1.Evaluation of Therapeutic Effect of Limaixian in Treating HP-related Gastroduodenopathy of Childhood

OBJECTIVE:To observe the therapeutic effect of limaixian in treating Helicobacterium pyloricus(HP)-related gastroduodenopathy of childhood and evaluate its role playing in triple short-term therapeutic scheme(TS scheme).METHODS:153 patients,who had definitely been diagnosed as HP-related gastroduodenopathy by gastroscopy or 13 C-UBT,were randomly divided into three groups(A,B,C)receiving ST scheme(omeprazole as the main drug).Group A:omeprazole 0.8mg/(kg*d),metronidazole 20mg/(kg*d) and amoxycillin 40mg/(kg*d); Group B omeprazole,metronidazole and klacid 15mg/(kg*d); Group C:omeprazole,metronidazole and limaixian 15mg/(kg*d).Therapeutic course was 2 wks and gastroscopy or 13C-UBT were carried out 4 wks after discontinuation of drugs.RESULTS:(1)HP eradication rates of group A,B and C were 62.7%,92.1% and 94.1% respectively.The rates of group B,C were higher than that of group A.There were significant differences among three groups(X2=31.8,P<0.01).Group B showed no significant different with group C.(2) The remission rates of pain of group B and C were markedly higher than that of group A one and two wks after treatment (X2=19.3,P<0.05),there was no significant difference between group B and C.CONCLUSION:As an anti- HP infection agent in TS scheme,limaixian is better than amoxycillin in therapeutic effect.Limaixian is comparable with imported klacid and inexpensive in price,it can be used as the first choice antibiotic in TS scheme.
2.Drug Resistance Supervision to Pathogens from Urinary Tract in Mountain Area of Western Hubei Province

OBJECTIVE To investigate the distribution and drug resistance status to pathogens from urinary tract and offer scientific evidence for reasonable usage of antibiotics.METHODS Totally 389 isolates derived from infected urinary tract in mountain area of western Hubei Province from 2002 to 2005 were identified and antibiotic susceptibility test was performed.RESULTS Escherichia coli accounted for 66.1% ranking the first.The isolates were all susceptible to imipenem and vancomycin.The ratio of ESBLs in E.coli and Klebsiella pneumoniae was 23.7% and 25.9%,respectively.CONCLUSIONS The drug resistance rate in isolates derived from infected urinary tract of patients in mountain area was lower than in plain region of Hubei Province.
3.Mutations analysis of SPG11 gene in Chinese Han patients with hereditary spastic paraplegia

Objective To identify disease-causing mutations in a large panel of Chinese Han patients with hereditary spastic paraplegia(HSP).Methods The coding sequence of the SPG11 gene in the probands of 28 families with ARHSP and 14 sporadic HSP patients was analyzed,and the identified changes in the sequence were tested to exclude being a benign polymorphism by sequencing 200 chromosomes from normal controls.Results Identified 13 causative mutations in SPG11 gene in 7 ARHSP and 3 sporadic HSP The mutations were:c.5977C>T/p.Q1993X、c.4668T>A/p.Y1556X、c.6898_6899delCT/p.L2300AfsX23.38、c.3719_3720delTA/p.11240VfsX263、c.733_734delAT/p.M245VfsX246、c.7088_7089insATTA/p.Y2363X、c.2163_2164insT/p.1722Yfsx731、c.7101-7102insT/p.K2368X、c.6790_6791insC/p.12264PfsX2339、c.654_655delinsG/p.S218RfsX219、c.7151+4_7151+7delAGTA/p.K2384fsX2386、c.6355-21_6355-18delTCT、c.3004C>T/p.G1002X.Among them,12 were novel mutations.The rate of mutation in the SPG11 gene was 25.0%(7/28)in ARHSP,6/6 in ARHSP-TCC and 3/14 in sporadic cases.Conclusions In Chinese Han population,patients with ARHSP-TCC and sporadic HSP-TCC should be screened for SPG11.Sequencing of the SPG11 gene in these patients with is valuable for clinical diagnostic testing.
4.Distribution and accumulation of vindoline, catharanthine and vinblastine in Catharanthus roseus cultivated in China.

OBJECTIVEThe content of vindoline, catharanthine and vinblastine in the root, stem, leaf, flower and fruit of Catharanthus roseus at various developmental stages were determined, and the biomass allocation was also determined to find the best harvest time.
METHODThe content of vindoline, catharanthine and vinblastine in the root, stem, leaf, flower and fruit of C. roseus were determined by HPLC.
RESULTThe content of these alkaloids were influenced by season and it varied in the different tissues of the plant. The content of vindoline and catharanthine in the leaves were the highest, and there was no vindoline detected in the root, but the content of vinblastine in the flower was the highest; the content of vindoline and catharanthine reached the maximum between the August and September, and the content of vinblastine reached the highest after the September. The biomass was the highest in the initial stage of September.
CONCLUSIONThe best harvest time was in the initial stage of September.

