1.Challenges and suggestions for human genetic resource management in international cooperation of medical institutions caused by the Implementation Rules of the Regulations on the Management of Human Genetic Resources: A case study of Peking University Cancer Hospital

Objective:According to the international cooperation project of Peking University Cancer Hospital on human genetic resource management practices, combined with the development direction of human genetic resource management laws and regulations, and propose reference suggestions for medical institutions to strengthen human genetic resource management.Methods:Sort out the projects that Peking University Cancer Hospital obtained international cooperation approval on the government platform of the Ministry of Science and Technology from July 2019 to June 2023, analyze the current situation of human genetic resource management in the hospital, summarize the challenges brought by the implementation of new regulations on human genetic resource management in medical institutions, and propose corresponding suggestions.Results:A total of 1276 international cooperation projects on human genetic resources have been approved, including 345 initial declarations and 931 change declarations. Involving 453 studies, including 286 clinical trials of drugs or devices on the market, accounting for 63.13%, and 100 clinical trials of Phase I drugs, accounting for 34.97% of the market studies. On average, there are 3.14 changes per project for listed research, and 1.56 changes per project for non listed research.Conclusions:Regulations on the Management of Human Genetic Resources ( short for Rules) limit the management scope of international cooperation projects involving human genetic resources and delegate management authority to medical institutions. Adjusting the scope of application for international cooperative clinical trial filing may result in some administrative approval projects being transferred to filing. The approval process for international cooperative scientific research projects on human genetic resources has been adjusted. Suggest medical institutions to strengthen the management of samples and intellectual property outside the scope of application of Rules.Strengthen the entire process management of international cooperation in scientific research. Pay attention to and timely communicate the dynamics of human genetic resource management.
2.Research on the standardized path of medical ultrasound equipment use and management based on total quality management

Objective:To study the standardization path of medical ultrasound equipment management based on total quality management,and to improve the service level of medical ultrasound equipment in clinical diagnosis and treatment activities.Methods:According to the process of medical ultrasound equipment use,the risk nodes were determined,the formation path and damage probability of equipment use risk were analyzed by probabilistic graphical model,and the standardized path of medical ultrasound equipment use was formulated based on time control and standardized use mode.35 medical ultrasound equipment in clinical use in The People's Hospital of Jianyang from 2022 to 2023 were selected,the use and management of 33 medical ultrasound equipment in 2022 were managed in the conventional management mode(33 units),and the use and management of 35 medical ultrasound equipment in 2023(2 new units)were adopted in the total quality management mode.The difference in equipment operation quality,risk ratio and service effect under different modes were compared.Results:The failure rates of medical ultrasound diagnostic equipment,medical ultrasound treatment equipment and other medical ultrasound equipment using the total quality management model were(0.763±0.68)%,(0.833±0.65)%and(0.969±0.64)%,respectively,which were lower than those of the conventional management mode,the difference was statistically significant(t=3.331,5.348,5.940,P<0.05).The proportion of risk problems such as component damage,delayed diagnosis and treatment,resource waste and social impact of medical ultrasound equipment using total quality management mode were(2.023±0.99)‰,(1.569±0.89)‰,(2.541±1.05)‰ and(1.238±0.63)‰,respectively,which were lower than those of the conventional management mode,the difference was statistically significant(t=3.935,3.676,5.699,3.189,P<0.05).The scores of clinicians,nurses,operating technicians,equipment engineers and management personnel involved in the use and management of medical ultrasound equipment were for the service effect of the total quality management model were(95.797±2.13)points,(93.880±2.12)points,(94.605±1.91)points,(91.387±3.20)points and(96.275±1.82)points,respectively,which were higher than those of the conventional management mode,the difference was statistically significant(t=4.536,5.477,6.869,2.943,4.673,P<0.05).Conclusion:The standardized path of medical ultrasound equipment management based on total quality management can improve the operation quality of equipment,reduce the incidence of clinical use risk,and enhance the level of clinical service.
3.Reflections on medical research management from judicial judgments on disputes related to service invention patents

