1.Evaluation of globle and regional left ventricular myocardial systolic function in patients with coronary stenosis by real-time three-dimensional speckle tracking imaging

Objective To explore the value of real‐time three‐dimensional speckle tracking imaging (RT‐3D‐STI) in the early detection of globle and regional left ventricular myocardial systolic function in patients of coronary stenosis without regional wall motion abnormality. Methods 64 coronary stenosis patients without regional wall motion abnormality were divided into 2 groups according to angiography (CAG):without coronary stenosis group( n = 20) and coronary stenosis group ( n = 44), and all the myocardial segments of the patients were divided into 5 groups:without coronary stenosis group normal myocardial segments ( n = 340 ), coronary stenosis group normal myocardial segments ( n = 235 ), mild coronary stenosis myocardial segments( n = 126), moderate coronary stenosis myocardial segments( n =213) and severe coronary stenosis myocardial segments( n =174). Real‐time three‐dimensional full volume of left ventricle was obtained, left ventricular global area strain (GAS)and regional area strain(AS) of 17 myocardial segments, etc, were measured by RT‐3D‐STI respectively. The parameters between the two groups and among the five sub groups were compared, the correlation between GAS and coronary artery Gensini score was also analyzed. Results GAS in coronary stenosis group significantly reduced compared with without coronary stenosis group( P <0 0.5) S.egmental AS gradually reduced with the increasing of the degree of the coronary stenosis. There was no statistically difference of segmental AS among without coronary artery group normal myocardial segments, coronary stenosis group normal myocardial segments and mild coronary stenosis myocardial segments( P >0 0.5) S.egmental AS in moderate and severe coronary stenosis myocardial segments significantly reduced ( P < 0.05 ), and that in severe coronary stenosis myocardial segments was lower than that in moderate coronary stenosis myocardial segments, the differences were statistically significant ( P < 0.05 ). In addition, GAS was significantly correlated with coronary artery Gensini score( r =0.828, P <0.01). Conclusions RT‐3D‐STI can quantitatively assess the early changes of left ventricular globle and regional myocardial systolic function in patients with coronary stenosis.

3.The efficacy evaluation of multiplane operations in obstructive sleep apnea-hypopnea syndrome.

OBJECTIVE:
To evaluate the therapeutic effect of multi-plane operations in treating obstructive sleep apnea-hypopnea syndrome (OSAHS).
METHOD:
One hundred and fifteen patients with OSAHS diagnosed by polysomnography were treated with uvuplopalatopharyngoplasty. Eighteen of them were treated combining with nasal septal construction. Twenty six patients also received nasal septal construction and partial inferior turbinectomy. One patients also received Genioglossus advancement and partial resection of corpus linguae. Five patients also received partial resection of corpus linguae. Some patients with the nasal disease and/or the lingual hypertrophy; AHI > 40 and/or BMI > 30 are received tracheotomy before general anaesthesia.
RESULT:
According to the postoperative follow-up 43 patients, were cured, 46 patients were improved, 26 patients were invalid, the effective power was 77.4%.
CONCLUSION
The operative effective power were increased by multi-plane operations in OSAHS patients. The serious complication were prevented through tracheotomy before general anaesthesia in multi-plane operations of severe OSAHS.

4.Establishment of rat cerebral hemorrhage model by type Ⅳcollagenase injection

Objective To establish a cerebral hemorrhage model in rats by local injection of type Ⅳcollagenase, and explore the factors affecting the speed and volume of hematoma formation Methods A total of 150 healthy male Wistar rats weighing from 200 to 250 g were randomly divided into 3 groups, control, collagenase, and collagenase/heparin groups Animals in those groups received injection of saline, collagenase (0 2 U/?l), and collagenase (same dose) and heparin (2 U/?l) with different volume respectively at the caudate nucleus The volume of the hematoma in the rats ( n =6 at each time point) was observed 6, 12, and 24 h, and 3, 7d after the injection Results Permeation of blood was found in 12 h after injection in control group Hematoma about 3 mm in diameter was found in 3 d after injection in collagenase group, while in collagenase/heparin group, it was found in 24 h Conclusion Cerebral hemorrhage model established by local injection of collagenase/heparin in saline solution is ideal and reliable, and the size of hematoma is in correlation with the volume of solutions injected into the brain
5.Effects of rhBNP on vascular remodeling and hs-CRP in rabbits after angioplasty

