998 min,260.0188 μg.min/L,5.9376 L/kg,and 0.334 L/(kg·min),respectively.ConclusionThe cerebral I/R could significantly increase AUC and Vd values,decrease CL values,and prolong the terminal half-life of baicalin.These findings suggest that the injuries of I/R could play an important role in pharmacokinetic process ofbaicalin.
4.Regional blood perfusion, metabolism and angiogenesis of breast cancer

Objective To evaluate the correlation between regional blood perfusion,metabolism and angiogenesis in breast cancer.Methods The PET/CT functional imaging technique was applied to quantitatively detect the central and marginal blood perfusion parameters including blood flow(BF),blood volume(BV)and permeability of surface(Ps),and metabolism index of standard uptake value(SUV)of the tumor in 33 breast cancer patients.The expression of CD31.CD105 and VEGF in paraffin section of breast cancer were detected by immunohistochemical method,then MVD(CD31)and MVD(CD105)were obtained.The relationship between the regional blood perfusion and metabolism and MVD(CD3 1),MVD(CD105)and the expression of VEGF were analyzed. Results There was significant correlation of MVD (CD31)with BF of marginal region(P<0.05).There were significant correlations of MVD(CD105)with BF.PS and SUV(P<0.05). Conclusions Regional blood perfusion,metabolism is correlated with angiogenesis in breast cancer tissue.PETT/CT regional blood perfusion and metabolic imaging iS a noninvasive method which can be used to estimate angiogenesis status clinically in breast cancer.

Objective The purpose of this study was to provide practical anatomic data for the imaging diagnosis and surgical treatment of the disease of the subphrenic spaces. Methods Sectional anatomy of the subphrenic spaces on the coronal plane were investigated on 30 sets of serial coronal sections of superior abdomen of Chinese adult cadavers and MR imaging of 2 abdomen of cadavers. Results Subphrenic spaces were divided into perihepatic spaces and perisplenic spaces on the coronal plane.The space between the anterior margin of left gastropancreatic fold and the posterior layer of hepatogastric ligament is the only direct communication between the superior and inferior recesses of the lesser sac.Its communicating type is divided into three types on the coronal plane.The right layer of the gastrophrenic ligament is continuous with the posterior layer of the lesser omentum,the left layer of the gastrophrenic ligament is continuous with the right layer of the phrenicosplenic ligament and the posterior layer of the gastrosplenic ligament.Left gastropancreatic fold is continued to the left and right layers of gastrophrenic ligament upward.The bare area of stomach is located between the left and right layers of gastrophrenic ligament,its existing rate is 100%.The bare area of spleen is located among phrenicosplenic ligament,gastrosplenic ligament,splenorenal ligament and splenocoloic ligament.Its the most width exists at the splenorenal ligament.It is divided into the part of the splenic hilus and the splenorenal part.Its maximum is 2 64?1 16 cm,4 16?2 24 cm respectively. Conclusion The coronal plane is dominant for showing left gastropancreatic fold and the communication relationship between superior and inferior recesses of the lesser sac.The superior and inferior recesses of the lesser sac that may communicate with each other is 73 3 percent of all the cases studied
6.Research progress in the role of TRPV channel-induced Ca 2+ signal in osteoclast differentiation and bone resorption

Osteoclast is a type of multinuclear giant cell, which plays a role in bone resorption in the homeostasis of bone mass. Excessive bone resorption leads to osteoporosis and other diseases characterized by decreased bone mass. Ca 2+ metabolism plays an important role as a second messenger in the differentiation, migration, fusion and bone resorption of osteoclast. The transient receptor potential vanilloid (TRPV) ion channel is expressed in a variety of cells, including osteoclast. Current studies have found that the TRPV ion channel can participate in the production and bone resorption function of osteoclast by increasing intracellular Ca 2+ concentration and calcium oscillations. This article reviews the relationship between the TRPV ion channel and the changes of Ca 2+ concentration and the potential mechanisms involved in osteoclast activity, so as to provide a reference for further research on diseases characterized by abnormally increased activation of osteoclast in the future.