Objective:To understand the legal disputes that are prone to arise in judicial practice regarding service inventions, this study aims to explore prevention strategies for patent disputes related to service inventions in medical research institutions from the perspective of scientific research management, guided by the standardization of intellectual property management and the promotion of achievement transformation.Methods:This study sorted out the judgments of patent related cases related to official inventions on the China Judgment Document Network from January 1, 2018 to December 31, 2023, analyzed the case situation, summarized the types of disputes, the reasons for disputes, and propose countermeasures.Results:A total of 469 dispute cases related to service invention patents were analyzed, with over 90% being ownership disputes. Among them, 413 cases involved patent ownership disputes, and 73.1% of the cases were judged as service inventions. There were only 14 cases involving disputes over the authorship rights of inventors and designers of inventions and creations, and 78.6% of cases had made judgments against disputed inventors and designers who were not the actual inventors or designers of the patent in question. There were 42 cases involving disputes over the distribution of benefits such as rewards and remuneration for inventors and designers in the field of service inventions and creations. In most cases, the judgment units should pay bonuses to inventors and designers.Conclusions:This study suggests that research institutions establish a disclosure system for job related scientific and technological achievements, guiding researchers to actively and timely disclose job related scientific and technological achievements to their respective research institutions, stimulate innovation enthusiasm, improve intellectual property management system and service mechanism, strengthen the construction of scientific research integrity and avoid scientific research integrity issues such as the naming of patent inventors and designers.
4.Clinical characteristics of different subtypes of severe influenza in 40 neonates from a hospital in He'nan Province

Abstract: Objective To compare and analyze the clinical characteristics, diagnosis, and treatment of different subtypes of severe influenza in neonates to provide a reference for the diagnosis and treatment of neonatal severe influenza. Methods A cohort of 40 neonates with severe influenza who were hospitalized in the neonatology ward of Children's Hospital Affiliated to Zhengzhou University between January 2019 to December 2023 were selected and divided into two groups based on the virus subtype: influenza A (n=23) and influenza B (n=17). A retrospective analysis was conducted to compare general information, clinical manifestations, auxiliary examinations, complications, and treatment outcomes of neonates with severe influenza A and B infection. Results The number of days of hospitalization was longer in cases of influenza A than that of influenza B. The proportion of neonates with severe influenza A who exhibited fever was higher than that for influenza B, and a higher percentage of those with fever had peak temperatures ranging from 38.1 ℃ to 39 ℃. Gastrointestinal symptoms, including vomiting and diarrhea leading to dehydration, were more evident in severe influenza B cases. The proportion of influenza A cases with abnormal creatine kinase-MB isoenzyme levels (>25 U/L) was higher than that of influenza B, and the differences were statistically significant (P<0.05). There were no significant differences between the two types of influenza in other clinical manifestations, the incidence of pneumonia/respiratory failure complications, peripheral blood leukocyte count and classifications, the proportion of abnormal aspartate aminotransferase (AST) (>40 U/L), alanine aminotransferase (ALT) (>40 U/L), and creatine kinase (CK) (>200 U/L), and lactate dehydrogenase (LDH) values (all P>0.05). In terms of treatment, neonates treated with Oseltamivir within 48 hours of onset mainly suffered from influenza A. Among those treated with Oseltamivir, the proportion of influenza A cases whose body temperature returned to normal within 24 hours was relatively higher, whereas, for those whose temperature returned to normal within 24-72 hours, the proportion was relatively higher in influenza B cases. These differences were statistically significant (all P<0.05). Conclusions Severe neonatal influenza usually occurs in winter and spring. After severe infection, fever is more obvious in neonates with influenza A, which is more likely to cause myocardial cell damage. Neonates with influenza A can be treated with Oseltamivir earlier and return to normal body temperature faster than those with influenza B after Oseltamivir treatment. Gastrointestinal symptoms are more common in neonates with severe influenza B.
5.Clinical characteristics and pathogen profile of 133 cases of community-acquired necrotizing pneumonia in children