ObjectiveTo reveal the relation between rhBNP and vascular remodeling,the relation between rhBNP and levels of serum high sensitive C reactive protein (hs-CRP).Methods20 male New Zealand rabbits were randomly divided into two groups.All the rabbits were constructed to be models of angioplasty with BC.After injury,the control group were feed with standard forage,the rhBNP group received standard forage plus sibeutaneous rhBNP [0.1mmg/(kg · day),20 days].The rabbits were killed 4weeks after initial injury.The injured segments were dissected free from arteria iliaca externa,processed for histological and morphological study by TD 2000 analysis system of pathology image.The data of vascular remodeling was determined.The levels of hs-CRP were measured at the beginning and the end of the experiment.ResultsThere was significant difference in AUIEM and AUEEM[ (643.2±134.06) μm2 vs (493.7±139.32) μm2 ; (1495.1±204.98)μm2 vs (1265.9±232.56)μm2,P < 0.05 ],extremely significant difference in residual cavity areas and ratios of stenosis[ (519.1±93.47) μm2 vs (308.6±86.50) μm2;(18.68±7.72) μm2 vs (38.04±8.32) μm2,P < 0.01 ].There was no significant difference between the areas of media (P> 0.05).rhBNP reduced the levels of hs-CRP on the 28th day.ConclusionsrhBNP can benefit vascular remodeling after angioplasty which might be through its anti -inflammatory action.
6.Applicatioin of micro-screw for mandibular incisors intrusion: a clinical and cephalometric study

Objective To evaluate the effects of micro-screw on the intrusion of lower incisors and the leveling of curve of Spee in the process of orthotherapy.Methods Fifteen patients (5 males,10 females) with deep-bite were selected,aged from 11.3 to 28.8 years,with average of 17.2 years.All of them were Angle Class Ⅱ Classification 1 malocclusion and had a curve of Spee larger than 3 mm.Orthotherapy of extraction was adopted for the first premolars of two upper mandible and second premolars of two lower mandible.All these patients had micro-screws implanted at root apex area between lower lateral incisor and canine bilaterally,and when the 0.457 mm× 0.635 mm (0.018 in ×0.025 in) Ni-Tisquare wire was internalized,the strength of intrusion from micro-screws was used.Lateral cephalometric radiographs were taken before loading the micro-screw and immediately when the curve of Spee was leveled.Two linear and two angular measurements were selected for cephalometric analysis.Results The following results were noted: the lower incisors were significantly intruded for a mean of (4.10±0.98) mm; the mandibular molars were not significantly extruded for a mean of (0.91±0.69) mm; some lower incisors proclined and some lower incisors retroclined for a mean of (-0.09±4.12)°.After bite opening,the mandibular plane angle was minimally altered for a mean of (0.25±0.70)°; the ratio of lower incisor intrusioin to lower molar extrusion was 4.5: 1.0.Conclusions The combined use of micro screw with Ni-Ti wire is capable of enhancing the intrusion of lower incisors and leveling the curve of Spee,and it has minimal side effects on the posterior teeth.
7.Efficient Screen of Toxic Herbal Medicine in Zhuang Gu Guan Jie Pill Based on Zebrafish Model

