1.Epidemiologic Study of Children Dental Fluorosis in Endemic Fluorosis Areas in Tianjin

Objective To know the epidemiological characteristics of children dental fluorosis in endemic fluorosis areas in Tianjin. Methods Ten prevalent areas were selected and 55 villages were collected by cluster sampling in 2008. Such indicators of endemic fluorosis as the content of water fluoride in the investigated villages,the state of dental fluorosis among the children aged 8-12 years,the concentration of urine fluoride were investigated. Results The average rates of dental fluorosis were different among different areas(?2=773.571,P=0.000) ,positively correlated with the content of water fluoride(r=0.924,P=0.000) . In serious prevalent areas,the average rates of dental fluorosis were different among age groups(?2=16.351,P=0.003) . The geometric mean of urine fluoride was different among different areas(H=75.356,P=0.000) ,positively correlated with the content of water fluoride(r= 0.266,P =0.000) . In the same area,no significant difference was observed in the geometric mean of urine fluoride among age groups. Conclusion In Tianjin,endemic fluorosis is still very serious and the efficient control will be promoted.
2.Efficiency Evaluation of Iodized Salt Supply in Prevention of Iodine Deficiency Disease in Tianjin in 1995-2005

Objective To evaluate the efficiency of ten years of iodized salt supply in prevention and control of iodine deficiency disease (IDD) in Tianjin. Methods 1 200 pupils aged 8-10 years with 1:1 of sex ratio were chosen in the regions where iodized salt supply had been conducted for 10 years (1995-2005), the prevalence rate of goiter, the level of urine iodine and the iodized salt consumption were investigated. Results The prevalence rate of goiter of children aged 8-10 years in Tianjin decreased from 32.6% to 1.8% after 10 years of iodized salt supply, the median of urinary iodine increased from 96 ?g/L to 228 ?g/L. In the high water iodine regions, the prevalence rate of goiter was 8.1%, the median of urinary iodine was 576 ?g/L. Conclusion 10 years of iodized salt supply can significantly prevent and control IDD in the regions where there is iodine deficiency in drinking water, but the problem should not be neglected that iodine excess in the regions where the concentration of iodine in drinking water is higher. Reasonable iodine supplement is the key to prevent and control IDD as a whole.
3.Incision concrescence of a nano-sized hydroxyapatite-zirconia bioceramical scalpel