In order to avoid the intra- and post-operative risks caused by massive blood loss, there are various clinical methods for evaluating the blood supply of the tumor and the distribution of blood vessels around the tumor before surgery, such as dynamic enhanced CT, dynamic enhanced magnetic resonance imaging, digital subtraction angiography, etc. And there are a variety of pre- and intra-operative methods to reduce tumor bleeding, such as transarterial vertebral tumor embolization, percutaneous or transpedicular injection of Onyx/NBCA, antifibrinolytic drugs, controlled deliberate hypotension, etc. This article reviews on spinal tumor blood supply assessment and the methods to reduce the amount of surgical bleeding.
8.Exosomes derived from mesenchymal stem cells in treatment of animals with acute liver failure:a meta-analysis

OBJECTIVE:To evaluate the efficacy of exosomes derived from mesenchymal stem cells on animal models of acute liver failure. METHODS:PubMed,Web of Science,Embase,The Cochrane Library,CBM,CNKI,WanFang,and VIP databases were retrieved from inception to January 16,2023.A series of animal experiments on the treatment of acute liver failure animal models by exosomes derived from mesenchymal stem cells were collected.Two evaluators screened the literature and extracted the data independently.The bias risk was evaluated by the SYRCLE tool.The extracted data were analyzed by Revmen 5.4.1 software and Stata 17.0 software. RESULTS:A total of 241 articles were retrieved and 9 animal experiments were included,with 219 animals:110 animals in the model group and 109 animals in the exosome group.The results showed that the survival rate of animals in the exosome group improved significantly[RR=9.34,95%CI(3.91,22.29),P<0.001],the levels of serum alanine transaminase[SMD=-5.31,95%CI(-7.43,-3.19),P<0.001]and aspartate aminotransferase[SMD=-4.47,95%CI(-5.85,-3.10),P<0.001]were reduced obviously.The expressions of interleukin-1β[SMD=-11.54,95%CI(-18.12,-4.95),P=0.000 6],interleukin-6[SMD=-5.75,95%CI(-8.08,-3.41),P<0.001]and tumor necrosis factor-α[SMD=-4.46,95%CI(-6.83,-2.09),P=0.000 2],were suppressed obviously. CONCLUSION:Exosomes derived from mesenchymal stem cells effectively inhibit the inflammatory response,ameliorate liver function of animals with acute liver failure,and improve their survival rate.The results of subgroup analysis showed that the shorter survival time of animals(≤24 hours),the lower dose of transplanted exosomes(<1 mg/kg)and the source of exosomes(adipose-derived mesenchymal stem cells)may affect the efficacy of the exosomes derived from mesenchymal stem cells in the animal model of acute liver failure.This conclusion and its clinical transformation still need to be confirmed by randomized controlled studies with large sample sizes and high quality.
9.Porphyromonas gingivalis exacerbates ulcerative colitis via Porphyromonas gingivalis peptidylarginine deiminase.

Ulcerative Colitis (UC) has been reported to be related to Porphyromonas gingivalis (P. gingivalis). Porphyromonas gingivalis peptidylarginine deiminase (PPAD), a virulence factor released by P. gingivalis, is known to induce inflammatory responses. To explore the pathological relationships between PPAD and UC, we used homologous recombination technology to construct a P. gingivalis strain in which the PPAD gene was deleted (Δppad) and a Δppad strain in which the PPAD gene was restored (comΔppad). C57BL/6 mice were orally gavaged with saline, P. gingivalis, Δppad, or comΔppad twice a week for the entire 40 days (days 0-40), and then, UC was induced by dextran sodium sulfate (DSS) solution for 10 days (days 31-40). P. gingivalis and comΔppad exacerbated DDS-induced colitis, which was determined by assessing the parameters of colon length, disease activity index, and histological activity index, but Δppad failed to exacerbate DDS-induced colitis. Flow cytometry and ELISA revealed that compared with Δppad, P. gingivalis, and comΔppad increased T helper 17 (Th17) cell numbers and interleukin (IL)-17 production but decreased regulatory T cells (Tregs) numbers and IL-10 production in the spleens of mice with UC. We also cocultured P. gingivalis, Δppad, or comΔppad with T lymphocytes in vitro and found that P. gingivalis and comΔppad significantly increased Th17 cell numbers and decreased Treg cell numbers. Immunofluorescence staining of colon tissue paraffin sections also confirmed these results. The results suggested that P. gingivalis exacerbated the severity of UC in part via PPAD.