This study aimed at efficiently screening out the toxic herbal medicines in Zhuang Gu Guan Jie pill (ZGGJ) based on the zebrafish model.Zebrafish embryos at 1 day post fertilization (dpf) were administered by ZGGJ and the water decoction (WD) and ethanol extract (EE) of 12 herbal drugs in it with various concentrations.The number of embryo deaths after the administration was recorded at 2-6 dpf and 2-8 dpf,respectively.Embryonic micro-morphology of zebrafish (3 dpf) was observed,LC50 value at 6 dpf was calculated by SPSS16.0.As a result,both the WD and EE of ZGGJ,fructus psoraleae,radix dipsaci,olibanum,myrrh,radix angelicae pubescentis and aucklandiae radix,and the EE of Caulis Spatholobo and Rhizoma Cibotii presented high organ toxicity to the juvenile zebrafish,featuring yolk sac swelling,deforming,blackening,indicating hepatotoxicity in the zebrafish.Their LC50 value at 6 dpf was from 33.2 μg·mL-1 to 392.7 μg·mL-1 (LD50 value was converted to crude drug.The same bellow.).The WD and EE of Epimedii Folium at 1,500 μg· mL-1 led to yolk sac swelling and blackening.The LC50 at 6 dpf varied at 1,100-1,350 μg·mL-1,showing a certain toxicity of Epimedii Folium.The WD of Caulis Spatholobo and Cibotii Rhizoma and both WD and EE of Rhizoma drynariae,taxilli herba and prepared rehmanniae radix didn' t change the toxicity to the organs in zebrafish.LC50 value were over 1,500 μg·mL-1,presenting favorable safety.In conclusion,it was demonstrated that the zebrafish model efficientlyidentified the toxicity of different herbal medicines from ZGGJ and efficiently screened out the potential toxicants in vivo with the advantages of real-time,dynamic and large scale for traditional Chinese medical compounds.
8.Effect of extract ethanol and water decoction of Psoralea corylifolia L.on bone development and their toxicities in zebrafish

OBJECTIVE To investigate the bone development activity and differences in safety of ethanol extract (EE) and water decoction (WD) of Psoralea corylifolia L.efficiently.METHODS Zebrafish larvae were co-exposed to prednisolone 25 μmol· L-1 and different concentrations of EE and WD (0.1,1.0,10 and 100 mg crude drug· L-1),and etidronate disodium (ED) 30 mg·L-1.All these groups were incubated at 28.5℃ until 9 dpf.The medium solution was changed every other day.Zebrafish skeleton at 9 dpf was stained with alizarin red and inspected under an optical microscope,in addition,the death toll and organ toxicity of zebrafish were also observed.The mRNA expression of osteoprotegrin (OPG) and receptor activator of NF-κB ligand (RANKL) in 9 dpf zebrafish were determined with fluorescence quantitative PCR.Zebrafish embryos (1 dpf) were exposed to various concentrations of EE (10,20,30,35,40,50 and 60 mg crude drug· L-1),WD (10,50,100,125,150,175,200 and 500 mg crude drug· L-1),psoralen (12.5,25.0,50.0,100.0,200.0 and 400.0 μmol·L-1) and bakuchiol (1,5,10,25 and 50 μmol· L-1).Embryonic morphology of zebrafish (3 dpf) was inspected with an optical microscope and the death toll of embryos or larvale was counted from 2 dpf to 9 dpf and LC50was calculated.Components of EE and WD ware analyzed by HPLC method.RESULTS Both EE (0.1 mg crude drug· L-1) and WD (1.0 mg crude drug· L-1) groups could increase the staining area and optical density values of zebrafish skeleton compared with prednisolone group (P<0.01),indicating the increase in bone mineralization;the OPG mRNA expression in both EE and WD (1.0 mg crude drug· L-1) groups increased,while the RANKL mRNA expression decreased (P<0.01) and the ratio of OPG/RANKL improved obviously (P<0.01).Embryos exposed to EE,WD,psoralen and bakuchiol showed swelling of the heart and yalk sac,and decrease in GOT.The LC50 of WD and psoralen was 5~8 and 5~21 times that EE and bakuchiol,respectively.The composition and relative content of EE and WD also varied considerably.CONCLUSION Bone development activity and toxicity of EE are both stronger than those of WD.The lipid soluble characteristic components of Psoralea corylifolia L.,may be critical components of toxicity.
9.The clinical research of low dose scanning protocol for aorto-iliac and lower extremity arteries CT angiography