BACKGROUND: Hydroxyapatite is a kind of ideal orthopedic material, but its Iow strength and brittleness need to improve. The research suggested that the improvement of the toughness and strength of bioceramics which composite hydroxyapatite (HAP) and zirconia (3% mol yttria-stabilized cubic zirconia) and fabricated ceramic scalpel possesses of the wound recover faster with smaller side. So it is significant that research on the incision concrescence of nano-sized hydroxyapatite-zirconia bioceramical scalpel.OBJECTIVE: To observe the effects of incision using nano-sized HAP-TZP bioceramics as scalpel.DESIGN: A single sample study.SETTING: Second Department of Orthopaedics, the Second Affiliated Hospital of Harbin Medical University and Harbin Institute of Technology.MATERIALS: The experiment was carried out in the Animal Laboratory of the Second Affiliated Hospital of Harbin Medical University (Provincial Laboratory) from March to May 2006. A total of 54 SD rats, of 4 months old, weighing 120-180 g, of both genders, were selected in this study. The experiment materials included nano-sized hydroxyapatite Quartz Clock Company), ammonia (Suihua Chemical Reagent Company), and anhydrous ethanol (Tianjin Chemical Reagent Company).METHODS: The nano-sized Hydroxyapatite/zirconia composite powder was synthesis by rubbing according to 40% hydroxyapatite + 60% zirconia powders (mass ratio) were mixed and milled. The knife-edge biomaterials, which fabricated by hot pressed sintering, was grinding and used in incision experiment after autoclaving. The back of the rats was shaved and removing a patch of skin under ether anesthesia made a length of 2 cm open excision wound and stitched up the tear by 1# string. The each 18 rats were sacrificed on 3, 7 and 14 days post-wounding and used for analyses respectively. A full-thickness specimen containing the wound was dissected out from each of the surgical sites. The hematoxylin-eosin (HE) staining was applied in the pathological observation.MAIN OUTCOME MEASURES: The tissue sections of back and the results of pathological observation. RESULTS: All 54 rats were involved in the results analysis without any loss. There was long gap with minimal cellular reaction in the scalpel wounds that has not healed on day 3. There was inflamed cells infiltration at the wounded margin edge. By week 1, the tiny detritus which some of them appear the repaired response of epidermis rend were still obviously found in the area of the scalpel wounds where the detritus was found to be significantly less, the wounds changed smooth and started to heal. The optical microscopy observed indicated that there was incomplete epithelialization and hyalinization of dermal collagen at the wounded area. There were obvious inflamed cells to infiltrate in the margin subcutaneous tissue of the wounds where the newborn loose collagenous fibers filled in scalpel wounds was the tissue repairing response phase. There was a great deal of the infiltrated white blood cells in the subcutaneous tissue. The tiny detritus of epidermis by the optical microscopy observed rend could not be found in the area of the scalpel wounds which were healed by week 2. The newborn capillary nets appeared in the newborn repaired collagen fibers tissue. The inflamed cells to be infiltrated were reduced. The compact and strong collagenous fibers filled in scalpel wounds were the normal wound repairing and tissue. The white blood cells infiltrated obvious reduction in the subcutaneous tissue. The normal blood cells in the newborn capillaries could be seen. CONCLUSION: The experiments prove that a HAP-ZTP bioceramic scalpel, which possesses excellent bioactive, is nontoxic, nonallergenic and noncarcinogenic for incisions.
4.Disinfection and Management of Oral Apparatus

OBJECTIVE To search for available mode of oral apparatus′s disinfection and management.METHODS To use new ways of oral apparatus′s disinfection and management that are enhancement of recognition,normal disinfecting procedure,available disinfection room,enhancement of management,new management system,building digitizing management and so on.RESULTS To build effective system and decrease abrasion and uselessness of oral apparatus.CONCLUSIONS Effective disinfection and management are very important for prevention of the cross infection and ensuring of daily use of oral apparatus.

7.Iodine nutritional status in high-risk areas of iodine deficiency disorders in Tianjin

Objective To investigate the nutritional status of children and women of childbearing age in high-risk areas of iodine deficiency disorders in Tianjin in 2008-2010 and to provide a scientific basis for iodine supplementation.Methods Hangu District was selected as survey point from 2008 to 2010,Dagang District and Jingan County were increased in 2010.Hangu District was divided into five areas,one township(town) that was not high iodine was selected from each area,four administrative villages were selected from each township(town),and 15 households were selected from each administrative village.Three township(town) were selected from Dagang and Jingan County,respectively.Two administrative villages were selected from each township(town),and 20 households were selected from each administrative village.Iodine level of salt samples from each household was determined.In 2009,one primary school was selected from each survey township(town),thyroid volume of 40 8-10-year-old students was examined,and urine iodine of 20 students was tested.In 2010,three township(town) were selected from each survey point,two primary school were selected from each survey township(town),40 8-l0-year-old students were selected from each primary school,urine iodine and thyroid volume of the students were determined.150,50 and 60 women of childbearing age from Hangu District were selected from 2008 to 2010,and 60 were selected from Dagang District and Jingan County,respectively.Urine iodine of the women was tested.Iodine content of salt was tested by a semi-quantitative test kit; diagnosis of endemic goiter was based on the Diagnostic Criteria of Endemic Goiter (WS 276-2007); urinary iodine test was based on the Arsenic Cerium Catalytic Spectrophotometry (WS/T 107-2006).Results Coverage rates of iodized salt in Hangu District in the 3 years were 65.0% (195/300),70.3% (211/300) and 90.8% (274/300),respectively.Coverage rates of iodized salt in Dagang District and Jinghai County were 40.0%(48/120) and 38.3%(46/120).In 2009,the median urinary iodine of children aged 8-10 in Hangu District was 205.4 μg/L and in 2010,the medians urinary iodine of children aged 8-10 of the three survey point were 193.0,293.1 and 216.8 μg/L.Goiter rates of children aged 8-10 in Hangu District were 4.24%(9/212) and 3.59% (9/251),and in Dagang District and Jinghai County were 3.60% (9/250) and 5.44%(13/239).The medians urinary iodine in women of childbearing age in Hangu District in the 3 years were 134.9,196.1 and 229.5 μg/L,respectively.The medians urinary iodine in women of childbearing age in Dagang District and Jinghai County were 316.9 and 190.9 μg/L.Conclusions The nutritional status of high-risk areas of iodine deficiency disorders in Tianjin are better than the national limit of enhanced iodine requirements.Due to higher water iodine in the same areas,some people are iodine excess,to which we should pay more attention.
8.Observation of the effect of ulinastatin in the treatment of children with severe pneumonia

