1.The effects of PHT,CT,Calcium and phospho- rus during the anesthesia and operative stress

The changes of parathyroid hormone (PTH),calcitonin(CT)of three different anesthesia methods were observed perioperatively in 38 patients who were divided into two age groups (22 ~ 50 and 51 ~ 60). Postoperative serum Ca, P levels decreased as compared with preoperative values,espe.cially in the old. age groups with no statistic significance. These results indicate that during anesthesia and operative stress the CDS is inhibited and the regulation of the autonomic nervous system is affected, with obscure the relationship between parathyroid hormone, calcitonin and Ca, P with the alteration of Ca,P metabolism. Serum Ca may increase to the baseline by supplement of calcium gluconate during and after operation, but the operation after 4 ~ 5 days, serum Ca may also return to normal without any intervention.
2.Determination of Baicalin in Ganteling Jiaonang by HPLC

Objective To establish the method for detemining the content of Baicalin in Ganteling Jiaonang by HPLC.Methods Using Diamonsil ODS1 C18 Column,methanol-water-phosphoric acid(50:50:0.2) as the mobile phase,detection wavelength as 280 nm and flow rate was 1.0 mL/min.Results The calibration curve was linear at the range of 0.125 6~1.004 8 ?g for Baicalin and linear equation was Y= 95 952X+8 108.6,r2=0.999 7.The average recovery was 98.8% and RSD was 1.64%(n=6).Conclusion This method was simple,accurate,proper,and the reduplication of the result was good,which provide scientific quantitative analysis method for the determination of Baicalin in Ganteling Jiaonang.
3.The effects of different methods of anesthesia on immune function during renal transplantation

Objective To compare the effects of different methods of anesthesia on immune function during renal transplantation. Methods Thirty-nine uremic patients of both sexes (20 male, 19 female) aged 19-65 yr, undergoing renal transplantation were studied. Patients who had infection, fever, immune system disease or had received immunoregulatory drugs were excluded. The patients were randomly divided into 3 equal groups: Ⅰ continuous epidural anesthesia group (CEA , n = 13); Ⅱ combined spinal-epidural anesthesia group (CSEA, n = 13) and Ⅲ general anesthesia group (GA , n = 13). In group I an epidural catheter was inserted at T12-L1 or L1-2 into epidural space and advanced in cephalad direction and a mixture of 2% lidocaine and 0.7% dicaine (1: 1) 11-12 ml was given. The height of block was T8 In group II CSEA was performed at L2-3 and a mixture of 1% dicaine, 10% glucose and 3% ephedrine (1 : 1 : 1) 3 ml was injected intrathecally. An epidural catheter was threaded in a cephalad direction. The block height was T8. 2 % lidocaine was given epidurally when the operation lasted more than 2 hours. In both group I and II pathidine 50 mg and droperidol 5 mg were given iv. In group III anesthesia was induced with fentanyl 5 ?g kg-1 , etomidate 0.3 mg ? kg-1 and vecuronium 0.08 mg ? kg-1 and maintained with isoflurane supplemented with intermittent iv boluses of vecuronium. Blood samples were obtained from peripheral vein for determination of CD+3 , CD+4 , CD+8 , CD+4 /CD+8 ratio and IgG, IgA, IgM, C3 , C4 before anesthesia (T0 ) , after anesthesia (T1), before blood transfusion (T2), 30 min and 1 h after blood transfusion (T3,4) ,when renal circulation was restored (T5), at the end of operation (T6 ) and 1 and 3 days after operation (T7.8) .Results There were no significant changes in T lymphocyte subgroups, immunoglobulins and complements measured after anesthesia (T1 ) as compared with the baseline values(T0) in the 3 groups. In group Ⅰ and Ⅱ CD+3 , CD+4 , IgG, IgA, IgM, C3 , C4 and CD+8 unchanged significantly as compared with the baseline values (T0 ) ; CD+4 /CD+8 ratio decreased after anesthesia ( P
4.A new monacolin analogue from Xuezhikang capsule.