Objective To study the feasibility of reducing radiation dose in aorto-iliac and lower extremity arteries CT angiography (CTA) with low tube voltage (100 kV) and automatic tube current modulation(ATCM).Methods Totally 61 patients requiting aorto-iliac and lower extremity arteries CTA for clinical reasons were prospectively enrolled in study.The patients were randomly assigned to 3 groups:Group A(120 kV),Group B (100 kV) and Group C (100 kV with automatic tube current modulation).Both quantitative and qualitative analysis were included in this research.Group C was divided into obese group(BMI≥ 24.9 kg/m2) and normal group(BMI < 24.9 kg/m2).The radiation doses were analyzed respectively among two groups.Results The subjective evaluations of image quality for axial,MIP and VR were good.There were no significant differences among group A,B and C in SNR and CNR (P >0.05).The effective dose of group A,B and C was 8.20 mSv,5.36 mSv,and was 7.48 mSv,respectively Group B was 34.6% less than group A.Group C was 39.5% more than group B,and there was no significant differences between group A and C (P > 0.05).The effective dose of group C1 was 7.11mSv,group C2 was 9.69 mSv,the E with group C1 were significantly less than group C2 (t =-3.163,P <0.05),the effective dose of group C1 was 13.3% less than group A (Z =-2.822,P < 0.05),but the group C2 was more than group A (P > 0.05) and group B (Z =-3.426,P < 0.05).Conclusions Lowkilovoltage (100 kV) CT scanning protocol is feasible in multi-detector CT angiography for aortoiliac and lower extremity arteries.Automatic low tube voltage (100 kV) with automatic tube current modulation (ATCM) scanning protocol can be used for someone with BMI less than 24.9 kg/m2.
10.Low-dose MSCT pulmonary angiography: low kVp with individual-adapted low tube current

Objective To assess the feasibility of low-dose MDCT pulmonary angiography (MDCTPA) by using low kVp combined with individual-adapted low tube current protocol and noise reduction filter algorithms.Methods Fifty-six consecutively patients with MDCTPA were enrolled in the study.The patients were randomly divided into group A (120 kVp,100 mAs) and group B (100 kVp,individual-adapted tube current about 1.0 mAs/kg).Two series of images were reconstructed in group B:B1 with standard filter algorithms (FC13,B1) and B2 with noise reduction filter algorithms (FC11,B2).The image noise,quality and radiation dose in two groups were compared.Results The CTDIvol,DLP and E of group B were 5.1 mGy,83.3 mGy· cm and 1.4 mSv respectively,which were reduced by 64.3%,66.4% and 66.7%,respectively (F =32.57,12.32,11.98,P < 0.05) when compared with those in group A.Compared with group A,the image noise in B1 was increased by 118.2% (t =10.05 P <0.05),the central and peripheral SNR and CNR and scores were increased by 51.5%,46.6%,50.8%,45.3%and 20.4%,respectively(t =7.20,6.30,6.58,5.54,8.35,P < 0.05).Compared with B1,the image noise in B2 was reduced by 32.5% (t =6.12,P <0.05),the central and peripheral SNR and CNR and scores were increased by 46.2%,46.2%,45.9%,46.9% and 18.4% (t =3.19,3.55,2.95,3.37,5.42,P <0.05).Compared with group A,the image noise in B2 group was increased by 47.3% (t =4.03,P <0.05),the central and peripheral SNR and CNR and scores were reduced by 29.1%,21.8%,28.2%,19.6% and 8.2%(t =4.06,2.82,3.68,2.22,3.02,P<0.05).Conclusions Keeping effective diagnostic image quality,the protocol with low kVp and individual-adapted low tube current for low-dose MDCT pulmonary angiography would be effective and feasible,and the radiation dose would be significantly reduced.