Objective To investigate the clinical data of pediatric community-acquired necrotizing pneumonia (NP) in 2023, summarizing its clinical characteristics and pathogen profile distribution. Methods The clinical data of hospitalized children with NP in 2023 at Henan Children's Hospital were retrospectively analyzed. Comparisons were made with those in the non-NP group to summarize the clinical symptoms, pathogenetic features, laboratory indices and imaging changes, treatment, and prognosis. Results Among the 133 NP cases, there were 64 males and 69 females, with a median age of 7 years. In the non-NP group, there were 61 cases, including 38 males and 23 females, with a median age of 6. The NP group showed significantly higher proportions of chest pain, dyspnea, poor mental response, refusal of food/dehydration signs, and extrapulmonary complications compared to the non-NP group (P<0.05). The hospitalization duration of NP children was 18 days, with a fever duration of 12 days. They had a peripheral blood leukocyte count of 11.44×109/L, C-reactive protein of 37.44 mg/L, procalcitonin of 0.207 ng/mL, blood sedimentation of 40 mm/h, D-dimer of 1.99 μg/mL, lactate dehydrogenase of 455 U/L, and serum ferritin of 233.8 μg/L, all significantly higher compared to the non-NP group (P<0.05). Enhanced chest CT in the NP group revealed necrotic foci on the (17.47±6.56) day of the disease, with some cases complicated by pleural effusion, pleural thickening, bronchial stenosis or dilatation, pulmonary atelectasis, pulmonary embolism, etc., accompanied by different degrees of impaired lung function. Bronchoscopy revealed mucosal congestion and edema, with some showing mucosal erosion and necrosis, phlegm thrombus obstruction, and rice soup-like secretions. Pathogens were detected in 129 (97.0%) children in the NP group, with the top five infectious agents being Mycoplasma pneumoniae, Streptococcus pneumoniae, adenovirus, rhinovirus, and influenza B virus. There were 76 cases (57.1%) of single pathogen infection, and 53 cases (39.8%) of mixed infection. The mixed infection group had higher peripheral blood leukocyte counts, poor mental response, refusal of food/dehydration signs, extrapulmonary complications, and application of gamma globulin compared to the non-mixed infection group (P<0.05). The prognosis was generally good after anti-infective and symptomatic supportive treatment, with some cases having residual respiratory sequelae. Conclusions Children with NP have a long course of disease, high inflammatory indexes, and are prone to intrapulmonary and extrapulmonary complications and lung function impairment. Chest CT enhancement scanning and bronchoscopy are helpful for early recognition of NP. The pathogen of NP infections in 2023 is dominated by drug-resistant Mycoplasma pneumoniae, with some children experiencing multi-pathogen mixed infections. The prognosis is generally good with aggressive treatment.
6.The Integrated Theory of Health Behavior Change in elderly patients with chronic obstructive pulmonary disease

Objective:To evaluate the effect of inhaled medication compliance intervention in elderly patients with chronic obstructive pulmonary disease (COPD) based on integrated theory of health behavior change.Methods:A total of 117 elderly COPD patients who visited Respiratory Department of Beijing Chaoyang Hospital Affiliated to Capital Medical University from July to December 2021 were selected by the convenient sampling method. According to the random number table method, they were divided into the experimental group and the control group. 7 cases fell off during follow-up, and a total of 110 cases were finally included in the study, of which 55 cases were in the experimental group and 55 cases in the control group. The control group adopted the conventional health education method, while the experimental group adopted the intervention plan of inhalation medication compliance of elderly COPD patients based on the integrated theory of health behavior change. The accuracy of inhalation device use, inhalant medication compliance, forced expiratory volume in the first second (FEV 1) and Chronic Obstructive Pulmonary Diseases Assessment Test (CAT) scores were compared between the two groups before intervention and 3 months after intervention. Results:After 3 months of intervention, the inhalation device use accuracy, inhalation medication compliance in experimental group were higher than those in the control group, the score of CAT in experimental group was lower than that in the control group, the differences were statistically significant ( P<0.05). There was no statistically significant difference in FEV 1 between two groups ( P>0.05) . Conclusions:The intervention plan for inhalation medication compliance in elderly patients with chronic obstructive pulmonary disease based on the integrated theory of health behavior changes can improve their inhalation medication compliance and improve their quality of life.
7.A scoping review of maternal readiness for hospital discharge