Objective To observe the effect of ulinastatin in the treatment of children with severe pneumonia. Methods 86 children with severe pneumonia were divided into two groups according to the hospital card number:treatment group(n=45) and control group(n=41).All of them were given routine treatment,while the patients of the treatment group were given ulinastatin(5 000 U· kg-1 · time-1 ,1-2times/d,7d) in addition.The time of tem-perature drop to normal,the time of dyspnea improve,the time of dyspnea disappear,the time of lung rale disappear and length of hospital stay were observed.Then,the therapeutic effect was evaluated after treatment for 7 days.Results The treatment group had 21 cases excellence,21 cases improvement and 3 cases failure, the total effective rate was 93.33%,the control group had 17 cases excellence,15 cases improvement and 9 cases failure,the total effective rate was 78.04%,there was significant difference between the two groups(χ2 =4.17,P<0.05).The time of temperature drop to normal,the time of dyspnea improve,the time of dyspnea disappear,the time of lung rale disappear and length of hospital stay in the treatment group were (3.37 ±1.51)d,(3.12 ±1.72)d,(7.15 ±2.45)d,(10.75 ±2.47)d, respectively,which in the control group were (5.02 ±1.78)d,(4.82 ±1.51)d,(9.08 ±2.85)d,(13.22 ±2.85)d, there were significant differences between the two groups(t=4.71,4.91,3.42,4.41,all P<0.01).Conclusion Ulinastatin has good effect on children with severe pneumonia.It has value in application.
9.Relationships among immune traits and MHC B-LBII genetic variation in three chicken breeds.

We have assessed the relationships between immune trait (antibody titers of Sheep red blood cell, SRBC; Avian influenza, AI; Newcastle disease, ND) and varieties of MHC B-LBHII Gene in local chicken breeds (Wenshang Barred chicken, LH; Laiwu Black chicken, LWH; and Jining Bairi chicken, BR). We selected 300 chickens randomly from the three indigenous chicken populations. The variations of MHC B-L BII gene were detected by directly DNA sequencing and polymerase chain reaction-single strand conformation polymorphism (PCR-SSCP). The results indicated that there were about 19-22 nucleotide mutations in the three local breeds, which could affect 16-18 amino acid variations. Another results indicated that there was significantly relationship between seven to eight SNPs of the MHC B-LBII region and some immune traits (P < 0.05 or P < 0.01). Both locus G97A and locus T138A were found in the three species, which were significantly related to the antibodies of SRBC, ND and AI antibody titers (P < 0.05). Among them, the locus G97A was significantly associated with ND antibody titers (P < 0.05) in BR chicken, with SRBC antibody titers (P < 0.05) in LWH chicken, and with H9 antibody titers (P < 0.05) in LH chicken. Furthermore, locus T138A was significantly associated with H9 antibody titers in BR and LH chickens (P < 0.05). All those results suggest relationships among the different varieties of MHC B-LBII and immune traits in the three local breeds.
