1.Influences of situation-background-assessment-recommendation communication mode on caring behavior and caring load of home caregivers for hemodialysis patients

Objective To explore the influences of situation-background-assessment-recommendation communication mode for caring behavior and caring load of hemodialysis patients home caregivers. Methods A total of 216 caregivers who took care of hemodialysis patients were selected in our hospital for treatment and were averagely divided into observation group ( implement personalized education plan for nursing education ) and control group ( implement routine nursing education) . We compared caring behavior and caring load between two groups by LC-GADs and caregiver′s pressure load scale. Results The 5 abstract behavior dimensions in patients of observation group including endorsement, evaluation, guidance, form a regular, keep balance were (3. 25 ± 0. 52), (3. 58 ± 0. 41), (3. 49 ± 0. 50), (3. 21 ± 0. 71), (3. 45 ± 0. 48) which were higher than those of the control group, the differences were statistically significant ( t = -7. 364, -4. 011, -9. 025, -4. 254,-3. 896;P<0. 01); the specific behavior scores in observation group were significantly higher than that of control group as well (P <0. 01). In the observation group, emotions and economic stress, the body and personal time pressure, psychological pressure and family relationship were (1. 82 ± 0. 35), (1. 93 ± 0. 35), (1.85 ±0.54) and (1. 68 ± 0. 45) which were lower than those of the control group (t =4. 365, 3. 563, 3. 657, 4. 615; P <0. 01). Conclusions Caregivers of hemodialysis patients receive situation-background-assessment-recommendation communication mode which can obviously improve caregivers caring behavior, reduce caregivers caring load, and improve the quality of care.
2.Effect observation of preventive effect of constraint-induced movement therapy in hemodialysis-related hypotension

Objective To explore the relation between constraint-induced movement therapy (CIMT) and hemodialysis-relative hypotension.Methods From April 2015 to March 2016,80 patients with hemodialysis therapy were divided into the control group (n=40) and the observation group (n=40). The observation group adopted CIMT, including position adaption training,respiratory control training,regular turning-over training,griping training,sitting up training,and standingup training. While patients in the control group received common hemodialysis nursing. Results The incidence rates of hypotension were 12.50% and 25.00% in the observation group and the control group respectively (χ2=2.885,P<0. 05). The average duration of hemodialysis in the observation group was (3.44±0.71) h,and it was (2.39±0.58) h in the control group (P<0.05). All patients in the observation group completed hemodialysis,and 80.0% patients in the control group completed hemodialysis (χ2=7.520,P<0.05). The adaptation of hemodialysis and degree of satisfaction in the observation group was significantly higher than those in the control group (P<0.05).Conclusions CIMT was effective in reducing hemodialysis-related hypotension,and it can ensure the procedure of hemodialysis.
3.Construction and practice of the training program of blood purification specialist nurses oriented by post competence

Objective:To construct a training program of blood purification specialist nurses oriented by post competence, and to explore its application effects.Methods:230 blood purification specialist nurses from 4 ClassⅢ Grade A hospitals in Harbin were selected as research objects from July to December 2018. The lottery method was used to randomly select nurses from two hospitals in control group ( n=110) , and nurses from the other two hospitals in experimental group ( n=120) . Control group was treated with traditional training, and experimental group was given a training program oriented by post competence. The training effect was evaluated with the score of theoretical knowledge, operational skills and Post Competency Assessment Scale for Blood Purification Specialist Nurses. Results:After training, scores of theoretical knowledge and operational skills of experimental group were higher than those of control group with statistical differences ( t=7.81, 5.05; P<0.01) . After training, the total score of the Post Competency Assessment Scale for Blood Purification Specialist Nurses of experimental group was higher than that of control group also with a statistical difference ( t=10.03, P<0.01) . Conclusions:The training program of blood purification specialist nurses oriented by post competence helps to improve nurses' theoretical knowledge, operational skills and job competence.