Xuezhikang capsule (ethanol extract of red yeast rice) which produced by Beijing WBL Peking University Biotech Co., Ltd., is a traditional Chinese medication with 3-hydroxy-3-methylglutaryl-coenzyme A (HMG-CoA) reductase inhibiting activity. Clinical trials indicated that Xuezhikang with lipid-lowering action could reduce the risk of cardiac events and total mortality of Chinese coronary heart disease patients. To exactly explain the clinical features of Xuezhikang, we undertook a complete study of the chemical constituents of Xuezhikang. This study resulted in the isolation of a new monacolin analogue, named alpha, beta-dehydromonacolin L (1), along with two known compounds: monacolin L (2) and 3-(2, 6-dimethyl-1, 2, 4a, 5, 6, 7, 8, 8a-octahydronaphthalen-1-yl)propanoic acid (3). The chemical structures were determined by extensive 1D and 2D NMR and MS spectroscopic analysis.
5.Construction of Bifidobacterium Infantis/CD tumor targeting gene therapy system.

BACKGROUNDTo construct the Bifidobacterium Infantis/CD tumor targeting gene therapy system.
METHODSPCR expanded CD gene and pGEX-1LambdaT plasmid was transferred into E.Coli. JM109. with TSS solution. Then a dual restriction enzyme (EcoR I and BamH I) digestion was carried out to cut CD gene and pGEX-1LambdaT plasmid. Two segments (4.9 kb and 1.3 kb) were selected and extracted from the gel. T4 DNA Ligase was added to these two segments' mixture. Finally, reconstruction fragment was transferred into Bifidobacterium Infantis by electroporation. The plasmid was extracted from the positive clone selected and testified by an agarose eletrophoresis after enzymed.
RESULTSA positive transferred Bifidobacterium Infantis with 6.2 kb long reconstruction plasmid fragment with CD gene was established which was in accordance with theoretical calculation.
CONCLUSIONSThe Bifidobacterium Infantis/CD tumor targeting gene therapy system could be successfully constructed.

OBJECTIVE: To verify the therapeutic effect and adverse reactions of ?-receptor blocker, domestic urapidil, on severe hypertension. METHODS: Observation was carried out in a multi-center, random sampling and controlled pattern. Drug was iv injected first and then infused. At the same time, the patients' systolic pressure, diastolic pressure, heart rate, EKG, blood & urine routine, serum GPT and urea nitrogen were measured and examined.RESULTS: Of 41 cases in this series. 16 satis- factory(39.0%), 23 improved (56.1%), 2 unsatisfactory(4.9%) .The total effective rate was 95.1%. 7.A national survey of ERCP training in China

Objective To investigate the current popularity of ERCP training in China.Methods A questionnaire-based survey was performed on a professional survey website.A total of 48 questions were designed,including personal information,training information,current status of ERCP performance and sug-gestions to ERCP training.The questionnaire was completed anonymously by physicians who fulfilled full-time ERCP training in any Chinese endoscopy center for at least 1 year before.Results A total of 464 phy-sicians,from all the regions of mainland China,among which 362 people completed all the questions.More than two thirds trainees thought that they had got fruitful training and 27.4% trainees had their ERCP vol-umes greatly increased and 64.3% trainees increased practice.According to related index,the physicians who were competent in routine ERCP performance were 84.5%,with outstanding operators of 12.2%.Con-clusion China has made great progress in the training program of ERCP technique in recent decades,but problems still remain such as various admission standard,insufficient training duration,incomprehensive program,as well as lack of evaluation and follow-up system.
8.Expression of junctophilin 2 and FGF23 in atrial tissue of rabbits with atrial fibrillation cardiomyopathy

