1.Effect of asarone injection combined with inhalation and psychological intervention on acute lung injury and its effect on pulmonary function

Objective To observe the efficacy of asarone injection combined with inhalation on the treatment of COPD acute phase.MethodsAccording to chronic obstructive pulmonary disease diagnosis and treatment guidelinesin the diagnostic criteria for chronic obstructive pulmonary disease in acute phase, 81 patients were randomized double-blindly divided into observation group of 40 cases and a control group 41 cases.Administering inhalation nebulizer patients in the control group, asarone injection combined with inhalation treatment in the observation group.Serum levels of interleukin-8 (IL-8), interferon-γ (IFN-γ), tumor necrosis factor (TNF-α) levels before and after treatment, using a blood gas analyzer patients arterial oxygen pressure (PaO2) and arterial carbon dioxide tension (PaCO2) level, measured using a spirometer patients accounted for one second forced expiratory percentage of predicted value (FEV1% pred) and total airway resistance, and a separate determination combination therapy regimen based on the above indicators Comparative efficacy.Results①IL-8, IFN-γ, TNF-α: After treatment, the observation group was significantly lower than the control group (t=11.498;t=10.279;t=11.576), the differences were statistically significant (P<0.05).②blood gas PaO2 and PaCO2: post-treatment observation group than the control group (t=11.021;t =8.868), the differences were statistically significant (P<0.05).③lung function FEV1% pred and total airway resistance: After treatment, the observation group than the control group (t=7.182;t=6.341), the differences were statistically significant (P<0.05).The total effective rate was significantly higher (χ2 =4.742) after ④observation group, the difference was significant (P<0.05).ConclusionAsarone injection combined with inhalation can reduce chronic inflammation in the body in patients with acute obstructive pulmonary disease, the body adjust blood gas, improve lung function, improve the treatment of patients with acute COPD.
2.Therapeutic effect of oxiracetam combined with dexamethasone for treatment of patients with severe acute carbon monoxide poisoning

Objective To investigate the therapeutic effect of oxiracetam combined with dexamethasone for treatment of patients with severe acute carbon monoxide poisoning (ACOP).Methods Eighty-seven patients with severe ACOP admitted to Affiliated Hospital of Xuzhou Medical University from January 2013 to July 2016 were enrolled, and they were divided into observation group (47 cases) and control group (30 cases) according to random number table method. The two groups were given conventional nerve nutrition, hyperbaric oxygen and symptomatic treatment, while in the observationgroup, on the basis of conventional treatment the patients received intravenous drip of dexamethasone 5 mg (with addition of normal saline 100 mL), once a day for consecutive 3 days, and intravenous drip of oxiracetam 3.0 g plus 100 mL normal saline, once a day for consecutive 7 days. The changes of mini-mental state examination (MMSE), Barthel index and Glasgow coma scale (GCS) were observed after discharge for 30 days, and the differences of the incidence of delayed encephalopathy after acute carbon monoxide poisoning (DEACMP) and hospitalization time between the two groups were compared.Results After treatment the MMSE and GCS scores of the two groups were higher than those before treatment, and the Barthel score was lower than that before onset. After discharge for 30 days, MMSE score (26.93±2.92 vs. 24.20±6.82), GCS (14.41±1.32 vs. 13.98±2.13), Barthel (94.78±12.62 vs. 85.25±19.57) of the observation group were significantly higher than those of control group, and the incidence of DEACMP was obviously lower than that in the control group [8.5% (4/47) to 17.5% (7/40)], the differences between the two groups being statistically significant (allP < 0.05); the hospitalizationtime of the observation group was shorter than that of the control group (days: 10.81±5.49 vs. 13.05±6.84), but there was no significant difference between the two groups (P > 0.05).Conclusion Early combined application of oxiracetam and dexamethasone can significantly improve the cognition and self-care abilities and reduce the incidence of DEACMP in patients with severe ACOP after discharge for 30 days.
3.Expression and clinical significance of peroxiredoxin Ⅰ in hepatocellular carcinoma with portal vein tumor thrombosis

Objective To investigate the expression of peroxiredoxin 1 (Prx 1) in hepatocellular carcinoma (HCC) with portal vein tumor thrombosis (PVTT) and to evaluate the relationship between the expressions of Prx 1 and the postoperative recurrence of this disease. Methods Immunohisto chemistry and Western blotting were performed to examine the expression of Prx 1 protein in 40 patients with HCC with PVTT. Experiments on Sprague Dawley (SD) rat hepatoma model were further carried out to observe the pathological changes of Prx 1 by immunohistochemistry. Clinical outcomes were analyzed to find a correlation between the recurrence and positive rate of Prx 1. Results The expression level of Prx 1 was significantly up-regulated in primary tumor tissues than in tumor thrombosis samples (P<0.01). Immunohistochemistry results showed that the positive rate of Prx 1 in primary tumor tissues were higher than that in tumor thrombosis. Western blotting confirmed a same trend in the level of Prx 1, the average luminosity of the blots were 1534.2 and 735.6, respectively. There was a significant difference in SD rat hepatoma model, the 4, 8, 12, 16, 20 and 24-week positive rates of Prx 1 in liver tumor tissues were 60%, 80%, 75% ,65%, 40% and 25% respectively. Clinical outcomes showed that the time to first postoperative recurrence of Prx 1 in the primary tumor positive group was significantly higher than that in the negative group (6. 3 vs 3. 7 months, P<0. 01). Conclusions Prx 1 protein was down-regulated in HCC with PVTT. There was a negative correlation between the expression of Prx 1 and recurrence.
4.The compression and storage of enhanced external counterpulsation waveform based on DICOM standard.