5.Efficacy and safety on upper ureteric lithotripsy by Shuo Tong ureteroscopy

Objective To investigate the efficacy and safety of Shuo Tong ureteroscopy in the treatment of upper ureteric stones.Methods The data of 832 patients,who underwent Shuo Tong ureteroscopy combined with holmium laser for the treatment of upper ureteric stones were collected between July 2014 and February 2017.The patients accepted general anesthesia under lithotomy position.After dilating the ureter,a ureteral access sheath was inserted along the guide wire.Finally,stones were broken and clean up by Shuo Tong ureteroscope.According to the stone size,823 patients were divided into ≤2.0 cm group (n=112,13.6%),2.1-3.0 cm group (n =395,48.0%),3.1-4.0 cm group (n =257,31.2%),> 4.0 cm group (n =59,7.2%)The stone free rate(SFR) and post-operative complications were observed.Results Among 983 operations,663 cases accepted one-stage operations and 160 cases accepted second stage operations.the operative time and postoperative hospital stay were 23-145 min(mean 74.8 ± 35.3) and 1-5 days(mean 1.9 ±0.8),respectively.The SFR in the first day and one month after operation was 75.6% and 83.8%,respectively.The stone free rate in each group were 89.3%,82.5%,61.1%,52.5% at first day after operation respectively(P < 0.05).While,the SFR after one month were 95.5%,87.6%,72.0%,61.0% in each group,respectively(P < 0.05).Fever rate after operation was 11.1%,ureteral injury rate was 1.9%,severe hematuria rate was 0.6%,stone street formation rate was 1.2%,perirenal hematoma rate was 0.4%.All patients were improved after conventional treatment.Conclusion Shuo Tong ureteroscopy is a safe treatment with high stone free rate for upper ureteric stones.
6.Study on the single-nucleotide substitution (c.-16C to T) of the PURATROPHIN-1 gene in Chinese patients with spinocerebellar ataxia.

OBJECTIVETo study the single-nucleotide substitution (c.-16C to T) of the PURATROPHIN-1 gene in spinocerebellar ataxia (SCA) patients in China.
METHODSThe single-nucleotide substitution (c.-16C to T) of the PURATROPHIN-1 gene was detected by PCR, digested with EcoN I, separated on 8% polyacrylamide gel in 68 probands of autosomal dominant SCA families and 119 sporadic SCA patients, who had been excluded CAG/CAA repeat expansion at the SCA1, 2, 3, 6, 7, 17 and dentatorubral-pallidolluysian atrophy (DRPLA) loci. The results were confirmed in four patients by direct sequencing.
RESULTSThe single-nucleotide substitution (c.-16C to T) of the PURATROPHIN-1 gene was not identified in authors' cohort.
CONCLUSIONThe mutation of c.-16C to T of the PURATROPHIN-1 gene might be rare in SCA patients in China.

7.Selection of inner ear fenestration strategy and surgical effect of patients with oval window atresia accompanied by facial nerve aberration

Objective:To summarize the clinical features and postoperative efficacy of patients with oval window atresia accompanied by facial nerve aberration.Methods:The clinical data of patients with congenital middle ear malformation with facial nerve aberration admitted to our hospital from January 2015 to March 2023 were retrospectively analyzed. There were 97 cases (133 ears) in total. Among them, 39 patients (44 ears) had complete follow-up data, including 27 male patients and 12 females, aged 7-48 years old, with an average age of 17.8 years old. Of these, 14 cases (16 ears) were patients combined with facial nerve aberration, and 25 cases (28 ears) were without facial nerve aberration. The results of imaging examination, pure-tone audiometry, selection of surgical strategy, intraoperative findings and postoperative hearing improvement were summarized and analyzed. The malformations of malleus, incus, stapes, oval window and facial nerve were recorded. Prism 9 software was used to statistically analyze the mean bone conductance and air-bone gap of patients before and after surgery.Results:All the 14 patients (16 ears) with middle ear malformation accompanied by facial nerve aberration and oval window atresia showed poor hearing and no facial palsy since childhood. High resolution CT (HRCT) examination of temporal bone, pure tone audiometry and Gelle test were performed before surgery. The malformations of malleus, incus, stapes, oval window and facial nerve were recorded. Preoperative high-resolution CT (HRCT) examination of temporal bone found 12 ears with 4 or more deformities, accounting for 75.00%, in the group of patients with facial nerve malformation. The preoperative average bone conductive threshold was (15.3±10.4) dB and the average air-bone gap was (46.3±10.6) dB in pure-tone audiometry (0.5, 1, 2, 4kHz). According to the different degrees of facial nerve and ossicle malformation, we performed three different hearing reconstruction strategies for the 14 patients (16 ears) with facial nerve aberration and oval window atresia, including 7 ears of incus bypass artificial stape implantation, 7 ears of Malleostapedotomy (MS) and 2 ears of Malleus-cochlear-prothesis (MCP). After 3 months to 18 months of follow-up, all patients showed no facial paralysis. The postoperative mean bone conductive threshold was (15.7±7.9) dB and air-bone gap was (19.8±8.5) dB. There were significant differences in mean air-bone gap before and after operation ( t=7.766, P<0.05), and there was no significant difference between the mean bone conductive threshold before and after surgery ( t=0.225, P=0.824). There was no significant difference of mean reduction of air-bone gap between patients with and without facial nerve aberration ( t=1.412, P=0.165). There was no significant difference between the three hearing reconstruction strategies. There was no significant displacement of the Piston examined by U-HRCT. Conclusion:For patients of middle ear malformation whose facial nerve cover the oval window partially, incus bypass artificial stape implantation or Malleostapedotomy (MS) can be selected according to the specific condition of auditory ossis malformation, and for patients whose facial nerve completely covers the oval window area, Malleus-cochlear-prothesis (MCP) can be selected. Three types of stapes surgery are safe and reliable for patients with oval window atresia accompanied by facial nerve aberration. There was no significant difference in efficacy between them. Preoperative HRCT assessment of middle ear malformation is effective. There is no significant difference of surgical effect with or without facial nerve aberration. The U-HRCT can be used to evaluate the middle ear malformation before surgery and the Piston implantation status after surgery. Due to the risks of surgery, those who do not want to undergo surgery can choose artificial hearing AIDS, such as hearing aid, vibrating soundbridge, bone bridge or bone-anchored hearing aid.