Objective To explore the expression of junctophilin 2(JP2)and fibroblast growth factor 23(FGF23)in a rabbit model of atrial fibrillation mediated-cardiomyopathy(AMC).Methods Rabbit models of atrial fibrillation(AF)were developed through rapid atrial stimulation and then divided into three groups:control group(pacemak-ers implanted without pacing,n=6),AF group(pacing with ejection fraction decrease<10%,n=5),and AMC group(pacing with ejection fraction decrease≥10%,n=6).Echocardiography was performed to detect left ventric-ular end-diastolic diameter(LVEDD),left ventricular end-systolic diameter(LVESD)and left ventricular ejection fraction(LVEF).JP2 and FGF23 were detected by ELISA method.Western blot and RT-qPCR were conducted to detect protein and mRNA expression of JP2 and FGF23.Results Left atrial diameter,right atrial diameter and right ventricular diameter increased and LVEF decreased in the AMC group as compared with the control group.AMC group had lower LVEF and larger aorta and right ventricle diameter.Compared with the control group,the ex-pression of FGF23(P<0.001)and JP2(P<0.01)in left atrial cardiomyocytes was significantly increased in the AF group,while the expression of JP2 was decreased in the AMC group(P<0.001).AMC group had lower expression of JP2 and FGF23 compared with AF group.Compared to the control group,plasma concentration of JP2 and FGF23 increased in the AF group and FGF23 plasma concentration increased in the AMC group.Plasma concentration of FGF23 and JP2 was lower in AMC group than that in AF group.Conclusions FGF23 expression increased and JP2 expression decreased as found in the rabbit AMC model.
9.Effects of pregnancy complicated with Takayasu arteritis on maternal and fetal outcomes

Objective:To explore the effects of pregnancy complicated with Takayasu arteritis (TA) on maternal and fetal outcomes.Methods:The clinical data of 17 TA patients with pregnancy and finally terminated admitted to the First Affiliated Hospital of Zhengzhou University from January 2012 to December 2020 were collected and the maternal and infant outcomes were retrospectively analyzed.Results:Among the 24 pregnancies in 17 TA patients in our hispital, 11 patients in our hospital were primiparous (46%, 11/24) and 13 patients were multiparous (54%, 13/24); 4 cases of chronic hypertension before pregnancy (17%, 4/24), and 20 cases of non-hypertension (83%, 20/24) were abserved. Apart from the thoracoabdominal artery type, 15 cases (63%, 15/24) of brachiocephalic type, 8 cases (33%, 8/24) of extensive type, and 1 case of pulmonary artery type (4%, 1/24) were included in TA classification; the renal artery was damaged in 7 cases (7/8) of the extensive type. Among the 24 pregnancies of 17 TA patients, 8 cases of pregnancy complications occurred, including 4 cases (17%, 4/24) of preeclampsia, one case of (4%, 1/24) new-onset hypertension during pregnancy, exacerbation of existing hypertension, thrombocytopenia and ischemic stroke respectively; 3 cases of induced abortion, 1 case of induced labor, and 20 cases of final delivery were found. Of the 20 final deliveries, 3 cases (15%, 3/20) were delivered through the vagina; 17 cases (85%, 17/20) were delivered by cesarean section; 3 cases (15%, 3/20) of fetal growth restriction and 6 cases of oligohydramnios (30%, 6/20) were occurred. The median gestational age of pregnancy termination of 17 full-term neonates (85%, 17/20) and 3 premature neonates (15%, 3/20) was 38.4 weeks (range:29.6-40.9 weeks). All premature women were complicated with pre-eclampsia or severe pre-eclampsia, which lead to premature birth; the birth weight of the neonates was (2 791±783) g. Neonatal asphyxia occurred in 3 cases (15%, 3/20), and 6 cases (30%, 6/20) were admitted to the neonatal intensive care unit. None of the newborns died. The patients were followed up for 3 months to 7 years. Except for one case who underwent surgical treatment 3 years after delivery and died of intraoperative hemorrhage, the other 16 patients were in stable condition; all the newborns grew and developed well. The incidences of maternal and infant adverse events were higher in those with chronic hypertension, renal artery involvement, and no use of hormones and aspirin during pregnancy, but the differences were not statistically significant (all P>0.05). Conclusions:Pregnancy with TA has adverse effects on maternal and infant outcomes. To reduce the occurrence of maternal and child adverse events, the condition of TA patients should be fully assessed before pregnancy, multidisciplinary cooperation, regular treatment, strengthened monitoring of the condition, timely treatment of complications, selection of appropriate delivery methods after pregnancy, and vigilance of postpartum complications should be properly carried out.