The development of external counterpulsation (ECP) local area network system and extensible markup language (XML)-based remote ECP medical information system conformable to digital imaging and communications in medicine (DICOM) standard has been improving the digital interchangeablity and sharability of ECP data. However, the therapy process of ECP is a continuous and longtime supervision which builds a mass of waveform data. In order to reduce the storage space and improve the transmission efficiency, the waveform data with the normative format of ECP data files have to be compressed. In this article, we introduced the compression arithmetic of template matching and improved quick fitting of linear approximation distance thresholding (LADT) in combimation with the characters of enhanced external counterpulsation (EECP) waveform signal. The DICOM standard is used as the storage and transmission standard to make our system compatible with hospital information system. According to the rules of transfer syntaxes, we defined private transfer syntax for one-dimensional compressed waveform data and stored EECP data into a DICOM file. Testing result indicates that the compressed and normative data can be correctly transmitted and displayed between EECP workstations in our EECP laboratory.

5.Correlation Analysis of XRCC1 rs25487 Polymorphism with the Occurrence of Lung Cancer

OBJECTIVE:To investigate the correlation of XRCC1 rs25487 polymorphism with the occurrence of lung cancer. METHODS:A total of 208 patients with primary lung cancer of Han nationality in Northern Jiangsu selected from the Affiliated Hospital of Xuzhou Medical University during Sept. 2015-Jul. 2016 were included in lung cancer group. A total of 214 healthy volunteers of the hospital underwent physical examination were included in control group. PCR-RFLP was used to detect the genotypes at XRCC1 rs25487 locus,and Logistic regression model was used to evaluate the correlation of genotypes with the occurrence of lung cancer. RESULTS:There was no statistical significance in the distribution of age and gender between 2 groups (P>0.05). The proportion of smoker in lung cancer group was significantly higher than control group,with statistical significance(P<0.05). AA,AG and GG genotypes were detected at rs25487 locus of XRCC1 gene. The frequency of AA,AG and GG genotype were 43.5%,41.1%and 15.4% in control group and 28.8%,48.6% and 22.6% in lung cancer group,respectively. The frequencies of genotypes in 2 groups were in line with Hardy-Weinberg equilibrium(P>0.05),but there was statistical significance in genotype distribution between 2 groups(P<0.05). Compared with AA genotype,the risk of lung cancer in individuals carrying AG genotype increased by 2.265 fold [OR=2.265,95%CI(1.299,3.950),P=0.040;after corrected with gender,age and smoking history OR=2.309,95%CI(1.274, 4.185),P=0.006],with statistical significance. The risk of lung cancer in individuals carrying GG genotype increased by 1.310 fold [OR=1.310,95%CI(0.771,2.228),P=0.318;after corrected OR=1.429,95%CI(0.811,2.518),P=0.217],without statistical significance. CONCLUSIONS:rs25487 locus mutant heterozy-gosity of XRCC1 gene is risk factor of lung cancer in Han nationality from Northern Jiangsu,and smoking can increase the risk of lung cancer.
6.Correlation Analysis of ADPRT rs1136410 Polymorphism with the Occurrence of Non-small Cell Lung Cancer in Han Nationa- lity from Northern Jiangsu

OBJECTIVE: To investigate the correlation of ADPRT rs1136410 polymorphism with the occurrence of non-small cell lung cancer (NSCLC) in Han nationality from northern Jiangsu. METHODS: A total of 283 patients with primary NSCLC of Han nationality in Northern Jiangsu were selected from the Affiliated Hospital of Xuzhou Medical University during Nov. 2015-Dec. 2018 as NSCLC group. A total of 210 healthy subjects underwent physical examination were included in control group. PCR-RFLP was utilized to determine the genotypes at ADPRT rs1136410 locus. Logistic regression model was used to evaluate the effect of polymorphism and its interaction with smoking on the occurrence of NSCLC. RESULTS: There was no statistical significance in age and gender between 2 groups (P>0.05). The proportion of smoker in NSCLC group was significantly higher than control group (P<0.05). TT, TC and CC genotypes were detected at rs1136410 locus of ADPRT gene. The frequency of TT, TC and CC genotype were 41.9%,44.8% and 13.3%, and those of allele T and C were 64.3% and 35.7% in control group. The frequency of TT, TC and CC genotype were 21.6%, 50.2% and 28.2%, and those of allele T and C were 46.6% and 53.4% in NSCLC group, respectively. The frequencies of genotypes in 2 groups were in accordance with Hardy-Weinberg equilibrium (P>0.05), while there was significant difference in genotype and allele frequencies between 2 groups (P<0.05). Compared with TT genotype, the risk of NSCLC in individuals carrying TC and CC genotypes raised by 1.179, 3.122 folds [ORTC=2.179, 95%CI (1.435, 3.309), P<0.05; ORCC=4.122,95%CI(2.401,7.075),P<0.05]. Compared with individuals carrying TT genotype, the risk of NSCLC occurrence in non-smokers carrying TC and CC genotypes increased by 0.371, 1.328 fold [ORTC=1.371,95%CI (0.927,3.428),P<0.05; ORCC=2.328,95%CI (1.249,4.622),P<0.05]; and the risk of NSCLC occurrence in smokers carrying TC and CC genotypes increased by 0.928, 2.182 folds [ORTC=1.928,95%CI (1.257,2.957), P<0.05;ORCC=3.182,95%CI (1.760,5.754), P<0.05]. CONCLUSIONS: The rs1136410 locus mutant genotype of ADPRT gene is the risk factor of NSCLC in Han nationality from Northern Jiangsu, and smoking raises this risk of NSCLC occurrence in individuals with mutation genotypes of ADPRT rs1136410.