Object:To conduct a scoping review of maternal readiness for hospital discharge, so as to provide references for future clinical research and practice on maternal and infant care.Methods:Using the scoping review guidelines of Joanna Briggs Institute in Australia as the methodological framework, the relevant studies on maternal readiness for hospital discharge were searched by computer on PubMed, Web of Science, Scopus, CNKI, Wanfang Database, Chinese Science and Technology Journal Database and China Biology Medicine disc. The search period was from the establishment of the databases to October 20, 2022.Results:A total of 24 artiles were included. The influencing factors of maternal readiness for hospital discharge included sociodemographic factors, maternity factors, psychological factors, children factors, family social factors, medical staff related factors and other factors. The intervention measures of maternal readiness for hospital discharge included the establishment of discharge preparation team, the measures from hospital admission to prenatal period, the measures from postpartum to the day of discharge and the measures after discharge. There were many kinds of scales for maternal readiness for hospital discharge, and there was no unified assessment standard.Conclusions:The level of maternal readiness for hospital discharge is different, and its influencing factors should be paid attention to. In the future, further research can be conducted on intervention measures and evaluation standards for maternal discharge readiness, providing references for improving the level of maternal discharge readiness.
8.Discussion on the strategy of the development of the cancer basic research in a cancer hospital under the new situation based on the supporting guidance of the NSFC

Objective:According to the characteristics of basic science research in the field of cancer during the 14 th five-year plan period, the scientific construction pattern which is available for the development of basic research in a cancer hospital is explored. Methods:A series of policy adjustments and the directions of research and development promulgated by the National Natural Science Foundation of China (NSFC) which supports basic and applied basic research in various scientific fields during the middle and late period of the 13 th five-year plan and the beginning year of the 14 th five-year plan is expounded and analyzed. Results:The cancer basic research will be more systematic, interdisciplinary and clinical fusion, and the clinical application of basic research achievements will be the key purposes under the new research pattern.Conclusions:The construction of basic research in the hospital should be combined with the characteristics of the new research pattern. The overall scientific and systematic layout of the research should be strengthened, the interdisciplinary research model should be established, and the clinical applied basic research should also be encouraged and supported, as well as the efficiency of transformation of basic research achievements needs to be further improved.
9.Research on the scientific research performance evaluation of young scientists in hospitals based on Analytic Hierarchy Process and Data Envelopment Analysis

Objective:To establish a performance evaluation index system and evaluation method for young scientists in hospitals, and to provide reliable evidence for management departments in evaluating, selecting outstanding young scientists and formulating tailored training programs.Methods:Literature analysis, expert consultation, analytic hierarchy process and data envelopment analysis were adopted in this article to evaluate the scientific research performance of young scientists in hospitals.Results:The scientific research data of young scientists under 45 years of age in Peking University Cancer Hospital from 2015 to 2019 were collected, and their scale efficiency and technical efficiency were analyzed by using index system and its weight and propose the improvement measures to achieve the purpose of evaluation and reform.Conclusions:This method is useful for the effective evaluation of the young scientists in the hospital, and is helpful to improve the level of scientific research management, as well as realize the virtuous circle of matching the input and output of scientific and technological innovation.
10.Problems and strategies on transformation of scientific and technological achievements in hospitals

Objective:To analyze some problems identified in the transformation of scientific and technological achievements in hospitals and put forward corresponding strategies in order to provide reference for other hospitals.Methods:This paper analyzes some problems identified in the transformation of scientific and technological achievements in hospitals.Results:The common problems identified in the research include: too many steps in the transformation of scientific and technological achievements, market information is asymmetry, there is also risks of leakage and infringement, lack of professional institutions and specialized talents. Several strategies to solve these problems were proposed.Conclusions:The management department needs to create a good external environment to adapt to the transformation of achievements. That is to integrate the intellectual property layout and the transformation of achievements; optimize the transformation model; implement the whole process of security supervision to protect the confidentiality; improve the construction of transformation platform; strengthen the construction of professional talent team. This can promote the "bench-to-bed" transition of medical scientific research, as well as assure the long-term development of scientific and technological innovation.
